Aim: A retrospective clinical audit was carried out on records of endodontic treatment performed by dental undergraduates. The audit was performed to evaluate the technical quality of root canal fillings performed by dental undergraduates and determine the associated factors. Methods: 140 records of patients who had received root canal treatment by dental undergraduates were evaluated through periapical radiographs by two examiners (κ =0.74). The root canal fillings had their quality evaluated according to extent, condensation and presence of procedural mishap. Possible factors associated with technical quality such as tooth type, canal curvature, student level and quality of record keeping were evaluated. Data were statistically analyzed using chi-square test (p<0.05). Results: Among the 140 root-filled teeth, acceptable extent, condensation and no-mishap were observed in 72.1%, 66.4% and 77.9% cases respectively. Overall, the technical quality of 68 (48.6%) root-filled teeth was considered acceptable. Overall, non-acceptable root canal fillings were significantly more likely to be observed in molars (69.2%), moderately and severely curved canals (71.4%) and junior students (61.5%). There was no association between acceptable root canal fillings and quality of record keeping. Conclusions: The technical quality of root canal fillings was acceptable in 48.6% cases and it was associated with tooth type, degree of canal curvature and student seniority.

Aim: To analyze the root canal organic tissue dissolution capacity promoted by irrigating solutions, with or without the use of different agitation techniques. Methods: Bovine pulp tissue fragments were initially weighed. The following irrigating solutions were tested: 2.5% sodium hypochlorite, 2% chlorhexidine digluconate solution, and distilled water. The irrigating protocols were: immersion, mechanical agitation with endodontic files, and ultrasonic or sonic systems (Endoactivactor® and Easy Clean®). At the end of the protocols, the pulps were weighed to determine their final weight. For comparison, the average percentage of tissue dissolution in relation to the groups was analyzed using the Kruskal-Wallis nonparametric test complemented by multiple comparisons test. The significance level was set at 5%. Results: Among the irrigation solutions, 2.5% sodium hypochlorite showed a higher dissolving power than 2% chlorhexidine digluconate and distilled water. Furthermore, ultrasonic and sonic systems were more effective irrigating protocols than immersion and mechanical agitation with endodontic files. Conclusions: The combination of sodium hypochlorite with an agitation system promotes a greater degree of tissue degradation.

O presente estudo visa avaliar a influência da espessura do filme de cimento sobre a resistência de união de pinos de fibra de vidro em diferentes regiões do conduto radicular (cervical, médio e apical), cimentados com cimento resinoso autoadesivo, com e sem a adição de Rodamina B, por meio do teste de push-out. Quarenta raízes foram incluídas em resina epóxi, submetidas a tratamento endodôntico e obturadas com guta percha e cimento endodôntico sem eugenol. Após sete dias, os condutos foram desobstruídos e aleatoriamente divididos em 4 grupos (n=10), de acordo com as brocas do sistema de pinos de fibra WhitePost DC (FGM) usadas: (G1) broca #2; (G2) broca #3; (G3) broca #4; (G4) broca #2. O preparo foi realizado a uma profundidade de 10 mm. A cimentação foi realizada com o cimento resinoso autoadesivo RelyX U100 (3M ESPE), e apenas nos grupos G1, G2 e G3 uma pequena quantidade de Rodamina B em pó foi usada como corante no cimento. Após uma semana, cada raiz foi seccionada em máquina de corte, obtendo-se 6 fatias de 1 0,1 mm de espessura. Antes do ensaio de push-out, imagens digitais foram obtidas, por meio de um estereomicroscópio, de ambas as faces de cada fatia, para determinação do raio dos pinos e da espessura do filme de cimento. Após o ensaio mecânico, novas imagens foram obtidas para determinação do modo de falha. Para determinar a espessura de cimento, foi desenvolvida uma rotina (macro) no software KS 400. Os dados foram estatisticamente analisados com análise de variância (ANOVA) 2 fatores (influência do diâmetro da broca e influência dos terços) e Kruskal-Wallis (influência da espessura do filme de cimento). Comparações múltiplas foram realizadas com o teste Duncan. Todos os testes foram aplicados com α=0,05. Houve diferenças significantes entre os grupos em relação ao diâmetro da broca (p<0,0001), sendo G2 (14,62 5,15 MPa) > G1 (10,04 5,13 MPa) > G3 (7,68 6,14 MPa). O terço do conduto exerceu influência significativa sobre a resistência adesiva (p<0,0001), sendo os maiores valores obtidos no terço apical. As espessuras do filme de cimento foram estatisticamente diferentes nos grupos. Os maiores valores de espessura de cimento foram obtidos no G3 (248,78 μm), seguido de G2 (185,91 μm) e G1 (110,16 μm), sendo o último estatisticamente semelhante ao G4 (119,99 μm). Os resultados de G1(10,04 5,13 MPa) e G4 (8,89 + 5,18 MPa) foram estatisticamente semelhantes, indicando que a presença da Rodamina B não influencia significativamente na resistência de união. O tipo de falha predominante no G1 foi mista, no G2, adesiva entre pino e cimento, e no G3 e G4, adesiva entre cimento e dentina. O diâmetro da broca influenciou significativamente nos resultados de resistência de união ao teste de push-out. O ligeiro aumento na espessura do filme de cimento promoveu um aumento nos valores de resistência, quando comparado com filmes de cimento muito finos ou muito espessos.

O objetivo do presente trabalho foi avaliar, em microtomografia computadorizada (CT), o preparo de canais de molares inferiores com diferentes sistemas acionados a motor. Foram selecionadas 58 raízes mesiais patentes, de diâmetro anatômico correspondente a #10. Para a avaliação em TC, um anel de aço inoxidável foi confeccionado nos moldes do suporte para amostra do microtomógrafo, para que servisse de fôrma para a inclusão das raízes em resina Duralay, a fim de padronizar a posição do espécime no escaneamento inicial e final. Os canais foram preparados com os sistemas Reciproc R25 (n=16); WaveOne Primary File (n=16); Twisted File (n=14), e HyFlex (n=12). Após serem escaneados, foram reconstruídos tridimensionalmente e avaliados quantitativamente quanto à variação de volume (mm3), área de superfície (mm2) e structure model index (SMI). Foi, ainda, realizada a avaliação qualitativa das seções transversais por terço e por quadrante (MV, ML, DV, DL), sendo avaliado o toque de paredes. Os dados paramétricos foram analisados estatisticamente pelos testes ANOVA e t para amostras pareadas (α=5%). Não foi observada diferença estatística nos parâmetros quantitativos avaliados para Reciproc (142,77 76,75; 42,22 19,22; e 14,68 17,69, respectivamente); WaveOne (105,09 64,82; 29,54 19,21; 14,81 9,10, respectivamente); Twisted File (111,83 43,09; 33,31 18,40; 9,16 6,57, respectivamente), e HyFlex (151,74 149,37; 43,08 41,44; 10,80 8,52, respectivamente) (p=0,423). Dentro de cada grupo, foi observada diferença significante entre os resultados pré e pós-operatórios. O teste não paramétrico de Kruskal Wallis foi aplicado para a avaliação relativa ao toque de paredes. Foi observado que o sistema HyFlex apresentou a maior porcentagem de toques (82,3 13,1), seguido por Reciproc (81,3 16,9), Twisted File (78,3 14,4) e, por fim, WaveOne (76,9 21,7) (p>0,05). Em relação aos terços não foi observada diferença significativa (p=0,424). Os resultados da avaliação dos quadrantes intergrupo não demonstraram diferenças, porém indicaram tendência do preparo em direção à parede distal no terço cervical. Ao final, pôde-se concluir que os sistemas testados se equivalem quanto ao preparo de canais mesiais de molares inferiores; porém, nenhuma das técnicas foi capaz de tocar completamente em todas as paredes do canal radicular.

Endodontic lubricants, irrigating solutions and medicaments help reduce the microbial load within root canals. Primary and secondary cases involve different microbes. Each'solution'or combinations thereof could play a significant role but no detailed guidelines exist on their use. An audit was undertaken to compare current practice in Belfast Dental School to the others across the UK and Republic of Ireland (ROI). This audit highlighted three main differences between Belfast and other dental schools. Many other institutions utilized other irrigants besides sodium hypochlorite (NaOCl), different intracanal medicaments, including calcium hydroxide, and higher concentrations of NaOCl. Having gathered this information, we ask, 'Is there sufficient evidence to change the endodontic regime currently used at Belfast Dental School?'. Using the findings from the literature review (Part 1), we introduce new evidence-based protocols for primary and secondary cases for use in Belfast Dental School. Clinical Relevance: In the absence of detailed clinical guidelines on the use of endodontic lubricants, irrigants and medicaments in primary and secondary cases, it is important to be aware of current practice in UK and ROI dental schools where dentists and specialists are trained.

Introduction: The aim of this study was to evaluate the ability of Resilon (Resilon Research, LLC, North Branford, CT) and 2 types of gutta-percha to fill simulated lateral canals when using the Obtura II system (Model 823-700; Obtura Spartan, Fenton, MO). Methods: Forty-five human single-rooted teeth were selected and subjected to root canal preparation. After that, simulated lateral canals were made at 2, 5, and 8 mm from the working length (WL). The specimens were divided into 3 groups (n = 15) according to the filling material used: Obtura Flow 150 gutta-percha (Obtura flow), Odous Endo Flow gutta-percha (Odous; Odous de Deus Ind e Corn. Ltda Belo Horizonte, MG, Brazil), and Resilon pellets (Resilon). Root canals were filled using the Obtura II system with the tip inserted to 3 mm from the WL. No sealer was used for root canal obturation. Specimens were subjected to a tooth decalcification and clearing method, and filling of the lateral canals was analyzed by digital radiography and photographs. The measurement of lateral canal filling was done using Image Tool software (UTHSCSA Image Tool for Windows version 3.0, San Antonio, TX). Data were statistically analyzed with the Kruskal-Wallis test at 5% significance. Results: All materials showed an ability to penetrate into the simulated lateral canals, with a minimum percentage of 73% in all thirds of the root canal. Conclusions: It was concluded that gutta-percha and Resilon are solid core materials with a lateral canal filling ability when used with the Obtura II system. (J Endod 2012;38:676-679)

The subject of this work is to observe if the employment of a cream (ENDO-PTC) during the root canal preparation contributes or not to the condensation of debris in the apical region. Twenty root canals of dogs teeth were divided into two experimental groups: in the first one, the biomechanical preparation was carried out up to the n. 50 Kerr file by using irrigation and aspiration with sodium hypochlorite. In the second experimental group the canal negotiation was done in the same way but using a cream (ENDO-PTC) with sodium hypochlorite as a lubrication substance. A final irrigation and aspiration with sodium hypochlorite was also done in this group. The animal was killed and the pieces prepared for histological analysis. The histological results show us that the employment of a cream during the negotiation of the root canal contributes to a higher condensation of debris in the apical area.

The purpose of this study was to carry out a scanning electron microscopic (SEM) analysis of the cleaning qualities and smear layer removal from root canal walls, instrumented and irrigated with 2.5% NaOCl, 2.0% chlorhexidine and saline solutions. Fifty extracted teeth were used in this study. All teeth were radiographed to determine the existence of a single canal. The crowns were cut at the cervical limit and the root canals were instrumented with K-type files up to size 45. During root canal preparation, irrigations were made with the different solutions being evaluated: Group 1: 2.5% NaOCl (10 roots); Group 2: 2.5% NaOCl and 17% EDTA for 2 minute (10 roots); Group 3: 2.0% chlorhexidine (10 roots); Group 4: 2.0% chlorhexidine and 17% EDTA for 2 minutes (10 roots); Group 5: saline solution (5 roots); Group 6: saline solution and 17% EDTA for 2 minutes (5 roots). After instrumentation, the canals were irrigated with each one of the solutions and the roots were cut in the buccolingual direction for SEM analysis, at the cervical, middle and apical thirds, to ascertain the presence or absence of smear layer and debris. SEM analysis was performed by three calibrated examiners and scores were submitted to Kruskal-Wallis test at the significance level of p = 5%. Results showed that the use of 17% EDTA decreased the smear layer significantly (p < 0.05) for all evaluated solutions in all thirds. When EDTA was not used, a significantly higher quantity of smear layer on the apical third was observed only in the NaOCl groups. The use of 17% EDTA was significant for debris removal except for the chlorhexidine groups. The following conclusion could be drawn: the use of 17% EDTA was necessary to enhance cleanness of the root canals.

Dentin hypersensitivity is a common painful condition observed in clinics. Dietary habits have been much associated with its development and persistence during and following periodontal treatment. The aim of this in vitro study was to evaluate the influence of vinegars on the removal of smear layer and exposure of dentinal tubules. Extracted human teeth were submitted to manual scaling with Gracey curettes in order to remove the cementum as well as to form a smear layer. Dentin samples with 3 mm(2) were obtained and distributed into six experimental groups: one control and five types of vinegars (alcohol, apple, rice, white wine and balsamic). Each group included two methods of vinegar application: topical and friction. After routine preparation for SEM analysis, photomicrographs were assessed by a calibrated and blind examiner using an appropriate index system. Kruskal-Wallis test indicated a significant influence of vinegars on smear layer removal. There was a statistically significant difference between groups treated with apple, white and rice vinegars and the control group (p < 0.05). Nevertheless, Mann-Whitney test indicated that removal of smear layer did not vary with the method of application (topical versus friction) for any of the tested substances. We can conclude that the contact of vinegar may remove smear layer and expose dentinal tubules, regardless of the type of application. However, balsamic vinegar was associated with less removal of smear layer after both methods of application.

This in vivo study compared the effect of mineral trioxide aggregate (MTA), IRM, Super EBA and ZOE in a puttylike consistency, used as retrofilling materials, in the healing process of periapical tissue of pulpless dogs' teeth submitted to a conventional retrofilling technique. Twenty-four premolars obtained from three dogs were used. At the first intervention, the animals were anesthetized, coronal access was obtained and pulpectomy was done. Root canals were kept open to the oral environment for 180 days to induce the formation of apical lesions. After surgical removal of the lesions with curettes, 2 mm of the apical root was cut out perpendicular to the long axis of the teeth, and root-end cavities were shaped with a low-speed round bur. The bone cavities were irrigated and dried, and the root-end cavities were filled with MTA, IRM, Super EBA and ZOE in a puttylike consistency. The bone cavities were passively filled with blood and flaps were sutured. The coronal access openings were cleaned and double-sealed with ZOE and amalgam. After 180 days, the animals were killed by anesthetic overdose, maxilla and mandible were removed and the pieces were processed for histomorphologic analysis. Data were evaluated blindly on the basis of several histopathologic events and the scores obtained were analyzed statistically using the Kruskal Wallis test. No significant differences were observed among MTA, Super EBA and IRM (p>0.05). However, ZOE had a significantly more negative influence on the apical healing (p<0.05). In conclusion, MTA, Super EBA and IRM had similar histopathologic effects among each other and better performance than ZOE used in a puttylike consistency. Furthermore, only MTA stimulated hard tissue deposition in direct contact with the retrofilling material, even when it was inserted under critical conditions.
