Background-Fetal atrioventricular (AV) block is an uncommon lesion with significant mortality. Because of the rarity of this disorder, the natural course, extensive evaluation of untreated fetuses, and late follow-up remain unclear. Methods and Results-Of the 116 consecutive cases of fetal AV block studied from 1988 to 2006, only 1 was terminated, and 75% were live births. Fifty-nine cases of AV block were associated with major structural heart disease, mainly left atrial isomerism (n = 40), with only 26% of neonatal survivors. Of the 57 fetuses with normal cardiac anatomy, 41 (72%) were positive for maternal antinuclear antibodies, and 32 of these seropositive mothers did not receive any treatment. This untreated group had live-birth and 1-year infant survival rates of 93% and 90%, respectively. Five fetuses from seronegative mothers showed regression to sinus rhythm during pregnancy. The presence of major structural heart disease, hydrops, an atrial rate <= 120 bpm, and a ventricular rate <= 55 bpm were identified as risk factors for mortality. Logistic regression analysis of the whole group showed that the presence of structural heart disease was the only independent predictor of death (P < 0.001). Conclusions-This long-term study confirms that fetal AV block has a poor outcome when associated with structural heart disease and that spontaneous regression of AV block is possible in seronegative forms. The survival rate of >90% of our untreated patients with isolated forms of AV block raises concerns about any decision to intervene with immunosuppressive agents.

In the present study we evaluated the toxic effects on the male adult rat prostate of DBP exposure during fetal and lactational periods, because although many studies have addressed the influence of phthalates on the male reproductive system, only a few have discussed their possible effects on prostate development. Pregnant females were distributed into two experimental groups: Control (C) and Treated (T). The females of the T group received DBP (100 mg/kg, by gavage) from gestation day 12 to postnatal day 21, while C rats received the vehicle (corn oil). In adulthood (90 days old), the animals were euthanized. The serum and testicular testosterone levels were measured. Ventral prostate was removed and weighed. Distal segment fragments of the ventral prostate were fixed and processed for histochemistry and immunohistochemistry to detect androgen receptor (AR) and Ki67 antigens. Protein extraction from ventral prostate fragments was performed for AR immunoblotting and Gelatin zymography for MMP-2 and MMP-9 (MMP, metalloproteinase). Stereological and histopathological analyses were also performed. Serum and testicular testosterone levels and prostate weight were comparable between groups. In the T group the relative proportions (%) of epithelial (C=32.86; T=42.04*) and stromal (C=21.61; T=27.88*) compartments were increased, while the luminal compartment was decreased (C=45.54; T=30.08*), *p < 0.05. In T, disseminated inflammatory infiltrate in the stroma, associated or not with epithelial dysplasia and PIN (Prostatic Intraepithelial Neoplasia), was observed. Increases in AR expression, proliferation index and metalloproteinase 9 (MMP-9) activity were noted in T animals. In some T animals, collagen fibrils accumulated adjacent to the epithelium. As far as we are aware, this is the first report in the literature showing that phthalates could play a role in proliferative and inflammatory disorders of the rat prostate. After the transition from in utero to newborn life, the neonate becomes solely reliant upon its own drug clearance processes to metabolise xenobiotics. Whilst most studies of neonatal hepatic drug elimination have focussed upon in vitro expression and activities of drug-metabolising enzymes, the rapid physiological changes in the early neonatal period of life also need to be considered. There are dramatic changes in neonatal liver blood how and hepatic oxygenation due to the loss of the umbilical blood supply, the increasing portal vein blood flow, and the gradual closure of the ductus venosus shunt during the first week of life. These changes which may well affect the capacity of neonatal hepatic drug metabolism. The hepatic expression of cytochromes P450 1A2, 2C, 2D6, 2E1 and 3A4 develop at different rates in the postnatal period, whilst 3A7 expression diminishes. Hepatic glucuronidation in the human neonate is relatively immature at birth, which contrasts with the considerably more mature neonatal hepatic sulfation activity. Limited in vivo studies show that the human neonate can significantly metabolise xenobiotics but clearance is considerably less compared with the older infant and adult. The neonatal population included in pharmacological studies is highly heterogeneous with respect to age, body weight, ductus venosus closure and disease processes, making it difficult to interpret data arising from human neonatal studies. Studies in the perfused foetal and neonatal sheep liver have demonstrated how the oxidative and conjugative hepatic elimination of drugs by the intact organ is significantly increased during the first week of life, highlighting that future studies will need to consider the profound physiological changes that may influence neonatal hepatic drug elimination shortly after birth.

Com a finalidade de conhecer as relações entre o hábito de fumar, o estado nutricional de gestantes, o peso do recém-nascido e seu crescimento no primeiro ano de vida, foi feito um estudo retrospectivo com dados obtidos nos prontuários de crianças atendidas em um consultório pediátrico. Verificou-se que filhos de desnutridas tiveram peso e altura inferiores quando comparados aos filhos de normais e obesas, e que filhos de fumantes também tiveram peso e altura inferiores, quando comparados aos filhos de não fumantes. Pôde-se constatar, ainda, que ao final do primeiro ano de vida essas diferenças praticamente desapareciam, mostrando que as influências maternas in utero não tiveram como conseqüência um retardo de crescimento pós-natal.

O desenvolvimento verificado nos processos de produção agrícola e industrial originou o aparecimento e acumulação de vários poluentes, como os compostos desreguladores endócrinos, os quais podem ser encontrados em diversos produtos como: plásticos, fitofarmacêuticos e compostos orgânicos, podendo os seus efeitos serem também provocados por metais pesados. Podem ainda, ter origem natural, como os fitoestrogéneos. Assim, a exposição a estes compostos pode materializar-se por diversas vias, das quais se destaca a alimentação. Ao interagir com o sistema endócrino, aqueles compostos podem originar efeitos noutros sistemas, nomeadamente no reprodutivo, imunológico e neurológico, para além de poderem ter efeitos cancerígenos, bem como contribuir para a obesidade. O aumento da incidência de cancros, como da mama e dos testículos, tem levado a uma preocupação crescente sobre a exposição in utero àqueles compostos e durante fases chave do desenvolvimento. Analisando três estudos, sobre os mencionados tipos de cancro, observou-se uma maior incidência nos países do Norte da Europa, assim como em grupos etários específicos. Comparando os dados de Portugal com a Europa, para 1997, observou-se uma menor incidência no nosso País. Por outro lado, a Comissão Europeia adoptou medidas que se baseiam no controlo de risco referente à exposição a estes compostos, através de três fases: identificação, avaliação e gestão do risco. Legislação Comunitária, como a Directiva-Quadro da Água ou respeitante ao controlo de compostos químicos, contribuem também para reduzir a exposição a compostos desreguladores endócrinos.

A ocorrência de microfilárias circulantes de Wuchereria bancrofti foi pesquisada em 304 gestantes da Unidade Mista Prof. Barros Lima e do Hospital das Clínicas da Universidade Federal de Pernambuco pelo Centro de Pesquisas Aggeu Magalhães. A microfilaremia materna foi investigada pela filtração de sangue venoso, sendo encontrados 13 casos positivos (4,2%). A pesquisa de microfilárias no sangue do cordão umbilical de suas crianças foi negativa (6/13), assim como no sangue periférico destas até 72 horas pós-parto e com 6 meses de vida. As amostras de leite coletadas destas mães também não apresentaram microfilárias. Os autores sugerem que a ocorrência de lesões placentárias seja um possível fator envolvido na passagem transplacentária de microfilárias, e que a exposição in utero a microfilárias e/ou antígeno filarial possam influenciar a resposta a uma infecção filarial adquirida posteriormente, sendo importante o acompanhamento clínico e laboratorial de crianças expostas previamente a antígenos filariais em períodos precoces do desenvolvimento humano.

The mortality rate is high and prognosis is worse among new-borns with prenatal diagnosis of heart malformation, mainly due to factors such as its association with other malformations, and a range of more severe diseases probably resulting from the predominance of the obstetric use of the four chamber view. In this study we retrospectively assessed the range of cardiopathies diagnosed by foetal echocardiography and their evolution, compared with previous years. From January 1994 to December 1995, 1173 foetal echocardiograms were performed at a gestation age of 24 weeks. Sixty-one foetuses (5.2%) had cardiac anomalies, structural in 56 and arrhythmia in 5. The risks and indications were maternal in 37%, foetal in 31%, familial in 17% and environmental in 15%. Three were false negatives (VSD:2; truncus arteriosus: 1). Five died in utero, and 18 were assessed after birth with a mean gestational age of 37 weeks and birth weight of 3 Kg, a caesarean section was performed in 9. All but one were born in central hospitals. Six children were operated on. Two children died, one after surgery. Compared with the four previous years of activity, indication due to foetal risk rose from 6 to 31%, the number of cases diagnosed with heart disease increased from 14 to 30 per year, and the mortality decreased from 59 to 11%. Despite this, we still observe that the vast majority of new-borns who are hospitalised due to a severe heart disease had no prenatal diagnosis, indicating the need to continue our educational policy in this field.
Vólvulo Intestinal en el Periodo Neonatal: 8 Años de Experiencia en un Hospital Pediátrico Terciario

Análise dos casos de volvo intestinal ocorridos no período neonatal nos últimos 8 anos (2002 a 2010). Material e métodos: Foram estudados os recém-nascidos admitidos na UCIN cujo diagnóstico de saída foi volvo intestinal. Foram estudados os seguintes parâmetros: idade gestacional e pós-natal, apresentação clínica e imagiológica, intervenção cirúrgica e resultados. Resultados: Foram identificados 15 doentes 7 dos quais no último ano do estudo. Sete RN eram pré-termo (PT) ou ex pré-termo. A mediana de peso ao nascer foi de 2665g (660-3900); 4 RN eram muito baixo peso. A mediana de idade de início dos sintomas foi 7 dias; em 5 RN a doença teve início nas primeiras 24 horas de vida; em 3 destes, o volvo ocorreu in utero. Sinais e sintomas: grande distensão abdominal-12; resíduo gástrico bilioso-11; alterações da parede abdominal-5; dejecções com sangue-4; instabilidade hemodinâmica-6. Imagiologia: grande distensão de ansas, sem ar ectópico- 10 doentes; ausência de ar no abdómen-4; trânsito intestinal contrastado sugestivo de malrotação e volvo-3; ecografia e Doppler abdominal com sinal de “whirlpool”-2. Todos foram submetidos a cirurgia de urgência, sendo o volvo confirmado intraoperatoriamente; foi necessária ressecção intestinal em 9 doentes; 3 ficaram com síndrome do intestino curto; registou-se um óbito por falência multi-orgânica no período pós-operatório. Conclusão: Foi encontrado um elevado número de casos de volvo intestinal em RN pré-termo ou ex pré-termo, de volvo in utero e de elevada ocorrência de casos no último ano do estudo. Resíduo gástrico bilioso e distensão abdominal foram os sintomas mais frequentes de volvo e devem ser tomados em consideração no diagnóstico diferencial com outras situações cirúrgicas abdominais. As sequelas são potencialmente graves.

Os teratomas sacrococcígeos (TSC) são os tumores mais frequentes do recém-nascido, sendo frequentemente diagnosticados in utero. Apresenta-se o caso de um recém-nascido com um volumoso TSC, diagnosticado às 22 semanas de gestação. A gravidez foi vigiada por uma equipa multidisciplinar, atendendo ao elevado risco de complicações materno-fetais. Realizou-se cesariana electiva às 38 semanas. Ao 8º dia de vida, procedeu-se à ressecção cirúrgica do TSC em bloco com o cóccix e reposicionamento posterior do ânus. Não existiram intercorrências no intra ou pós-operatório. O seguimento da gravidez por uma equipa pluridisciplinar foi fulcral, per - mitindo a vigilância de complicações, planeamento anestésico-cirúrgico do nascimento e antecipação do tratamento apropriado, nomeadamente dos cuidados perinatais e da cirurgia neonatal.

Se proponen tres líneas de Proyecto: 1) Efectos del estrés crónico intermitente (180 días) sobre el metabolismo y la actividad cardiovascular. En este Proyecto se dará continuidad a la línea de trabajos relativos a los efectos del estrés agudo y crónico sobre el estrés oxidativo, el metabolismo lipídico, las lipoproteínas plasmáticas; y la aterosclerosis (Bianco, M. et al. 2008; Scoppa, H. et al, 2005 y 2006). La retención de sodio y agua demostrada en nuestro laboratorio podría explicar la hipertensión generada por el estrés (Bensi, N. et al, 1997; Bertuzzi, M. et al. 2003; Niebylski, A. et al. 2008). Es en esta búsqueda que se investigan los mecanismos endócrinos y/o nerviosos involucrados en cada caso. 2) Efectos del estrés prenatal sobre la función reproductiva de la madre gestante y sus crías. A partir de los resultados previos como alteraciones morfofuncionales de la hipofisis materna, de la migración de células GnRH de las crías y sus probables impactos funcionales y conductuales, se continuará con la evaluación placentaria de las hembras gestantes y los cambios de la función adrenal y adenohipofisiaria (Mugnaíni, MT, et al. 2006; Bozzo A. et al, 2006; Romanini MC. et al, 2007 y Soñez, C. Tesis Doctorales). 3) El estrés prenatal produce alteraciones neurológicas, conductuales y reproductivas en el hombre y animales; además afecta la producción en los animales domésticos, fundamentalmente cuando son criados en condiciones intensivas, bajo las cuales tienen mayor exposición a agentes infecciosos, produciendo grandes pérdidas económicas. En nuestro laboratorio hemos hallado que la aplicación de estrés por inmovilización en plancha (IMO) en las ratas preñadas produce en la cría estresada prenatalmente (EP) al estado adulto, hiperactividad del eje HHA, en condiciones basales y habituación cuando el animal es sometido, al mismo estrés. Algunos estudios muestran que la actividad del eje HHA podría producir alteraciones en algunos parametros inmunes de la crías adultas EP. El objetivo de este trabajo es investigar el efecto del estrés agudo, por nado forzado, en crías adultas EP por IMO, sobre la actividad del eje HHA y su relación con las células efectoras de la inmunidad. Para ello se utilizaron ratas Wistar, macho, de 90 días de edad, EP y sus controles no estresados (C). Se determinará la concentración de COR plasmática en respuesta a la aplicación de un estrés agudo distinto al aplicado in utero y su relación con la variación de los valores porcentuales de los leucocitos sanguíneos y la proliferación de linfocitos T.adrenal y adenohipofisiaria (Mugnaíni, MT,et al. 2006;

Adverse events in utero are associated with the occurrence of chronic diseases in adulthood. We previously demonstrated in mice that perinatal hypoxia resulted in altered pulmonary circulation in adulthood, with a decreased endothelium-dependent relaxation of pulmonary arteries, associated with long-term alterations in the nitric oxide (NO)/cyclic GMP pathway. The present study investigated whether inhaled NO (iNO) administered simultaneously to perinatal hypoxia could have potential beneficial effects on the adult pulmonary circulation. Indeed, iNO is the therapy of choice in humans presenting neonatal pulmonary hypertension. Long-term effects of neonatal iNO therapy on adult pulmonary circulation have not yet been investigated. Pregnant mice were placed in hypoxia (13% O2) with simultaneous administration of iNO 5 days before delivery until 5 days after birth. Pups were then raised in normoxia until adulthood. Perinatal iNO administration completely restored acetylcholine-induced relaxation, as well as endothelial nitric oxide synthase protein content, in isolated pulmonary arteries of adult mice born in hypoxia. Right ventricular hypertrophy observed in old mice born in hypoxia compared to controls was also prevented by perinatal iNO treatment. Therefore, simultaneous administration of iNO during perinatal hypoxic exposure seems able to prevent adverse effects of perinatal hypoxia on the adult pulmonary circulation.

Several factors appear to affect vertical HIV-1 transmission, dependent mainly on characteristics of the mother (extent of immunodeficiency, co-infections, risk behaviour, nutritional status, immune response, genetical make-up), but also of the virus (phenotype, tropism) and, possibly, of the child (genetical make-up). This complex situation is compounded by the fact that the virus may have the whole gestation period, apart from variable periods between membrane rupture and birth and the breast-feeding period, to pass from the mother to the infant. It seems probable that an extensive interplay of all factors occurs, and that some factors may be more important during specific periods and other factors in other periods. Factors predominant in protection against in utero transmission may be less important for peri-natal transmission, and probably quite different from those that predominantly affect transmission by mothers milk. For instance, cytotoxic T lymphocytes will probably be unable to exert any effect during breast-feeding, while neutralizing antibodies will be unable to protect transmission by HIV transmitted through infected cells. Furthermore, some responses may be capable of controlling transmission of determined virus types, while being inadequate for controlling others. As occurence of mixed infections and recombination of HIV-1 types is a known fact, it does not appear possible to prevent vertical HIV-1 transmission by reinforcing just one of the factors, and probably a general strategy including all known factors must be used. Recent reports have brought information on vertical HIV-1 transmission in a variety of research fields, which will have to be considered in conjunction as background for specific studies.

In 2011, new oral anticoagulants for atrial fibrillation are available and the ABCD3-I score predicting stroke after TIA updates the ABCD2 score. New McDonald criteria allow faster MS diagnosis and the first oral treatment (fingolimod) for MS can be prescribed. A new anti-antiepileptic drug (retigabine) is available and sodium valproate has long term neurological adverse effects after in utero exposure. Among Parkinson disease treatments, deep brain stimulation is extending applications and dopamine agonists with extended release are as efficient and well tolerated as standard forms at long term scale. Monoclonal antibodies and immunosuppressant agents are proposed as good alternatives in the treatment of chronic dysimmune polyneuropathies. Gene therapy for the treatment of genetic myopathies is progressing.

A report on Environmental Inequalities in the UK. Part of the Burden of disease. A clean and healthy environment is a vital component of public health. This is particularly so for children. They are more sensitive to most stressors during development and growth and receive relatively more exposure than adults due to behaviour patterns, lack of awareness, size and biological metabolisms.A study of the contribution of environmental pollutants to the incidence, prevalence, mortality and costs of four categories of paediatric disease in American children estimated total annual costs to be $54.9 billion comprising $43.4 billion for lead poisoning, $2.0 billion for asthma, $0.3 billion for childhood cancer, and $9.2 billion for neurobehavioral disorders; 2.8 % of total U.S. health care costs. As well as childhood conditions, some adult diseases, even those that emerge much later in life, e.g. hypertension, hyperlipidemia, insulin resistance, type 2 diabetes, ischemic heart disease, breast cancer and prostate cancer have some of their origins in utero and childhood. Childhood exposures to environmental health hazards may therefore constitute a source of inequity between generations .

The aim of this work is to describe the techniques that have been used for preparation and analysis of whole fetal liver extracts destined for in utero transplantation. Nine fetal livers between 12 and 17 weeks of gestation were prepared: cell counts and assessment of the hematopoietic cell viability were performed on cell suspensions. Hepatocytes represented 40 to 80% of the whole cell population. The remaining cells were constituted by hematopoietic cells (mainly erythroblasts), as well as by endothelial cells. The latter expressed CD34 on their surface, interfering with the assessment of CD34+ hematopoietic cells by flow cytometry. Direct visual morphologic control using alkaline phosphatase anti-alkaline phosphatase techniques was needed to differentiate hematopoietic from extra-hematopoietic CD34+ cells. Between 3.0 and 34.6 x 10(6) CD34+ viable hematopoietic cells were collected per fetal liver. Adequate differentiation of these cells into burst-forming units erythroid (BFU-E), colony-forming units granulocyte-macrophage (CFU-GM), and colony-forming units granulocyte erythroid macrophage megakaryocyte (CFU-GEMM) has been shown for each sample in clonogeneic cultures. In conclusion, fetal liver is a potential source of hematopoietic stem cells. Their numeration, based on the presence of CD34, is hampered by the expression of this antigen on other cells contained in the liver cell extract, in particular endothelial cells.
