BACKGROUND: Polycystic ovary syndrame (PCOS) has been shown to cause a reduction in quality of life. Little is known on the psychosocial aspects associated with PCOS, especially on women s own experiences regarding the syndrome. Therefore, the aim of this study was to analyze the psychosocial impact of PCOS by measuring the health-related quality of life and exploring the women s own experiences of PCOS. METHODS: A two-phase model of combination of quantitative and qualitative research approaches was conducted, a crass-sectional survey to pravide quantitative data on the quality of life and a qualitative approach to understand the women s experience in relation to the PC OS sympt?ms. For the first phase, 109 women with PC OS and 104 healthy contrais were evaluated with the 36-item short-form health survey (SF-36). Additionally, semi-structured interviews (n=30) were conducted to facilitate in-depth exploration of PC OS women s experience. RESUL TS: PCOS patients showed significant reductions in almost ali SF-36 domains of quality of life when compared with healthy contrais (physical function 76.5:!:20.5 and 84.6:!:15.9, respectively; physical rale function 56.4:!:43.3 and 72.6:!:33.3; general health 55.2:!:21.0 and 62.5:!:17.2; vitality 49.6:!:21.3 and 55.3:!:21.3; social function 55.3:!:32.4 and 66.2:!:26.7; emotional rale function 34.2:!:39.7 and 52.9:!:38.2; mental health 50.6:!:22.8 and 59.2:!:20.2). Thematic analysis revealed reports of feeling abnormal , sadness, afraid and anxiety. These feelings were related to four symptoms commonly experienced by women with PCOS: excess hair grawth; irregular or absent menstruation, infertility and obesity. CONCLUSIONS: Polycystic ovary syndrame impacts women both physically and psychosocially. Owing to this, women with PCOS need not only medical treatment for. the reproductive, esthetic and metabolic repercussions, but also the care of a multidisciplinary team, since PCOS is not just a physical condition, but also an important psychosocial problem that affects various aspects of a woman s life

PURPOSE. To compare the effectiveness of posterior sub-Tenon's infusion (STi) and intravitreal injection (IVI) of triamcinolone acetonide (TA) for treatment of refractory diffuse diabetic macular edema.METHODS. Thirty-six phakic diabetic patients with refractory diffuse diabetic macular edema were prospectively enrolled. Patients randomly received either 40 mg STi or 4 mg IVI of TA. Comprehensive ophthalmic evaluation was performed at baseline and 1, 2, 4, 8 +/- 1, 12 +/- 2 and 24 +/- 2 weeks after treatment. Macular morphologic changes detected by optical coherence tomography and visual acuity, intraocular pressure, and lens status were evaluated.RESULTS. Twenty-eight patients (28 eyes) completed the 24-week study. Central macular thickness was significantly reduced in the IVI group when compared with the STi group at 2, 4, 8, 12, and 24 weeks after treatment (P < 0.01). Mean visual acuities (in logarithm of the minimum angle of resolution [logMAR]) at week-4, -8, and -12 follow-up examinations were significantly higher in the IVI group (0.74, 0.75, and 0.82, respectively) when compared with the STi group (0.88, 0.88, and 0.90, respectively; P < 0.01). A significant change from baseline in mean intraocular pressure (mm Hg) was seen at weeks 4 (+/- 3.21) and 8 (+/- 3.35) in STi the group (P < 0.01), and at week 8 (+/- 2.78) in the IVI group (P < 0.05). No patient had cataract progression during the study.CONCLUSIONS. Although the number of patients and length of follow-up in this preliminary study were limited, the changes in central macular thickness and visual acuity observed after treatment suggest that IVI TA may be more effective than STi for the management of refractory diffuse diabetic macular edema. Further studies are needed to confirm these preliminary findings.

We investigated the influence of myocardial collagen volume fraction (CVF, %) and hydroxyproline concentration (mu g/mg) on rat papillary muscle function. Collagen excess was obtained in 10 rats with unilateral renal ischemia for 5 wk followed by 3-wk treatment with ramipril (20 mg . kg(-1) . day(-1)) (RHTR rats; CVF = 3.83 +/- 0.80, hydroxyproline = 3.79 +/- 0.50). Collagen degradation was induced by double infusion of oxidized glutathione (GSSG rats; CVF 5 2.45 +/- 0.52, hydroxyproline = 2.85 +/- 0.18). Nine untreated rats were used as controls (CFV = 3.04 +/- 0.58, hydroxyproline = 3.21 +/- 0.30). Active stiffness (AS; g . cm(-2) . %L-max(-1)) and myocyte cross-sectional area (MA; mu m(2)) were increased in the GSSG rats compared with controls [AS 5.86 vs. 3.96 (P< 0.05); MA 363 +/- 59 vs. 305 +/- 28 (P< 0.05)]. In GSSG and RHTR groups the passive tension-length curves were shifted downwards, indicating decreased passive stiffness, and upwards, indicating increased passive stiffness, respectively. Decreased collagen content induced by GSSG is related to myocyte hypertrophy, decreased passive stiffness, and increased AS, and increased collagen concentration causes myocardial diastolic dysfunction with no effect on systolic function.

In Experiment 1, effects of an intravaginal progesterone insert (CIDR) for synchronization of estrus in non-suckled cycling primiparous Bos indicus beef cows were evaluated. Cows received either no treatment (Control), prostaglandin F(2 alpha) (PGF), or CIDR (6 d)+prostaglandin F(2 alpha) (CIDR+PGF). There were greater (P<0.05) percentages of cows inseminated and pregnant in 4 d of breeding season in CIDR+PGF (81.6%; 48.7%) than in PGF (53.8%; 32.5%) and Control cows (14.6%; 8.5%). In Experiment 2, efficacy of a treatment with CIDR (6 d)+prostaglandin F(2 alpha)+48 h temporary weaning (CIDR+PGF+TW) associated or not with equine chorionic gonadotropin (eCG) for synchronization of estrus in suckled primiparous B. indicus beef cows was evaluated. Cows were assigned to receive either no treatment (Control), CIDR+PGF+TW, CIDR+PGF+TW+200 IU of eCG, or CIDR+PGF+IW+400 IU of eCG. There were greater (P<0.05) percentages of insemination and pregnancy in 4 d of breeding season in treatment groups (47.0%; 27.5%) than in Control cows(4.5%; 3.0%) and in cows treated with eCG (50.9%; 29.4%) than in cows treated only with CIDR+PGF+TW (39.4%: 23.7%). No effects of eCG dosage were detected. The percentage of cows inseminated and pregnant in a 30 d breeding season were greater (P<0.05) in cows assigned to synchronization treatments (53.3%; 37.2%) than in Control cows (35.3%; 21.4%). In the beginning of the BS, non-suckled primiparous cows treated with CIDR+PGF with estrous cycles having been initiated following calving had greater estrous detection and pregnancy rates than PGF and Control cows, and suckled primiparous cows had improved estrous detection and pregnancy rates when eCG was associated with CIDR+PGF+TW. In order to know which stage of fruit development is better for acerola industrialization, we studied the PME specific activity, pectin content and vitamin C at various stages of development. The acerola fruits were classified according to colour and weight in five stages: immature green (2.62-3.21 g), green (4.04-4.83 g), mature green/yellow (5.03-5.88 g), pale-red (6.16-6.77 g) and ripe mature (6.92-8.37 g). The results showed that the highest content of pectin and vitamin C occurred at the immature green stage, 4.51 +/- 0.1% yield, 2424 mg/100 g of pulp and decreased as fruit ripened, 2.99 +/- 0.03% yield, 957 +/- 0.0 mg/100 g of pulp, respectively. However, at the same stages, the values of PME specific activity were lowest, 0.61 +/- 0.01 and 0.55 +/- 0.0 units g(-1)/g of pulp, respectively. The highest value of PME activity was 2.08 +/- 0.01 units g(-1)/g of pulp in the green stage. (C) 2001 Elsevier B.V. Ltd. All rights reserved.

We hypothesized that reducing the size of the ovulatory follicle using aspiration and GnRH would reduce the size of the resulting CL, reduce circulating progesterone concentrations, and alter conception rates. Lactating dairy cows (n=52) had synchronized ovulation and AI by treating with GnRH and PGF(2 alpha) as follows: Day -9, GnRH (100 mug); Day -2, PGF(2 alpha) (25 mg); Day 0, GnRH (100 mug); Day 1, AI. Treated cows (aspirated group; n=29) had all follicles > 4 mm in diameter aspirated on Days -5 or -6 in order to start a new follicular wave. Control cows (nonaspirated group; n=23) had no follicle aspiration. The size of follicles and CL were monitored by ultrasonography. The synchronized ovulation rate (ovulation rate to second GnRH injection; 42/52=80.8%) and double ovulation rate of synchronized cows (6/42=14.3%) did not differ (P > 0.05) between groups. Aspiration reduced the size of the ovulatory follicle (P < 0.0001; 11.5 +/- 0.2 vs 14.5 +/- 0.4 mm), and serum estradiol concentrations at second GnRH treatment (P < 0.0002; 2.5 +/- 0.4 vs 5.7 +/- 0.6 pg/mL). The volume of CL was less (P < 0.05) for aspirated than nonaspirated cows on Day 7 (2,862 +/- 228 vs 5,363 +/- 342 mm(3)) or Day 14 (4,652 +/- 283 vs 6,526 +/- 373 mm(3)). Similarly, serum progesterone concentrations were less on Day 7 (P < 0.05) and Day 14 (P < 0.10) for aspirated cows. Pregnancy rate per AI for synchronized cows was lower (P < 0.05) for aspirated (3/21=14.3%) than nonaspirated (10/21=47.6%) cows. In conclusion, ovulation of smaller follicles produced lowered fertility possibly because development of smaller CL decreased circulating progesterone concentrations. (C) 2001 by Elsevier B.V.

We analyzed clinicopathologic data, immunophenotype, and Epstein-Barr virus (EBV) status in 96 cases of Hodgkin disease (HD) in juveniles (younger than 20 years) and adults (20 years or older) from 2 distinctive states in Brazil. We studied 34 juvenile (group 1) and 16 adult (group 2) cases from Ceara and 31 juvenile (group 3) and 15 adult (group 4) cases from São Paulo. Ceara has a socioeconomic profile similar to a developing country; São Paulo is in better economic condition. Mixed cellularity (MC) was the major histologic subtype among groups 1 (22 [65%]), 3 (21 [68%]), and 4 (7 [47%]); nodular sclerosis (NS) was more frequent in group 2 (8 [50%]). EBV infection was observed in 61 cases (64%), including the following (among others): group 1, MC, 22 (65%) and NS, 4 (12%); group 2, NS, 3 (19%) and MC, 2 (12%); group 3, MC, 16 (52%) and NS, 1 (3%); and group 4, MC, 7 (47%). There was predominance of EBV+ HD cases in group 1 compared with group 3. HD in Brazilian patients is highly associated with EBV infection, but geographic differences reflect histologic subtypes and age distribution.

The higher concentration during exercise at which lactate entry in blood equals its removal is known as maximal lactate steady state (MLSS) and is considered an important indicator of endurance exercise capacity. The aim of the present study was to determine MLSS in running rats. Adult male Wistar sedentary rats, which were selected and adapted to treadmill running for three weeks, were used. After becoming familiarized with treadmill running, the rats were submitted to five exercise tests at 15, 20, 25, 30 and 35 m/min velocities. The velocity sequence was distributed at random. Each test consisted of continuous running for 25 min at one velocity or until the exhaustion. Blood lactate was determined at rest and each 5 min of exercise to find the MLSS. The running rats presented MLSS at the 20 m/min velocity, with blood lactate of 3.9±1.1 mmol/L. At the 15 m/min velocity, the blood lactate also stabilized, but at a lower concentration (3.2±1.1 mmol/L). There was a progressive increase in blood lactate concentration at higher velocities, and some animals reached exhaustion between the 10 th and 25 th minute of exercise. These results indicate that the protocol of MLSS can be used for determination of the maximal aerobic intensity in running rats.
