RESUMO - Introdução: Os problemas do sono, designadamente a insónia, os sintomas de insónia, os padrões de sono inadequados e a sonolência diurna, são frequentes na adolescência. Estes problemas estão frequentemente associados a múltiplos fatores, entre os quais estilos de vida e fatores ambientais, e apresentam consequências significativas na vida do adolescente e posteriormente na idade adulta. O sono e as suas perturbações deveriam constituir uma preocupação para os profissionais da saúde e da educação com o objetivo de tornar os hábitos de sono saudáveis num estilo de vida - com benefícios calculáveis como os associados a outros estilos de vida saudáveis (alimentação e exercício físico). Em Portugal, os estudos sobre problemas do sono em adolescentes são escassos, bem como as intervenções individuais e comunitárias no âmbito da higiene do sono. Os objetivos desta investigação foram estimar a prevalência de insónia e de sintomas de insónia em adolescentes, identificar fatores de risco e protetores dos sintomas de insónia, analisar as repercussões dos sintomas de insónia, caracterizar os padrões de sono dos adolescentes do distrito de Viseu e elaborar uma proposta de intervenção destinada à promoção da higiene do sono adaptada às características dos adolescentes do distrito de Viseu. Métodos: Realizou-se um estudo transversal onde se avaliaram alunos de vinte e seis escolas públicas do terceiro ciclo e secundário do distrito de Viseu, durante ano letivo 2011/2012. A recolha dos dados foi efetuada através de um questionário autoaplicado e respondido pelos alunos em sala de aula. Foram considerados elegíveis para participar no estudo todos os alunos que frequentassem entre o 7.º e o 12.º ano de escolaridade e tivessem idades entre os 12 e os 18 anos. Dos 9237 questionários distribuídos recolheu-se 7581 (82,1%). Foram excluídos da análise os questionários relativos a adolescentes com idade inferior a 12 ou superior a 18 anos e os questionários devolvidos por preencher. A amostra global foi constituída por 6919 adolescentes, sendo 3668 (53,2%) do sexo feminino. A insónia foi definida com base na presença, no mês prévio, dos sintomas de insónia definidos nos critérios do DSM-IV (dificuldade em adormecer, dificuldade em manter o sono, acordar muito cedo e ter dificuldade em voltar a adormecer e sono não reparador) com uma frequência de pelo menos três vezes por semana e associados a consequências no dia-a-dia. A qualidade de vida foi avaliada com recurso à escala de qualidade de vida SF-36; a sintomatologia depressiva através do Inventário de Depressão de Beck para adolescentes (BDI-II) e a sonolência diurna utilizando a Escala de Sonolência de Epworth (ESE). Para responder ao último objetivo foi elaborada uma proposta de intervenção individual e comunitária no âmbito da higiene do sono. A proposta resulta da evidência científica, dos resultados da presente investigação e de reuniões com profissionais da saúde e da educação. Resultados: No total da amostra, a prevalência de insónia foi de 8,3% e de sintomas de insónia foi de 21,4%. A prevalência de insónia foi superior no sexo feminino (10,1% vs. 5,9%; p<0,001) assim como a prevalência de sintomas de insónia (25,6% vs. 15,8%; p<0,001). Individualmente, todos os sintomas foram mais prevalentes no sexo feminino, sendo a diferença estatisticamente significativa (p<0,001). Em média os adolescentes dormiam, durante a semana, 8:04±1:13 horas. A prevalência de sono insuficiente (< 8 horas) foi de 29%. Apenas 6,4% dos adolescentes indicaram que se deitavam todas as noites à mesma hora. A prevalência de sintomatologia depressiva foi de 20,9% (26,0% nas raparigas e 15,1% nos rapazes, p<0,001). A prevalência de sonolência diurna foi de 33,1%, apresentando o sexo feminino um risco superior (OR=1,40; IC95%: 1,27-1,55). A prevalência de sintomatologia depressiva e de sonolência diurna foi superior entre os adolescentes com sintomas de insónia (48,2% vs. 18,8%, p<0,001 e 42,4% vs. 33,0%, p<0,001, respetivamente). Os adolescentes com sintomas de insónia apresentavam igualmente pior qualidade de vida. Em relação a outras repercussões no dia-a-dia, foram os adolescentes com sintomas de insónia que referiam mais vezes sentir dificuldade em levantar-se de manhã, acordar com cefaleias, acordar cansado e recorrer a medicação para dormir. Nos rapazes os sintomas de insónia associaram-se com o IMC. Após o ajustamento para o sexo e idade com recurso à regressão logística verificou-se uma associação entre sintomas de insónia e sexo feminino [OR ajustado(idade)= 1,82; IC95%: 1,56-2,13], idade ≥16 anos [OR ajustado(sexo)= 1,17; IC95%: 1,01-1,35], residência urbana (OR ajustado= 1,30; IC95%: 1,04-1,63), consumo de café (OR ajustado= 1,40; IC95%: 1,20-1,63), consumo de bebidas alcoólicas (OR ajustado= 1,21; IC95%: 1,03-1,41) e sintomatologia depressiva (OR ajustado= 3,59; IC95%: 3,04-4,24). Quanto à escolaridade dos pais, verificou-se uma redução do risco com o aumento da escolaridade dos pais (5º-6º ano OR ajustado= 0,82; IC95%: 0,64- 1,05; 7º-12º ano OR ajustado= 0,77; IC95%: 0,61-0,97; >12º ano OR ajustado= 0,64; IC95%: 0,47-0,87). Após uma análise multivariada, o modelo preditivo para a ocorrência de sintomas de insónia incluiu as variáveis sexo feminino, viver em meio urbano, consumir café e apresentar sintomatologia depressiva. Este modelo apresenta uma especificidade de 84,2% e uma sensibilidade de 63,6%. O sono insuficiente associou-se, após ajuste para o sexo e idade, com o ano de escolaridade, estado civil dos pais, determinados estilos de vida (consumo de café, tabagismo, consumo de álcool, consumo de outras drogas, sair à noite, presença de TV no quarto e número de horas despendido a ver televisão e no computador), latência do sono, sesta > 30 minutos, horários de sono irregulares e com a toma de medicamentos para dormir. Os resultados deste estudo constituem um diagnóstico de situação relativamente aos problemas de sono em adolescentes no distrito de Viseu. Tendo por base os princípios da Carta de Ottawa relativamente à promoção da saúde, a proposta elaborada visa a implementação de estratégias de prevenção agrupadas em intervenções individuais, comunitárias e sobre os planos curriculares. As intervenções baseiam-se na utilização das tecnologias da informação e comunicação, no contexto da nova arquitetura na esfera pública da saúde conducente aos sistemas personalizados de informação em saúde (SPIS). Conclusões: Registou-se uma elevada prevalência de insónia e sintomas de insónia entre os adolescentes do distrito de Viseu, superior no sexo feminino. A presença de sintomas de insónia esteve associada, sobretudo, a determinados estilos de vida e à ausência de higiene do sono. Os problemas de sono em adolescentes, devido à sua frequência e repercussões, devem constituir uma preocupação em termos de saúde pública e constituir uma prioridade nas estratégias de educação para a saúde. Os 9 princípios da intervenção delineada visam uma abordagem preventiva de problemas de sono - através da ação conjunta de profissionais da saúde e da educação, de elementos da comunidade e com o indispensável envolvimento dos adolescentes e da família -, procurando instituir os hábitos de sono saudáveis como um estilo de vida.

RESUMO - As desigualdades em saúde estão relacionadas com as condições em que as pessoas se desenvolvem. Estas condições podem ser afetadas, principalmente, por diferenças de rendimento, background socioeconómico, educação e estilos de vida. A adolescência é um período de mudança em que os jovens podem experienciar comportamentos de risco que podem prevalecer na idade adulta. Uma posição socioeconómica inferior, do adolescente, pode estar associada a um aumento na prevalência de consumo de tabaco. Este trabalho tem como objetivo: verificar a associação entre estatuto socioeconómico e consumo de tabaco nos adolescentes em Portugal. Utilizaram-se dados colhidos por questionário, em Abril e Outubro de 2013 na cidade de Coimbra. O questionário insere-se no projeto europeu SILNE. Para avaliar o estatuto socioeconómico foram analisadas cinco variáveis: Family Affluence Scale, privação material, posição social subjetiva, semanada, educação e emprego dos pais. Para estratificar o comportamento do fumador foram desenvolvidas três variáveis: não fumador, experimenter e fumador regular. A análise foi separada por sexo. Constata-se que as raparigas fumam, em média, menos que os rapazes (F =17.1%; M =22.1%). Concluímos que o estatuto socioeconómico, avaliado pela Family Affluence Scale, posição social subjetiva, privação material, educação e emprego dos pais não têm impacto no consumo de tabaco no adolescente. As desigualdades socioeconómicas no consumo de tabaco nos adolescentes estão sim, relacionadas com o próprio estatuto socioeconómico do adolescente, traduzido pela semanada que este recebe. Foi encontrado no desempenho escolar um efeito mediador entre desigualdades socioeconómicas e consumo de tabaco na adolescência.

RESUMO - Introdução: A inatividade física é um dos determinantes major das doenças crónicas não transmissíveis sendo a quarta maior causa de mortalidade no mundo, nomeadamente para as doenças vasculares. A prática regular de atividade física produz adaptações vasculares responsáveis por efeitos benéficos na prevenção e tratamento dos diferentes fatores de risco vascular, nomeadamente através do seu efeito no metabolismo das lipoproteínas. Objetivos: Analisar a interferência da atividade física no perfil lipídico de uma população residente em Portugal. Métodos: Estudo observacional descritivo transversal exploratório com 1027 indivíduos (idade: 18 aos 80 anos, 49% mulheres). Os dados foram analisados em SPSS (versão 20), tendo-se utilizado métodos de estatística descritiva e de análise bivariável entre os factores de risco vascular e as variáveis do perfil lipídico e ainda uma análise multivariável de regressão logística binária para medir a razão de riscos pelo odds ratio. O nível de significância foi estabelecido em 5%. Resultados: Na análise da relação entre atividade física e os biomarcadores do perfil lipídico verificou-se que existem benefícios no que diz respeito ao aumento dos níveis de HDL e de apoA1 e na diminuição dos níveis de TG com a prática regular de atividade física. Conclusões: A atividade física apresenta um papel importante na regulação do perfil lipídico evidenciando a necessidade de implementar estratégias multissectoriais de prevenção dos fatores de risco vascular, nomeadamente na área dos estilos de vida saudáveis que são fundamentais para a prevenção destas condições de saúde e para gerar ganhos em saúde.

A sample of 445 consumers resident in distinct Lisbon areas was analyzed through direct observations in order to discover each lifestyle’s current proportion, applying the Whitaker Lifestyle™ Method. The findings of the conducted hypothesis tests on the population proportion unveil that Neo-Traditional and Modern Whitaker lifestyles have the significantly highest proportion, while the overall presence of different lifestyles varies across neighborhoods. The research further demonstrates the validity of Whitaker observation techniques, media consumption differences among lifestyles and the importance of style and aesthetics while segmenting consumers by lifestyles. Finally, market opportunities are provided for firms operating in Lisbon.

ABSTRACT - Background: From a public health perspective, the study of socio-demographic factors related to physical activity is important in order to identify subgroups for intervention programs. Purpose: This study also aimed to identify the prevalence and the socio-demographic correlates related with the achievement of recommended physical activity levels. Methods: Using data from the European Social Survey round 6, physical activity and socio-demographic characteristics were collected from 39278 European adults (18271 men, 21006 women), aged 18-64 years, from 28 countries in 2012. Meeting physical activity guidelines was assessed using World Health Organization criteria. Results: 64.50% (63.36% men, 66.49% women) attained physical activity recommended levels. The likelihood of attaining physical activity recommendations was higher in age group of 55-64 years (men: OR=1.22, p<0.05; women: OR=1.66, p<0.001), among those who had completed high school (men: OR=1.28, p<0.01; women: OR=1.26, p<0.05), among those who lived in rural areas (men: OR=1.20, p<0.001; women: OR=1.10, p<0.05), and among those who had 3 or more people living at home (men: OR=1.40, p<0.001; women: OR=1.43, p<0.001). On the other hand, attaining physical activity recommendations was negatively associated with being unemployed (men: OR=0.70, p<0.001; women: OR=0.87, p<0.05), being a student (men: OR=0.56, p<0.001; women: OR=0.64, p<0.01), being a retired person (men: OR=0.86, p<0.05) and with having a higher household income (OR=0.80, p<0.001; women: OR=0.81, p<0.01). Conclusion: This research helped clarify that, as the promotion of physical activity is critical to sustain health and prevent disease, socio-demographic factors are important to consider when planning the increase of physical activity.

The nature tourism experienced a great expansion of its market with the appearance of different lifestyles. In this Work Project a study regarding the website direct sales of Rota Vicentina was developed. Its website shows the idea of being solely an information structure and not a purchase one, leading to a current absence of online sales. Hence, it is suggested the modification of its business model, using different instruments and channels. Some digital marketing recommendations were developed in order to boost website sales, such as a platform for online reviews, remarketing campaigns and social media activity.

OBJECTIVE: To describe food habits and dietary intakes of athletic and non-athletic adolescents in Switzerland. SETTING: College, high schools and professional centers in the Swiss canton of Vaud. METHOD: A total of 3,540 subjects aged 9-19 y answered a self-reported anonymous questionnaire to assess lifestyles, physical plus sports activity and food habits. Within this sample, a subgroup of 246 subjects aged 11-15 also participated in an in-depth ancillary study including a 3 day dietary record completed by an interview with a dietician. RESULTS: More boys than girls reported engaging in regular sports activities (P<0.001). Adolescent food habits are quite traditional: up to 15 y, most of the respondents have a breakfast and eat at least two hot meals a day, the percentages decreasing thereafter. Snacking is widespread among adolescents (60-80% in the morning, 80-90% in the afternoon). Food habits among athletic adolescents are healthier and also are perceived as such in a higher proportion. Among athletic adolescents, consumption frequency is higher for dairy products and ready to eat (RTE) cereals, for fruit, fruit juices and salad (P<0.05 at least). Thus the athletic adolescent's food brings more micronutrients than the diet of their non-athletic counterparts. Within the subgroup (ancillary study), mean energy intake corresponds to requirements for age/gender group. CONCLUSIONS: Athletic adolescents display healthier food habits than non-athletic adolescents: this result supports the idea that healthy behavior tends to cluster and suggests that prevention programs among this age group should target simultaneously both sports activity and food habits.

This article presents selected findings and lessons from a cardiovascular research and prevention program initiated in 1989 in the Republic of Seychelles, a country in demographic and epidemiological transition. Rapid and sustained aging of the population (e.g., two-fold increase of people aged 30-39 from 1979 to 1995) implies, over the next few decades, further dramatic increase of the burden of chronic diseases, particularly cardiovascular disease (CVD). Epidemiological surveillance shows high age-specific rates of CVD (particularly stroke), high prevalence of peripheral atherosclerosis (plaques in carotid and femoral arteries), high prevalence of classical modifiable risk factors in the adult population (particularly hypertension), and substantial proportions of children with overweight. Stagnant life expectancy in men and an increase in women have been observed over the last two decades; this occurred despite largely improved health services and reduced infant mortality rates, and may reflect the large CVD burden found in middle-aged men (less so in middle-aged women). A national program of prevention of CVD has been initiated since 1991, which includes a mix of interventions to reduce risk factors in the general population and in high-risk individuals. Substantial research to back the prevention program indeed shows, at the moment, epidemiological patterns in Seychelles similar to those observed in Western countries (e.g., an association between peripheral atherosclerosis [as a proxy of CVD] and low density lipoprotein-cholesterol, smoking, diabetes, and [inversely] walking). This clearly supports the view that promotion of healthy lifestyles and control of conventional risk factors should be the main targets for CVD prevention and control.

To evaluate the socio-demographic as well as the health and psychiatric profiles of adolescents hospitalised for suicide attempt or overwhelming suicide ideation and to assess repetition of suicide attempt over a period of 18 months. Between April 2000 and September 2001, all patients aged 16 to 21 years admitted to the University Hospitals of Geneva and Lausanne for suicide attempt or ideation were included in the study. At this time (T0) semi-structured face to face interviews were conducted to identify socio-demographic data, mental health and antecedents regarding suicidal conducts. Current psychiatric status was assessed with the MINI (Mini International Neuropsychiatric Instrument). At T1 and T2, reassessments included psychiatric status (MINI) as well as lifestyles, socio-professional situation and suicidal behaviours. At T0, 269 subjects met the study criteria, among whom 83 subjects (56 girls and 27 boys) left the hospital too quickly to be involved or refused to participate in the study (final sample at T0: 149 girls; 37 boys). The participation rate at T1 and T2 was respectively 66% and 62% of the original sample. The percentage of adolescents meeting the criteria for psychiatric diagnoses (91%) was high: affective disorder (78%); anxiety disorder (64%); substance use disorder (39%); eating disorder (9%); psychotic disorder (11%); antisocial personality (7%) with most subjects (85%) having more than one disorder. Around 90% of the subjects interviewed at T1, and/or T2, had received follow-up care after their hospitalisation, either by a primary care physician or a psychotherapist or both. Two subjects died of violent death and 18% made a further suicide attempt. Most adolescents hospitalised for suicidal episodes suffer from psychiatric problems which should be addressed by a careful psychiatric assessment, followed up if needed by a structured after care plan.

This paper aims at evaluating the compatibility of coercive climate policies with liberal neutrality. More precisely, it focuses on the doctrine of state neutrality as associated with the "harm principle". It argues that given the difficulty of attributing causal responsibilities for climate harms to individuals, the harm principle doesn't work in this case, at least if one endorses a liberal atomistic ontology. Furthermore, the definition of what constitutes climate harms implies making moral assumptions, which makes it impossible to justify climate policies in a neutral way. Finally, the paper shows another consequence of applying neutrality to the case of climate change, that is the risk of a shift from political forms of decision-making to technocracy. Focusing too much on liberty of choice may (paradoxically) be to the detriment of political freedom. The paper concludes that climate change is an intrinsically moral issue and that it should be the occasion of a political debate about our current values and lifestyles. It should not be reduced to a mere question of carbon metric.

Physical inactivity poses a huge burden on Canada's health care system and is detrimental to the health of Canadians (Katzmarzyk & Janssen, 2004). Walking is a viable option for individuals to become physically active on a daily basis and is in fact the most commonly reported leisure time physical activity. It has been associated with many health benefits including weight loss/weight control, reduced risk of coronary artery disease and diabetes, lowered blood pressure, and improved psychological wellbeing (Brisson & Tudor-Locke, 2004). Specifically, individuals' stage of change, selfefficacy and health related quality of life (HRQL) are three psychological constructs that can be greatly improved with increased physical activity (Dishman, 1991; Penedo & Dahn, 2005; Poag & McAuley, 1992). Public health physical activity recommendations exist but many individuals find these difficult to meet due to overly busy lifestyles (Public Health Agency of Canada, 2003). Pedometers are inexpensive devices that can monitor individual bouts of walking so that the incorporation of physical activity into one's daily life is more plausible. They are also excellent tools for motivation, goalsetting, and immediate feedback (Brisson & Tudor-Locke, 2004). Since many people spend a large proportion of their time at their places of employment, workplaces have begun to be a common site for the development of physical activity interventions. These programs have been growing in popUlarity and have shown numerous benefits for both employees and employers (Voit, 2001). The purpose of the current study was to implement and evaluate the use of a pedometer-based physical activity intervention incorporating goal-setting and physical activity logs in a workplace setting, and to examine the relationship between different types of self-efficacy (task, barrier, and scheduling) and different phases of the intervention. Twenty male participants from a local steel manufacturing plant who exhibited health risk factors (e.g. hypertension, diabetes, etc.) were assigned to one of two groups (group A or group B). All participants were asked to wear pedometers on their waists, record their daily steps, set goals that were outlined on a step-tracking sheet (detennined by their baseline number of steps), and keep track of their work days, wakelbed time, sedentary time, and time spent doing other physical activity. Group A began the intervention immediately following the baseline measures, whereas group B continued with their regular routine for 4 weeks before beginning. Physiological measures (height, weight, blood pressure, relative body fat, waist and hip circumference, and body mass index) were taken and a battery of questionnaires that assessed barrier, task and scheduling self-efficacy, HRQL, and stage of change administered at baseline, week 5 (end of intervention for group A), week 9 (end of intervention for group B; follow-up for group A) and week 13 (follow-up for both groups). Results showed that this workplace physical activity intervention was successful at increasing the participants' daily steps, that task self-efficacy is a significant predictor of participants' exercise adherence during the initial stages of participation (intervention phase), and that the participants felt that this intervention was effective. Finally, further exploratory analyses showed that this intervention was effective for all participants, but most valuable for participants most in need of improvement - that is, those who were most sedentary prior to the intervention. This intervention is an inexpensive use of simple and effective tools (e.g. pedometers), has the potential to attract a wide variety of participants and become a pennanent part of any health promotion initiative.

As children are becoming increasingly inactive and obese, there is an urgent need for effective early prevention and intervention programs. One solution is a comprehensive school health (CSH) program, a health promotion initiative aimed at educating students about healthy behaviours and lifestyles, which also provides a link between the school, students, families, and the surrounding community. The purpose of this study was to explore the relationship between different components of CSH programs, as well as three determinants of health (gender, social support, socio-economic status), and physical activity, on the aerobic fitness and body mass index (BMI) of children. A newly developed and pilot-tested survey derived from Health Canada's fourpart CSH model (instruction, social support, support services, and a healthy physical environment) was sent to elementary school principals. Data on the gender, physical activity, parental education, and social support levels of students from these schools were gathered from a previous study. Multiple regression procedures were conducted to estimate the relationships between CSH components, the social determinants of health, physical activity, and BMI and aerobic fitness. Results showed that three CSH components were significantly associated with both BMI and aerobic fitness values in children, but accounted for less than 5% of the variance in both variables. Physical activity partially mediated the relationship between the significant CSH components, BMI, and particularly aerobic fitness. Furthermore, the social determinant and physical activity variables played independent roles in aerobic fitness values. No moderating effects of the social determinants were discovered.

The purpose of this cross sectional survey design was to examine self-reported health status and lifestyle behaviours of the residents of the Town of Fort Erie, Ontario, as related to the Canadian Community Health Survey. Using a mail-out survey, entitled the Fort Erie Survey of Health (FESH), a probability cluster sampling technique was used to measure self-reported health status (present health, health conditions, health challenges, functional health limitations) and lifestyle behaviour (smoking, alcohol use, drug use, physical activity, fruit and vegetable consumption, body weight, and gaming). Each variable was described and analyzed in relation to socio-economic variables, age and gender. The findings from this study were compared to the Canadian Community Health Survey 2000/2001. Overall, 640 surveys were completed. The majority of Fort Erie residents rated their present health as good and were satisfied with their overall health and quality of life. The main chronic conditions reported were arthritis, back pain and heart disease. Other main health problems reported were vision, sleeping and chronic pain. Overall, 14.6% smoke; 58.8% engaged in physical activity either occasionally or never as opposed to regularly engaging in physical activity; 52.1% did not eat the required daily fruits and vegetables; and 40.0% were in the overweight category. Persons who practiced one healthy lifestyle behaviour were more likely to practice other healthy promoting behaviours. Therefore, health promotion programs are best designed to address multiple risk factors simultaneously. The ffiSH was generally consistent with the Canadian Community Health Survey in the overall findings. A small number of inconsistencies were identified that require further exploration to determine if they are unique to this community.

The purpose of this study is to examine the psychographic (product attributes, motivation opinions, interest, lifestyle, values) characteristics of wine tourists along the Niagara wine r,~ute, located in Ontario, Canada, using a multiple case study method. Four wineries were selected, two wineries each on the East, and West sides of the wine route during the shoulder-season (January, February, 2004). Using a computer generated survey technique, tourists were approached to fill out a questionnaire on one of the available laptop computers, where a sample ofN=321 was obtained. The study findings revealed that there are three distinct wine tourist segments in the Niagara region. The segments were determined using an exploratory factor analysis (EFA) and a K-means cluster analysis: Wine Lovers, Wine Interested, and Wine Curious wine tourists. These three segments displayed significant differences in their, motivation for visiting a winery, lifestyles, values, and wine purchasing behaviour. This study also examined differences between winery locations, on the East and West sides of the Niagara wine route, with respect to the aforementioned variables. The results indicated that there were significant differences between the regions with respect to these variables. The findings suggest that these differences present opportunities for more effective marketing strategies based on the uniqueness of each region. The results of this study provide insight for academia into a method of psychographic market segmentation of wine tourists and consumer behaviour. This study also contributes to the literature on wine tourism, and the identification of psychographic characteristics of wine tourists, an area where little research has taken place.
