A difficult transition to a new paradigm of Democratic Security and the subsequent process of military restructuring during the nineties led El Salvador, Honduras, Guatemala and Nicaragua to re-consider their old structures and functions of their armed forces and police agencies. This study compares the institutions in the four countries mentioned above to assess their current condition and response capacity in view of the contemporary security challenges in Central America. This report reveals that the original intention of limiting armies to defend and protect borders has been threatened by the increasing participation of armies in public security. While the strength of armies has been consolidated in terms of numbers, air and naval forces have failed to become strengthened or sufficiently developed to effectively combat organized crime and drug trafficking and are barely able to conduct air and sea operations. Honduras has been the only country that has maintained a proportional distribution of its armed forces. However, security has been in the hands of a Judicial Police, supervised by the Public Ministry. The Honduran Judicial Police has been limited to exercising preventive police duties, prohibited from carrying out criminal investigations. Nicaragua, meanwhile, possesses a successful police force, socially recognized for maintaining satisfactory levels of security surpassing the Guatemalan and El Salvadoran police, which have not achieved similar results despite of having set up a civilian police force separate from the military. El Salvador meanwhile, has excelled in promoting a Police Academy and career professional education, even while not having military attachés in other countries. Regarding budgetary issues, the four countries allocate almost twice the amount of funding on their security budgets in comparison to what is allocated to their defense budgets. However, spending in both areas is low when taking into account each country's GDP as well as their high crime rates. Regional security challenges must be accompanied by a professionalization of the regional armies focused on protecting and defending borders. Therefore, strong institutional frameworks to support the fight against crime and drug trafficking are required. It will require the strengthening of customs, greater control of illicit arms trafficking, investment in education initiatives, creating employment opportunities and facilitating significant improvements in the judicial system, as well as its accessibility to the average citizen.

Fatigue fracture is an overuse injury commonly encountered in military and sports medicine, and known to relate to intensive or recently intensified physical activity. Bone responds to increased stress by enhanced remodeling. If physical stress exceeds bone s capability to remodel, accumulation of microfractures can lead to bone fatigue and stress fracture. Clinical diagnosis of stress fractures is complex and based on patient s anamnesis and radiological imaging. Bone stress fractures are mostly low-risk injuries, healing well after non-operative management, yet, occurring in high-risk areas, stress fractures can progress to displacement, often necessitating surgical treatment and resulting in prolonged morbidity. In the current study, the role of vitamin D as a predisposing factor for fatigue fractures was assessed using serum 25OHD level as the index. The average serum 25OHD concentration was significantly lower in conscripts with fatigue fracture than in controls. Evaluating TRACP-5b bone resorption marker as indicator of fatigue fractures, patients with elevated serum TRACP-5b levels had eight times higher probability of sustaining a stress fracture than controls. Among the 154 patients with exercise induced anterior lower leg pain and no previous findings on plain radiography, MRI revealed a total of 143 bone stress injuries in 86 patients. In 99% of the cases, injuries were in the tibia, 57% in the distal third of the tibial shaft. In patients with injury, forty-nine (57%) patients exhibited bilateral stress injuries. In a 20-year follow-up, the incidence of femoral neck fatigue fractures prior to the Finnish Defence Forces new regimen in 1986 addressing prevention of these fractures was 20.8/100,000, but rose to 53.2/100,000 afterwards, a significant 2.6-fold increase. In nineteen subjects with displaced femoral neck fatigue fractures, ten early local complications (in first postoperative year) were evident, and after the first postoperative year, osteonecrosis of the femoral head in six and osteoarthritis of the hip in thirteen patients were found. It seems likely that low vitamin D levels are related to fatigue fractures, and that an increasing trend exists between TRACP-5b bone resorption marker elevation and fatigue fracture incidence. Though seldom detected by plain radiography, fatigue fractures often underlie unclear lower leg stress-related pain occurring in the distal parts of the tibia. Femoral neck fatigue fractures, when displaced, lead to long-term morbidity in a high percentage of patients, whereas, when non-displaced, they do not predispose patients to subsequent adverse complications. Importantly, an educational intervention can diminish the incidence of fracture displacement by enhancing awareness and providing instructions for earlier diagnosis of fatigue fractures.

Little research has examined the impact of being an accompanying spouse on British military foreign postings. The aim of this qualitative study was to investigate the experiences of 13 military spouses from 11 different overseas locations. Data were collected via an online forum and thematic content analysis was conducted. Key findings revealed that, regardless of the location, reactions to overseas posting varied considerably and were related to the military spouse's personality and personal circumstances, as well as their relationship with family, husband and their support networks. Spouses experienced a loss of control over their lives that was in some cases psychologically distressing. The findings corroborate and extend the findings from a previous study that was limited to one location, further highlighting the need for pre-established support resources from the military and healthcare professionals to be readily accessible for all military spouses. Importantly, such support provision may also facilitate the military spouse in regaining some control over their everyday life, enhancing their well-being and the experience for the family.

Objective: The goal of this study was to identify rates, characteristies, and predictors of mental health treatment seeking by military members with posttraumatic stress disorder (PTSD). Method: Our sample was drawn from the 2002 Canadian Community Health Survey-Canadian Forces Supplement (CCHS-CF) dataset. The CCHS-CF is the first epidemiologic survey of PTSD and other mental health conditions in the Canadian military and includes 8441 nationally representative Canadian Forces (CF) members. Of those, 549 who met the criteria for lifetime PTSD were included in our analyses. To identify treatment rates and characteristics, we examined frequency of treatment contact by professional and facility type. To identify predictors of treatment seeking, we conducted a binary logistic regression with lifetime treatment seeking as the outcome variable. Results: About two-thirds of those with PTSD consulted with a professional regarding mental health problems. The most frequently consulted professionals, during both the last year and lifetime, included social workers and counsellors, medical doctors and general practitioners, and psychiatrists. Consultations during the last year most often took place in a CF facility. Treatment seeking was predicted by cumulative lifetime trauma exposure, index traumatic event type, PTSD symptom interference, and comorbid major depressive disorder. Those with comorbid depression were 3.75 times more likely to have sought treatment than those without. Conclusions: Although a significant portion of military members with PTSD sought mental health treatment, 1 in 3 never did. Trauma-related and illness and (or) need factors predicted treatment seeking. Of all the predictors of treatment seeking, comorbid depression most increased the likelihood of seeking treatment.

Understanding the complex relationships between quantities measured by volcanic monitoring network and shallow magma processes is a crucial headway for the comprehension of volcanic processes and a more realistic evaluation of the associated hazard. This question is very relevant at Campi Flegrei, a volcanic quiescent caldera immediately north-west of Napoli (Italy). The system activity shows a high fumarole release and periodic ground slow movement (bradyseism) with high seismicity. This activity, with the high people density and the presence of military and industrial buildings, makes Campi Flegrei one of the areas with higher volcanic hazard in the world. In such a context my thesis has been focused on magma dynamics due to the refilling of shallow magma chambers, and on the geophysical signals detectable by seismic, deformative and gravimetric monitoring networks that are associated with this phenomenologies. Indeed, the refilling of magma chambers is a process frequently occurring just before a volcanic eruption; therefore, the faculty of identifying this dynamics by means of recorded signal analysis is important to evaluate the short term volcanic hazard. The space-time evolution of dynamics due to injection of new magma in the magma chamber has been studied performing numerical simulations with, and implementing additional features in, the code GALES (Longo et al., 2006), recently developed and still on the upgrade at the Istituto Nazionale di Geofisica e Vulcanologia in Pisa (Italy). GALES is a finite element code based on a physico-mathematical two dimensional, transient model able to treat fluids as multiphase homogeneous mixtures, compressible to incompressible. The fundamental equations of mass, momentum and energy balance are discretised both in time and space using the Galerkin Least-Squares and discontinuity-capturing stabilisation technique. The physical properties of the mixture are computed as a function of local conditions of magma composition, pressure and temperature.The model features enable to study a broad range of phenomenologies characterizing pre and sin-eruptive magma dynamics in a wide domain from the volcanic crater to deep magma feeding zones. The study of displacement field associated with the simulated fluid dynamics has been carried out with a numerical code developed by the Geophysical group at the University College Dublin (O’Brien and Bean, 2004b), with whom we started a very profitable collaboration. In this code, the seismic wave propagation in heterogeneous media with free surface (e.g. the Earth’s surface) is simulated using a discrete elastic lattice where particle interactions are controlled by the Hooke’s law. This method allows to consider medium heterogeneities and complex topography. The initial and boundary conditions for the simulations have been defined within a coordinate project (INGV-DPC 2004-06 V3_2 “Research on active volcanoes, precursors, scenarios, hazard and risk - Campi Flegrei”), to which this thesis contributes, and many researchers experienced on Campi Flegrei in volcanological, seismic, petrological, geochemical fields, etc. collaborate. Numerical simulations of magma and rock dynamis have been coupled as described in the thesis. The first part of the thesis consists of a parametric study aimed at understanding the eect of the presence in magma of carbon dioxide in magma in the convection dynamics. Indeed, the presence of this volatile was relevant in many Campi Flegrei eruptions, including some eruptions commonly considered as reference for a future activity of this volcano. A set of simulations considering an elliptical magma chamber, compositionally uniform, refilled from below by a magma with volatile content equal or dierent from that of the resident magma has been performed. To do this, a multicomponent non-ideal magma saturation model (Papale et al., 2006) that considers the simultaneous presence of CO2 and H2O, has been implemented in GALES. Results show that the presence of CO2 in the incoming magma increases its buoyancy force promoting convection ad mixing. The simulated dynamics produce pressure transients with frequency and amplitude in the sensitivity range of modern geophysical monitoring networks such as the one installed at Campi Flegrei . In the second part, simulations more related with the Campi Flegrei volcanic system have been performed. The simulated system has been defined on the basis of conditions consistent with the bulk of knowledge of Campi Flegrei and in particular of the Agnano-Monte Spina eruption (4100 B.P.), commonly considered as reference for a future high intensity eruption in this area. The magmatic system has been modelled as a long dyke refilling a small shallow magma chamber; magmas with trachytic and phonolitic composition and variable volatile content of H2O and CO2 have been considered. The simulations have been carried out changing the condition of magma injection, the system configuration (magma chamber geometry, dyke size) and the resident and refilling magma composition and volatile content, in order to study the influence of these factors on the simulated dynamics. Simulation results allow to follow each step of the gas-rich magma ascent in the denser magma, highlighting the details of magma convection and mixing. In particular, the presence of more CO2 in the deep magma results in more ecient and faster dynamics. Through this simulations the variation of the gravimetric field has been determined. Afterward, the space-time distribution of stress resulting from numerical simulations have been used as boundary conditions for the simulations of the displacement field imposed by the magmatic dynamics on rocks. The properties of the simulated domain (rock density, P and S wave velocities) have been based on data from literature on active and passive tomographic experiments, obtained through a collaboration with A. Zollo at the Dept. of Physics of the Federici II Univeristy in Napoli. The elasto-dynamics simulations allow to determine the variations of the space-time distribution of deformation and the seismic signal associated with the studied magmatic dynamics. In particular, results show that these dynamics induce deformations similar to those measured at Campi Flegrei and seismic signals with energies concentrated on the typical frequency bands observed in volcanic areas. The present work shows that an approach based on the solution of equations describing the physics of processes within a magmatic fluid and the surrounding rock system is able to recognise and describe the relationships between geophysical signals detectable on the surface and deep magma dynamics. Therefore, the results suggest that the combined study of geophysical data and informations from numerical simulations can allow in a near future a more ecient evaluation of the short term volcanic hazard.

El motivo de abordar esta Tesis responde al hecho personal de haber vivido históricamente momentos cumbre de la Construcción Naval en España y no dejar de plantearme, a pesar de mi distancia del sector desde el punto de vista profesional, en las opciones de futuro para volver a ser competitivos en un sector que ha tenido un peso tan importante en la economía española, y que tanto ha ilusionado a centenares de profesionales y compañeros a lo largo de muchas décadas con el optimismo de estar aportando valor para que España fuera competitiva. A lo largo de los últimos años, y la relación con el ámbito de la dirección de Empresas desde el punto de vista de una Escuela de Negocios, así como el contacto con enfoques estratégicos en sectores muy diversos me animó a plantearme la Tesis con el objetivo de reflejar el estado actual del sector y poder valorar alternativas de futuro para la Construcción Naval española, aun sabiendo que son muchos los “maestros” realmente autorizados en nuestros país, con amplios conocimientos y experiencia, que muchas veces, a pesar de las propuestas y de los esfuerzos que han realizado para impulsar el sector, se han encontrado con situaciones adversas, bien de tipo económico, social‐laboral, político ‐a nivel nacional, europeo o global‐, etc., que han impedido un fortalecimiento del sector como todos hubiéramos deseado. La presión histórica ejercida por los países competidores en Construcción naval del ámbito asiático y lejano oriente, así como los compromisos derivados de las Directivas europeas, han obligado al sector de la Construcción Naval en España a buscar unos nuevos posicionamientos estratégicos presentes y, sobre todo, de futuro. Partiendo de un análisis del sector naval, tanto del mercado nacional como del internacional, con especial foco en los países líderes, se plantea investigar, siguiendo el modelo de Porter, las fuerzas competitivas que han influido en estas últimas décadas y que han conducido a la situación actual, valorando la estructura competitiva, el entorno relevante y los efectos de la globalización, con las amenazas de los nuevos y actuales competidores y las barreras existentes. Para abordar esta investigación se ha realizado un análisis del sector naval con la siguiente metodología: 1. Análisis del estado actual de la construcción naval en España. 2. Análisis del estado actual de la construcción naval en el mundo. 3. Estudio de la demanda en el mercado y evolución de la misma en los últimos años: muy centrada en los países líderes y más competitivos. 4. Estudio de las perspectivas de negocio en el sector marítimo y oceánico: estudio particular del transporte marítimo y una comparativa con la explotación de recursos oceánicos. Finalmente se decidió no abordar por falta de datos de futuro las construcciones militares. 5. Estudio de características de la industria naval española y capacidad de los astilleros. Se ha focalizado especialmente en la construcción para valorar la capacidad de futuro. 6. Análisis de fuerzas competitivas de la industria naval española a partir del modelo de Porter. En esta parte se incluyen alguno de los factores críticos externos e internos que ayudan a identificar barreras y estrategias en el entorno de la construcción naval como sector global. 7. Identificación de las oportunidades de negocio hacia el 2050. 8. Alternativas para una estrategia competitiva de actuación frente a las oportunidades de futuro en el 2050. Con esta Tesis se aporta un estudio competitivo actualizado, de acuerdo con el modelo de Porter, con el fin de proponer una posible estrategia competitiva de futuro, que posicione competitivamente la industria naval y el sector marítimo en España en las próximas décadas. ABSTRACT The reason for addressing this thesis responds to my personal experience about having lived historical moments summit Shipbuilding in Spain. Despite my distance from this industry from a professional point of view, I have never stopped wonder myself which are the options for the future to become competitive in an industry that has had such an important weight in the Spanish economy, which has excited so hundreds of professionals and peers through many decades with optimism to be adding value to Spain in order to be competitive again. Over recent years, and the relationship with the field of business management from the point of view of a business school, as well as contact with strategic approaches in diverse sectors encouraged me to wonder Thesis order to reflect the current state of the sector and to evaluate future alternatives for the Spanish Shipbuilding, knowing that many "teachers" really allowed in our country, with extensive knowledge and experience that often, despite proposals and the efforts that have been made to boost the sector, have met with adverse situations, whether economic, social and labor, political kind ‐at national, European or global‐level, etc., that have prevented a strengthening of all sectors we wished. The historical pressure from competing countries in Shipbuilding Asian area and Far East, as well as commitments arising from EU directives, have forced the shipbuilding industry in Spain to seek a new strategic positions present and, above all, future. Starting from an analysis of the shipbuilding sector, both national and international market, with special focus on the leading countries, we propose to investigate, following the model of Porter, the competitive forces that have influenced recent decades and have led to the current situation, assessing the competitive structure of the relevant environment and the effects of globalization, with the threat of new and existing competitors and barriers. To address this research has analyzed the naval sector with the following methodology: 1. Analysis of the current state of shipbuilding in Spain. 2. Analysis of the current state of shipbuilding in the world. 3. Study of the demand in the market and evolution of the same in recent years: very focused on the leading and most competitive countries. 4. Study of business prospects in the maritime and oceanic sector: private study of maritime transport and a comparison with the exploitation of ocean resources. Finally it was decided not to address a lack of data future military construction. 5. Study of characteristics of the Spanish shipbuilding and shipyard capacity. It is particularly focused on building the capacity to assess future. 6. Analysis of competitive forces of the Spanish shipbuilding industry from the model of Porter. In this part they include some critics of the external and internal factors that help identify barriers and strategies in the environment of global shipbuilding sector. 7. Identification of business opportunities by 2050. 8. Alternatives to a competitive strategy of action against future opportunities in 2050. This thesis has sought to provide a competitive study updated according to Porter's model, in order to propose a possible future competitive strategy to reach a competitive position at the shipbuilding industry and the maritime sector in Spain in the coming decades. Post-traumatic stress disorder (PTSD) has emerged as a key concern for military and veteran populations. This article describes what is being done programmatically and therapeutically to treat PTSD in military personnel and veterans returning from deployment. This scoping review demonstrates that (1) research published in this area has been rapidly increasing since its inception in the 1980s; (2) the vast majority of articles focus on cognitive-behavioral approaches to treatment, and this area of the literature presents strong evidence for these approaches; and (3) there is a lack of randomized controlled trials for treatments, such as art therapies and group therapies.
