Schizophrenia is an idiopathic mental disorder with a heritable component and a substantial public health impact. We conducted a multi-stage genome-wide association study (GWAS) for schizophrenia beginning with a Swedish national sample (5,001 cases and 6,243 controls) followed by meta-Analysis with previous schizophrenia GWAS (8,832 cases and 12,067 controls) and finally by replication of SNPs in 168 genomic regions in independent samples (7,413 cases, 19,762 controls and 581 parent-offspring trios). We identified 22 loci associated at genome-wide significance; 13 of these are new, and 1 was previously implicated in bipolar disorder. Examination of candidate genes at these loci suggests the involvement of neuronal calcium signaling. We estimate that 8,300 independent, mostly common SNPs (95% credible interval of 6,300-10,200 SNPs) contribute to risk for schizophrenia and that these collectively account for at least 32% of the variance in liability. Common genetic variation has an important role in the etiology of schizophrenia, and larger studies will allow more detailed understanding of this disorder.

Psychotic-like experiences (PLEs) have been associated with increased risk of suicidality, but it is unclear whether the level of risk varies with different types of PLE. A cross-sectional online survey was completed by 1610 university students. Respondents completed the Community Assessment of Psychic Experiences-15 (CAPE-P15) assessing PLEs on three subscales: Perceptual Abnormalities (PA), Persecutory Ideation (PI) and Bizarre Experiences (BE). Lifetime suicidal ideation, plans and attempts, cannabis, ecstasy and methamphetamine use and family history of mental disorder were also assessed. Multinomal logistic regression was used to examine unique determinants of lifetime suicidality, defined as any history of (i) suicidal ideation or plans and (ii) any attempt, relative to no lifetime history of suicidality. A lifetime history of PA and PI provided significant unique contributions to the prediction of suicide risk, after control for other significant predictors. BE were not associated with any suicide variable demonstrating the variation in risk of suicidality with different types of PLEs. Perceptual abnormalities and persecutory ideation as measured by the CAPE-P15 are the PLEs associated with a higher risk of lifetime suicidality.

A quarter of Australia’s sunflower production is from the central highlands region of Queensland and is currently worth six million dollars ($AUD) annually. From the early 2000s a severe necrosis disorder of unknown aetiology was affecting large areas of sunflower crops in central Queensland, leading to annual losses of up to 20%. Other crops such as mung bean and cotton were also affected. This PhD study was undertaken to determine if the causal agent of the necrosis disorder was of viral origin and, if so, to characterise its genetic diversity, biology and disease cycle, and to develop effective control strategies. The research described in this thesis identified Tobacco streak virus (TSV; genus Ilarvirus, family Bromoviridae) as the causal agent of the previously unidentified necrosis disorder of sunflower in central Queensland. TSV was also the cause of commonly found diseases in a range of other crops in the same region including cotton, chickpea and mung bean. This was the first report from Australia of natural field infections of TSV from these four crops. TSV strains have previously been reported from other regions of Australia in several hosts based on serological and host range studies. In order to determine the relatedness of previously reported TSV strains with TSV from central Queensland, we characterised the genetic diversity of the known TSV strains from Australia. We identified two genetically distinct TSV strains from central Queensland and named them based on their major alternative hosts, TSV-parthenium from Parthenium hysterophorus and TSV-crownbeard from Verbesina encelioides. They share only 81 % total-genome nucleotide sequence identity. In addition to TSV-parthenium and TSV-crownbeard from central Queensland, we also described the complete genomes of two other ilarvirus species. This proved that previously reported TSV strains, TSV-S isolated from strawberry and TSV-Ag from Ageratum houstonianum, were actually the first record of Strawberry necrotic shock virus from Australia, and a new subgroup 1 ilarvirus, Ageratum latent virus. Our results confirmed that the TSV strains found in central Queensland were not related to previously described strains from Australia and may represent new incursions. This is the first report of the genetic diversity within subgroup 1 ilarviruses from Australia. Based on field observations we hypothesised that parthenium and crownbeard were acting as symptomless hosts of TSV-parthenium and TSV-crownbeard, respectively. We developed strain-specific multiplex PCRs for the three RNA segments to accurately characterise the range of naturally infected hosts across central Queensland. Results described in this thesis show compelling evidence that parthenium and crownbeard are the major (symptomless) alternative hosts of TSV-parthenium and TSV-crownbeard. While both TSV strains had wide natural host ranges, the geographical distribution of each strain was closely associated with the respective distribution of their major alternative hosts. Both TSV strains were commonly found across large areas of central Queensland, but we only found strong evidence for the TSV-parthenium strain being associated with major disease outbreaks in nearby crops. The findings from this study demonstrate that both TSV-parthenium and TSV-crownbeard have similar life cycles but some critical differences. We found both TSV strains to be highly seed transmitted from their respective major alternative hosts from naturally infected mother plants and survived in seed for more than 2 years. We conclusively demonstrated that both TSV strains were readily transmitted via virus-infected pollen taken from the major alternative hosts. This transmission was facilitated by the most commonly collected thrips species, Frankliniella schultzei and Microcephalothrips abdominalis. These results illustrate the importance of seed transmission and efficient thrips vector species for the effective survival of these TSV strains in an often harsh environment and enables the rapid development of TSV disease epidemics in surrounding crops. Results from field surveys and inoculation tests indicate that parthenium is a poor host of TSV-crownbeard. By contrast, crownbeard was naturally infected by, and an experimental host of TSV-parthenium. However, this infection combination resulted in non-viable crownbeard seed. These differences appear to be an effective biological barrier that largely restricts these two TSV strains to their respective major alternative hosts. Based on our field observations we hypothesised that there were differences in relative tolerance to TSV infection between different sunflower hybrids and that seasonal variation in disease levels was related to rainfall in the critical early crop stage. Results from our field trials conducted over multiple years conclusively demonstrated significant differences in tolerance to natural infections of TSV-parthenium in a wide range of sunflower hybrids. Glasshouse tests indicate the resistance to TSV-parthenium identified in the sunflower hybrids is also likely to be effective against TSV-crownbeard. We found a significant negative association between TSV disease incidence in sunflowers and accumulated rainfall in the months of March and April with increasing rainfall resulting in reduced levels of disease. Our results indicate that the use of tolerant sunflower germplasm will be a critical strategy to minimise the risk of TSV epidemics in sunflower.

Meckel syndrome (MKS, MIM 249000) is a severe developmental disorder that leads to death already in utero or shortly after birth. MKS diagnosis can be established by a careful ultrasound examination already at 11-14 weeks of gestation. The main features of MKS are occipital meningoencephalocele, cystic kidney dysplasia and fibrotic changes of the liver. In addition, polydactyly is frequently reported in the cases. The aim of the study was to characterize the molecular and functional defects in MKS. In this study we were able to identify two major MKS mutations in Finnish population, which cover over 90% of the cases. The first mutation is a 29 bp intronic deletion in the MKS1 gene (c.1483-7_35del) that is found in 70% of the families and the second is a C>T substitution in the coding region of CC2D2A (c.1762C>T), that is found in 20% of the MKS families. Both of these mutations result in abnormal splicing. The discovery of the disease genes has revealed that MKS is caused by primary cilia dysfunction. MKS1 gene has a conserved B9 domain, and it is found in the predicted ciliary proteome. CC2D2A protein is also found in the predicted ciliary proteome and it has a Ca2+ binding domain. The number of genes behind MKS has increased rapidly in the past years and to date, mutations have been identified in five genes (MKS1, TMEM67/MKS3, CEP290/MKS4, RPGRIP1L/MKS5 and CC2D2A/MKS6). Identification of the disease genes mutations has also revealed that MKS is an allelic disorder with other syndromes with overlapping phenotypes. Disorders that are caused by primary cilia dysfunction are collectively known as ciliopathies. Sequence analysis of all the known MKS genes in Finnish and non-Finnish families available to us, where the mutation was still unknown, revealed mutations in 14 out of the 30 families included in the study. When we collected all the reported mutations in MKS genes in different syndromes we could see that there was clearly a genotype-syndrome correlation between the mutations and the syndromes, since the same pair of mutations has never been reported in different syndromes. The basic molecular events behind MKS will not only give us information of this syndrome, but also significant novel information on early fetal development in general.

There is growing evidence, especially in the USA and UK, that creative writing can form an important part of the recovery experience of people affected by severe mental illness. In this chapter, I consider theoretical models that explain how creative writing might contribute to recovery, and discuss the potential for creative writing in psychosocial rehabilitation. It is argued that the rehabilitation benefits of creative writing might be optimized through focus on process and technique in writing, rather than expression or content alone, and that consequently, the involvement of professional writers might be important. I will explore the recent history of theoretical frameworks and explanatory models that link creative writing and recovery, and examine such empirical evidence as is available on the contribution of creative writing to recovery from severe mental illness.

This study is part of the Mood Disorders Project conducted by the Department of Mental Health and Alcohol Research, National Public Health Institute, and consists of a general population survey sample and a major depressive disorder (MDD) patient cohort from Vantaa Depression Study (VDS). The general population survey study was conducted in 2003 in the cities of Espoo and Vantaa. The VDS is a collaborative depression research project between the Department of Mental Health and Alcohol Research of the National Public Health Institute and the Department of Psychiatry of the Peijas Medical Care District (PMCD) beginning in 1997. It is a prospective, naturalistic cohort study of 269 secondary-level care psychiatric out- and inpatients with a new episode of Diagnostic and Statistical Manual of Mental Disorders, 4th edition (DSM-IV) MDD. In the general population survey study, a total of 900 participants (300 from Espoo, 600 from Vantaa) aged 20 70 years were randomly drawn from the Population Register Centre in Finland. A self-report booklet, including the Eysenck Personality Inventory (EPI), the Temperament and Character Inventory Revised (TCI-R), the Beck Depression Inventory and the Beck Anxiety Inventory was mailed to all subjects. Altogether 441 participants responded (94 returned only the shortened version without TCI-R) and gave their informed consent. VDS involved screening all patients aged 20-60 years (n=806) in the PMCD for a possible new episode of DSM-IV MDD. 542 consenting patients were interviewed with a semi-structured interview (the WHO Schedules for Clinical Assessment in Neuropsychiatry, version 2.0). 269 patients with a current DSM-IV MDD were included in the study and further interviewed with semi-structured interviews to assess all other axis I and II psychiatric diagnoses. Exclusion criteria were DSM-IV bipolar I and II, schizoaffective disorder, schizophrenia or another psychosis, organic and substance-induced mood disorders. In the present study are included those 193 (139 females, 54 males) individuals who could be followed up at both 6 and 18 months, and their depression had remained unipolar. Personality was investigated with the EPI. Personality dimensions associated not only to the symptoms of depression, but also to the symptoms of anxiety among general population and in depressive patients, as well as to comorbid disorders in MDD patients, supporting the dimensional view of depression and anxiety. Among the general population High Harm Avoidance and low Self-Directedness associated moderately, whereas low extraversion and high neuroticism strongly with the depressive and anxiety symptoms. The personality dimensions, especially high Harm Avoidance, low Self-Directedness and high neuroticism were also somewhat predictive of self-reported use of health care services for psychiatric reasons, and lifetime mental disorder. Moreover, high Harm Avoidance associated with a family history of mental disorder. In depressive patients, neuroticism scores were found to decline markedly and extraversion scores to increase somewhat with recovery. The predictive value of the changes in symptoms of depression and anxiety in explaining follow-up neuroticism was about 1/3 of that of baseline neuroticism. In contrast to neuroticism, the scores of extraversion showed no dependence on the symptoms of anxiety, and the change in the symptoms of depression explained only 1/20 of the follow-up extraversion compared with baseline extraversion. No evidence was found of the scar effect during a one-year follow-up period. Finally, even after controlling for symptoms of both depression and anxiety, depressive patients had a somewhat higher level of neuroticism (odds ratio 1.11, p=0.001) and a slightly lower level of extraversion (odds ratio 0.92, p=0.003) than subjects in the general population. Among MDD patients, a positive dose-exposure relationship appeared to exist between neuroticism and prevalence and number of comorbid axis I and II disorders. A negative relationship existed between level of extraversion and prevalence of comorbid social phobia and cluster C personality disorders. Personality dimensions are associated with the symptoms of depression and anxiety. Futhermore these findings support the hypothesis that high neuroticism and somewhat low extraversion might be vulnerability factors for MDD, and that high neuroticism and low extraversion predispose to comorbid axis I and II disorders among patients with MDD.

Background: Opiod dependence is a chronic severe brain disorder associated with enormous health and social problems. The relapse back to opioid abuse is very high especially in early abstinence, but neuropsychological and neurophysiological deficits during opioid abuse or soon after cessation of opioids are scarcely investigated. Also the structural brain changes and their correlations with the length of opioid abuse or abuse onset age are not known. In this study the cognitive functions, neural basis of cognitive dysfunction, and brain structural changes was studied in opioid-dependent patients and in age and sex matched healthy controls. Materials and methods: All subjects participating in the study, 23 opioid dependents of whom, 15 were also benzodiazepine and five cannabis co-dependent and 18 healthy age and sex matched controls went through Structured Clinical Interviews (SCID) to obtain DSM-IV axis I and II diagnosis and to exclude psychiatric illness not related to opioid dependence or personality disorders. Simultaneous magnetoencephalography (MEG) and electroencephalography (EEG) measurements were done on 21 opioid-dependent individuals on the day of hospitalization for withdrawal therapy. The neural basis of auditory processing was studied and pre-attentive attention and sensory memory were investigated. During the withdrawal 15 opioid-dependent patients participated in neuropsychological tests, measuring fluid intelligence, attention and working memory, verbal and visual memory, and executive functions. Fifteen healthy subjects served as controls for the MEG-EEG measurements and neuropsychological assessment. The brain magnetic resonance imaging (MRI) was obtained from 17 patients after approximately two weeks abstinence, and from 17 controls. The areas of different brain structures and the absolute and relative volumes of cerebrum, cerebral white and gray matter, and cerebrospinal fluid (CSF) spaces were measured and the Sylvian fissure ratio (SFR) and bifrontal ratio were calculated. Also correlation between the cerebral measures and neuropsychological performance was done. Results: MEG-EEG measurements showed that compared to controls the opioid-dependent patients had delayed mismatch negativity (MMN) response to novel sounds in the EEG and P3am on the contralateral hemisphere to the stimulated ear in MEG. The equivalent current dipole (ECD) of N1m response was stronger in patients with benzodiazepine co-dependence than those without benzodiazepine co-dependence or controls. In early abstinence the opioid dependents performed poorer than the controls in tests measuring attention and working memory, executive function and fluid intelligence. Test results of the Culture Fair Intelligence Test (CFIT), testing fluid intelligence, and Paced Auditory Serial Addition Test (PASAT), measuring attention and working memory correlated positively with the days of abstinence. MRI measurements showed that the relative volume of CSF was significantly larger in opioid dependents, which could also be seen in visual analysis. Also Sylvian fissures, expressed by SFR were wider in patients, which correlated negatively with the age of opioid abuse onset. In controls the relative gray matter volume had a positive correlation with composite cognitive performance, but this correlation was not found in opioid dependents in early abstinence. Conclusions: Opioid dependents had wide Sylvian fissures and CSF spaces indicating frontotemporal atrophy. Dilatation of Sylvian fissures correlated with the abuse onset age. During early withdrawal cognitive performance of opioid dependents was impaired. While intoxicated the pre-attentive attention to novel stimulus was delayed and benzodiazepine co-dependence impaired sound detection. All these changes point to disturbances on frontotemporal areas.

Em 2008, a partir da Portaria 1707 do Ministério da Saúde, foi instituído no âmbito do Sistema Único de Saúde o Processo Transexualizador que estabeleceu as bases para a regulação do acesso de transexuais aos programas para realizar os procedimentos de transgenitalização. Esta Portaria, que tem como base o reconhecimento de que a orientação sexual e a identidade de gênero são determinantes da situação de saúde e que o mal-estar e sentimento de inadaptação por referência ao sexo anatômico do transexual devem ser abordados dentro da integralidade da atenção preconizada pelo SUS, significou avanços expressivos na legitimação da demanda de transexuais por redesignação sexual e facilitou o acesso dessa população à assistência de saúde. Embora a proposta da atenção a transexuais instituída no Brasil seja a de uma política de saúde integral que ultrapassa a questão cirúrgica e considera fatores psicossociais desta experiência, é possível observar que a mesma está baseada em um modelo biomédico que considera a transexualidade um transtorno mental cujo diagnóstico é condição de acesso ao cuidado e o tratamento está orientado para a realização da cirurgia de redesignação sexual. Nesse sentido, apenas os sujeitos que se enquadram na categoria nosológica de Transtorno de Identidade de Gênero e, consequentemente, expressam o desejo de adequar seu corpo ao gênero com o qual se identificam por meio de modificações corporais têm seu direito à assistência médica garantido. Diante disso, considerando que no Brasil a atenção a transexuais está absolutamente condicionada a um diagnóstico psiquiátrico que, ao mesmo tempo em que legitima a demanda por redesignação sexual e viabiliza o acesso a cuidados de saúde é um vetor de patologização e de estigma que restringe o direito à atenção médica e limita a autonomia, o presente estudo pretende discutir os desafios da despatologização da transexualidade para a gestão de políticas públicas para a população transexual no país. A partir de uma pesquisa sobre as questões históricas, políticas e sociais que definiram a transexualidade como um transtorno mental e dos processos que associaram a regulamentação do acesso aos serviços de saúde ao diagnóstico de transexualismo, espera-se problematizar o atual modelo de assistência a pessoas trans e construir novas perspectivas para a construção de políticas inclusivas e abrangentes que garantam o direito a saúde e o exercício da autonomia para pessoas trans.

Background: The aims of this study were to evaluate the prevalence of HIV and its associated demographic and clinical factors among psychiatric inpatients of a general hospital. Methods: This was a single-center, observational, cross-sectional study that included patients consecutively admitted to our unit aged 16 years or older and with no relevant cognitive problems. The patients were evaluated using a semistructured interview and an appropriate test for HIV infection. Results: Of the 637 patients who were screened, 546 (86%) who consented to participate were included in the analyses. Twenty-five (4.6%, 95% confidence interval [CI] 3.0-6.8) patients were HIV-positive. The prevalence was higher among patients with substance misuse (17.4%, 95% CI 9.7-28.8). All except one of the 25 patients knew of their seropositive condition prior to participation in the study. Only 14 (56%) of the 25 seropositive patients had previously received pharmacological treatment for their infection. According to the multiple logistic regression analysis, the likelihood of HIV infection was lower in patients with higher levels of education and higher among patients who were single, had history of intravenous drug use, and had an HIV-positive partner, particularly if they did not use condoms. Among the patients with HIV infection, 18 (72%) had a history of suicide attempts compared with 181 (34.7%) of the patients without HIV infection (relative risk 2.1, 95% CI 1.6-2.7; P<0.001). Conclusion: HIV infection is highly prevalent in patients admitted to a psychiatric unit, especially those with a diagnosis of substance misuse. Seropositive patients show very poor treatment adherence. The risk of suicide seems to be very high in this population. Implementing interventions to reduce the suicide risk and improve adherence to antiretroviral therapy and psychotropic medications seems crucial.

Este estudo tenciona discutir o problema da conceituação das doenças mentais, a partir dos DSMs e dos diferentes paradigmas que os embasaram. O DSM (manual estatístico e diagnóstico de transtornos mentais) é um manual, de influência internacional, para profissionais da saúde mental, que lista diferentes categorias de transtornos mentais, de acordo com a Associação Psiquiátrica Norte-Americana. Desde a sua primeira publicação, em 1952, já foi submetido a cinco revisões (DSM II, DSM III, DSM III-R, DSM IV e DSM IVTR). Escolhemos a categoria diagnóstica da depressão, objetivando realizar um rastreamento conceitual, desde o DSM II - modelo até então marcado pela psicanálise, depois ressaltando o DSM III, que, em 1980, promove uma mudança de paradigma no conhecimento psiquiátrico, ao apresentar um modelo que se propõe descritivo e ateórico até o DSM IV-TR. Dessa perspectiva, são assinaladas algumas considerações e pontos de discussão entre a chamada psiquiatria biológica e a psicanálise, no que diz respeito às suas respectivas influências na forma de entender o diagnóstico psiquiátrico, enfatizando a categoria diagnóstica da depressão.

O Transtorno do estresse pós-traumático (TEPT) é um transtorno mental que ocorre em resposta a um evento traumático que coloca em risco a vida do indivíduo ou de outras pessoas. O TEPT no período pós-parto foi documentado pela primeira vez em 1978. Porém, há poucos estudos sobre o tema, principalmente em gestantes de alto risco materno e fetal. Visando preencher essa lacuna, essa dissertação tem por objetivo estimar a magnitude de TEPT no período pós-parto em uma maternidade de alto risco fetal no município do Rio de Janeiro e identificar subgrupos vulneráveis ao transtorno. Trata-se de um estudo transversal, cuja população de estudo foi composta por 456 mulheres que tiveram o parto no Instituto Fernandes Figueira e realizaram a consulta de revisão pós-parto entre fevereiro e julho de 2011. Casos suspeitos de TEPT foram identificados por meio de dois instrumentos: Trauma History Questionnaire (THQ) utilizado para a captação de situações potencialmente traumáticas ao longo da vida e Post-Traumatic Stress Disorder Checklist (PCL-C) para rastreio de sintomas de TEPT. A prevalência agregada de TEPT no período pós-parto foi de 9,4%. Subgrupos considerados vulneráveis foram: mulheres com três ou mais partos anteriores (15,1%), com o recém-nascido com APGAR menor ou igual a 7 no primeiro minuto (13,6%), com histórico de psicopatologia anterior (29,0%) ou concomitante à gestação (36,7%), com depressão pós-parto (31,5%), mulheres que sofreram violência física (19,8%) e psicológica (11,6%) perpetrada por parceiro íntimo durante a gestação, mulheres que sofreram abuso sexual na infância (25,7%) e com histórico de 5 ou mais situações traumáticas anteriores (25,9%). A elevada prevalência de TEPT encontrada entre as mulheres entrevistadas pode ser, em parte, atribuída às particularidades da população assistida nessa instituição, de reconhecido risco materno e fetal. A alta prevalência de casos suspeitos de depressão pós-parto entre as mulheres com suspeição de TEPT é um fator de preocupação adicional, já que dificulta o manejo clínico dos casos e afasta a mulher e a criança dos serviços de saúde. TEPT no período pós-parto não é um evento raro e merece atenção. Rápido diagnóstico e tratamento são fundamentais para a melhor qualidade de vida da mãe tornando-a apta aos cuidados do recém-nascido.

Excesso de peso (sobrepeso e obesidade) e transtornos mentais comuns são importantes problemas de saúde pública no Brasil e no mundo. A associação entre ambos tem sido investigada por pesquisadores, porém os resultados ainda são conflitantes. Estudos realizados com nutricionistas têm dado maior ênfase à prática de atuação, entretanto, poucos abordaram questões de saúde desses profissionais, principalmente sobre o excesso de peso e sofrimento psíquico. Objetivo - Analisar a associação entre sobrepeso, obesidade e transtornos mentais comuns nesses profissionais. Métodos - Estudo seccional, realizado com 289 nutricionistas da rede pública de hospitais do município do Rio de Janeiro, no período de outubro de 2011 a agosto de 2012. A avaliação do excesso de peso corporal foi realizada com base no Índice de Massa Corporal (kg/m2) através da aferição de peso e altura, e os transtornos mentais comuns através do General Health Questionarie (GHQ-12). Variáveis sócio-demográficas, laborativas e de saúde também foram incluídas no estudo. Resultados - A prevalência de sobrepeso foi de 32,3% e de obesidade, 15,3%. A prevalência de transtornos mentais comuns (TMC) foi de 37,7%. A análise bruta demonstrou uma associação negativa entre transtornos mentais comuns e sobrepeso (OR 0,68; IC95% 0,39 1,20) e positiva para obesidade (OR 1,34; IC95% 0,65 2,75) que não se modificou quando ajustado pelas variáveis socioeconômicas (SES), laborativas e de saúde (OR= 0,60 IC95% 0.32 1,10) para sobrepeso e para a obesidade (OR= 1.09 IC95% 0,50 2,37). Conclusão - Os resultados do estudo destacam as altas prevalências de sobrepeso, obesidade e transtornos mentais comuns, bem como, a magnitude da associação entre os eventos, ambos sem significância estatística. Sugerimos novos estudos em que se possam identificar os mecanismos envolvidos nesta relação, bem como os fatores relacionados às condições de trabalho e de vida que possam estar afetando a saúde do nutricionista que é formado para cuidar da saúde população muitas vezes em detrimento da sua própria saúde.

Com o objetivo de compreender a busca por cuidado de uma pessoa portadora de transtorno mental em situação de comorbidade clínica, um trabalho de campo foi desenvolvido junto a uma usuária da rede de atenção em saúde do município do Rio de Janeiro-RJ. O atendimento desses casos é um desafio para o campo da saúde mental, pois demanda a elaboração de projetos terapêuticos que rompam com o dilema corpo/mente e promovam cuidado integral. A metodologia adotada foi a construção do itinerário terapêutico seguido pela usuária, tendo sido utilizadas as técnicas de observação participante, entrevista aberta e análise documental, ao longo de 6 meses do ano de 2013, envolvendo também os familiares e profissionais no estudo. Os resultados apontam que a usuária transitou tanto pelo Sistema Único de Saúde quanto pelo Sistema Único de Assistência Social ao longo do itinerário, tendo sido atendida em um centro de referência especializado, uma emergência médica, um hospital psiquiátrico, uma policlínica e uma clínica da família. Além disso, nos serviços de saúde foi descrita principalmente do ponto de vista biomédico, o alcoolismo e a demência predominando como índice de comorbidade psiquiátrica e a hanseníase como índice de comorbidade clínica, o transtorno mental esquizofrenia paranóide sendo o diagnóstico principal. A usuária apresentava-se aos profissionais como uma mãe que gostaria de viver em companhia dos filhos e ao mesmo tempo como alguém com vício de bebida forte, enquanto era considerada pelos familiares bêbada e maloqueira, habitando as ruas do centro da cidade do Rio de Janeiro há aproximadamente dez anos. Por meio da narrativa de uma parcela da trajetória de vida e de grande parte do itinerário terapêutico da usuária, nota-se trajetos entre serviços de saúde, casa e rua, bem como passagens marcadas pela vivência de que os profissionais de saúde manifestam asqueiro quando de sua presença. Com base nessa abordagem teórico-metodológica centrada na usuária, nota-se que os seus trajetos e passagens pela cidade e serviços de saúde repercutiram em seu cotidiano de uma forma que sua busca por cuidado converteu-se numa fuga do cuidado

Supported housing for individuals with severe mental illness strives to provide the services necessary to place and keep individuals in independent housing that is integrated into the community and in which the consumer has choice and control over his or her services and supports. Supported housing can be contrasted to an earlier model called the “linear residential approach” in which individuals are moved from the most restrictive settings (e.g., inpatient settings) through a series of more independent settings (e.g., group homes, supervised apartments) and then finally to independent housing. This approach has been criticized as punishing the client due to frequent moves, and as being less likely to result in independent housing. In the supported housing model (Anthony & Blanch, 1988) consumers have choice and control over their living environment, their treatment, and supports (e.g., case management, mental health and substance abuse services). Supports are flexible and faded in and out depending on needs. Results of this systematic review of supported housing suggest that there are several well-controlled studies of supported housing and several studies conducted with less rigorous designs. Overall, our synthesis suggests that supported housing can improve the living situation of individuals who are psychiatrically disabled, homeless and with substance abuse problems. Results show that supported housing can help people stay in apartments or homes up to about 80% of the time over an extended period. These results are contrary to concerns expressed by proponents of the linear residential model and housing models that espoused more restrictive environments. Results also show that housing subsidies or vouchers are helpful in getting and keeping individuals housed. Housing services appear to be cost effective and to reduce the costs of other social and clinical services. In order to be most effective, intensive case management services (rather than traditional case management) are needed and will generally lead to better housing outcomes. Having access to affordable housing and having a service system that is well-integrated is also important. Providing a person with supported housing reduces the likelihood that they will be re-hospitalized, although supported housing does not always lead to reduced psychiatric symptoms. Supported housing can improve clients’ quality of life and satisfaction with their living situation. Providing supported housing options that are of decent quality is important in order to keep people housed and satisfied with their housing. In addition, rapid entry into housing, with the provision of choices is critical. Program and clinical supports may be able to mitigate the social isolation that has sometimes been associated with supported housing.
