Although the oral cavity is easily accessible to inspection, patients with oral cancer most often present at a late stage, leading to high morbidity and mortality. Autofluorescence imaging has emerged as a promising technology to aid clinicians in screening for oral neoplasia and as an aid to resection, but current approaches rely on subjective interpretation. We present a new method to objectively delineate neoplastic oral mucosa using autofluorescence imaging. Autofluorescence images were obtained from 56 patients with oral lesions and 11 normal volunteers. From these images, 276 measurements from 159 unique regions of interest (ROI) sites corresponding to normal and confirmed neoplastic areas were identified. Data from ROIs in the first 46 subjects were used to develop a simple classification algorithm based on the ratio of red-to-green fluorescence; performance of this algorithm was then validated using data from the ROIs in the last 21 subjects. This algorithm was applied to patient images to create visual disease probability maps across the field of view. Histologic sections of resected tissue were used to validate the disease probability maps. The best discrimination between neoplastic and nonneoplastic areas was obtained at 405 nm excitation; normal tissue could be discriminated from dysplasia and invasive cancer with a 95.9% sensitivity and 96.2% specificity in the training set, and with a 100% sensitivity and 91.4% specificity in the validation set. Disease probability maps qualitatively agreed with both clinical impression and histology. Autofluorescence imaging coupled with objective image analysis provided a sensitive and noninvasive tool for the detection of oral neoplasia.

BACKGROUND: Optical spectroscopy is a noninvasive technique with potential applications for diagnosis of oral dysplasia and early cancer. In this study, we evaluated the diagnostic performance of a depth-sensitive optical spectroscopy (DSOS) system for distinguishing dysplasia and carcinoma from non-neoplastic oral mucosa. METHODS: Patients with oral lesions and volunteers without any oral abnormalities were recruited to participate. Autofluorescence and diffuse reflectance spectra of selected oral sites were measured using the DSOS system. A total of 424 oral sites in 124 subjects were measured and analyzed, including 154 sites in 60 patients with oral lesions and 270 sites in 64 normal volunteers. Measured optical spectra were used to develop computer-based algorithms to identify the presence of dysplasia or cancer. Sensitivity and specificity were calculated using a gold standard of histopathology for patient sites and clinical impression for normal volunteer sites. RESULTS: Differences in oral spectra were observed in: (1) neoplastic versus nonneoplastic sites, (2) keratinized versus nonkeratinized tissue, and (3) shallow versus deep depths within oral tissue. Algorithms based on spectra from 310 nonkeratinized anatomic sites (buccal, tongue, floor of mouth, and lip) yielded an area under the receiver operating characteristic curve of 0.96 in the training set and 0.93 in the validation set. CONCLUSIONS: The ability to selectively target epithelial and shallow stromal depth regions appeared to be diagnostically useful. For nonkeratinized oral sites, the sensitivity and specificity of this objective diagnostic technique were comparable to that of clinical diagnosis by expert observers. o artigo descreve o programa de rastreamento de câncer de colo de útero em uma população de mulheres de 20-59 anos, usuárias de um serviço de Atenção Primária à Saúde (APS), em Porto Alegre. Esta coorte histórica de 5 anos foi constituída usando os registros de família de três unidades do Serviço de Saúde Comunitária do Grupo Hospitalar Conceição, em Porto Alegre, Brasil. O estudo caracteriza a associação entre a detecção de hipertensão, diabetes, depressão e ansiedade nestas mulheres, e as freqüências de sua captação e adesão ao programa de rastreamento do câncer de colo de útero. As mulheres com 50 anos ou mais tiveram um risco relativo de 1,70 (IC95%=1,40-2,06) de não serem captadas pelo programa de rastreamento, quando comparadas com as mais jovens. Os resultados sugerem a necessidade de intensificar o rastreamento de rotina às mulheres de 50 anos ou mais. A captação e a adesão de usuárias poderiam ser usadas como indicadores da qualidade do processo de trabalho. São necessários novos estudos para o estabelecimento de inferência causal e para definir a captação e a adesão como indicadores da qualidade do processo de trabalho em Atenção Primária à Saúde.

Base teórica: Para pacientes com câncer de colo uterino em estádios iniciais (IA2-IIA) e fatores de risco para recorrência, a radioterapia pós-operatória diminui a incidência de recorrência local, embora sem impacto na sobrevida. Os fatores de risco incluem metástases em linfonodos, invasão do espaço linfovascular, invasão com profundidade maior do que 10mm, invasão microscópica de paramétrios, histologia não-escamosa e margens cirúrgicas comprometidas. Além disso, essas pacientes possivelmente estejam sob risco de disseminação subclínica da doença, o que não seria afetado pela radioterapia direcionada à pelve. Desta forma, esta revisão sistemática foi realizada com o objetivo de avaliar as evidências disponíveis para a adição de quimioterapia ao tratamento adjuvante radioterápico de pacientes com câncer de colo uterino em estádios iniciais com fatores de risco de mau prognóstico. Objetivos: Avaliar a sobrevida, a sobrevida livre de progressão e as taxas de recorrência do câncer de colo uterino em estádios iniciais (estádios IA2-IIA) com fatores de risco para recorrência, tratado com quimioterapia e radioterapia adjuvantes versus apenas radioterapia adjuvante. Estratégias de busca: Foram realizadas buscas no Cochrane Gynaecological Cancer Group Trials Register (busca realizada em dezembro de 2004), CENTRAL (a partir de 1993), MEDLINE (a partir de 1966), EMBASE (a partir de 1980), LILACS (a partir de 1982), Biological Abstracts (a partir de 1990), CINHAL (a partir de 1984), SciSearch (a partir de 1991) e Cancerlit (a partir de 1963). Também foram realizadas buscas em resumos de congressos. Critérios de seleção: Foram incluídos todos os ensaios clínicos randomizados controlados comparando quimioterapia e radioterapia adjuvantes (grupo intervenção) com radioterapia adjuvante apenas (grupo controle) no tratamento do câncer de colo uterino em estádio inicial. Extração dos dados e análise: Dois revisores avaliaram de maneira independente os critérios de elegibilidade e de qualidade de cada estudo e extraíram os dados. Resultados: Dois estudos randomizados preencheram os critérios de seleção, incluindo um total de 314 pacientes. As pacientes apresentaram diminuição significativa no risco de morte em 48 meses (hazard ratio 0,43, intervalo de confiança – IC 95% 0,25 – 0,76), o que representou uma redução de 57% no risco de morte e um benefício absoluto de 17%. Em 48 meses, o risco para sobrevida livre de progressão foi estimado em 0,45 (IC 95% 0,28 - 0,74), o que representa uma redução de 55% na razão de chances de progressão da doença e um benefício absoluto de 17%. A recorrência local em 48 meses foi menor no grupo da intervenção (hazard ratio 0,50; IC 95% 0,26 – 0,98). O risco de recorrência à distância não foi diferente entre os dois grupos de tratamento (hazard ratio 0,74; IC 95% 0,36 – 1,52). A recorrência global favoreceu o grupo de intervenção, com razão de chances (RC) de 0,54 (IC 95% 0,33 – 0,90) em 48 meses. A razão de chances para toxicidade grau 3 foi maior no grupo que recebeu quimioterapia (RC 5,19; IC 95% 2,90 – 9,29), da mesma forma que a razão de chances para toxicidade grau 4 (RC 4,62; IC 95% 1,96 - 10,86). Não foram encontrados dados a respeito de qualidade de vida nos estudos avaliados. Conclusão dos revisores: Nesta revisão sistemática, as evidências sugerem haver benefício clínico com a adição de quimioterapia ao tratamento adjuvante radioterápico de pacientes com câncer de colo uterino em estádios iniciais e fatores de risco para recorrência. No entanto, as evidências são limitadas devido ao pequeno número de pacientes incluídas nos estudos e ao curto período de seguimento. Há a necessidade de novos ensaios clínicos randomizados nesta área, com um maior número de pacientes, para que os desfechos possam ser adequadamente avaliados.

Sexually transmitted diseases (STDs) are among the largest public health problems, especially in developing countries. The acquisition of these infections during early sexual activity is common and many infections have a benign course. However, in some pathogens remain in the state of latency can be reactivated and cause productive infection that may progress to severe forms. In addition, some of them are transmitted vertically resulting in congenital infection, causing immediate damage or long-term child. The classic risk factors for sexually transmitted agents are: early onset of sexual and reproductive health, multiple sexual partners throughout life, use of oral contraceptives and co-infections with different pathogens. We present the results of a cross-sectional study aimed to estimate the prevalence of genital infection by human papillomavirus (HPV), Herpes simplex virus (HSV) and Chlamydia trachomatis (CT) in a segment of the female population of the metropolitan area Christmas, among those who enrolled voluntarily sought, Basic Health Units for the examination of cancer screening cervix in the period 2008 to 2010. All participants, a total of 261 women answered a standard questionnaire by which identified the socio-demographic characteristics, classical risk factors for STDs, reproductive and sexual activity and smoking. Of each patient were obtained two samples, one for the completion of the Pap test for detection of cellular changes and the other processed for DNA extraction and analyzed by PCR (polymerase chain reaction) to detect the three pathogens studied. The population of the study was composed of sexually active women aged between 13 and 79 years, mean 38.7 years, most of them being married, low education levels and low incomes. The majority (87%) had normal results on cytology and only 2.7% had low-grade cytological abnormalities. Prevalence rates were 37.9% for HPV, 4.6% for CT and 26% for HSV. HPV prevalence was higher in women under 25, unmarried and in those who had multiple sexual partners. Women with simultaneous infection by HSV-1 and 2 had higher prevalence of HPV infection. The prevalence of HSV infection showed no association whatsoever with the risk factors analyzed and HSV-1 was the predominant type among the cases of genital HSV infection. The overall prevalence of C. Trachomatis was relatively low, thus providing greater value in younger women aged less than or equal to 20 years

Cancer of the cervix (cervical cancer) is the second most prevalent cancer among Brazilian women. The high rates of cervical cancer in Brazil justify the implementation of effective strategies to control this, which include actions to promote health, primary prevention, early detection, screening, treatment and palliative care. Despite the existence of the National Programme for Control of the CCU there was no reduction in the incidence and mortality of this disease in Brazil. The Family Health Strategy (FHS) has the potential to facilitate such control and, in this context, one should consider that nurses play a central role. The study aimed to know the general intervention strategies used by nurses FHS of Natal / RN in CCU control, and how specific: analyzing the knowledge of these nurses on the CCU, the actions developed in the ESF for the control of CCU and identify the difficulties faced by them to perform it. This is a descriptive exploratory quantitative developed through a structured interview guide with 106 nurses who have experience in controlling the CCU in FHS teams of Natal / RN. Data analysis was performed using descriptive statistic s. The results pointed to actions taken in the FHS to control the CCU, collection of cervical cancer screening, health education activities, nursing consultation, referral of suspected cases for medical monitoring and active women with abnormal test result . The actions that were not mentioned by the nurses included: forming groups of prevention and health promotion; expand coverage of exams and office hours of consultations, establishment of alternatives to end the pent-up demand in the health units, participation in treatment or rehabilitation process users with the CCU; interventions for pain management, alliances and partnerships with schools, in dustry and the use of protocols. This study can be seen that the practice nurses partially shares to the CCU in Natal / RN. The participants of this study, when asked about the CCU, specifically for signs and symptoms of disease and risk factors in general showed important gaps. Difficulties such as lack of materials for collection of Pap smear; inadequate physical space in the Health Units; pent-up demand in the service, delay in arrival of the test results; obstacles in the actions of referral and counter-referral and cultural factors make the CCU control is compromised. It is believed in this research contributed to a reflection on the importance of the role of nurses in the development of the ESF control actions CCU, pointing out the factors that affect these. It is important to involve all nurses who comprise the ESF as knowledgeable of the risk factors, signs and symptoms, and existing tools for the early detection of cervical cancer in the pursuit of quality improvement actions to promote women`s health, contributing in planning future interventions that may reduce mortality from this disease in Natal / RN.

Introdução: O exame colpocitológico ou teste de Papanicolaou permite o diagnóstico precoce do câncer de colo do útero. O êxito no rastreamento desta doença dependerá, além de outros fatores, da acuidade diagnóstica do exame colpocitológico. Objetivos: Avaliar a acurácia diagnóstica da citologia cervicovaginal na detecção de lesões cervicais através da comparação citoistopatológica. Material e métodos: Avaliou-se o grau de concordância entre os exames citológico e histopatológico de 373 pacientes atendidas em um hospital universitário no período de 1990 a 1997 e foram calculados os indicadores: sensibilidade, especificidade, valores preditivos positivos e negativos e as taxas de falsos positivos e falsos negativos do exame. Resultados: A taxa bruta de concordância citoistopatológica foi de 65,1%. A sensibilidade do exame foi alta (96,0%), no entanto sua especificidade foi baixa (51,5%), significando a inclusão de muitos resultados falsos positivos. A taxa de falsos positivos foi de 48,4%, enquanto a de falsos negativos foi de 4%. Discussão: Apesar de a sensibilidade do teste ser alta, sua especificidade é baixa, significando que muitas mulheres seriam falsamente diagnosticadas como doentes, levando a um número elevado de resultados falsos positivos e custos desnecessários, além do potencial caráter iatrogênico que esta ação poderia assumir. No entanto, um exame altamente sensível é o teste de escolha para programas de rastreamento de câncer cervical na população feminina. Conclusões: Para garantir a acuidade diagnóstica do teste de Papanicolaou são essenciais atividades de controle de qualidade nos laboratórios, tanto do procedimento colpocitológico quanto do histopatológico, permitindo, assim, o êxito na detecção precoce e no tratamento das lesões cervicais.

The cancer of the uterine cervix remains a serious public health problem in developing countries with low social-economic conditions. Papanicolaou smear has been used to screen women for cervical cancer since 1940, and it is widely recognized as an effective method for preventing cervical cancer. In Brazil, conventional Papanicolaou is the most common method used in diagnosis cytopathological routine, involving the morphological cell analysis by microscopic examination of cervical samples smeared on glass slides. On the other hand, the liquid cytology is an advantageous method whose specimen collected is put into a fluid alcohol based to guarantee the potential use of residual cellular material in cytobrush. Moreover, this method permits additional diagnostic studies, preserving the morphological cell features as the conventional one does. The aim of the present study was report the historical evolution of different cervical cytologic tests, emphasizing the advantages and disadvantages between conventional Papanicolaou method and new technologies in liquid based cytology.

Objectives: To evaluate if the prevalence of cervical smear results varies between pregnant and nonpregnant women stratified by age group.Study design: Observational analytical study with a total sample of 1,336,180 pregnant and nonpregnant women, aged between 20 and 34 years, who underwent cervical cancer screening in the Primary Health Care of the national health system in the area of Campinas in Brazil during the period of 2005-2009. The source is the information system for cervical cancer screening. Data collected on abnormal cervical smears were analyzed using the Chi-square test and Fisher's exact test and the magnitude of the association between pregnancy and high-grade squamous epithelial lesions was analyzed by odds ratio (OR) and estimated values with confidence intervals (CI) of 95%.Results: 15,190 pregnant women and 395,961 non-pregnant women were analyzed and fulfilled the inclusion criteria. Regardless of age, no statistical differences were observed for high-grade squamous intraepithelial lesion prevalence (OR 0.90; CI 0.66-1.23). Taking into account the five-year age groups, however, low-grade squamous intraepithelial lesion was less prevalent in pregnant women aged 20-24 (OR 0.71; 0.54-0.95) and 25-29 years (OR 0.56; 0.35-0.89); also, atypical squamous cells of undetermined significance was more prevalent in non-pregnant women aged 25-29 years (OR 0.72; 0.54-0.97).Conclusion: The study showed that the cytological prevalence of high-grade squamous intraepithelial lesion was similar in pregnant and non-pregnant women, regardless of age. The results indicate that there are no reasons for specific approaches to cervical cancer screening for pregnant women. The examination should be carried out only on pregnant women who have not been tested according to current recommendations. (C) 2014 Elsevier Ireland Ltd. All rights reserved.
