Although most prospective cohort studies do not support an association between coffee consumption and pancreatic cancer, the findings for alcohol are inconsistent. Recently, a large prospective cohort study of women reported statistically significant elevations in risk of pancreatic cancer for both coffee and alcoholic beverage consumption. We obtained data on coffee, alcohol, and other dietary factors using semiquantitative food frequency questionnaires administered at baseline (1986 in the Health Professionals Follow-Up Study and 1980 in the Nurses’ Health Study) and in subsequent follow-up questionnaires. Data on other risk factors for pancreatic cancer, including cigarette smoking, were also available. Individuals with a history of cancer at study initiation were excluded from all of the analyses. During the 1,907,222 person-years of follow-up, 288 incident cases of pancreatic cancer were diagnosed. The data were analyzed separately for each cohort, and results were pooled to compute overall relative risks (RR). Neither coffee nor alcohol intakes were associated with an increased risk of pancreatic cancer in either cohort or after pooling the results (pooled RR, 0.62; 95% confidence interval, 0.27–1.43, for >3 cups of coffee/day versus none; and pooled RR, 1.00; 95% confidence interval, 0.57–1.76, for >=30 grams of alcohol/day versus none). The associations did not change with analyses examining different latency periods for coffee and alcohol. Similarly, no statistically significant associations were observed for intakes of tea, decaffeinated coffee, total caffeine, or alcoholic beverages. Data from these two large cohorts do not support any overall association between coffee intake or alcohol intake and risk of pancreatic cancer.

We describe a functional and biochemical link between the myogenic activator MyoD, the deacetylase HDAC1, and the tumor suppressor pRb. Interaction of MyoD with HDAC1 in undifferentiated myoblasts mediates repression of muscle-specific gene expression. Prodifferentiation cues, mimicked by serum removal, induce both downregulation of HDAC1 protein and pRb hypophosphorylation. Dephosphorylation of pRb promotes the formation of pRb-HDAC1 complex in differentiated myotubes. pRb-HDAC1 association coincides with disassembling of MyoD-HDAC1 complex, transcriptional activation of muscle-restricted genes, and cellular differentiation of skeletal myoblasts. A single point mutation introduced in the HDAC1 binding domain of pRb compromises its ability to disrupt MyoD-HDAC1 interaction and to promote muscle gene expression. These results suggest that reduced expression of HDAC1 accompanied by its redistribution in alternative nuclear protein complexes is critical for terminal differentiation of skeletal muscle cells.

The conditions under which blink startle facilitation can be found in anticipation of a reaction time task were investigated to resolve inconsistent findings across previous studies. Four groups of participants (n = 64) were presented with two visual stimuli, one predicting a reaction time task (S+) and the second presented alone (S-). Participants were asked to make a speeded response to the offset of the S+ (S1 paradigm) or were asked to respond to a tactile stimulus presented at the offset of the S+ (S1-S2 paradigm). Half of the participants in each paradigm condition received performance feedback. Overall, blink latency shortening and magnitude facilitation were larger during S+ than during S-. More detailed analyses, however, found these differences to be reliable only in the Feedback conditions. Ratings of S+ pleasantness did not change across the experiment. Electrodermal responses to S+ were larger than to S- in all groups with differential electrodermal responding emerging earlier in the S1 paradigm. Taken together, the data support the notion that startle facilitation can occur during non-aversive Pavlovian conditioning. (C) 2002 Elsevier Science B.V. All rights reserved.

Two experiments investigated the effects of the sensory modality of the lead and of the blink-eliciting stimulus during lead stimulus modality change on blink modulation at lead intervals of 2500 and 3500 ins. Participants were presented with acoustic, visual, or tactile change stimuli after habituation training with lead stimuli from the same or a different sensory modality. In Experiment 1, latency and magnitude of the acoustic blink were facilitated during a change to acoustic or visual lead stimuli, but not during a change to tactile lead stimuli. After habituation to acoustic lead stimuli, blink magnitude was smaller during tactile change stimuli than during habituation stimuli. The latter finding was replicated in Experiment 2 in which blink was elicited by electrical stimulation of the trigeminal nerve. The consistency of the findings across different combinations of lead stimulus and blink-eliciting stimulus modalities does not support a modality-specific account of attentional blink modulation. Rather, blink modulation during generalized orienting reflects modality non-specific processes, although modulation may not always be found during tactile lead stimuli. (C) 2002 Elsevier Science B.V. AB Study Design. A cross-sectional study was conducted. Objective. To determine the activity of the deep and superficial fibers of the lumbar multifidus during voluntary movement of the arm. Summary of Background Data. The multifidus contributes to stability of the lumbar spine. Because the deep and superficial parts of the multifidus are near the center of lumbar joint rotation, the superficial fibers are well suited to control spine orientation, and the deep fibers to control intervertebral movement. However, there currently are limited in vivo data to support this distinction. Methods. Electromyographic activity was recorded in both the deep and superficial multifidus, transversus abdominis, erector spinae, and deltoid using selective intramuscular electrodes and surface electrodes during single and repetitive arm movements. The latency of electromyographic onset in each muscle during single movements and the pattern of electromyographic activity during repetitive movements were compared between muscles. Results. With single arm movements, the onset of electromyography in the erector spinae and superficial multifidus relative to the deltoid was dependent on the direction of movement, but the onset in the deep multifidus and transversus abdominis was not. With repetitive arm movements, peaks in superficial multifidus and erector spinae electromyography occurred only during flexion for most subjects, whereas peaks in deep multifidus electromyography occurred during movement in both directions. Conclusions. The deep and superficial fibers of the multifidus are differentially active during single and repetitive movements of the arm. The data from this study support the hypothesis that the superficial multifidus contributes to the control of spine orientation, and that the deep multifidus has a role in controlling intersegmental motion. Studies with the myogenic basic helix-loop-helix and MADS box factors suggest that efficient transactivation is dependent on the recruitment of the steroid receptor coactivator (SRC) and the cofactors p300 and p300/CBP-associated factor. SRCs have been demonstrated to recruit CARM1 (coactivator-associated arginine methyltransferase-1), a member of the S-adenOSyl-L-methionine-dependent PRMTI-5 (protein-arginine N-methyltransferase-1-5) family, which catalyzes the methylation of arginine residues. This prompted us to investigate the functional role of CARM1/PRMT4 during skeletal myogenesis. We demonstrate that CARM1 and the SRC cofactor GRIP-1 cooperatively stimulate the activity of myocyte enhancer factor-2C (MEF2C). Moreover, there are direct interactions among MEF2C, GRIP-1, and CARM1. Chromatin immunoprecipitation demonstrated the in vivo recruitment of MEF2 and CARM1 to the endogenous muscle creatine kinase promoter in a differentiation-dependent manner. Furthermore, CARM1 is expressed in somites during embryogenesis and in the nuclei of muscle cells. Treatment of myogenic cells with the methylation inhibitor adenosine dialdehyde or tet-regulated CARM1 antisense expression did not affect expression of MyoD. However, inhibition of CARM1. inhibited differentiation and abrogated the expression of the key transcription factors (myogenin and MEF2) that initiate the differentiation cascade. This work clearly demonstrates that the arginine methyltransferase CARM1 potentiates myogenesis and supports the positive role of arginine methylation in mammalian differentiation.

This study investigated the influence of a concurrent cognitive task on the compensatory stepping response in balance-impaired elders and the attentional demand of the stepping response. Kinetic, kinematic and neuromuscular measures of a forward recovery step were investigated in 15 young adults, 15 healthy elders and 13 balance-impaired elders in a single task (postural recovery only) and dual task (postural recovery and vocal reaction time task) situation. Results revealed that reaction times were longer in all subjects when performed concurrently with a compensatory step, they were longer for a step than an in-place response and longer for balance-impaired older adults compared with young adults. An interesting finding was that the latter group difference may be related to prioritization between the two tasks rather than attentional demand, as the older adults completed the step before the reaction time, whereas the young adults could perform both concurrently. Few differences in step characteristics were found between tasks, with the most notable being a delayed latency and reduced magnitude of the early automatic postural response in healthy and balance-impaired elders with a concurrent task. University students spelled low-frequency words to dictation and subsequently made lexical decisions to them. In Experiment I, lexical decisions were slower on words students had spelled incorrectly relative to words they had spelled correctly, and there A as a larger repetition benefit 101 incorrectly spelled words. In experiment 2, the latency advantage for items spelled correctly was replicated when words were presented for only 200 ms and also in a spelling recognition task, In Experiment 3. masked identity and form priming effects were similar for words that had been spelled correctly and incorrectly, Item spelling accuracy tracked word frequency effects in the way chat it combined with repetition and priming effects. we inter that an individuals learning with a word's orthography underlies word frequency and item spelling accuracy effects and that a single orthographic lexicon serves visual word recognition and spelling. O objetivo deste trabalho é a apresentação de um caso de um indivíduo de 9 anos de idade, do sexo masculino, com queixa de distúrbio de aprendizagem, para o qual a efetividade da fonoterapia pôde ser avaliada através de testes objetivos e comportamentais, compreendendo audiometria tonal, imitanciometria, potenciais auditivos evocados de tronco encefálico, P300 e Avaliação do Processamento Auditivo Central. Foram encontrados resultados normais nos exames otorrinolaringológico e audiológico. O P300 foi realizado mostrando tempo de latência aumentada. A avaliação do Processamento Auditivo Central revelou uma desordem em grau severo, caracterizada por alterações nos processos de codificação, organização e memória, assim como dificuldade significativa para atenção seletiva e fechamento auditivo. Foi diagnosticado Desordem do Processamento Auditivo Central, sendo que o indivíduo foi encaminhado para acompanhamento fonoaudiológico com o objetivo de desenvolvimento das habilidades auditivas alteradas. Após um período de 4 meses de fonoterapia, repetidos os exames acima descritos, observou-se melhora nas latências do P300, a desordem permaneceu em grau moderado, com prejuízo mais significativo no processo de organização e não apresentou dificuldade para o fechamento auditivo. Podemos concluir com este estudo a efetividade da terapia fonoaudiológica para o desenvolvimento das habilidades auditivas, podendo ser verificada através da avaliação objetiva e comportamental.

O Lúpus Eritematoso Sistêmico (LES) é uma doença inflamatória crônica sistêmica, de etiologia desconhecida, multifatorial, caracterizada imunologicamente pela presença de múltiplos auto-anticorpos, sendo as manifestações clínicas bastante polimórficas. Essa doença pode comprometer múltiplos órgãos e sistemas. Os comprometimentos mais comuns são: articular, cutâneo, vascular, renal, neurológico, cardíaco, gastrointestinal, hematológico, ocular e auditivo. OBJETIVO: Investigar a função auditiva central de indivíduos com Lúpus Eritematoso Sistêmico. MATERIAL E MÉTODO: Foi realizado estudo de série, no qual foram avaliados 60 indivíduos do sexo feminino, com idades entre 21 a 46 anos, sendo 30 no grupo controle e 30 no grupo pesquisa. Os participantes foram submetidos a Anamnese, Avaliação Audiológica (Audiometria Tonal, Logoaudiometria e Medidas de Imitância Acústica), e pesquisa dos Potenciais Evocados Auditivos de Curta (PEATE), Média (PEAML) e Longa Latências (PEALL). Os dados obtidos foram analisados estatisticamente. RESULTADOS: Não foram observadas diferenças estatisticamente significantes entre os dois grupos avaliados, em nenhuma das avaliações realizadas. CONCLUSÕES: Não há diferença nos Potenciais Evocados Auditivos de Curta (PEATE), Média (PEAML) e Longa Latência (P300) entre os indivíduos dos grupos controle e pesquisa.

A deficiência auditiva como déficit sensorial mais comum tem dentre suas diferentes etiologias as alterações genéticas. Assim, é importante que a investigação audiológica se associe à busca do diagnóstico etiológico. OBJETIVO: Relatar o perfil audiológico e genético de três irmãos portadores de deficiência auditiva neurossensorial não-sindrômica. MATERIAL E MÉTODO: Estudo de caso de três irmãos, do sexo masculino, com 3, 5 e 16 anos, respectivamente, submetidos à avaliação audiológica comportamental e eletrofisiológica, e estudo molecular. RESULTADOS: Os achados audiológicos mostraram: audiometria do tipo neurossensorial, bilateral, simétrica, de grau moderado a moderadamente severo e configuração descendente acentuada. EOAT e EOAPD ausentes nos dois irmãos mais novos. PEATE compatível com perda auditiva moderadamente severa a severa. Presença do P300 com latências dentro da normalidade bilateralmente no irmão mais velho. Os achados do exame molecular mostraram que as duas crianças mais novas eram heterozigotos para a mutação 35delG no gene GJB2 e o mais velho não apresentava essa mutação. CONCLUSÃO: A associação das avaliações fonoaudiológicas e genéticas permite o diagnóstico etiológico de perdas auditivas que à primeira vista são semelhantes, mas que não obedecem à mesma estrutura genética. Os estudos moleculares devem ser abrangentes, evitando diagnósticos precipitados que prejudiquem o aconselhamento genético.

Indivíduos com transtornos psiquiátricos podem apresentar distúrbios perceptuais, de atenção e memória, questionando-se a presença de perdas auditivas periféricas e/ou centrais. Assim, o objetivo deste trabalho é descrever os resultados obtidos nas avaliações audiológica e eletrofisiológica, verificando a ocorrência de alterações auditivas periféricas e/ou centrais nesta população. CASUÍSTICA E MÉTODOS: Foram avaliados 20 indivíduos com autismo e síndrome de Asperger, e 20 indivíduos em desenvolvimento típico, entre oito e 19 anos. RESULTADOS: Todos os indivíduos apresentaram resultados normais na avaliação audiológica. No PEATE, 50% dos indivíduos com autismo e 30% com síndrome de Asperger apresentaram alterações, havendo diferença estatisticamente significante na análise dos dados quantitativos em ambos os grupos. Em todos os grupos verificaram-se alterações no PEAML e P300. No PEAML, não houve diferença estatisticamente significante entre os grupos na análise dos dados quantitativos e qualitativos. No P300, observou-se diferença estatisticamente significante quando comparados os grupos controle e síndrome de Asperger na análise dos dados quantitativos. CONCLUSÃO: Verificou-se grande ocorrência de alterações nos potenciais evocados auditivos em crianças autistas e com síndrome de Asperger, embora em algumas análises realizadas não tenha sido constatada diferença estatisticamente significante. Enfatiza-se a importância da investigação minuciosa da função auditiva em indivíduos com transtornos psiquiátricos.

OBJECTIVE: To examine the effects of the length and timing of nighttime naps on performance and physiological functions, an experimental study was carried out under simulated night shift schedules. METHODS: Six students were recruited for this study that was composed of 5 experiments. Each experiment involved 3 consecutive days with one night shift (22:00-8:00) followed by daytime sleep and night sleep. The experiments had 5 conditions in which the length and timing of naps were manipulated: 0:00-1:00 (E60), 0:00-2:00 (E120), 4:00-5:00 (L60), 4:00-6:00 (L120), and no nap (No-nap). During the night shifts, participants underwent performance tests. A questionnaire on subjective fatigue and a critical flicker fusion frequency test were administered after the performance tests. Heart rate variability and rectal temperature were recorded continuously during the experiments. Polysomnography was also recorded during the nap. RESULTS: Sleep latency was shorter and sleep efficiency was higher in the nap in L60 and L120 than that in E60 and E120. Slow wave sleep in the naps in E120 and L120 was longer than that in E60 and L60. The mean reaction time in L60 became longer after the nap, and faster in E60 and E120. Earlier naps serve to counteract the decrement in performance and physiological functions during night shifts. Performance was somewhat improved by taking a 2-hour nap later in the shift, but deteriorated after a one-hour nap. CONCLUSIONS: Naps in the latter half of the night shift were superior to earlier naps in terms of sleep quality. However performance declined after a 1-hour nap taken later in the night shift due to sleep inertia. This study suggests that appropriate timing of a short nap must be carefully considered, such as a 60-min nap during the night shift.

As radiculopatias lombares referem-se a um processo patológico envolvendo as raízes nervosas espinais, causando sintomas radiculares ao nível dos membros inferiores. As respostas-F são ondas tardias que advêm de descargas recorrentes de neurónios motores despolarizados antidromicamente, que podem ser úteis na avaliação de lesões radiculares. Com o objetivo de avaliar a utilidade dos estudos de condução nervosa e respostas- F do nervo peronial no diagnóstico de radiculopatia de L5 e a sua correlação com o seu grau de gravidade, foram estudados 47 sujeitos que padeciam de radiculopatia de L5 e foram comparados com um grupo de controlo, constituído por 28 sujeitos saudáveis. Foram estudadas as amplitudes do PAMC do nervo peronial profundo, do PANS do peronial superficial, bem como as latências mínima, média e máxima, cronodispersão e persistência das Respostas-F. Foi realizada ainda uma avaliação eletromiográfica, com o intuito de classificar o acometimento da raiz em termos de gravidade. Registaram-se diferenças significativas entre os dois grupos na amplitude do PAMC do peronial profundo (p<0,0001), na F-mínima, F-média, F-máxima e cronodispersão (p<0,0001), e ainda na persistência (p 0,014). Todos estes parâmetros também se correlacionaram significativamente com o grau de gravidade da radiculopatia, sendo as latências das ondas-F e a cronodispersão progressivamente maiores nos sujeitos com um grau de afeção mais grave, e a persistência e a amplitude dos estudos de condução motora mais baixas. O fator que se evidenciou mais sensível no diagnóstico desta patologia foi a latência-máxima das ondas-F, 31,25%, e o menos sensível a persistência, apenas alterada em 9,34% dos indivíduos. Tendo em consideração todos os parâmetros avaliados nas respostas-F atingiu-se uma sensibilidade global desta técnica de 42,19%. Com esta investigação concluímos que os estudos de condução nervosa e as respostas-F poderão ser úteis como complemento na avaliação das radiculopatias lombares, apresentando uma sensibilidade considerável para esta patologia. Não se deve limitar o estudo desta técnica à avaliação das latências mínima das ondas-F, mas incluir sempre todos os outros parâmetros, aumentando assim a sua sensibilidade. Estas técnicas deverão ser incluídas no estudo das radiculopatias de L5.
