This article examines the longitudinal relation between positive affect (PA) and sleep in 126 spousal Alzheimer's disease caregivers. Caregivers underwent 4 yearly assessments for the Positive and Negative Affect Schedule, the self-rated Pittsburgh Sleep Quality Index, and actigraphy to objectify nighttime total sleep time, wake after sleep onset, and percentage of sleep. Increased levels of PA and a greater positivity (i.e., positive-to-negative affect) ratio were significantly associated with better subjective sleep over the entire study period. Yearly increases in PA-even when controlling for negative affect (NA)-and in the positivity ratio were also associated with better subjective sleep. PA and actigraphy measures showed no significant relations. Increased PA is longitudinally associated with better sleep in dementia caregivers largely independent of NA.

This dissertation presents the concept of Deliberative Transformative Moment and the instrument to identify it, in a further attempt to bridge the gap between deliberation theory and practice. A transformative moment in the deliberative process occurs when the level of deliberation is either lifted from low to high or drops from high to low. In order to identify such a moment, one has to look at the context and dynamics of the group discussion. This broadening of the unit of analysis is a big difference from other existing instruments to measure the level of deliberation, such as the Deliberative Quality Index –DQI, which focuses primarily on the individual speech acts. Consistent with the theoretical framework of consociational and deliberation approaches, the observed discussions took place among two deeply divided groups, Colombian ex-combatants from both the extreme left and the extreme right. Moving beyond a pure Habermasian perspective, this study finds that besides pure rational arguments, there are some contexts in which personal stories, jokes and self-interests, acting as justification of arguments, have either a positive or a negative impact on deliberative transformative moments. Although this research has a strongly qualitative orientation, reliability tests scored high, giving it strength as a reliable and valid research method that shedding some light on the sort of speech acts that enhance deliberation and those that detract from it.

In a cohort study among 2751 members (71.5% females) of the German and Swiss RLS patient organizations changes in restless legs syndrome (RLS) severity over time was assessed and the impact on quality of life, sleep quality and depressive symptoms was analysed. A standard set of scales (RLS severity scale IRLS, SF-36, Pittsburgh Sleep Quality Index and the Centre for Epidemiologic Studies Depression Scale) in mailed questionnaires was repeatedly used to assess RLS severity and health status over time and a 7-day diary once to assess short-term variations. A clinically relevant change of the RLS severity was defined by a change of at least 5 points on the IRLS scale. During 36 months follow-up minimal improvement of RLS severity between assessments was observed. Men consistently reported higher severity scores. RLS severity increased with age reaching a plateau in the age group 45-54 years. During 3 years 60.2% of the participants had no relevant (±5 points) change in RLS severity. RLS worsening was significantly related to an increase in depressive symptoms and a decrease in sleep quality and quality of life. The short-term variation showed distinctive circadian patterns with rhythm magnitudes strongly related to RLS severity. The majority of participants had a stable course of severe RLS over three years. An increase in RLS severity was accompanied by a small to moderate negative, a decrease by a small positive influence on quality of life, depressive symptoms and sleep quality.

BACKGROUND Pyoderma gangrenosum (PG) is a rare, neutrophilic, ulcerative skin disease that is difficult to treat, especially when unresponsive to steroids. OBJECTIVES To determine whether canakinumab is an effective and safe treatment in PG. METHODS Five adult patients with clinically and histologically confirmed steroid-refractory PG were enrolled in this prospective open-label study. They received canakinumab 150 mg subcutaneously at week 0 with an optional 150 mg at week 2 in case of an inadequate response [Physician's Global Assessment (PGA) ≥ 2], and an optional 150-300 mg at week 8 depending on PGA. The primary clinical end point was clinical improvement (PGA at least -1 from baseline) and/or complete remission (PGA 0 or 1) at week 16. Real-time quantitative polymerase chain reaction was performed on skin samples to quantify cytokine mRNA levels. RESULTS Interleukin (IL)-1β and its known target genes IL6, CXCL8 and IL36A were significantly increased in lesional skin of PG. Under canakinumab therapy, four of five patients showed a decrease in target-lesion size, PGA and Dermatology Life Quality Index (DLQI), and three of five achieved complete remission. The mean diameter of target lesions decreased from 4·32 ± 2·6 cm at visit 1 to 0·78 ± 1·3 cm at visit 7 (P = 0·03). Mean DLQI decreased from 15 ± 5 at visit 1 to 8 ± 4 by visit 7 (P = 0·01). Adverse effects were reported in two patients: fatigue in one and worsening of disease at a nontarget lesion in the other. CONCLUSIONS Our data indicate that IL-1β plays a key pathogenic role in PG and canakinumab may represent a therapeutic option for steroid-refractory PG.

BACKGROUND Hand eczema (HE) is a common skin disease with major medical psychological and socio-economic implications. Onset and prognosis of HE are determined by individual as well as environmental factors. So far, most epidemiological data on HE have been reported from Scandinavian and recently German studies. OBJECTIVE To investigate the characteristics and medical care of patients with chronic HE (CHE) in Switzerland, and identify risk factors. METHODS In this cross-sectional study, data from patients with chronic HE were obtained by means of medical history, dermatological examination and patient questionnaires. Multiple logistic regression analysis was applied to identify risk factors for high severity and dermatology life quality index (DLQI). RESULTS In seven dermatology departments, 199 patients (mean age 40.4 years, 50.8% female) with CHE (mean duration 6.6 years) were enrolled. Moderate to severe HE was reported by 70.9% of patients, and was associated with age <30 or >50 years, localization of lesions and pruritus. Because of the CHE, 37.3% of patients were on sick leave over the past 12 months, 14.8% had changed or lost their job. Practically all patients applied topical therapy, 21% were treated with alitretinoin, and 21% with psoralen plus UVA light (PUVA). The effects on the health-related quality of life was moderate to large in 33.7% and 39.4% of CHE patients, respectively. Factors associated with a high impact on DLQI (mean 9.7 ± 5.8) were female sex, lesions on back of the hands and pruritus as well as mechanical skin irritation and wearing gloves. CONCLUSION In agreement with recent studies, the Swiss data demonstrate the high impact of CHE on medical well-being, patient quality of life and work ability. As it is associated with an intense use of health care services, high rate of sick leave, job loss and change, CHE may cause a high socio-economic burden.

Little is known about how sleep disruption impacts physical health among the homeless. The association between homelessness, quality of sleep and physical health were investigated in the current study. Convenience sampling was used to select participants from a pool of people attending the programs of Ecclesia Ministries. Interviews were conducted with 32 persons from the Boston metropolitan area, of whom 23 were currently homeless. The researcher assessed level of sleep disturbance, number of health problems and degree of homelessness using a standard demographic questionnaire, the General Health Questionnaire-12 (GHQ-12) and the Pittsburgh Sleep Quality Index (PSQI). Our results found evidence of significant sleep disturbance as well as significant mental and physical health problems in the sample. Correlational analyses provided partial support for the hypothesis that degree of homelessness impacts both sleep quality and physical health. Future work should investigate whether change in homelessness status alters sleep quality and physical health and also whether interventions may be utilized in this understudied and vulnerable population.

Se realizó un seguimiento de la maduración de kiwis (Actinidia celiciosa (A. Chev.) Liang et Ferguson cv. Hayward) en una plantación comercial del sudeste de la provincia de Buenos Aires (Argentina), con la finalidad de definir índices de madurez y calidad y compararlos con los estándares vigentes en Nueva Zelanda y Chile. A intervalo semanal, desde las 13 semanas después de plena floración, se tomaron muestras de frutos para evaluar las siguientes variables: peso fresco, volumen, porcentaje de semillas negras, contenido de materia seca, acidez titulable y pH. De acuerdo con los mismos, el valor mínimo de sólidos solubles y el porcentaje de semillas negras exigido se alcanzaron durante la semana 21 después de plena floración y la firmeza de pulpa presentaba aún valores superiores al umbral recomendado. El momento óptimo de cosecha correspondió a plena temporada. Los resultados sugieren que la acidez titulable podría constituir otro índice de madurez. El porcentaje de materia seca no sería útil en fruta de plena temporada dado que los incrementos significativos se registran tempranamente. Sin embargo, puede ser útil como índice de calidad. Se plantea la posibilidad de utilizar la relación MS/AT como índice de calidad y de cosecha.

En el noroeste del Chubut, los álamos se encuentran establecidos en cortinas de protección en predios de pastoreo o donde se cultivan pasturas y forrajes en secano. El Populus nigra ‘Italica’ es el clon más difundido en plantaciones lineales, ubicadas en diferentes calidades de sitio. No existen antecedentes sobre crecimiento en altura e Índice de Sitio en la zona. Se seleccionaron ocho sitios de muestreo, donde fueron apeados 24 árboles dominantes, a los que se les realizó análisis fustal. Con los pares de datos de edad-altura, resultantes del análisis de fuste de los árboles muestra, se ajustó el modelo de Chapman-Richards mediante técnicas de regresión no lineal. Se construyó una familia de curvas de crecimiento en altura, según la metodología de curva guía. Las curvas de Índice de Sitio, basadas en altura y edad, se construyeron mediante deducción matemática a partir de la función de crecimiento en altura, tomándose como edad de referencia 25 años a la altura del pecho. Se definieron cinco calidades de sitio, con un rango de 4 metros, con Índices de Sitio comprendidos entre 19 y 35 m.

The injection of gas into sub-seabed aquifers may lead to the displacement of hypoxic and hypersaline fluids (reservoir formation water) major environmental risk. To investigate this risk, the impact of formation water release on the macrofaunal community in a mesocosm experiment at Solbergstrand was conducted. 20 boxcores were exposed to 4 treatments (high salinity, hypoxic, mixed and tidal) during two weeks. The abundance of macrofauna was quantified for each treatment and richness, eveness and biodiversity indices calculated. The data are reported in this dataset.

A land classification method was designed for the Community of Madrid (CM), which has lands suitable for either agriculture use or natural spaces. The process started from an extensive previous CM study that contains sets of land attributes with data for 122 types and a minimum-requirements method providing a land quality classification (SQ) for each land. Borrowing some tools from Operations Research (OR) and from Decision Science, that SQ has been complemented by an additive valuation method that involves a more restricted set of 13 representative attributes analysed using Attribute Valuation Functions to obtain a quality index, QI, and by an original composite method that uses a fuzzy set procedure to obtain a combined quality index, CQI, that contains relevant information from both the SQ and the QI methods.

Debido al carácter innovador de los objetivos científico-tecnológicos planteados al inicio de la presente Tesis doctoral, además de la primera recopilación bibliográfica, a lo largo del desarrollo de la misma se ha realizado una actualización bibliográfica continua centrada en los desarrollos relativos a la instrumentación, las aplicaciones y los procesos de interpretación de las técnicas de tomografía eléctrica. Durante el desarrollo del presente trabajo, se han realizado una serie de campañas diferenciadas, tanto por los ambientes geológicos y las condiciones antrópicas en los que se han efectuado los perfiles geoeléctricos, como por la profundidad y los objetivos de la prospección. En esas campañas se han obtenido, mediante el programa Res2DInv, las secciones tomográficas con la configuración de variables por defecto y se ha realizado una interpretación estructural de las mismas. Como parte central del desarrollo de esta investigación se han efectuado varios procesos de optimización: Comparación y optimización de los dispositivos empleados para la medición de perfiles de resistividad de cara a la obtención de secciones tomográficas que presenten la mayor resolución en distintos ambientes geológicos. Selección de los rangos de representación (lineales y logarítmicos) y de la gradación cromática que, pudiendo adoptarse de forma generalizada, permita la mejor interpretación gráfica para usuarios no avanzados. Diseño de criterios para la selección del factor de amortiguamiento, como parámetro crítico en los procesos de inversión tomográfica de perfiles geoeléctricos de superficie, de forma que los modelos resultantes alcancen la mejor relación entre fiabilidad y error respecto a los valores medidos. Paralelamente a los procesos anteriores se ha definido un índice de calidad de la inversión, que es directamente normalizado y que permite evaluar el grado en el que pueden considerarse fiables tanto los valores de resistividad obtenidos como su distribución espacial. El índice desarrollado (IRI) está basado en la influencia que tiene la resistividad inicial del modelo de inversión en los valores de cada una de las celdas de la malla de inversión. Tras un exhaustivo análisis estadístico se ha verificado que, a diferencia de otros índices utilizados hasta la fecha, dicho índice no está afectado ni por la profundidad de investigación y la resolución de los dispositivos, ni por la distribución de resistividades que presente el subsuelo. ABSTRACT Due to the innovative character of the scientific and technological objectives proposed at the beginning of this thesis, in addition to preparatory bibliographic reference, a constant bibliographic update on developments related to instrumentation, applications and interpretation processes for electric tomography has been performed all through the development of the work. Several measuring campaigns were performed during the development of the thesis. These campaigns, in which the geoelectric profiles were measured, varied in several aspects, both in the geological and anthropic conditions, and also in the depth and objectives of the prospection. Tomographic sections were obtained and interpretated by making use of the Res2DInv software configured with the default variables. As part of the core development in this research, several optimization processes have been done: Comparison and optimization of the devices used in the measurement of resistivity profiles. This optimization allows increasing the resolution of tomographic sections in different geological conditions. Selection of the best visual representation for the tomographic sections. This involves the selection of the range of values and type of scale (linear vs. logarithmic) as well as the chromatic graduation. This selection allows improving the interpretation of tomographic sections for the non-expert eye. Devising of criteria for the selection of the damping factor, as the critical parameter in processes of tomographic inversion of superficial geoelectrical profiles. The use of these criteria allows selecting the best factors to obtain the best ratio between reliability and error (in relation to the mean values) for the resulting model. In addition to the aforementioned processes, and as part of the work within this thesis, it has been defined a normalized quality index for inversion. This index allows evaluating the degree of reliability of the obtained resistivity values and spatial distribution. The developed index, termed IRI in the thesis, is based on the influence shown by the initial resistivity of the inversion model on the values of each of the cells of the inversion mesh. After an exhaustive statistical analysis it was verified that the IRI index is neither affected by the investigation depth, nor the resolution of the devices, nor the resistivity’s distribution of the sub soil. This independency is a unique characteristic of the IRI index, not present in other indexes developed and used till date.
