The clinical demand for a device to monitor Blood Pressure (BP) in ambulatory scenarios with minimal use of inflation cuffs is increasing. Based on the so-called Pulse Wave Velocity (PWV) principle, this paper introduces and evaluates a novel concept of BP monitor that can be fully integrated within a chest sensor. After a preliminary calibration, the sensor provides non-occlusive beat-by-beat estimations of Mean Arterial Pressure (MAP) by measuring the Pulse Transit Time (PTT) of arterial pressure pulses travelling from the ascending aorta towards the subcutaneous vasculature of the chest. In a cohort of 15 healthy male subjects, a total of 462 simultaneous readings consisting of reference MAP and chest PTT were acquired. Each subject was recorded at three different days: D, D+3 and D+14. Overall, the implemented protocol induced MAP values to range from 80 ± 6 mmHg in baseline, to 107 ± 9 mmHg during isometric handgrip maneuvers. Agreement between reference and chest-sensor MAP values was tested by using intraclass correlation coefficient (ICC = 0.78) and Bland-Altman analysis (mean error = 0.7 mmHg, standard deviation = 5.1 mmHg). The cumulative percentage of MAP values provided by the chest sensor falling within a range of ±5 mmHg compared to reference MAP readings was of 70%, within ±10 mmHg was of 91%, and within ±15mmHg was of 98%. These results point at the fact that the chest sensor complies with the British Hypertension Society (BHS) requirements of Grade A BP monitors, when applied to MAP readings. Grade A performance was maintained even two weeks after having performed the initial subject-dependent calibration. In conclusion, this paper introduces a sensor and a calibration strategy to perform MAP measurements at the chest. The encouraging performance of the presented technique paves the way towards an ambulatory-compliant, continuous and non-occlusive BP monitoring system.

Dans les années trente, Ella Maillart et Annemarie Schwarzenbach quittent la Suisse pour l'Afghanistan. Deux décennies plus tard, L. Pestelli et N. Bouvier s'embarquent à leur tour sur les routes d'Orient. Filles et fils de grands industriels, d'universitaires ou de diplomates, ces quatre écrivains-voyageurs mettent un point d'honneur à s'éloigner d'une conception bourgeoise du voyage en présentant leur départ comme un moyen de se définir dans l'ailleurs, c'est-à-dire en dehors de leur héritage social, familial, national et occidental. L'Orient leur semble le lieu des possibles. Or, malgré leur désir de table rase, ils s'aperçoivent vite que, quoi qu'ils fassent, leur corps porte les stigmates de l'Occident. Ils vont donc tenter de le modifier. Si le but du voyage n'est pas de faire totalement disparaître le corps, ce n'est qu'en le risquant, en l'offrant au monde, pour le meilleur et souvent pour le pire, que les voyageurs croient pouvoir goûter aux délices du dehors. Mais ce bonheur charnel, physique, sensuel voire érotique, comment le dire ? Et quelle langue adopter pour rendre compte de sa présence physique au monde ? La réunion de ces quatre auteurs d'époques, de genres et de plumes différents, nous permet d'observer l'évolution des représentations du corps dans le récit de voyage au vingtième siècle tout en questionnant nos habitudes de lecture. Quelle représentation attendons-nous du corps dans un récit de voyage ? Celui-ci est-il vraiment le lieu privilégié pour remettre le corps à l'ouvrage ? The Boby at work. Body representations ìn the narratives of Eila Maillart, Annemarie Schwarzenbach, Nicolas Bouvier and Lorenzo Pestelli. In the 1930s, Ella Maillart and Annemarie Schwarzenbach left Switzerland for Afghanistan. Two decades later, Lorenzo Pestelli and Nicolas Bouvier set out on the routes to the East. Daughters and sons of prominent industrialists, academics or diplomats, these four writer-travellers made a point of straying away from the bourgeois conception of travel by presenting their departure as a way of defining the self away from social, family, national and Western inheritance. The East appears to them as the location of many possibilities. Yet, in spite of their desire for a clean slate, they soon realise that, no matter what they do, their body carries the stigma of the West. They will thus try to modify it. If the aim of their travelling is not to make the body disappear completely, it is only by putting it at risk and by offering it to the world, for better and often for worse, that the travellers believe they can taste the delights from the outside. But how to put in words this carnal, physical, sensual and even erotic pleasure? And what language can be chosen to account for one's presence in the world? Working jointly on four writers, from different eras, genres and styles, helps us to observe the evolution of the representations of the body in travel literature in the 20`h century and at the same time it questions our reading habits. What representations of the body do we expect in travel literature? Is travel literature really the privileged location to put the body back to work?

Testimonies disclosed that a 44-year-old pedestrian was struck head-on by a truck while she was roaming on the motorway; at the time of collision, the truck was travelling at a speed of about 90 km/h. In the second phase of the collision, the pedestrian was projected about 100 m before her body was run over by the truck and then by a car. The autopsy revealed extensive mutilations, making it impossible to verify the testimonies of witnesses to the collision as regards the pedestrian's position at the moment of the first impact. However, the reports produced by the technical expert and the forensic pathologist were able to confirm the testimonies, based on an impact zone on the front panel of the cab of the truck, where part of the pedestrian's face was reproduced like a "modern holy shroud".

Amb l’objectiu d’analitzar i comprendre d’una manera més adequada els mecanismes pels quals els potencials turistes que venen d’Anglaterra prenen la decisió de viatjar fins la Costa Daurada, en el context de la introducció de les línies aèries de baix cost a Espanya i la creació de noves rutes, s’ha dut a terme una enquesta a l’aeroport de Reus a un total de 445 turistes que estaven a punt de tornar de les seves vacances cap a Anglaterra tenint l'aeroport d'East Midlands (EMA) com a destinació i viatjant amb la companyia aèria de baix cost RyanAir. Amb el mètode utilitzat en aquest projecte s'ha mesurat, per mitjà de l'enquesta esmentada, el pes de la causalitat entre la presència de vols barats des de EMA fins a Reus amb la decisió dels turistes de passar les seves vacances en una regió turística de primer ordre a Catalunya com és la Costa Daurada. Segons els resultats de les dues preguntes causals que formen part de l’enquesta (pregunta 18 i pregunta 19), gairebé un 30% de l'impacte econòmic turístic total és degut a la disponibilitat de vols barats mitjançant RyanAir. Tanmateix, al voltant d’un 60% de l'impacte estaria relacionat amb el fet que els turistes visitarien la Costa Daurada independentment de la disponibilitat de vols de RyanAir. Això indicaria que tot i que els vols de Ryanair són innegablement atractius pels turistes, hi ha un desig important de visitar la Costa Daurada que podria ser satisfet per altres mitjans.

Résumé La mobilité ne signifie plus uniquement se mouvoir d'un point à un autre ; il s'agit d'un concept lui-même en constante évolution, grâce au progrès technique et à l'innovation sociale notamment. Aujourd'hui, la recherche de la vitesse n'est plus le seul enjeu au coeur de nos préoccupations. Elle a été remplacée par un retour au voyage enrichi par l'expérience et ce quelle que soit sa durée. Cet enrichissement s'est principalement fait par le truchement des technologies de l'information et de la communication et peut prendre plusieurs formes liées aux problématiques contemporaines de la ville et du territoire. Citons comme exemple la valorisation du temps de déplacement, grâce à un meilleur accès à l'information (travail, réseaux sociaux, etc.) et à la recherche d'une plus grande cohérence entre l'acte de se mouvoir et l'environnement proche ou lointain. Cette « recontextualisation » du mouvement nous interpelle dans notre rapport à l'espace et nous donne également des pistes pour repenser le métier d'urbaniste de la ville intelligente. Abstract Mobility issues do not only involve the act of moving nowadays. The concept itself evolves continuously thanks to technological and social innovations. The main stakes do not focus anymore on improving speed, but on enriching the experience of travelling, even in the case of short trips. One of the main factors that fosters this evolution is the progressive adoption of information and communication technologies that help to reshape the issues of contemporary cities. For example, the quality of travel time has improved thanks to the ubiquitous accessibility to information, and by offering a better coherence between the trip and the immediate social environment. The "recontextualisation" of everyday activities (working, interacting, etc.) challenges the relationship individuals have with space and offers many clues in regard to the required skills that urban planners and designers of the smart city should possess.

Background. New recommendations for rabies postexposure prophylaxis (PEP) were published by the Centers for Disease Control and Prevention and the World Health Organization in 2010. In view of these new recommendations, we investigated the adequacy of rabies PEP among patients consulting our travel clinic. Methods. A retrospective analysis of the files of all patients who consulted for rabies PEP at the Travel Clinic of the University Hospital in Lausanne, Switzerland, between January 2005 and August 2011 was conducted. Results. A total of 110 patients who received rabies PEP were identified. The median age of the patients was 34 years (range, 2-79 years), and 53% were women. Ninety subjects were potentially exposed to rabies while travelling abroad. Shortcomings in the management of these patients were (1) late initiation of rabies PEP in travelers who waited to seek medical care until returning to Switzerland, (2) administration of human rabies immunoglobulin (HRIG) to only 7 of 50 travelers (14%) who sought care abroad and for whom HRIG was indicated, and (3) antibody levels <0.5 IU/mL in 6 of 90 patients (6.7%) after 4 doses of vaccine. Conclusions. Patients do not always receive optimal rabies PEP under real-life conditions. A significant proportion of patients did not develop adequate antibody levels after 4 doses of vaccine. These data indicate that the measurement of antibody levels on day 21 of the Essen PEP regimen is useful in order to verify an adequate immune response.

Actualment internet és un gran aparador i des de fa temps, les empreses han vist en aquest una manera de treure’n profit. És per això, junt amb el seu baix cost, que fa que qualsevol empresa disposi del seu propi espai a la xarxa. D’aquestes idees sorgeix el projecte de fer la web per AutoSuministres Motor. La principal funció de la web es donar-se a conèixer i ensenyar els seus productes, que en aquest cas, són autocaravanes i caravanes. Tot i així, la pàgina web contindrà un espai de reportatges on l’usuari podrà conèixer més a fons un model concret d’autocaravana o caravana i on també podrà demanar més informació mitjançant un formulari de contacte. Per tant, la pàgina web serà totalment administrable perquè els comercials puguin introduir els vehicles corresponents i l’usuari final pugui consultarlos correctament d’una manera fàcil i senzilla. Per tal de dur a terme aquesta web s’ha tingut en compte utilitzar eines de programació de distribució lliure com és el llenguatge PHP, la base de dades MySQL i jQuery.

BACKGROUND: Studies carried out on specific travelers' groups such as students describe an increase in the consumption of alcohol and drugs during travel and vacation time. The present study investigates the risk behaviors (alcohol and drugs) in a general adult population in Switzerland travelling abroad who visited a travel clinic before departure. METHODS: This retrospective study was conducted in a travel clinic between January 2006 and December 2008. 14,496 patients came to the clinic for a pre-travel consultation. 3,537 of them answered a questionnaire about their life habits in Switzerland and during their last trip. The only exclusion criterion was an age inferior to 18 years old.The consumption habits of drugs and at-risk alcohol intake (8 standard drinks (SD) per week for women and 15 SD for men) was analyzed according to gender, sex, destination and profession. Predictors of adopting a risky behavior between habits in Switzerland and during their previous trip were also analyzed. RESULTS: 7% (229/3477) of participants declared having at-risk alcohol consumption in Switzerland and 14% (473/3275 [95% CI 13-16]) during their trip. 9% (332/3527) of the participants used drugs in Switzerland and 5% (178/3481) during their trip. Risk factors for at-risk alcohol consumption during a trip were: at-risk alcohol consumption in Switzerland (OR 31[95% CI 21-45]), smoking (1.7 [95% CI 1-2]), use of drugs in Switzerland (OR 2.2 [95% CI 2-3]), leisure travel (OR 1.6 [95% CI 1-2]) and managerial professions (OR 1.8 [95% CI 1-3]). Risk factors for the use of drugs during a trip were: alcohol consumption in Switzerland (OR 2.1 [95% CI 1-4]), smoking (OR 1.9 [95% CI 1-3]), and use of drugs in Switzerland (OR 29.7 [95% CI 19-45]). CONCLUSIONS: At-risk alcohol consumption and, to a lesser extent, use of drugs, affect a large number of travelers which expose them to health problems during a trip. Exploring the alcohol and drugs consumption patterns of people visiting a travel clinic should be part of the pre-travel routine consultation and would allow to identifying people who would benefit most from a specific prevention.

The aim of this article is to prove the real possibility of travelling intellectually to the Platonic image of the cave from different films. In this sense, one can speak of explicit references as in The Conformist by B. Bertolucci or in Shadowlands by R. Attenborough -if one bears in mind the Chronicles of Narnia by C. S. Lewis- or The Picture of Dorian Gray ¿if one bears in mind the well-known O. Wilde¿s novel-, but, on other occasions, although the Platonic influence cannot be proved, for instance in The Truman Show, A Room with a View or Brideshead Revisited, one can perfectly think of these films in order to guide the contemporary audiences to that Platonic image, since Plato himself affirms that it deals with an image which can be easily applied and, in first place, to his idealistic philosophy.

In coordination with a Technical Advisory Committee (TAC) consisting of County Engineers and Iowa DOT representatives, the Iowa DOT has proposed to develop a set of standards for a single span prefabricated bridge system for use on the local road system. The purpose of the bridge system is to improve bridge construction, accelerate project delivery, improve worker safety, be cost effective, reduce impacts to the travelling public by reducing traffic disruptions and the duration of detours, and allow local forces to construct the bridges. HDR Inc. was selected by the Iowa DOT to perform the initial concept screening of the bridge system. This Final Report summarizes the initial conceptual effort to investigate potential systems, make recommendations for a preferred system and propose initial details to be tested in the laboratory in Phase 2 of the project. The prefabricated bridge components were to be based on the following preliminary criteria set forth by the TAC. The criteria were to be verified and/ or modified as part of the conceptual development. - 24’ and 30’ roadway widths - Skews of 0o, 15o, and 30o - Span lengths of 30’ – 70’ in 10’ increments using precast concrete beams - Voided box beams could be considered - Limit precast element weight to 45,000 pounds for movement and placement of beams - Beams could be joined transversely with threaded rods - Abutment concepts may included precast as well as an option for cast-in-place abutments with pile foundations In addition to the above criteria, there was an interest to use a single-width prefabricated bridge component to simplify fabrication as well as a desire to utilize non-prestressed concrete systems where possible to allow for precasting of the beam modules by local forces or local precast plants. The SL-1 modular steel bridge rail was identified for use with this single span prefabricated bridge system.

Background: Anterior ciliary arteries travelling along recti muscles provide anterior segment vascularization, which can be compromised by surgery involving more than 2 muscles.Patients and Methods: We studied retrospectively the files of 10 patients in whom a fluorescein angiography of the iris had been performed as a pre-operative assessment prior to a second or third oculomotor surgery.Results: The median age of the patients was 47.5 years (range 15 to 73 years). Relative iris ischemia was present in 4 patients following multiple surgeries, none of them presenting any general cardiovascular risk. The initial surgical protocol was modified according to angiographic results in these 4 patients.Conclusion: When further surgery has to be performed on previously multi-operated patients, anterior segment angiography can be useful in the planning of surgery in order to minimize the risks of anterior segment ischemia.

The goal of this research project was to develop a method to measure the performance of a winter maintenance program with respect to the task of providing safety and mobility to the travelling public. Developing these measures required a number of steps, each of which was accomplished. First, the impact of winter weather on safety (crash rates) and mobility (average vehicle speeds were measured by a combination of literature reviews and analysis of Iowa Department of Transportation traffic and Road Weather Information System data. Second, because not all winter storms are the same in their effects on safety and mobility, a method had to be developed to determine how much the various factors that describe a winter storm actually change safety and mobility. As part of this effort a storm severity index was developed, which ranks each winter storm on a scale between 0 (a very benign storm) and 1 (the worst imaginable storm). Additionally a number of methods of modeling the relationships between weather, winter maintenance actions and road surface conditions were developed and tested. The end result of this study was a performance measure based on average vehicle speed. For a given class of road, a maximum expected average speed reduction has been identified. For a given storm, this maximum expected average speed reduction is modified by the storm severity index to give a target average speed reduction. Thus, if for a given road the maximum expected average speed reduction is 20 mph, and the storm severity for a particular storm is 0.6, then the target average speed reduction for that road in that storm is 0.6 x 20 mph or 12 mph. If the average speed on that road during and after the storm is only 12 mph or less than the average speed on that road in good weather conditions, then the winter maintenance performance goal has been met.

Introduction: As part of the roadside development along the Interstate Highway System, the Iowa State Highway Commission has constructed eight pair of rest area facilities. Furthermore, two pair are presently under construction with an additional two pair proposed for letting in 1967. An additional nine and one-half pairs of rest areas are in the planning phase, a grand total of 45 rest Brea buildings. The facilities existing were planned and designed in a relatively short period of time. The rest area facilities are unusual in terms of water use, water demand rates, and the fact that there are no applicable guidelines from previous installations. Such facilities are a pioneering effort to furnish a service -which the travelling public desires and will use. The acceptance and current use of the existing facilities shows that the rest areas do provide a service the public will use and appreciate. The Iowa State Highway Commission is to be congratulated for this· pioneering effort. However there are problems, as should be expected when design of a new type of facility has no past operating experience to use as a guide. Another factor which enters is that a rest area facility is quite different and rather unrelated to engineering in the highway field of practice. Basically, the problems encountered can be resolved into several areas, namely 1) maintenance problems in equipment due to 2) insufficient capacity of several other elements of the water systems, and 3) no provisions for water quality control. This study and report is supposed to essentially cover the review of the rest areas, either existing and under construction or letting. However, the approach used has been somewhat different. Several basic economically feasible water system schemes have been developed which are· adaptable to the different well capacities and different water qualities encountered. These basic designs are used as a guide in recommending modifications to the existing rest area water systems, anticipating that the basic designs will be used for future facilities. The magnitude of the problems involved is shown by the fact that the projected water use and demand variations of each rest area building is equivalent to the water supply for a community of about 100 people. The problems of proper operation and maintenance of an eventual thirty to forty-five such facilities are gigantic. For successful operation the rest area water systems must have a high degree of standardization and interchangeability of all elements of the water systems, even if it means a limited degree of over-design in some rest area facilities.

New methods and devices for pursuing performance enhancement through altitude training were developed in Scandinavia and the USA in the early 1990s. At present, several forms of hypoxic training and/or altitude exposure exist: traditional 'live high-train high' (LHTH), contemporary 'live high-train low' (LHTL), intermittent hypoxic exposure during rest (IHE) and intermittent hypoxic exposure during continuous session (IHT). Although substantial differences exist between these methods of hypoxic training and/or exposure, all have the same goal: to induce an improvement in athletic performance at sea level. They are also used for preparation for competition at altitude and/or for the acclimatization of mountaineers. The underlying mechanisms behind the effects of hypoxic training are widely debated. Although the popular view is that altitude training may lead to an increase in haematological capacity, this may not be the main, or the only, factor involved in the improvement of performance. Other central (such as ventilatory, haemodynamic or neural adaptation) or peripheral (such as muscle buffering capacity or economy) factors play an important role. LHTL was shown to be an efficient method. The optimal altitude for living high has been defined as being 2200-2500 m to provide an optimal erythropoietic effect and up to 3100 m for non-haematological parameters. The optimal duration at altitude appears to be 4 weeks for inducing accelerated erythropoiesis whereas <3 weeks (i.e. 18 days) are long enough for beneficial changes in economy, muscle buffering capacity, the hypoxic ventilatory response or Na(+)/K(+)-ATPase activity. One critical point is the daily dose of altitude. A natural altitude of 2500 m for 20-22 h/day (in fact, travelling down to the valley only for training) appears sufficient to increase erythropoiesis and improve sea-level performance. 'Longer is better' as regards haematological changes since additional benefits have been shown as hypoxic exposure increases beyond 16 h/day. The minimum daily dose for stimulating erythropoiesis seems to be 12 h/day. For non-haematological changes, the implementation of a much shorter duration of exposure seems possible. Athletes could take advantage of IHT, which seems more beneficial than IHE in performance enhancement. The intensity of hypoxic exercise might play a role on adaptations at the molecular level in skeletal muscle tissue. There is clear evidence that intense exercise at high altitude stimulates to a greater extent muscle adaptations for both aerobic and anaerobic exercises and limits the decrease in power. So although IHT induces no increase in VO(2max) due to the low 'altitude dose', improvement in athletic performance is likely to happen with high-intensity exercise (i.e. above the ventilatory threshold) due to an increase in mitochondrial efficiency and pH/lactate regulation. We propose a new combination of hypoxic method (which we suggest naming Living High-Training Low and High, interspersed; LHTLHi) combining LHTL (five nights at 3000 m and two nights at sea level) with training at sea level except for a few (2.3 per week) IHT sessions of supra-threshold training. This review also provides a rationale on how to combine the different hypoxic methods and suggests advances in both their implementation and their periodization during the yearly training programme of athletes competing in endurance, glycolytic or intermittent sports.

Matkailualan palvelujen kehitystä on pitkään hidastanut standardien ja yhtenäisten toimintatapojen puute. Tässä diplomityössä tarkastellaan standardien merkitystä teknisesti kehittyneille matkailupalveluille ja esitellään TravelGate Finland -hankkeen tuloksena syntynyttä mobiilisovellusta. Kehittyneiden matkaviestinverkkojen ja älypuhelimien ansiosta yhä useammilla ihmisillä on mahdollisuus käyttää matkustaessaan erilaisia mobiilipalveluja. Matkailualan yritysten tarjoamia palveluita ei kuitenkaan ole juurikaan saatavilla, koska niiden kehittäminen on standardien puuttumisen takia hankalaa ja kallista. TravelGate Finland -hankkeen tuloksena matkailualan käyttöön määriteltiin uusi rajapintakirjasto, jonka avulla uusien Internet- ja mobiilipalvelujen kehittäminen helpottuu. Uusien rajapintojen ja toimintatapojen avulla toteutettiin Symbian-älypuhelimissa toimiva mobiilisovellus, jonka avulla matkailijat voivat etsiä heitä lähellä olevien matkailukohteiden sijainnin ja perustiedot veloituksetta.
