El diagnóstico de la apendicitis aguda continúa siendo uno de los problemas clínico-quirúrgicos más difíciles en los departamentos de urgencia. El valor diagnóstico de la proteína C Reactiva y la Citología peritoneal por Punción, fueron evaluados en un estudio clínico-descriptivo de 81 pacientes administrados en el servicio de emergencia del Hospital Vicente Corral Moscoso, de la ciudad de Cuenca con diagnóstico presuntivo de apendicitis aguda. Las pruebas fueron realizadas en muestras de sangre y aspirado peritoneal, obtenidas en el momento de la admisión. 52 pacientes fueron intervenidos, de estos 39 tuvieron apendicitis, 5 presentaron otro tipo de patología quirúrgica y 8 fueron innecesariamente operados. Los 29 pacientes que no fueron intervenidos, se recuperaron satisfactoriamente en forma espontánea o luego de tratamiento alternativo respectivo. La sensibilidad, especificidad y valores predictivos de ambas pruebas y su combinación fueron calculadas; así como su comparación con el recuento y fórmula leucocitaria. Usando intervalos de referencia estándar, la PCR, demostró la mayor sensibilidad [92 por ciento] y el mejor valor predictivo negativo [91 por ciento]. Al combinar PCR y CPP, se mejoro la sensibilidad y el valor predictivo [100 por cieto para ambas], con una especificidad aceptable [69 por ciento]. Concluimos que al usar las pruebas por separado o combinadas, tiene un valor limitado para predecir apendicitis, sin embargo, la combinación entre el recuento leucocitario, PCR y CPP, o las dos últimas simplemente, denotaron un valor predictivo negativo de 100 por ciento, indicando esto que la apendicitis no es posible cuando las pruebas son simultáneamente negativas. Por ello nuestra recomendación es la de usar las pruebas en combinación para reducir el índice de laparotomías negativas en pacientes con sospecha de apendicitis

The primary cementing is an important step in the oilwell drilling process, ensuring the mechanical stability of the well and the hydraulic isolation between casing and formation. For slurries to meet the requirements for application in a certain well, some care in the project should be taken into account to obtain a cement paste with the proper composition. In most cases, it is necessary to add chemicals to the cement to modify its properties, according to the operation conditions and thus obtain slurries that can move inside the jacket providing a good displacement to the interest area. New technologies of preparation and use of chemicals and modernization of technological standards in the construction industry have resulted in the development of new chemical additives for optimizing the properties of building materials. Products such as polycarboxylate superplasticizers provide improved fluidity and cohesion of the cement grains, in addition to improving the dispersion with respect to slurries without additives. This study aimed at adapting chemical additives used in civil construction to be used use in oilwell cement slurries systems, using Portland cement CPP-Special Class as the hydraulic binder. The chemical additives classified as defoamer, dispersant, fluid loss controller and retarder were characterized by infrared absorption spectroscopy, thermogravimetric analyses and technological tests set by the API (American Petroleum Institute). These additives showed satisfactory results for its application in cement slurries systems for oil wells. The silicone-based defoamer promoted the reduction of air bubbles incorporated during the stirring of the slurries. The dispersant significantly reduced the rheological parameters of the systems studied. The tests performed with the fluid loss controller and the retarder also resulted in suitable properties for application as chemical additives in cement slurries

Oligophenylenes (polyphenylenes) are constituted by an array of conjugated benzenes where inter-ring electron delocalization tends to extend over the whole chain (linear conjugation) being intrinsically limited, among other factors, by terminal effects. Alternatively, cyclic conjugation is envisaged as the unlimited free-boundary versionofconjugation which will impact the structure of molecules in rather unknown ways. The cyclic version of oligophenylenes, cycloparaphenylenes ([n]CPPs with n the number of phenyl rings) were first synthesized in 2008 by Beztozzi and Jasti.1 Today the whole [n]CPP series from [5]CPP to [18]CPP has been prepared. [n]CPPs represent ideal models to investigate new insights of the electronic structure of molecules and cyclic conjugation when electrons or charges circulate in a closed circuit without boundaries. Radical cations and dications of [n]CPP from n=5 to n=12 have been prepared and studied by Raman spectroscopy.2 Small [n]CPP dications own their stability to the closed-shell electronic configuration imposed by cyclic conjugation. However, in large [n]CPP dications cyclic conjugation is minimal and these divalent species form open-shell biradicals. The Raman spectra reflect the effect of cyclic conjugation in competition with cyclic strain and biradicaloid aromatic stabilization. Cyclic conjugation provokes the existence of a turning point or V-shape behavior of the frequencies of the G bands as a function of n. Although there are a wide variety of additives that act in fresh state, to adjust the properties of cement, there is also a search by additions that improve the tenacity of the cement in the hardened state. This, in turn, can often be increased by inserting fibers, which act on the deflection of microcracks. This study aimed to use a microfiber glass wool (silica-based) as an additive reinforcing the cement matrix, improving the rupture tenacity, in order to prevent the propagation of microcracks in the cement sheath commonly found in oil wells submitted to high temperatures. The fibers were added at different concentrations, 2 to 5% (BWOC) and varied average sizes, grinding for 90 s, 180 s, 300 s, 600 s. The cement slurries were made with a density of 1,90 g/ cm3 (15,6 lb/gal), using Portland cement CPP- Special Class as the hydraulic binder and 40% silica flour. The characterization of the fiber was made by scanning electron microscopy (SEM), particle size by sieving, X-ray fluorescence (XRF), X-ray diffraction (XRD) and thermogravimetry (TG / DTG). Were performed technological tests set by the API (American Petroleum Institute) by rheology, stability, free water, compressive strength, as well as testing rupture energy, elastic modulus and permeability. The characterization results showed good thermal stability of the microfiber glass wool for application in oil wells submitted to steam injection and, also, that from the particle size data, it was possible to suggest that microfibers milled up to 300 s, are ideal to act as reinforcement to the cement slurries. The rheological parameters, there was committal of plastic viscosity when larger lengths were inserted of microfiber (F90). The values obtained by free water and stability were presented according to API. The mechanical properties, the incorporation of microfiber to the cement slurries gave better rupture tenacity, as compared to reference cement slurries. The values of compressive strength, elastic modulus and permeability have been maintained with respect to the reference cement slurries. Thus, cement slurries reinforced with microfiber glass wool can ensure good application for cementing oil wells submitted to steam injection, which requires control of microcracks, due to the thermal gradients

The study aim was to test whether a 12-week publically rebated group programme, based upon Steketee and Frost's Cognitive Behavioural Therapy-based hoarding treatment, would be efficacious in a community-based setting. Over a 3-year period, 77 participants with clinically significant hoarding were recruited into 12 group programmes. All completed treatment; however, as this was a community-based naturalistic study, only 41 completed the post-treatment assessment. Treatment included psychoeducation about hoarding, skills training for organization and decision making, direct in-session exposure to sorting and discarding, and cognitive and behavioural techniques to support out-of-session sorting and discarding, and nonacquiring. Self-report measures used to assess treatment effect were the Savings Inventory-Revised (SI-R), Savings Cognition Inventory, and the Depression, Anxiety and Stress Scales. Pre-post analyses indicated that after 12 weeks of treatment, hoarding symptoms as measured on the SI-R had reduced significantly, with large effect sizes reported in total and across all subscales. Moderate effect sizes were also reported for hoarding-related beliefs (emotional attachment and responsibility) and depressive symptoms. Of the 41 participants who completed post-treatment questionnaires, 14 (34%) were conservatively calculated to have clinically significant change, which is considerable given the brevity of the programme judged against the typical length of the disorder. The main limitation of the study was the moderate assessment completion rate, given its naturalistic setting. This study demonstrated that a 12-week group treatment for hoarding disorders was effective in reducing hoarding and depressive symptoms in an Australian clinical cohort and provides evidence for use of this treatment approach in a community setting. KEY PRACTITIONER MESSAGE: A 12-week group programme delivered in a community setting was effective for helping with hoarding symptoms with a large effect size. Hoarding beliefs (emotional attachment and responsibility) and depression were reduced, with moderate effect sizes. A third of all participants who completed post-treatment questionnaires experienced clinically significant change. Suggests that hoarding CBT treatment can be effectively translated into real-world settings and into a brief 12-session format, albeit the study had a moderate assessment completion rate.

Binge eating occurs primarily on highly palatable food (PF) suggesting that the reward value of food has an important role in this behaviour. Bingeing also leads to reward dysfunction in rats and humans. The rewarding effect of binge eating may involve opioid mechanisms as opioid antagonists reduce PF consumption in animals that binge eat and binge eating produces neuroadaptations of opioid receptors in rodents. We tested this hypothesis by using the conditioned place preference (CPP) paradigm. First we established a sucrose CPP in male and female Long-Evans rats (n=8 for each group) using 1%, 5%, 15%, or 30% sucrose solution. Next, rats underwent the sucrose bingeing model in which separate groups of rats (n=8 for each group) received 12hr and 24hr access to 10% sucrose solution and chow, 12hr access to 0.1% saccharin solution and chow, or 12hr access to chow only every day for 28 days. Immediately following these sessions, rats were conditioned and tested in the CPP paradigm using a 15% sucrose solution. Finally, we examined whether the sucrose bingeing model altered morphine reward in female rats. Rats (n=8 for each group) received 12hr and 24hr access to 10% sucrose solution and chow every day for 28 days. Immediately following this access period, rats were conditioned to morphine (6mL/kg) or saline solution in the CPP paradigm and tested for a CPP. In all experiments, rats drank more sucrose solution than water during conditioning sessions. Male rats did not develop a CPP to any concentration of sucrose solution and females developed a CPP to 15% sucrose solution only. Following the sucrose bingeing protocol, sucrose CPP was attenuated in male rats that binged on sucrose and in all female rats. Sucrose bingeing in females did not affect the development of a CPP to morphine. These results suggest that sucrose consumption and sucrose CPP are measures of different psychological components of reward. Furthermore, sucrose bingeing reduces the rewarding effect of sucrose, but not morphine, suggesting that opioid reward is still intact.

Normalmente el desarrollo de un país se ha explicado desde una perspectiva tradicional en términos de su crecimiento económico, teniendo en cuenta indicadores macroeconómicos como el PIB, la inflación y el desempleo. Poca atención se le ha puesto a la importancia que para el desarrollo de un país representan el capital humano y el proceso de liderazgo. Debido a lo anterior, mediante este estudio de caso, se pretende entender el éxito de la estrategia de crecimiento por exportaciones de Japón entre los años 1960-1980 teniendo en cuenta estos aspectos. Así, se busca sustentar que la incorporación de un tipo de liderazgo transformacional- transaccional y los elementos propios de su cultura como el confucianismo y el budismo, le imprimieron una perspectiva no economicista al éxito del modelo de desarrollo como parte de la triada empresa-estado-universidad. Lo anterior se realizará partiendo de un análisis cualitativo y con un enfoque en la economía política internacional y en el liderazgo. No pantanal mato-grossense a pesca artesanal tem acontecido por consecutivas gerações e, de maneira geral, sempre contribuiu para a subsistência das famílias ribeirinhas. Ospescadores profissionais artesanais comercializam, por vezes, em mercados restritos ou sujeitam-se aos preços de atravessadores, como correlato, convivem sem o acesso àmelhores condições de saúde, moradia e educação. Diante do exposto a presente investigação teve por objetivo a descrição e análise dos aspectos econômicos dos pescadores artesanais no município de Poconé (16015`25” S e 560 37`22” W) localizadona micro-região Alto Pantanal, meso-região sul do estado de Mato Grosso, na região Centro-Oeste do Brasil. Após revisão de literatura pertinente, efetivou-se a coleta de dados em campo, por meio das técnicas de entrevistas não-estruturadas e semiestruturadas. A unidade amostral foi a Colônia de Pescadores Z11 no município de Poconé. Os discursos foram transcritos e analisados na perspectiva da pesquisa qualitativa. Concluindo-se que apesar de estarem organizados em colônias, os índices deacesso às políticas públicas de melhoria de qualidade de vida ainda são precários, uma vez que participam de mercados econômicos incompletos e do circuito inferior da economia.

Este artigo analisa a pecuária no Pantanal mato-grossense a partir da abordagem da sustentabilidade. Os resultados apresentados são fruto de um projeto de pesquisa financiado pelo Instituto Nacional de Áreas Úmidas – INAU / Centro de Pesquisas do Pantanal – CPP[1]. A pecuária é a principal atividade econômica do Pantanal brasileiro e tem sido desenvolvida na região há pelo menos dois séculos. O caráter extensivo da atividade pecuária no Pantanal, alicerçado em outros elementos da questão agrária nacional, contribuiu por muito tempo para a manutenção do bioma sem grandes impactos ambientais. Contudo, com as transformações agrárias e produtivas da agropecuária brasileira, ocorridas a partir da década de 1970, a pecuária pantaneira tem sofrido alterações e se tornado mais impactante no bioma. Ao mesmo tempo, a região apresenta um quadro de estagnação econômica e baixa qualidade de vida da maior parte da população, já que a atividade pecuária é dominada por um pequeno grupo que concentra as terras. Assim, como pensar a pecuária no contexto do desenvolvimento regional sustentável? Quais as perspectivas para a atividade na região e quais os sentidos das alterações de suas práticas para a sustentabilidade, se é que isso é possível?

O presente trabalho teve como objetivo determinar a produtividade agrícola e a qualidade tecnológica do caldo das variedades da cana-de-açúcar, submetidas a diferentes regimes hídricos. O delineamento experimental foi em blocos ao acaso com três repetições em parcelas subdivididas, com três variedades RB855156, RB835486 e RB867515 (parcela), e sete regimes hídricos correspondendo a 0, 7, 17, 36, 46, 75 e 100% da ETc (subparcela). Foram avaliadas as características de produção: comprimento do entrenó (CE), peso do colmo (PC), número de perfilhos por hectare e produtividade; e tecnológicos do caldo: ºBrix do caldo, Pol do caldo (Teor de sacarose), pureza (PZA), AR (teor de açúcares redutores do caldo), ARC (açúcares redutores da cana), fibras, Pol da cana (PCC), açúcares totais recuperáveis (ATR) e o valor da megagrama da cana (VMgC). O máximo de produtividade da cana-de-açúcar foi de 182,31 Mg ha-1 registrado com irrigação equivalente a 69,01% da ETc; A cana-de-açúcar cultivada com irrigação equivalente a 75% da ETc não proporcionou diferença significativa para a qualidade industrial das variedades testadas em relação ao cultivo em sequeiro (0% da ETc). Não houve diferença significativa para os valores do teor de açúcares redutores (AR), açúcares redutores da cana (ARC) e a pureza do caldo (PZA) entre as variedades. ABSTRACT: This work aimed to determine the agricultural productivity and the technological quality of the juice of the sugarcane varieties, which were submitted to different hydrological regimes. The experimental design was made in randomized blocks with three repetitions in subdivided plots, with three varieties: RB855156, RB835486 and RB867515 (plot), and seven hydrological regimes corresponding to 0, 7, 17, 36, 46, 75 and 100% of ETc (subplot). The following production characteristics were evaluated: length of internode (LI), stem weight (SW), number of tillers per hectare and productivity; and technological components of the juice: ºBrix of the juice, Pol of the juice (sucrose levels), purity (PTY), RS (amount of reducing sugars of the juice), RSC (reducing sugars of the sugarcane plant), fibers, Pol of the sugarcane plant (CPP), total retrievable sugars (TRS) and the value of the sugarcane megagram (Mg). The maximum productivity of sugarcane was 182.31 Mg ha-1, which was registered with irrigation equivalent to 69.01% of ETc. The sugarcane cultivated with irrigation equivalent to 75% of ETc did not provide significant difference for the industrial quality of varieties tested in relation to the non-irrigated cultivation (0% of ETc). There was no significant difference for the values of reducing sugars (RS), the reducing sugars of the sugarcane plant (RSC) and the juice purity (PTY) among varieties.
