Objectif: Évaluer la concordance entre l'évolution de la perception des patients diabétiques type 2 et l'évolution de mesures quantifiables, dans le contexte d'un programme d'activité physique adapté. Ce programme se base sur l'accompagnement interdisciplinaire dans un concept éducatif et motivationnel (36 séances d'activité physique et 6-8 h d'atelier). Matériels et méthodes: Évaluation de la perception des patients portant sur : activité physique, condition physique, contrôle métabolique, gestion des corrections hypo/hyperglycémie, autonomisation et bien-être. Nous avons utilisé une cible d'auto-évaluation, composée d'échelles de Likert de 1 à 10 (1 = mauvais 10 = excellent). Concernant la condition physique nous avons mesuré : endurance, vitesse de marche, force, équilibre et souplesse. En fin de programme un questionnaire de satisfaction comprenant 5 items a été distribué. Résultats: Analyse des données de 40 patients, âge 59 ± 10 ans, 60 % femmes. Avant programme, 60 % des patients s'estiment insuffisants (moyenne < 5/10) face à la pratique de l'activité, la condition physique et le contrôle métabolique. Le bien-être se situe en moyenne à 5,4/10. Après programme, 75 % des patients montrent une progression dans tous les domaines (moyenne 7,4/10). Une corrélation positive apparaît entre l'amélioration de la condition physique et le bienêtre. Tous les paramètres physiques mesurés se sont aussi améliorés. L'amélioration de la condition physique perçue est corrélée avec celle de la force (p = 0,006). Le travail interdisciplinaire réalisé a été perçu positivement par 87,9 % des patients. La communication était de bonne qualité pour 78,1 % ainsi que le climat d'apprentissage (82,4 %). La majorité des patients (64,7 %) est très satisfaite du programme. Conclusion: Ce programme est prometteur, il montre l'amélioration de la perception et de la condition physique avec une concordance entre les deux. Cette amélioration laisse imaginer que les plus confiants sur leur capacité à agir puissent s'impliquer d'avantage dans la gestion quotidienne et dans la poursuite d'un projet d'activité.

OBJECTIVES: Exercise combined with nicotine therapy may help smoking cessation and minimise weight gain after quitting. Low participation in vigorous-intensity physical activity programmes precludes their population-wide applicability. In a randomised controlled trial, we tested whether a population-based moderate-intensity physical activity programme increases quit rates among sedentary smokers receiving nicotine therapy. METHODS: Participants (n=481; 57% male; mean age, 42.2 years (SD 10.1); mean cigarette consumption, 27 (SD 10.2) per day) were offered a nine-week smoking cessation programme consisting of a weekly 15-minute counselling session and the prescription of nicotine replacement therapy. In addition, participants in the physical activity group (n=229) also took part in a programme of moderate-intensity physical activity implemented at the national level, and offering nine weekly 60-minute sessions of physical activity. To ensure equal contact conditions, participants in the control group (n=252) attended weekly 60-minute health behaviour education sessions unrelated to physical activity. The primary outcome was continuous CO-verified smoking abstinence rates at 1-year follow-up. RESULTS: Continuous smoking abstinence rates were high and similar in the physical activity group and the control group at the end of the intervention (47% versus 46%, p=0.81) and at 1-year follow-up (27% versus 29%, p=0.71). The mean weight gain after one year was 4.4 kg and 6.2 kg among sustained quitters of the physical activity and control groups, respectively (p=0.06). CONCLUSION: Participation in a population-based moderate-intensity physical activity programme for 9 weeks in addition to a comprehensive smoking cessation programme did not significantly increase smoking cessation rates. A non-significant reduction in weight gain was observed among participants who quit smoking in the physical activity group. Physical activity has a beneficial effect on the cardiovascular risk and the well-being in patients with type 2 diabetes. Thereby, both aerobic physical activities and resistance activities are recommended. DIAfit is a programm for patients with type 2 diabetes that is being implemented in Switzerland. Its objective is to allow the initiation of a structured physical activity in the setting of a pluridisciplinary team to promote a healthy lifestyle.

This is the second annual report on the One UN Programme in Cape Verde, covering the year 2010. The report high-lights joint efforts and achievements of the UN System, the Government and Civil Society within the framework of the One Programme. It includes both programmatic and financial reporting. It gives concrete examples of the development impact of the “Cape Verde Transition Fund” -- both how those funds were used, and how they complemented additional resources available to the UN system (e.g. core funds plus contributions resulting from other resource mobilization efforts). The report shows how UN system activities have supported national priorities as defined in the Cape Verdean Growth and Poverty Reduction Strategy Paper (GPRSP II).

This paper identifies selected issues and lessons learned from the implementation of a national program of prevention and control of non-communicable diseases (NCD) during the past 20 years in the Seychelles, a small island state in the African region. As early as in 1989, population-based surveys demonstrated high levels of several cardiovascular risk factors, which prompted an organized response by the government. The early creation of a NCD unit within the Ministry of Health, coupled with cooperation with international partners, enabled incremental capacity building and coherent development of NCD programs and policy. Information campaigns and screening for hypertension and diabetes in work/public places raised awareness and rallied increasingly broad awareness and support to NCD prevention and control. A variety of interventions were organized for tobacco control and comprehensive tobacco control legislation was enacted in 2009 (including total bans on tobacco advertising and on smoking in all enclosed public and work places). A recent School Nutrition Policy prohibits the sale of soft drinks in schools. At primary health care level, guidelines were developed for the management of hypertension and diabetes (these conditions are managed in all health centers within a national health system); regular interactive education sessions were organized for groups of high risk patients ("heart health club"); and specialized "NCD nurses" were trained. Decreasing prevalence of smoking is evidence of success, but the raising "diabesity epidemic" calls for strengthened health care to high-risk patients and broader multisectoral policy to mould an environment conducive to healthy behaviors. Key components of NCD prevention and control in Seychelles include effective surveillance mechanisms supplemented by focused research; generating broad interest and consensus on the need for prevention and control of NCD; mobilizing leadership and commitment at all levels; involving local and international expertise; building on existing efforts; and seeking integrated, multi-disciplinary and multisectoral approaches.

L'importance des déficits neurocognitifs des patients schizophrènes a conduit la section «E. Minkowski» du Département universitaire de psychiatrie adulte de Lausanne à développer un programme de remédiation cognitive pour jeunes patients schizophrènes. Les résultats préliminaires de ce programme indiquent une très nette amélioration des performances cognitives chez la plupart des patients. Les mécanismes cérébraux responsables de ces progrès ne sont cependant pas élucidés. De nouveaux éléments concernant les mécanismes de plasticité synaptique sont évoqués pour expliquer les modifications cérébrales associées aux fonctions cognitives exercées.
