Introduction: Trastuzumab (T) is a cornerstone in the treatment of patients with HER2-overexpressing advanced breast cancer and development of resistance to T is a major therapeutic problem. HER-2 is part of a highly interactive signaling network that may impair efficacy of endocrine therapy. A sequential treatment design was chosen in this trial to ensure complete resistance to single agent therapy before receiving both a non-steroidal aromatase inhibitor (AI) and T. Any kind of clinical activity with combined treatment of AI and T after progression of single agent treatments could indicate restoration of sensitivity as a consequence of cross-talking and networking between both pathways. Methods: Key eligibility criteria included postmenopausal patients (pts.) with advanced, measurable, HER-2 positive (assessed by FISH, ratio (≥2)), HR positive disease and progression on prior treatment with a non-steroidal AI, e.g. letrozole or anastrozole, either in an adjuvant or advanced setting. Pts. received standard dose T monotherapy either weekly or three-weekly in step 1 and upon disease progression, continued T in combination with letrozole in step 2. The primary endpoint was clinical benefit response (CBR: CR, PR or SD for at least 24 weeks (+/- 1 week) according to RECIST) in step 2. Results: Thirteen pts. were enrolled in five centers in Switzerland. In step 1, six pts. (46%) achieved CBR. Median time to progression (TTP) was 161 days (Range: 50 - 627). Based on data collected until the end of May 2010, CBR was observed in seven out of the eleven evaluable pts. (64%) in step 2, including one pt. with partial response. Four of the seven pts. within step 2 that achieved CBR also had CBR in step 1. Seven out of eleven pts. have documented tumor progression during step 2 treatment. Median TTP for all eleven pts. was 184 days (range 61 - 471). Mean time on study treatment (TTP in step 1 plus TTP in step 2) for pts. reaching step 2 was 380 days (range 174 - 864). Adverse events were generally mild. Conclusion: Results of this proof-of-principle trial suggest that complete resistance to both AI and T can be overcome in a proportion of pts. by combined treatment of AI and T, as all pts. served as their own control. Our results appear promising for a new treatment strategy which offers a chemotherapy-free and well-tolerated option for at least a subset of the pts. with HR positive, HER-2 positive breast cancer. Further trials will need to corroborate this finding.

This control apparatus is equivalent to the ERAD system that regulates sterol synthesis in yeasts and mammals but that uses distinct E3 ubiquitin ligases, of the HMGR degradation 1 (HRD1) type, to direct destruction of HMGR. Hence, the general principles for the management of sterol and triterpene saponin biosynthesis are conserved across eukaryotes but can be controlled by divergent regulatory cues.

Cancer-related inflammation has emerged in recent years as a major event contributing to tumor angiogenesis, tumor progression and metastasis formation. Bone marrow-derived and inflammatory cells promote tumor angiogenesis by providing endothelial progenitor cells that differentiate into mature endothelial cells, and by secreting pro-angiogenic factors and remodeling the extracellular matrix to stimulate angiogenesis though paracrine mechanisms. Several bone marrow-derived myelonomocytic cells, including monocytes and macrophages, have been identified and characterized by several laboratories in recent years. While the central role of these cells in promoting tumor angiogenesis, tumor progression and metastasis is nowadays well established, many questions remain open and new ones are emerging. These include the relationship between their phenotype and function, the mechanisms of pro-angiogenic programming, their contribution to resistance to anti-angiogenic treatments and to metastasis and their potential clinical use as biomarkers of angiogenesis and anti-angiogenic therapies. Here, we will review phenotypical and functional aspects of bone marrow-derived myelonomocytic cells and discuss some of the current outstanding questions.

Les maladies rhumatismales sont fréquemment observées chez les personnes âgées et ont un impact considérable sur la qualité de vie des personnes en souffrant. Peu d'études suisses sur la situation épidémiologique et sur l'impact de ce type de maladies sur la consommation des services de santé existent. Cette thèse a pour but d'étudier les connaissances actuelles à partir de la littérature suisse et étrangère et d'établir un bilan en Suisse au moyen d'une enquête de population effectuée en 1997. Une revue systématique de littérature a été effectuée. En dépit d'une grande variété des approches méthodologiques rendant délicates les comparaisons géographiques et temporelles, la prévalence des maladies rhumatismales chez les personnes de 65 ans et plus semble être homogène entre pays et stable temporellement. particulièrement dès 1980. Celle-ci est élevée et augmente rapidement avec le très grand âge. De plus, les femmes sont plus fréquemment atteintes que les hommes. Notre revue montre que le recours aux soins ambulatoires (médecins, chiropraticiens, traitements ambulatoires des hôpitaux) lié aux maladies rhumatismales est important. L'impact de ces maladies sur les hospitalisations est, par contre, moins clairement établi. Les nouvelles générations de personnes en souffrant semblent avoir plus recours aux services de santé que les précédentes. Ces maladies sont aussi à l'origine d'une forte consommation d'anti-inflammatoires non-stéroïdiens avec comme conséquence une multitude de complications. En dépit de son efficacité et de son utilité, le recours à l'arthroplastie est sous- utilisé. Notre analyse se base sur une enquête réalisée auprès d'un échantillon représentatif des individus âgés de 15 ans et plus résidant de manière permanente en Suisse en 1997 : la prévalence des maladies rhumatismales en Suisse s'élève à 41 % chez les personnes âgées de 65 ans et plus, dont 48 % chez les femmes et 31 % chez les hommes. Ces prévalences sont inférieures à celles relevées dans la littérature probablement en raison de notre définition relativement restrictive des maladies rhumatismales. Ces dernières augmentent de 50 % le nombre attendu de consultations chez un médecin ou un chiropraticien et de 30 % le nombre attendu d'hospitalisations. Les personnes souffrantes ont. en outre, une probabilité de recours aux services de Soins à domicile 1,7 fois plus élevé que les autres. Aucun impact sur le nombre de traitements ambulatoires en milieu hospitalier n'a été trouvé. Nos résultats sont comparables à ceux relevés dans la littérature internationale et suisse, sauf pour les traitements ambulatoires des hôpitaux. En 1990, sur les 983'400 personnes de 65 ans et plus (recensement fédéral de la population de 1990), 403'200 personnes souffraient de maladies rhumatismales. Quelque 5'334'900 consultations chez un médecin ou un chiropraticien, 4'959'300 consultations chez un médecin et 216'800 hospitalisations étaient imputables aux personnes de 65 ans et plus toutes causes de consultations confondues, dont 1'008'000 consultations chez un médecin/chiropraticien, 927'300 chez un médecin et 98'500 hospitalisations imputables aux maladies rhumatismales. Selon ie scénario (( tendance )) des projections démographiques publiées par l'Office Fédéral de la Statistique. d'ici 2040, le nombre de personnes souffrant de maladies rhumatismales en Suisse risque d'augmenter de 80 % (en supposant que la prévalence reste stable), affectant 726'500 sur 1'772'000 personnes de 65 ans et plus. Cette augmentation est la conséquence de l'accroissement prévu de la population de 65 ans et plus dans la population générale. Le nombre global de consultatiordhospitalisations risque d'augmenter dans les mêmes proportions si le recours aux services de santé reste stable. En effet. en 2040, quelque 9'613'100 consultations chez un médecinichiropraticien, 8'936'200 consultations chez un médecin et 390'700 hospitalisations pourraient être imputables aux personnes de 65 ans et plus. dont 1'8 16'300 consultations chez un médecin/chiropraticien, 1'67 1'000 consultations chez un médecin et 1 90'600 hospitalisations en raison de maladies rhumatismales. Une légère diminution du nombre de personnes atteintes de maladies rhumatismales. ainsi que du recours aux services de santé engendré par ces maladies. est attendue dès 3040. Le nombre de personnes souffrant de maladies rhumatismales et le nombre de consultations/ hospitalisations associées risquant d'augmenter de façon considérable, il est nécessaire de freiner cette progression. Des mesures préventives primaires, secondaires ou tertiaires peuvent diminuer la prévalence des maladies rhumatismales et l'impact de celles-ci sur la consommation des services de santé.<br/><br/>Rheumatic diseases are frequently observed in elderly people and have an important impact on tlieir life qurlity. There are fe1.v Swiss stuciies on the epiciemio!ogica! situttien and on the impact of such diseases on the use of health services. This thesis aims at studying the current knowledge based on Swiss and international literature and at establishing the situation in Switzerland from a population survey conducted in 1997. A systeinatic literature review lias been carried out. Despite a large range of methods making a comparisoii diffcult, the prevalence of rheumatic diseases seems to be homogeneous in different countries and stable. especially since 1980. It is high and increases rapidly with age. Furthermore, \niorneil suffer more frequently thaii men. Our review shows that the use of ambulatory care linked to rheumatic diseases is important. On the contrary, the impact of such diseases on hospitalization is less clearly established. New generations seem to consult more. Rheumatic diseases are also at the origin of a strong consumptioii of non-steroidal anti- inflammatory drugs \vitIl potential severe consequences. Despite its effectiveness and efficiency, arthroplasty is underused. Our analysis is based 011 a survey of Swiss permanent residents aged 15 or more in 1997. Based on Our analysis, the prevalence of rheumatic diseases in Switzerland is 41 % for elderly people (48 96 for women and 31 % for men). Theses prevalences are smaller than those found in the literature because of our relatively strict definition of rheumatic diseases. The latter diseases increase of about 50 o/o the expected number of consultations (chiropractor included or not) and of about 30 960 the expected number of hospitalizations. The affected persons have a probability of home care use 1.7 times higlier than the others. No impact on the number of outpatient care provided by hospitals has been found. Our results are comparable to those found in the international and Swiss literature, except for hospital outpatient care. This increase is due to the increase of the percentage of persons 65 and older in the population. The global number of consultationshospitalizations will increase similarly if the use of health services stays stable. In 2040, 9,613,l 00 consultations by a physiciaidchiropractor, 8,936,200 Consultations by a physician and 390,700 hospitalizations could be attributed to the persons aged 65 and older, of which 1,816,300 consultations by a physician, 1,671,000 consultations by a physician/chiropractor and 109,600 hospitalizations will be due to the rheumatic diseases. However a small decrease of the number of affected perçons and of the subsequent use of health services is expected after 2040. The number of affected elderly people and the volume of conçultations/hospitalizations are expected to increase and it ir necessx-y to slow down this progression. Preventive interventions, primary, secondary or tertiary, can decrease the prevalence of rheumatic diseases and the impaci on the consumption of health services.

Les plantes médicinales représentent la seule source de médicaments pour près de 90 % de la population de certains pays d?Afrique. Le savoir-faire des guérisseurs traditionnels, d?une valeur inestimable, représente un point de départ pour l?investigation pharmacologique et phytochimique de ces médicaments naturels. Dans le cadre de ce travail, nous nous sommes dans un premier temps intéressés à valider l?utilisation en médecine traditionnelle de deux plantes, Diuscorea sylvatica (Dioscoreaceae) et Urginea altissima (Liliaceae), qui produisent, lorsqu?elles sont frottées sur la peau, une inflammation et des démangeaisons. Ces réactions cutanées ont pu être expliquées, au moins en partie, par la présence d?aiguilles acérées d?oxalate de calcium dans les organes souterrains. Ces microtraumatismes répétés de l?épiderme risquent de provoquer, lors d?une utilisation prolongée, des lésions granulomateuses. L?histamine n?a pas été détectée, mais d?autres substances pourraient être impliquées dans le processus inflammatoire. La seconde partie de ce travail a consisté en la détection, l?isolement et la caractérisation de nouveaux composés naturels présentant un intérêt thérapeutique potentiel. 70 extraits provenant de 28 plantes supérieures du Zimbabwe ont été soumis à un criblage chimique et biologique. Les extraits méthanoliques des parties aériennes de Jamesbrittenia fodina et J. elegantissima (Scrophulariaceae) ont été sélectionnés sur la base de leurs nombreuses activités. Le fractionnement guidé par l?activité de J. fudina a permis l?isolement des saponines A et B, responsables des activités antifongique, antibactérienne et molluscicide de l?extrait. De plus, les deux saponines ont montré une activité équivalente en tant qu?inhibiteurs de l?acétylcholinestérase, propriété encore non décrite pour cette classe de composés. Une analyse LC/uv/MS de l?extrait a permis d?attribuer l?activité antiradicalaire au verbascoside, un dérivé du phenylpropane; cette analyse a de plus montré la présence d?une série de dérivés de l?acide cinnamique, dont l?isolement a été entrepris. Deux problèmes d?instabilité sont apparus, empêchant l?isolement des composés par des méthodes chromatographiques de pointe, en dépit de très bonnes conditions de séparations. Des analyses LC/?H-NMR combinées à des analyses RMN classiques des mélanges ont permis d?attribuer ces instabilités d?une part à une isomérisation cis/trans induite par la lumière, et d?autre part à une transacylation du groupe cinnamoyl sur une unité de sucre. Ceci a permis l?identification de 12 esters cinnamiques d?iridoïdes, dont 8 nouveaux produits naturels. Ces dérivés présentent un intérêt thérapeutique, car des composés similaires ont montré des propriétés anti-inflammatoires significatives dans différents modèles in vivo. Deux flavanones ont aussi été isolées de l?extrait. Cette classe de composés n?a jamais été rapportée chez un membre des Scrophulariaceae. Une analyse LC/UV/MS comparative des extraits polaires des deux espèces, J. fodina et J. elegantissima, a été effectuée pour détecter la présence éventuelle de compos.és communs. Les saponines A et B et le verbascoside ont été identifiés dans l?extrait de J. elegantissima. Trois flavonoïdes ont de plus été isolés de ce dernier par CPC et HPLC semi-préparative.<br/><br/>In certain African countries, medicinal plants represent the unique source of to 90% of the population. The knowledge of traditional healers represents a basis for the pharmacological and phytochemical investigation of these natural medicines. This work first focused on the validation of use of two plants frequently employed in traditional medicine, Dioscorea sylvatica (Dioscoreaceae) and Urginea altissimu (Liliaceae), which produce mild inflammation and itching when rubbed on the skin. These cutaneous reactions were shown to be due, at least in part, to the presence of sharp needles of calcium oxalate, implying the risk of granulomatous lesions following a long term use. Histamine was not detected, but other compounds could be involved in the inflammatory process. The second part of this work consisted of the detection, isolation and characterisation of new natural compounds of potential therapeutic interest from African plants. Seventy extracts obtained from 28 higher plants of Zimbabwe were submitted to a chemical and biological screening. The methanol extracts of the whole plants of Jamesbrittenia fodina and J. elegantissima (Scrophulariaceae) were selected for their various activities. An activity-guided fractionation of J. fodina led to the isolation of the saponins A and B, responsible for the antifungal, antibacterial and molluscicidal properties. Both saponins were equally active as inhibitors of acetylcholinesterase, a property that has, to our knowledge, never been described for this class of compounds. A LC/UV/MS analysis of the extract allowed the identification of verbascoside as the product with radical scavenging activity, and indicated the presence of a series of potentially interesting cinnamic acid derivatives. Two types of instability problems occurred in the course of their isolation, as some compounds could not be separated despite very good chromatographic conditions. LC/'H-NMR analyses combined with in-mixture NMR analyses enabled the attribution of the cause of the instability in one case to a cidtrans light-induced isomerisation, and in the other case to a transacylation of the cinnamoyl moiety on a sugar residue. These problems of instability have not been the object of previous studies. 12 cinnamic iridoid esters could be characterised, 8 of these being new natural compounds. Several similar substances have displayed significant anti-inflammatory properties in different in vivo models, suggesting a therapeutic interest for these new derivatives. Two flavanones were isolated from the same extract. This class of compound has not been previously reported from species of the Scrophulariaceae family. A comparative LCAJVNS study of the polar extracts of the two species J. elegantissima and J. fodina was performed in order to detect possible common compounds. Saponins A and B and verbascoside were thus identified in .J. elegantissima. Moreover, three supplementary flavonoids were isolated from J. elegantissima..

OBJECTIVES: Non-steroidal anti-inflammatory drugs (NSAIDs) may cause kidney damage. This study assessed the impact of prolonged NSAID exposure on renal function in a large rheumatoid arthritis (RA) patient cohort. METHODS: Renal function was prospectively followed between 1996 and 2007 in 4101 RA patients with multilevel mixed models for longitudinal data over a mean period of 3.2 years. Among the 2739 'NSAID users' were 1290 patients treated with cyclooxygenase type 2 selective NSAIDs, while 1362 subjects were 'NSAID naive'. Primary outcome was the estimated glomerular filtration rate according to the Cockroft-Gault formula (eGFRCG), and secondary the Modification of Diet in Renal Disease and Chronic Kidney Disease Epidemiology Collaboration formula equations and serum creatinine concentrations. In sensitivity analyses, NSAID dosing effects were compared for patients with NSAID registration in ≤/>50%, ≤/>80% or ≤/>90% of assessments. FINDINGS: In patients with baseline eGFRCG >30 mL/min, eGFRCG evolved without significant differences over time between 'NSAID users' (mean change in eGFRCG -0.87 mL/min/year, 95% CI -1.15 to -0.59) and 'NSAID naive' (-0.67 mL/min/year, 95% CI -1.26 to -0.09, p=0.63). In a multivariate Cox regression analysis adjusted for significant confounders age, sex, body mass index, arterial hypertension, heart disease and for other insignificant factors, NSAIDs were an independent predictor for accelerated renal function decline only in patients with advanced baseline renal impairment (eGFRCG <30 mL/min). Analyses with secondary outcomes and sensitivity analyses confirmed these results. CONCLUSIONS: NSAIDs had no negative impact on renal function estimates but in patients with advanced renal impairment.

Psoriatic arthritis is a chronic inflammatory disease. It affects up to 40% of patients suffe- ring from skin psoriasis. Joint involvement is relatively heterogeneous. Some clinical manifestations are similar to those of rheumatoid arthritis, others are close to spondylarthritis manifestations and are therefore considered as part of this entity. Treatment depends on initial presentation (peripheral or axial) but often begins with non-steroidal anti-inflammatory drugs and methotrexate, followed by anti-TNFalpha if needed. New therapeutic op- tions are available or under evaluation, parti- cularly targeting cytokines involved in psoriatic arthritis (IL-12/IL-23 and IL-17).

OBJECTIVE: This study aimed to assess the long-term outcome of functional endoscopic sinus surgery for Samter's triad patients using an objective visual analogue scale and nasal endoscopy. METHOD: Using a retrospective database, 33 Samter's triad patients who underwent functional endoscopic sinus surgery were evaluated pre- and post-operatively between 1987 and 2007 in Hospital of La Chaux-de-Fonds, Switzerland. RESULTS: A total of 33 patients participated in the study, and the mean follow-up period was 11.6 years (range 1.2-20 years). Patients were divided into two groups based on visual analogue scale scores of the five parameters with the greatest difference in intensity of symptoms between the beginning and end of follow up. Group 1 included patients with a mean visual analogue scale score of 6 and below at the end of follow up and group 2 included patients with a mean visual analogue scale score of more than 6. The only statistically significant difference noted between the two groups was the endonasal findings: stage III-IV polyposis was present in 1 out of 24 patients (4 per cent) in group 1 and in 5 out of 9 patients (56 per cent) in group 2. CONCLUSION: The results of our study indicate that functional endoscopic sinus surgery helps stabilise disease progression. Stage III-IV polyposis had a significant adverse effect on long-term outcome.

The detection of testosterone abuse in sports is routinely achieved through the 'steroidal module' of the Athlete Biological Passport by GC-MS(/MS) quantification of selected endogenous anabolic androgenic steroids (EAAS) from athletes' urines. To overcome some limitations of the "urinary steroid profile" such as the presence of confounding factors (ethnicity, enzyme polymorphism, bacterial contamination, and ethanol), ultrahigh performance liquid chromatography (UHPLC) measurements of blood concentrations of testosterone, its major metabolites, and precursors could represent an interesting and complementary strategy. In this work, two UHPLC-MS/MS methods were developed for the quantification of testosterone and related compounds in human serum, including major progestogens, corticoids, and estrogens. The validated methods were then used for the analyses of serum samples collected from 19 healthy male volunteers after oral and transdermal testosterone administration. Results from unsupervised multiway analysis allowed variations of target analytes to be assessed simultaneously over a 96-h time period. Except for alteration of concentration values due to the circadian rhythm, which concerns mainly corticosteroids, DHEA, and progesterone, significant variations linked to the oral and transdermal testosterone administration were observed for testosterone, DHT, and androstenedione. As a second step of analysis, the longitudinal monitoring of these biomarkers using intra-individual thresholds showed, in comparison to urine, significant improvements in the detection of testosterone administration, especially for volunteers with del/del genotype for phase II UGT2B17 enzyme, not sensitive to the main urinary marker, T/E ratio. A substantial extension of the detection window after transdermal testosterone administration was also observed in serum matrix. The longitudinal follow-up proposed in this study represents a first example of 'blood steroid profile' in doping control analysis, which can be proposed in the future as a complement to the 'urinary module' for improving steroid abuse detection capabilities.

The year 2014 was rich in significant advances in all areas of internal medicine. Many of them have an impact on our daily practice and on the way we manage one problem or another. From the use of the ultrasound for the diagnosis of pneumonia to the choice of the site of venous access and the type of line, and the increasing complexity of choosing an oral anticoagulant agent, this selection offers to the readers a brief overview of the major advances. The chief residents in the Service of internal medicine of the Lausanne University hospital are pleased to share their readings.

An isolation method of the steroidal alkaloid solasodine was applied to the fruits of seven species of the Solanum genus from the Federal District. Two of these species show promising yields. The purity of the isolated alkaloid allows it to be transformed it into an intermediate for steroidal drugs production, 16-deshydropregnenolone, and to the lactone vespertiline.

In the last years, several research groups have been working on the synthesis of new steroidal plant hormones called brassinosteroids (BS), which promote plant growth and better crops. Many synthetic targets and applications of these compounds and their analogues have been described in the literature. From Solanum species of the Distrito Federal, we isolated the steroidal alkaloid solasodine, which was then converted into our starting material, vespertiline. By functionalization of rings A and B, we have synthesized a new analogue of BS, with a 2alpha,3alpha-dihydroxy-6-one structure, typical of the naturally occurring BS castasterone, the immediate biosynthetic precursor of brassinolide.
