Moulds are considered central elements in daily exposure of poultry workers and can be the cause of an increased risk of occupational respiratory diseases, like allergic and non-allergic rhinitis and asthma. The objective is to evaluate the exposure to different species of moulds in poultries and relate them with respiratory symptoms in poultry workers. Seven Portuguese poultries were analyzed in order to assess air fungal contamination, as well as to evaluate the existence of clinical symptoms associated with asthma and other allergy diseases by European Community Respiratory Health Survey questionnaire.

OBJETIVO: Estimar a prevalência de ações de promoção, prevenção e cuidado da hipertensão arterial em adultos e identificar sua associação com estado descompensado de hipertensão. MÉTODOS: Estudo epidemiológico transversal de base populacional realizado por meio de entrevista com 12.324 adultos, de 20 a 59 anos, em 100 municípios brasileiros. As variáveis independentes, consideradas como promoção, prevenção e cuidado, foram: ter recebido orientações sobre a manutenção do peso ideal e sobre atividade física; ter consultado um médico e ter realizado eletrocardiograma no último ano. Pressão arterial acima de 140/90 mmHg foi considerada estado descompensado, sendo a variável dependente para a avaliação da qualidade do cuidado. RESULTADOS: Do total, 16,3% (n = 2.004) referiram diagnóstico médico de hipertensão. As maiores prevalências de hipertensão foram observadas na categoria de idade de 50 a 59 anos, concentradas nas regiões Sudeste e Centro-Oeste. Mais da metade (66,1%) esteve em consulta médica por hipertensão no último ano, da qual metade (52,4%) realizou eletrocardiograma. Dos hipertensos que tiveram sua pressão arterial aferida na entrevista (74,6%), menos da metade (42,4%) apresentava cifras tensionais descompensadas. CONCLUSÕES: Não houve associação entre haver consultado médico no último ano e cifras tensionais descompensadas. A proporção de hipertensos descompensados foi significativamente menor entre os que foram orientados para manter o peso ideal, realizar atividade física e os que fizeram eletrocardiograma. Ser do sexo masculino, ter idade acima de 40 anos e habitar na região Sul mostraram-se associados a estado descompensado da hipertensão.

A dynamical approach to study the behaviour of generalized populational growth models from Bets(p, 2) densities, with strong Allee effect, is presented. The dynamical analysis of the respective unimodal maps is performed using symbolic dynamics techniques. The complexity of the correspondent discrete dynamical systems is measured in terms of topological entropy. Different populational dynamics regimes are obtained when the intrinsic growth rates are modified: extinction, bistability, chaotic semistability and essential extinction.

The snail density, levels of infection and the monthly production of Schistosoma mansoni cercariae by Biomphalaria glabrata were determined in a focus of Barreiro de Baixo (Belo Horizonte, MG, Brazil). During a period of 38 months (1984 to 1987) 5,366 snails were collected of which 324 (6.03%) were infected with S. mansoni. The total number of cercariae shed was 5,667,312. Each snail shed an average of 17,422 cercariae during the time that it was under study in the laboratory. The greatest longevity of infected snails was 218 days. Natural cure was observed in 42 (12.9%) of the infected specimens about 130 days after collection. The average snail density in the focus during the period of study was 16.3 snails per scoop. The shedding of cercariae by snails collected from the field was compared with laboratory bred specimens infected in mass with the LE strain of S. mansoni from Belo Horizonte. The laboratory infected snails shed an average of 6,061 cercariae each, a value 2.8 times less than the field specimens due to a shorter life span. The prevalence of schistosomiasis in the focus was 14.3%.

From June to July 1999 an outbreak of acute respiratory illness occurred in the town of Iporanga. Out of a total of 4,837 inhabitants, 324 cases were notified to the Regional Surveillance Service. Influenza virus was isolated from 57.1% of the collected samples and 100% seroconversion to influenza A (H1N1) was obtained in 20 paired sera tested. The isolates were related to the A/Bayern/07/95 strain (H1N1). The percentages of cases notified during the outbreak were 28.4%, 29.0%, 20.7%, 6.2% and 15.7% in the age groups of 0-4, 5-9, 10-14, 15-19 and older than 20 years, respectively. The highest proportion of positives was observed among children younger than 14 years and no cases were notified in people older than 65 years, none of whom had been recently vaccinated against influenza. These findings suggest a significant vaccine protection against A/Bayern/7/95, the H1 component included in the 1997-98 influenza vaccine for elderly people. This viral strain is antigenically and genetically related to A/Beijing/262/95, the H1 component of the 1999 vaccine. Vaccines containing A/Beijing/262/95 (H1N1) stimulated post-immunization hemagglutination inhibition antibodies equivalent in frequency and titre to both A/Beijing/262/95-like and A/Bayern/7/95-like viruses. Thus, this investigation demonstrates the effectiveness of vaccination against influenza virus in the elderly.

INTRODUCTION: The definition of subclinical hypothyroidism (SH) is an asymptomatic state in which free thyroxine (T4) is normal and thyroid-stimulating hormone (TSH) levels are elevated. Its relationship with coronary disease is not clear and has been the subject of recent interest. Current evidence is conflicting and there is a lack of studies supported by coronary angiography. OBJECTIVE: To assess the relationship between SH and the presence and extent of coronary disease diagnosed by angiography. METHODS: We prospectively studied 354 consecutive patients referred for elective coronary angiography. Those with known thyroid disease, documented coronary disease or previous myocardial infarction were excluded. Fasting blood specimens were collected to measure thyroid hormones, lipid profile, high-sensitivity C-reactive protein, fibrinogen and NT-proBNP. Patients with SH were compared with those without to assess differences in clinical characteristics and biochemical and angiographic results. Significant coronary disease was defined as the presence of at least one lesion with > or = 50% luminal stenosis. Lesions with <50% stenosis were considered minimal. RESULTS: SH was diagnosed in 32 (9%) patients. Mean age was similar between the groups. There were more women (66% vs. 39%; p=0.003) and atrial fibrillation was more frequent (25% vs. 11%; p=0.016) in the group of patients with SH. There were no significant differences in the other baseline clinical parameters, and blood biochemistry results were similar in the two groups, with the exception of higher levels of NT-proBNP in SH patients, although without statistical significance. The angiographic results were as follows: significant coronary disease (SH 28.1% vs. non-SH 43.8%; p=0.087); three-vessel disease (9.4% vs. 9.9%; p=0.919); two-vessel disease (12.5% vs. 13.4%; p=0.892); single-vessel disease (6.3% vs. 29.5%; p=0.051); minimal lesions (9.4% vs. 10.9%; p=0.794); and no coronary disease (62.4% vs, 45.3%; p=0.064). CONCLUSION: In this population SH was not associated with the presence or extent of coronary disease diagnosed by coronary angiography.

Introdução: O Programa Nacional de Vacinação (PNV) é um programa universal e gratuito, sendo uma das suas características a acessibilidade sem qualquer tipo de barreira. Apesar do inquestionável êxito do PNV desde o seu início em 1965, poderão persistir assimetrias sociais na sua aplicação, com grupos populacionais com níveis de protecção inferiores ao desejado e risco de desenvolvimento de bolsas de susceptíveis,possibilitando a reemergência de doenças já controladas ou mesmo eliminadas no nosso país, situações que urge diagnosticar e prevenir. Objectivo: Avaliar o estado vacinal de crianças internadas numa enfermaria de Pediatria Geral e na Unidade de Infecciologia do Hospital Dona Estefânia, durante um ano, e detectar obstáculos à vacinação, quer relacionados com serviços de saúde, quer relacionados com as características sócio-demográficas da população. Adicionalmente, pretendeu-se avaliar a adesão a algumas vacinas não contempladas no PNV à data do estudo. Material e Métodos: Estudo transversal que decorreu entre Janeiro e Dezembro de 2004. Incluiu o preenchimento de um inquérito pelos pais e a análise dos dados do boletim de vacinas. As perguntas aos pais incluíam características sociais e a auto-avaliação da acessibilidade à vacinação no Centro de Saúde. Para este estudo definiu-se atraso vacinal como o não cumprimento da vacinação nas datas estabelecidas, independentemente da duração do atraso. Resultados e conclusões: Nos 324 inquéritos analisados, 90% das crianças apresentava o calendário vacinal actualizado. Os factores de risco associados ao incumprimento do PNV foram a raça negra, a etnia cigana, a baixa escolaridade dos pais e a ausência de seguimento médico. Das vacinas extra-PNV à data do estudo analisadas, a vacina contra Neisseria meningitidis C (NmC) foi administrada a 30% das crianças e a vacina conjugada heptavalente contra Streptococcus pneumoniae (Pn7) a 23%, sendo que 18% das crianças tinham ambas as vacinas. Estas vacinas foram administradas predominantemente às crianças de raça caucasiana (94%), com agregados familiares pequenos (79%), seguidas por pediatra (75%)e cujos pais tinham pelo menos o 9º ano de escolaridade. Apenas 2 % dos inquiridos classificaram a acessibilidade à vacinação no Centro de Saúde como difícil.

A dermatoscopia constitui uma técnica de diagnóstico não-invasiva, in vivo, que permite complementar a observação clínica de lesões cutâneas não-pigmentadas de etiologia diversa. Nos tumores cutâneos não-pigmentados, a dermatoscopia facilita a observação de estruturas vasculares, aumentando a acuidade no seu diagnóstico, distinguindo tumores melanocíticos e não-melanocíticos, benignos e malignos. Na parte II descreve-se a variabilidade da aplicação clínica da dermatoscopia em lesões cutâneas não-pigmentadas tumorais.

OBJECTIVE/BACKGROUND: The association between socioeconomic status (SES), presentation, and outcome after vascular surgery is largely unknown. This study aimed to determine the influence of SES on post-operative survival and severity of disease at presentation among vascular surgery patients in the Dutch setting of equal access to and provision of care. METHODS: Patients undergoing surgical treatment for peripheral artery disease (PAD), abdominal aortic aneurysm (AAA), or carotid artery stenosis between January 2003 and December 2011 were retrospectively included. The association between SES, quantified by household income, disease severity at presentation, and survival was studied using logistic and Cox regression analysis adjusted for demographics, and medical and behavioral risk factors. RESULTS: A total of 1,178 patients were included. Low income was associated with worse post-operative survival in the PAD cohort (n = 324, hazard ratio 1.05, 95% confidence interval [CI] 1.00-1.10, per 5,000 Euro decrease) and the AAA cohort (n = 440, quadratic relation, p = .01). AAA patients in the lowest income quartile were more likely to present with a ruptured aneurysm (odds ratio [OR] 2.12, 95% CI 1.08-4.17). Lowest income quartile PAD patients presented more frequently with symptoms of critical limb ischemia, although no significant association could be established (OR 2.02, 95% CI 0.96-4.26). CONCLUSIONS: The increased health hazards observed in this study are caused by patient related factors rather than differences in medical care, considering the equality of care provided by the study setting. Although the exact mechanism driving the association between SES and worse outcome remains elusive, consideration of SES as a risk factor in pre-operative decision making and focus on treatment of known SES related behavioral and psychosocial risk factors may improve the outcome of patients with vascular disease.

INTRODUCTION: In the past two decades members of the genus Enterococcus have emerged as important nosocomial pathogens worldwide. This study prospectively analyzed the distribution of species and trends in antimicrobial resistance among clinical isolates of enterococci in a Brazilian tertiary hospital from 2006-2009. METHODS: Enterococcal species were identified by conventional biochemical tests. The antimicrobial susceptibility profile was performed by disk diffusion in accordance with the Clinical and Laboratory Standards Institute (CLSI). A screening test for vancomycin was also performed. Minimal inhibitory concentration (MIC) for vancomycin was determined using the broth dilution method. Molecular assays were used to confirm speciation and genotype of vancomycin-resistant enterococci (VRE). RESULTS: A total of 324 non-repetitive enterococcal isolates were recovered, of which 87% were E. faecalis and 10.8% E. faecium. The incidence of E. faecium per 1,000 admissions increased significantly (p < 0.001) from 0.3 in 2006 to 2.3 in 2009. The VRE rate also increased over time from 2.5% to 15.5% (p < 0.001). All VRE expressed high-level resistance to vancomycin (MIC >256µg/ mL) and harbored vanA genes. The majority (89.5%) of VRE belonged to E. faecium species, which were characteristically resistant to ampicillin and quinolones. Overall, ampicillin resistance rate increased significantly from 2.5% to 21.4% from 2006-2009. Resistance rates for gentamicin, chloramphenicol, tetracycline, and erythromycin significantly decreased over time, although they remained high. Quinolones resistance rates were high and did not change significantly over time. CONCLUSIONS: The data obtained show a significant increasing trend in the incidence of E. faecium resistant to ampicillin and vancomycin.

Analisam a estrutura da floresta equatorial úmida próxima a Manaus, introduzindo a qualidade de fuste na. análise. Utilizam 8 amostras de 20m x 500m para indivíduos com DAP > 20cm, 24 subamostras de 10m x 100m para. indivíduos com DAP entre 5cm e 20cm e 120 subamostras de 2m x 10m para indivíduos com DAP < 5cm. Determinam valores mínimos de 2,6 e 22 amostras para composição florística é de 6,18 e 80 para a abundância em cada fração da floresta. Concluem que a floresta é muito heterogênea, com 324 espécies, 173 gêneros e 57 fiamílias e com grande diferença entre a regeneração e o povoamento adulto. Eschweilera odora, Corythophora alta., Protium apiculatum e Radlkoferella sp. são as espécies mais importantes. A floresta em geral apresenta a estrutura diamétrica regular esperada para florestas nativas devido às espécies do estrato inferior e sub-bosque.

OBJECTIVE: To determine the levels of total cholesterol in a significant sample of the Brazilian population. METHODS: Blood cholesterol was determined in 81.262 individuals > 18 years old (51% male, 44.7 ± 15.7 years), using Accutrend equipment, in the cities São Paulo, Campinas, Campos do Jordão, São José dos Campos, Santos, Santo André, Ribeirão Preto, Porto Alegre, Rio de Janeiro, Belo Horizonte, Curitiba, Brasília, Salvador and documented in the presence of other risk factors (RF) for coronary artery disease (CAD) (systemic hypertension, CAD in the family, smoking, and diabetes). Participants were classified according to sex, age, and the presence or absence of RF, respectively, as 0 RF, 1 RF and > 2 RF. The percentage of individuals with cholesterol > 200 mg/dL and > 240 mg/dL was evaluated. RESULTS: The prevalence of individuals with 0, 1, and > 2 risk factors was 30% (n = 24,589), 36% (n =29,324), and 34% (n = 27,349) respectively, (P=0.657), and the mean total cholesterol of the population was 199.0 ± 35.0 mg/dL. Cholesterol levels above 200 and 240 mg/dL were found, respectively, in 40% (n = 32,515) and 13% (10.942) of individuals. The greater the number of risk factors the higher the levels of cholesterol (P<0.0001) and the greater the proportion of individuals with cholesterol > 200 mg/dL (P=0.032). No difference existed in the proportion of individuals with cholesterol > 240 mg/dL (P=0.11). CONCLUSION: A great percentage of individuals with cholesterol levels above those recommended to prevent coronary artery disease was found.
