The staff of 20 substance abuse treatment facilities were administered the Ward Atmosphere Scale, an instrument which measures treatment environment. Ten facilities were freestanding and ten were hospital based, and were drawn from a large, not-for-profit national chain using a random selection process. Controlling for several staff and facility attributes, it was found that no substantial effects on treatment environment existed due to facility type, freestanding or hospital-based. Implications of the study exist in selection of facility type for purchasers of substance abuse treatment and for the hiring and training of clinical staff for treatment facilities. Study findings also suggest that inadequate or insufficient measures exist to examine the construct 'treatment environment'. ^

There is a growing interest in the location of Treatment, Storage, and Disposal (TSDF) sites in relation to minority communities. A number of studies have been completed, and the results of these studies have been varied. Some of the studies have shown a strong positive correlation between the location of TSDF sites and minority populations, while a few have shown no significance in that relationship. The major difference between these studies has been in the areal unit used.^ This study compared the minority populations of Texas census tracts and ZIP codes containing a TSDF using the associated county as the comparison population. The hypothesis of this study was that there was no difference between using census tracts and ZIP codes to analyze the relationship of minority populations and TSDF's. The census data used was from 1990, and the initial list of TSDF sites was supplied by the Texas Natural Resource Conservation Commission. The TSDF site locations were checked using graphical information systems (GIS) programs, in order to increase the accuracy of the identity of exposed ZIP codes and census tracts. The minority populations of the exposed areal units were compared using proportional differences, crosstables, maps, and logistic regression. The dependent variable used was the exposure status of the areal units under study, including counties, census tracts, and ZIP codes. The independent variables used included minority group proportion and grouping of the proportions, educational status, household income, and home value.^ In all cases, education was significant or near significant at the.05 level. Education rather than minority proportion was therefore the most significant predictor of the exposure status of a census tract or ZIP code. ^

Population growth is always increasing, and thus the concept of smart and cognitive cities is becoming more important. Developed countries are aware of and working towards needed changes in city management. However, emerging countries require the optimization of their own city management. This chapter illustrates, based on a use case, how a city in an emerging country can quickly progress using the concept of smart and cognitive cities. Nairobi, the capital of Kenya, is chosen for the test case. More than half of the population of Nairobi lives in slums with poor sanitation, and many slum inhabitants often share a single toilet, so the proper functioning and reliable maintenance of toilets are crucial. For this purpose, an approach for processing text messages based on cognitive computing (using soft computing methods) is introduced. Slum inhabitants can inform the responsible center via text messages in cases when toilets are not functioning properly. Through cognitive computer systems, the responsible center can fix the problem in a quick and efficient way by sending repair workers to the area. Focusing on the slum of Kibera, an easy-to-handle approach for slum inhabitants is presented, which can make the city more efficient, sustainable and resilient (i.e., cognitive).

Purpose. To examine the association between living in proximity to Toxics Release Inventory (TRI) facilities and the incidence of childhood cancer in the State of Texas. ^ Design. This is a secondary data analysis utilizing the publicly available Toxics release inventory (TRI), maintained by the U.S. Environmental protection agency that lists the facilities that release any of the 650 TRI chemicals. Total childhood cancer cases and childhood cancer rate (age 0-14 years) by county, for the years 1995-2003 were used from the Texas cancer registry, available at the Texas department of State Health Services website. Setting: This study was limited to the children population of the State of Texas. ^ Method. Analysis was done using Stata version 9 and SPSS version 15.0. Satscan was used for geographical spatial clustering of childhood cancer cases based on county centroids using the Poisson clustering algorithm which adjusts for population density. Pictorial maps were created using MapInfo professional version 8.0. ^ Results. One hundred and twenty five counties had no TRI facilities in their region, while 129 facilities had at least one TRI facility. An increasing trend for number of facilities and total disposal was observed except for the highest category based on cancer rate quartiles. Linear regression analysis using log transformation for number of facilities and total disposal in predicting cancer rates was computed, however both these variables were not found to be significant predictors. Seven significant geographical spatial clusters of counties for high childhood cancer rates (p<0.05) were indicated. Binomial logistic regression by categorizing the cancer rate in to two groups (<=150 and >150) indicated an odds ratio of 1.58 (CI 1.127, 2.222) for the natural log of number of facilities. ^ Conclusion. We have used a unique methodology by combining GIS and spatial clustering techniques with existing statistical approaches in examining the association between living in proximity to TRI facilities and the incidence of childhood cancer in the State of Texas. Although a concrete association was not indicated, further studies are required examining specific TRI chemicals. Use of this information can enable the researchers and public to identify potential concerns, gain a better understanding of potential risks, and work with industry and government to reduce toxic chemical use, disposal or other releases and the risks associated with them. TRI data, in conjunction with other information, can be used as a starting point in evaluating exposures and risks. ^

Very few studies have described MUP-1 concentrations and measured prevalence of Laboratory Animal Allergy (LAA) at such a diverse institution as the private medical school (MS) that is the focus of this study. Air sampling was performed in three dissimilar animal research facilities at MS and quantitated using a commercially available ELISA. Descriptive data was obtained from an anonymous laboratory animal allergy survey given to both animal facility employees and the researchers who utilize these facilities alike. Logistic regression analysis was then implemented to investigate specific factors that may be predictive of developing LAA as well as factors influencing the reporting of LAA symptoms to the occupational health program. Concentrations of MUP-1 detected ranged from below detectable levels (BDL) to a peak of 22.64 ng/m3 . Overall, 68 employees with symptoms claimed they improved while away from work and only 25 employees reported their symptoms to occupational health. Being Vietnamese, a smoker, not wearing a mask, and working in any facility longer than one year were all significant predictors of having LAA symptoms. This study suggests a LAA monitoring system that relies on self-reporting can be inadequate in estimating LAA problems. In addition, efforts need to be made to target training and educational materials for non-native English speaking employees to overcome language and cultural barriers and address their specific needs. ^

This study provides data which can contribute to improving services and delivering quality health care in government health facilities in the state of Qatar. To measure the satisfaction with current care of selected patients who receive care in Hamad General Hospital and the Khalifa Town Health Center in the city of Doha, a cross-sectional survey and a self-administered questionnaire were used.^ Analysis was performed on data from 444 patients on eight dimensions of patient satisfaction with medical care. These include: general satisfaction, availability of services, convenience of services, facilities, humaneness of doctors, quality of care, continuity of care, and aspects of the last visit. Patient satisfaction parameters were compared for males vs. females, for citizens vs. non-citizens, and for patients seen in the hospital vs. those seen in the health center.^ Results indicate that patients seen in the hospital were more satisfied with care than patients seen in the health center, that non-citizens were more satisfied than citizens, and that males were slightly more satisfied than females with medical services. ^

One of the broad objectives of the Nigerian health service, vigorously being pursued at all levels of government, is to make comprehensive health care available and accessible to the population at the lowest possible cost, within available resources. Some state governments in the federation have already introduced free medical service as a practical way to remove financial barriers to access and in turn to encourage greater utilization of publicly funded care facilities.^ To aid health planners and decision makers in identifying a shorter corridor through which urban dwellers can gain access to comprehensive health care, a health interview survey of the metropolitan Lagos was undertaken. The primary purpose was to ascertain the magnitude of access problems which urban households face in seeking care from existing public facilities at the time of need. Six categories of illness chosen from the 1975 edition of the International Classification of Disease were used as indicators of health need.^ Choice of treatment facilities in response to illness episode was examined in relation to distance, travel time, time of use and transportation experiences. These were graphically described. The overall picture indicated that distance and travel time coexist with transportation problems in preventing a significant segment of those in need of health care from benefitting in the free medical service offered in public health facilities. Within this milieu, traditional medicine and its practitioners became the most preferred alternative. Recommendations were offered for action with regard to decentralization of general practitioner (GP) consultations in general hospitals and integration of traditional medicine and its practitioners into public health service. ^

It has been well documented that inmates incarcerated in prisons and correctional facilities exhibit higher incidence and prevalence of mycobacterium tuberculosis (TB) disease than the general population. This has public health implications because correctional systems may serve as reservoirs for TB disease that can lead to TB outbreaks in the facilities or can be spread to the general public once inmates are released. Although Texas has one of the largest correctional systems in both the US and the world, little is known about TB prevalence and incidence among Texas inmates. The purpose of this study was to elucidate the relationship between TB incidence and incarceration in Texas correctional facilities and investigate differences in various demographic factors. ^ The study used the national TB database from the US Centers for Disease Control and Prevention (CDC) to calculate and compare the overall incidences of TB disease among correctional facility inmates and similar non-inmates in Texas during 2005–2009. Data were also stratified by age, gender, race/ethnicity, birth status, and HIV status and compared between inmates and non-inmates using chi-squared analysis and relative risks with 95% confidence intervals to assess any significant differences. ^ Results suggest that the overall TB incidence among Texas correctional facility inmates per year (88.6 per 100,000) was significantly higher than that of Texas non-inmates (6.3 per 100,000); a 14 fold difference. Relative risk analyses by gender, race/ethnicity, and those with HIV infection found that the TB incidences for all these demographics were significantly and consistently higher in inmates compared to non-inmates. In particular, Hispanic inmates were more likely to develop TB than their non-inmate counterparts by a relative risk of 23.9 (95% CI 19.4–29.4). Likewise, both male and female inmates were more likely to develop TB than non-inmates (RR = 10.2, 95% CI 8.5–12.2; RR = 20.8, 95% CI 12.2–25.3, respectively), although female inmates unconventionally exhibited a higher TB incidence and relative risk than males inmates, which has not been shown. Among those with HIV infections, correctional facility inmates were 2.6 times were likely to develop TB disease than non-inmates (95% CI 1.5–4.4). ^ Inmates in Texas correctional facilities have a higher incidence of TB than non-inmates. Part of this higher risk may be because a large proportion of inmates come from populations already at high risks for TB, such as foreign born immigrants, those infected with HIV, and low SES groups such as many racial/ethnic minorities. Thus, these results may be used as a basis for more controlled and detailed research in the area, and to further characterize incarceration as a risk factor for TB incidence. They may also bring much needed attention about this health disparity to public health officials, legislators, and health administrators to expand and improve TB control in Texas correctional facilities, particularly among inmates released to the community, and reduce the risk of TB transmission to the general population.^

This paper proposes a general equilibrium model of a monocentric city based on Fujita and Krugman (1995). Two rates of transport costs per distance and for the same good are introduced. The model assumes that lower transport costs are available at a few points on a line. These lower costs represent new transport facilities, such as high-speed motorways and railways. Findings is that new transport facilities connecting the city and hinterlands strengthen the lock-in effects, which describes whether a city remains where it is forever after being created. Furthermore, the effect intensifies with better agricultural technologies and a larger population in the economy. The relationship between indirect utility and population size has an inverted U-shape, even if new transport facilities are used. However, the population size that maximizes indirect utility is smaller than that found in Fujita and Krugman (1995).

The European HiPER project aims to demonstrate commercial viability of inertial fusion energy within the following two decades. This goal requires an extensive Research &Development program on materials for different applications (e.g., first wall, structural components and final optics). In this paper we will discuss our activities in the framework of HiPER to develop materials studies for the different areas of interest. The chamber first wall will have to withstand explosions of at least 100 MJ at a repetition rate of 5-10 Hz. If direct drive targets are used, a dry wall chamber operated in vacuum is preferable. In this situation the major threat for the wall stems from ions. For reasonably low chamber radius (5-10 m) new materials based on W and C are being investigated, e.g., engineered surfaces and nanostructured materials. Structural materials will be subject to high fluxes of neutrons leading to deleterious effects, such as, swelling. Low activation advanced steels as well as new nanostructured materials are being investigated. The final optics lenses will not survive the extreme ion irradiation pulses originated in the explosions. Therefore, mitigation strategies are being investigated. In addition, efforts are being carried out in understanding optimized conditions to minimize the loss of optical properties by neutron and gamma irradiation

CATR facilities are attractive antenna measurement facilities. Main reasons which contribute to this fact lie on its inherent reduced volume, on-the-fly measurements and the extension of both to a wide range of frequencies. However, these features rely on the assumption that the field collimation scheme is able to generate a plane wave distribution (quiet zone) where the AUT is to be placed and operated in RX mode. Unfortunately, electromagnetic theory states that this field distribution is not possible to be generated by a finite size scatterer operated as the collimator of a nonzero wavelength time-harmonic propagating field. This is the background of this paper, where two well-known electromagnetic field collimators will be discussed: the serrated edge reflector and the blended rolled edge reflector. To reach this purpose, electromagnetic hybrid analysis techniques developed at Technical University of Madrid will be applied.

En este trabajo se han cubierto diferentes asuntos del diseño neutrónico de los aspectos radiológicos de las dos instalaciones del proyecto HiPER. El proyecto HiPER es un proyecto europeo concebido en el marco del programa ESFRI (European Scientific Facilities Research Infrastructure). Está destinado al desarrollo de la energía de fusión nuclear inercial mediante el uso de láseres y el esquema iluminación directa. Consecuentemente, se trata de una instalación con fines exclusivamente civiles. Se divide en dos fases, correspondientes con dos instalaciones: HiPER Engineering y HiPER Reactor. La instalación HiPER Engineering desarrollará las tecnologías implicadas en la ignición de alta repetición de cápsulas de DT por iluminación directa. El HiPER Reactor será una planta demostradora que produzca electricidad haciendo uso de las tecnologías desarrolladas durante la fase HiPER Engineering. El HiPER Engineering se centrará en las tecnologías relevantes para las igniciones a alta repetición de cápsulas de DT usando la iluminación directa. El principal esfuerzo de desarrollo tecnológico se hará en todos los asuntos directamente relacionados con la ignición: láseres, óptica, inyector, y fabricación masiva de cápsulas entre otros. Se espera una producción de entre 5200 MJ/año y 120000 MJ/año dependiendo del éxito de la instalación. Comparado con la energía esperada en NIF, 1200 MJ/año, se trata de un reto y un paso más allá en la protección radiológica. En este trabajo se ha concebido una instalación preliminar. Se ha evaluado desde el punto de vista de la protección radiológica, siendo las personas y la óptica el objeto de protección de este estudio. Se ha establecido una zonificación durante la operación y durante el mantenimiento de la instalación. Además, se ha llevado a cabo una evaluación de la selección de materiales para la cámara de reacción desde el punto de vista de gestión de residuos radiactivos. El acero T91 se ha seleccionado por, siendo un acero comercial, presentar el mismo comportamiento que el acero de baja activación EUROFER97 al evaluarse como residuo con el nivel de irradiación de HiPER Engineering. Teniendo en cuenta los resultados obtenidos para la instalación preliminar y las modificaciones de la instalación motivadas en otros campos, se ha propuesto una instalación avanzada también en este trabajo. Un análisis más profundo de los aspectos radiológicos, así como una evaluación completa de la gestión de todos los residuos radiactivos generados en la instalación se ha llevado a cabo. La protección radiológica se ha incrementado respecto de la instalación preliminar, y todos los residuos pueden gestionarse en un plazo de 30 sin recurrir al enterramiento de residuos. El HiPER Reactor sera una planta demostradora que produzca electricidad basada en las tecnologías de ignición desarrolladas durante la fase HiPER Engineering. El esfuerzo de desarrollo tecnológico se llevará a cabo en los sistemas relacionados con la generación de electricidad en condiciones económicas: manto reproductor de tritio, ciclos de potencia, vida y mantenimiento de componentes, o sistemas de recuperación de tritio entre otros. En este trabajo la principal contribución a HiPER Reactor está relacionada con el diseño de la cámara de reacción y sus extensiones en la planta. La cámara de reacción es la isla nuclear más importante de la planta, donde la mayoría de las reacciones nucleares tienen lugar. Alberga la primera pared, el manto reproductor de tritio y la vasija de vacío. Todo el trabajo realizado aquí ha pivotado en torno al manto reproductor de tritio y sus interacciones con el resto de componentes de la planta. Tras una revisión profunda de la bibliografía de los diseños recientes de cámaras de reacción con características similares a HiPER Reactor, se ha propuesto y justificado un esquema tecnológico innovador para el manto reproductor de tritio. El material fértil selecconado es el eutéctico 15.7 at.% Litio – 84.3 at.% Plomo, LiPb, evitando el uso de berilio como multiplicador neutrónico mientras se garantiza el ajuste online de la tasa de reproducción de tritio mediante el ajuste en el enriquecimiento en 6Li. Aunque se podría haber elegido Litio purom el LiPb evita problemas relacionados con la reactividad química. El precio a pagar es un reto materializado como inventario radiactivo de Z alto en el lazo de LiPb que debe controlarse. El material estructural seleccionado es el acero de baja activación EUROFER97, que estará en contacto directo con le LiPb fluyendo a alta velocidad. En este esquema tecnológico, el LiPb asegurará la autosuficiente de tritio de la planta mientras el propio LiPb extrae del manto el calor sobre él depositado por los neutrones. Este esquema recibe el nombre de manto de Litio-Plomo auto-refrigerado (SCLL por sus siglas en inglés). Respecto de los conceptos SCLL previos, es destacable que nos e requieren componentes del SiC, puesto que no hay campos magnéticos en la cámara de reacción. Consecuentemente, el manto SCLL propuesto para HiPER presenta riesgo tecnológicos moderados, similares a otros dispositivos de fusión magnética, como el HCLL, e incluso inferiores a los del DCLL, puesto que no se require SiC. Los retos que se deben afrontar son el control del inventario de Z alto así como las tasas de corrosión derivadas de la interacción del LiPb con el EUROFE97. En este trabajo se abordan ambos aspectos, y se presentan los respectivos análisis, junto con otros aspectos neutrónicos y de activación, tales como la protección de la vasija de vacío por parte del material fértil para garantizar la resoldabilidad de por vida en la cara externa de la vasija. También se propone y se estudio un ciclo de potencia de Brayton de Helio para dos configuraciones diferentes de refrigeración del sistema primera pared-manto reproductor. Las principales conclusiones de estos estudios son: i) el inventario de Z alto puede controlarse y es comparable al que se encuentra en dispositivos de fusión similares, ii)la vasija de vacío requiere una mayor protección frente a la radiación neutrónica y iii) las tasas de corrosión son demasiado altas y la temperatura media de salida del LiPb es demasiado baja. Tiendo en cuenta estos resultados juntos con otras consideraciones relacionadas con el mantenimiento de componentes y la viabilidad constructiva, se ha propuesto una evolución de la cámara de reacción. Las evoluciones más destacables son la introducción de un reflector neutrónico de grafito, la modificación de la configuración de la óptica final, la forma y el tamaño de la cámara de vacío y una nueva subdivisión modular del manto. Se ha evaluado desde el punto de vista neutrónico, y su análisis y posterior evolución queda fuera del objeto de este trabajo. Los códigos utilizados en este trabajo son: CATIA para la generación de geometrías 3D complejas MCAM para la traducción de archivos de CATIA a formato de input de MCNP MCNP para el transporte de la radiación (neutrones y gammas) y sus respuestas asociadas ACAB para la evolución del inventario isotópico y sus respuestas asociadas MC2ACAB para acoplar MCNP y ACAB para el cómputo de dosis en parada usando la metodología R2S basada en celda. Moritz para visualizar los reultados de MCNP FLUENT para llevar a cabo cálculos de fluido-dinámica Para llevar a cabo este trabajo, han sido necesarias unas destrezas computacionales. Las más relevantes utilizadas son: generación de geometrás 3D complejas y transmisión a MCNP, diferentes tñecnica de reducción de varianza como importancia por celdas y weight windows basado en malla, metodología Rigorous-two-Steps basada en celdas para el cálculo de dosis en parada y la modificación del código ACAB para el cálculos con múltiples espectros en la misma simulación. Como resumen, la contribución de este trabajo al proyecto HiPER son dos diseños conceptuales de instalación: una para HiPER Engineering y otra para HiPER Reactor. La primera se ha estudio en profundidad desde el punto de vista de protección radiológica y gestión de residuos, mientras que la segunda se ha estudiado desde el punto de vista de operación: seguridad, comportamiento, vida y mantenimiento de componentes y eficiencia del ciclo de potencia.

There are many industries that use highly technological solutions to improve quality in all of their products. The steel industry is one example. Several automatic surface-inspection systems are used in the steel industry to identify various types of defects and to help operators decide whether to accept, reroute, or downgrade the material, subject to the assessment process. This paper focuses on promoting a strategy that considers all defects in an integrated fashion. It does this by managing the uncertainty about the exact position of a defect due to different process conditions by means of Gaussian additive influence functions. The relevance of the approach is in making possible consistency and reliability between surface inspection systems. The results obtained are an increase in confidence in the automatic inspection system and an ability to introduce improved prediction and advanced routing models. The prediction is provided to technical operators to help them in their decision-making process. It shows the increase in improvement gained by reducing the 40 % of coils that are downgraded at the hot strip mill because of specific defects. In addition, this technology facilitates an increase of 50 % in the accuracy of the estimate of defect survival after the cleaning facility in comparison to the former approach. The proposed technology is implemented by means of software-based, multi-agent solutions. It makes possible the independent treatment of information, presentation, quality analysis, and other relevant functions.

The present Master/Doctorate in Nuclear Science and Technology programme implemented in the Department of Nuclear Engineering of the Universidad Politécnica de Madrid (NED-UPM) has the excellence qualification by the Spanish Ministry of Education. One of the main of this programme is the training for the development of methodologies of simulation, design and advanced analysis, including experimental tools, necessary in research and in professional work in the nuclear field.
