Este trabajo está basado en las prácticas del trabajo final de máster (TFM) de “Envejecimiento Activo y Satisfactorio” de la Universidad de Vic. Partiendo de la base que según la OMS el Envejecimiento Activo (en adelante EA) “pretende optimizar las oportunidades de salud, participación y seguridad a fin de mejorar la calidad de vida de las personas mayores”, en la Residencia Nord Egara de Castellar del Vallés (Barcelona) promoví un conjunto de Buenas Prácticas (en adelante BBPP), centrando la atención en los usuarios y considerando las dificultades derivadas de su situación de fragilidad y/o dependencia. En el Centro se promovieron dos tipos de BBPP: 7 BBPP de mejora enfocadas a las intervenciones en el Centro y 3 BBPP de recomendaciones para la buena praxis profesional en la vida cotidiana. La implantación de las BBPP implicó repensar en el ámbito organizativo y relacional del Centro, mejorando, entre otros aspectos, la manera de actuar de los profesionales, aspectos de organización interna, ampliación de horarios profesionales y las formas cotidianas de relación interpersonal. En concreto, las BBPP de mejora reportaron avances tales como ofrecer la oportunidad para que los usuarios, en función de sus capacidades y deseos, pudieran elegir entre varias alternativas, implicar a los usuarios y/o familias en las tomas de decisiones sobre aspectos que conciernen a la vida del propio usuario, aumentar la relación de los usuarios con el exterior (tanto abriendo el Centro a la comunidad como aumentando la integración de los usuarios en el entorno) y aumentar la implicación de las familias de los usuarios en la vida del Centro. Por otra parte, las BBPP como recomendaciones sirvieron de complemento para que los profesionales se evaluaran y reflexionaran sobre sus formas de actuación y de relación, concretando cómo debería actuar el equipo profesional si quería seguir avanzando, después de mi intervención en el Centro, en las áreas de mejora tratadas con las BBPP desarrolladas. Al finalizar el presente TFM, el Centro estaba en pleno proceso de implantación de prácticamente todas las BBPP. Por este motivo, con el paso de los meses, el equipo tendrá que ir revisando, evaluando y modificando (si precisa) dichas BBPP. A pesar de no estar todas completamente implantadas, éstas ya han dejado constancia de mejoras y beneficios en la calidad de vida de los usuarios.

Con la llegada del nuevo paradigma del “envejecimiento activo” planteado por la Organización Mundial de la Salud (OMS, 2002), estamos asistiendo a una nueva manera de enfocar el progresivo envejecimiento de la población. Este enfoque trata de fomentar la consecución de una esperanza de vida saludable y libre de discapacidad, gracias a la promoción de políticas y programas cuyos objetivos principales se centran en la independencia y en la autonomía a lo largo del proceso de envejecimiento. Acorde con este enfoque pensamos que las ocupaciones significativas son una buena herramienta para mantener la autonomía de estas personas al fomentar la prevención y la promoción de su salud, motivo por el cual desarrollamos este estudio. Los objetivos principales del presente estudio son conocer y comprender que ocupaciones adquieren un significado y satisfacción personal en las personas mayores-jóvenes, que han concluido su etapa laboral, que cuentan con buena salud y condiciones psicofísicas favorables y que viven de manera autónoma en la comunidad, así como explorar también si se establece en estas personas alguna vinculación entre las actividades que eligen desarrollar en la actualidad con su perfil e historia ocupacional previa. Por último, este estudio pretende conocer el tipo de gestión que cada persona prefiere para llevar a cabo las actividades cotidianas que les son más significativas, a partir de los contextos en los que se inscribe. El marco teórico se desarrolla a partir de la conceptualización aportada por diversos autores sobre el envejecimiento activo y la ciencia de la ocupación. La metodología utilizada es de carácter cualitativo, enfocado desde un paradigma constructivista. Los resultados del estudio muestran que se inicia tímidamente un cambio de tendencias en la elección y el significado que las personas mayores-jóvenes atribuyen a sus ocupaciones. Muestran también que las ocupaciones elegidas como más significativas por estas personas mantienen una vinculación con las que ya realizaban con anterioridad a la jubilación, con su historia o perfil 6 ocupacional previo. Y finalmente demuestran que el tipo de gestión que eligen para efectuarlas influye en la elección del entorno donde las llevan a término.

Introducción: los cambios epidemiológicos sufridos en los países occidentales en el siglo XX han propiciado un aumento del número absoluto y de la proporción de ancianos en todos ellos. Pacientes y Método: se realizó un estudio descriptivo transversal de prevalencia de enfermedades bucodentales en una población de 3.460 individuos en 7 Comunidades Autónomas españolas, tanto institucionalizados (10%) como no institucionalizados (90%). Resultados: la edad media de los encuestados fue de 73,6±7 años. Un 31,4% de los individuos del estudio eran desdentados totales. Los individuos dentados presentaban un promedio de 14,8±8 dientes. El CAOD de los individuos de la muestra fue de 20,8±10, siendo de 23,7±9,8 en los institucionalizados. La última visita al dentista había sido de media 4 años y medio antes, pero los institucionalizados hacía 7 años. Discusión: es totalmente inaceptable que la prevención y el tratamiento las enfermedades bucodentales. especialmente entre las personas de edad más avanzada, se sigan considerando como un aspecto totalmente optativo para el mantenimiento de la salud del individuo. Estudios epidemiológicos. bacteriológicos e Inmunológicos demuestran que la enfermedad periodontal o la caries pueden ser las causantes de un importante aumento de la morbi-mortalidad especialmente entre las personas de más edad.

En este trabajo se analiza el modelo markoviano de transiciones anuales entre estados de dependencia asumiendo la hipótesis de estacionariedad. Se suponen conocidas las tasas de mortalidad de la población autónoma y las tasas de prevalencia de los tres estados de dependencia considerados. La indeterminación del modelo se resolverá incorporando restricciones en forma de hipótesis en las interrelaciones, a partir de las cuales se obtienen las matrices de transición por edades y se analiza el comportamiento de las mismas. Se realizan aplicaciones numéricas utilizando distribuciones de mortalidad y de prevalencia que pueden ser adecuadas para la población española y que han surgido de un análisis preliminar. Por último, se efectúa un análisis de sensibilidad de los resultados respecto al cambio de hipótesis en las mencionadas interrelaciones. Abstract

Complex psychopathological and behavioral symptoms, such as delusions and aggression against care providers, are often the primary cause of acute hospital admissions of elderly patients to emergency units and psychiatric departments. This issue resembles an interdisciplinary clinically highly relevant diagnostic and therapeutic challenge across many medical subjects and general practice. At least 50% of the dramatically growing number of patients with dementia exerts aggressive and agitated symptoms during the course of clinical progression, particularly at moderate clinical severity. METHODS: Commonly used rating scales for agitation and aggression are reviewed and discussed. Furthermore, we focus in this article on benefits and limitations of all available data of anticonvulsants published in this specific indication, such as valproate, carbamazepine, oxcarbazepine, lamotrigine, gabapentin and topiramate. RESULTS: To date, most positive and robust data are available for carbamazepine, however, pharmacokinetic interactions with secondary enzyme induction limit its use. Controlled data of valproate do not seem to support the use in this population. For oxcarbazepine only one controlled but negative trial is available. Positive small series and case reports have been reported for lamotrigine, gabapentin and topiramate. CONCLUSION: So far, data of anticonvulsants in demented patients with behavioral disturbances are not convincing. Controlled clinical trials using specific, valid and psychometrically sound instruments of newer anticonvulsants with a better tolerability profile are mandatory to verify whether they can contribute as treatment option in this indication.

During the last three decades, the number of tourism events has been growing in Catalan coastal resorts because of the recover of Catalan cultural traditions, festivals and folklore, and also because of tourism growth. Catalan tourism resorts use events as catalysers for new supply and as a mean to differentiate and singularize themselves from their competitors. The tourism potential of cultural events is undeniable but there are some problems that prevent a more effective impact as economic and regional development agents. This paper reflects some discussions and conclusions obtained from the analysis of 264 valid responses of a survey made to different Catalan event organizers in 2008 and 2009. We describe and characterize cultural event supply in coastal resorts in order to study the events tourism importance, their capacity to generate and spread economic development, and their managerial model. The analysis is made in a geographical basis, comparing the results of the territorial organization of events of the city of Barcelona, coastal and inland municipalities. Finally some considerations about event regional tourism policy and tourism development are discussed.

This paper explores the distinctive characteristics of mobile telephone use among the elderly population using the most recent European country-level data on individual use of mobile telephony and advanced mobile services, Eurostat 2008. Through a cluster analysis of mobile phone use data across 30 countries, it is possible to confirm that mobile telephone occupies a peripheral position for the elderly in Europe.

High blood pressure (BP) has been ranked as the most important risk factor worldwide regarding attributable deaths. Dietary habits are major determinants of BP. Among them, frequent intake of low-fat dairy products may protect against hypertension. Our aim was to assess the relationship between low-fat dairy product intake and BP levels and their changes after 12 month follow-up in a cohort of asymptomatic older persons at high cardiovascular risk recruited into a large-scale trial assessing the effects of Mediterranean diets on cardiovascular outcomes. Data from 2290 participants, including 1845 with hypertension, were available for analyses. Dairy products were not a specific part of the intervention; thus, data were analysed as an observational cohort. Dietary information was collected with validated semi-quantitative FFQ and trained personnel measured BP. To assess BP changes, we undertook cross-sectional analyses at baseline and at the end of follow-up and longitudinal analyses. A statistically significant inverse association between low-fat dairy product intake and systolic BP was observed for the 12-month longitudinal analysis. In the longitudinal analysis, the adjusted systolic and diastolic BP were significantly lower in the highest quintile of low-fat dairy product intake ( 2 4·2 (95% CI 2 6·9, 2 1·4) and 2 1·8 (95% CI 2 3·2, 2 0·4) mmHg respectively), whereas the point estimates for the difference in diastolic BP indicated a modest non-significant inverse association. Intake of low-fat dairy products was inversely associated with BP in an older population at high cardiovascular risk, suggesting a possible protective effect against hypertension.

Les Fitxes Operatives d’Elements de Risc han de ser una eina molt valida en el món operatiu del cos de Bombers, i aquí s’ha plantejat el disseny d’una nova tipologia, les FOER de Massissos Forestals per intervencions en Grans Incendis Forestals. Unes FOER molt diferents a les fetes fins a l’actualitat. També s’ha plantejat senyalar punts crítics en el territori, punts que esdevenen possibles zones de gestió forestals emmarcada en la prevenció d’incendis.
