Não existe consenso sobre a estratégia nutricional mais adequada para tratar a Síndrome Metabólica (SM), de tal forma que ocorra redução do risco cardiovascular. O presente estudo foi desenhado para avaliar a força de evidência dos benefícios de diferentes intervenções nutricionais na remissão da SM. A busca virtual foi realizada nas bases de dados Medline, Cochrane Library e PubMed, de ensaios clínicos randomizados publicados no período entre 1999 a 2009, em qualquer língua, em indivíduos com 18 anos ou mais e diagnóstico de SM, independente do critério. O operador booleano and foi utilizado na combinação dos MeSH terms "Metabolic Syndrome", "Síndrome x Metabólica" e "Metabolic Syndrome X"; dos entry terms "Dysmetabolic Syndrome X", Metabolic Cardiovascular Syndrome", "Metabolic X Syndrome", "Syndrome X, Metabolic" adicionados dos termos "diet", "intervention and diet", "treatment and diet" e "supplementation". Para cada estudo incluído na revisão foi estimada a Prevalência de SM e o Cálculo da Eficácia após o período de seguimento. Medidas de risco relativo para cada estudo foram descritas pelo Forest Plot. Foram identificados 131 artigos, os quais após critérios de elegibilidade resultaram em 15 estudos. Estes foram separados em quatro grupos: dieta normocalórica associada a exercícios; dieta normocalórica isolada; dieta hipocalórica associada a exercícios; e dieta hipocalórica isolada. Os ensaios com dieta hipocalórica associada à prática de exercícios apresentaram valores mais elevados de eficácia, colaborando para ressaltar os aspectos globais da mudança do estilo de vida no tratamento da SM, onde a alimentação saudável e reduzida em calorias deve ser complementada com a prática de atividade física.

FUNDAMENTO: Em face de definições de variáveis e critérios de amostragem, a real prevalência de hipertensão resistente em ambiente clínico é desconhecida. OBJETIVO: Investigar a prevalência de real hipertensão resistente em uma clínica de hipertensão arterial. MÉTODOS: Hipertensão resistente verdadeira foi diagnosticada quando fenômeno do jaleco branco, insuficiente adesão ao tratamento e hipertensão secundária foram excluídos em pacientes com Pressão Arterial (PA) ≥ 140/90 mmHg em duas visitas consecutivas, usando três de fármacos anti-hipertensivos, incluindo um diurético. RESULTADOS: No total, 606 pacientes, com 35 a 65 anos de idade, a maioria mulheres, com PA de 156,8 ± 23,8 mmHg por 91,9 ± 15,6 mmHg e IMC de 29,7 ± 5,9 Kg/m² foram sequencialmente avaliados. Cento e seis pacientes em uso de três agentes anti-hipertensivos estavam com pressão arterial não controlada (17,5% da amostra total) na primeira visita. Oitenta e seis pacientes (81% dos pacientes com PA não controlada na primeira avaliação) retornaram para a avaliação de confirmação: 25 estavam com PA controlada; 21 tinham evidência de baixa adesão ao tratamento; 13 tinham fenômeno do jaleco branco; e 9 tinham hipertensão secundária, restando 18 pacientes (20,9% dos não controlados na consulta de confirmação e 3% da amostra total) com verdadeira hipertensão resistente. Considerando pacientes com hipertensão secundária como casos de hipertensão refratária, a prevalência de hipertensão resistente aumentou para 4,5%. CONCLUSÃO: A frequência de hipertensão resistente verdadeira em pacientes não idosos é baixa em um ambiente clínico, e não é substancialmente aumentada com a inclusão de pacientes com hipertensão secundária. Background: Postprandial Lipemia (PPL) is a physiological process that reflects the ability of the body to metabolize lipids. Even though the influence of oral contraceptives (OC) on PPL is not known, it is a known fact that their use increases fasting lipid values. Objective: To compare the PPL between women who are on OC and those who are not. Methods: A prospective analytical study which assessed eutrophic women, aged between 18 and 28 years old, who were irregularly active and with fasting triglycerides ≤150 mg/dL. They were divided into two groups: oral contraceptive group (COG) and non-oral contraceptive group (NCOG). Volunteers were submitted to the PPL test, in which blood samples were collected in time 0 (12-hour fasting) and after the intake of lipids in times 180 and 240 minutes. In order to compare the triglyceride deltas, which reflect PPL, the two-tailed Mann-Whitney test was used for independent samples between fasting collections and 180 minutes (Δ1) and between fasting and 240 minutes (Δ2). Results: Forty women were assessed and equally divided between groups. In the fasting lipid profile, it was observed that HDL did not present significant differences and that triglycerides in COG were twice as high in comparison to NCOG. Medians of Δ1 and Δ2 presented significant differences in both comparisons (p ≤0.05). Conclusion: The results point out that women who are irregularly active and use OC present more PPL in relation to those who do not use OC, which suggests that in this population, its chronic use increases the risk of heart conditions.

Background: Although exercise training is known to promote post-exercise hypotension, there is currently no consistent argument about the effects of manipulating its various components (intensity, duration, rest periods, types of exercise, training methods) on the magnitude and duration of hypotensive response. Objective: To compare the effect of continuous and interval exercises on hypotensive response magnitude and duration in hypertensive patients by using ambulatory blood pressure monitoring (ABPM). Methods: The sample consisted of 20 elderly hypertensives. Each participant underwent three ABPM sessions: one control ABPM, without exercise; one ABPM after continuous exercise; and one ABPM after interval exercise. Systolic blood pressure (SBP), diastolic blood pressure (DBP), mean arterial pressure (MAP), heart rate (HR) and double product (DP) were monitored to check post-exercise hypotension and for comparison between each ABPM. Results: ABPM after continuous exercise and after interval exercise showed post-exercise hypotension and a significant reduction (p < 0.05) in SBP, DBP, MAP and DP for 20 hours as compared with control ABPM. Comparing ABPM after continuous and ABPM after interval exercise, a significant reduction (p < 0.05) in SBP, DBP, MAP and DP was observed in the latter. Conclusion: Continuous and interval exercise trainings promote post-exercise hypotension with reduction in SBP, DBP, MAP and DP in the 20 hours following exercise. Interval exercise training causes greater post-exercise hypotension and lower cardiovascular overload as compared with continuous exercise.

Seven new species of Trichorhina Budde-Lund, 1908 are described, six from Southeastern Brazil, T. biumbonata sp. nov., T. lenkoi sp. nov., T. myrmecophila sp. nov., T. orensis sp. nov., T. sexdens sp. nov., T. tropidocerata sp. nov., and one from Central Brazil, T. crassisetae sp. nov. An emended diagnosis for the genus and a key to the 19 species recorded from Brazil are provided. Trichorhina acuta Araújo & Buckup, 1994 is recorded to state of Mato Grosso do Sul.

Os métodos de imunoperoxidase têm muito em comum com os da imunofluorescência para a demonstração de antígenos teciduais e celulares como no campo das doenças renais relacionadas a imunoglobinas e imunocomplexos. Neste trabalho os autores fazem um estudo comparativo da sensibilidade dos dois métodos (IF e PAP) em tecido de congelação e blocos parafinizados de material de biópsia renal. A análise estatística dos resultados mostrou uma concordãncia significativa entre a IF de congelação e a imunoperoxidase em tecido parafinizado com exceções para a detecção de frações de complemento e de fibrinogênio. Não houve concordância entre a IF em congelação e IF em tecido parafinizado.

Os autores descrevem a padronização de técnicas histoquímicas, fosfatase ácida, alfa-naftil-acetato esterase e peroxidase para identificação de linfócitos e macrófagos em tecido. Recomenda-se a fixação em formol sacarose tamponado e inclusão em goma de Holt como a mais eficaz ja que não só conserva a atividade enzimática, como também e de realização acessivel.

A study of colicinogeny was made in 748 strains of Salmonella (97 serovars) isolated from different sources; human (291), animal (119), environmental (141), food (102) and animal feed (95). Colicin production was detected in 64 strains (8.6%), particularly isolated from foods (30.4%). Col. E1 (53) and Ia (44) were the most frequently observed, especially in S. agona for environment and food sources. Col V production was identified in 5 strains of S. typhimurium within 8 producer cultures isolated from humans. Its relationship with the sources and serovars of Salmonella are discussed.

Tandemly repeated DNA sequences are found in the genome of higher eukaryotes, and have also been demonstrated in Trypanosoma cruzi. Repeated DNA sequences are potentially useful for the diagnostic detection of T. cruzi (A. Gonzales et al., 1984, Proc. Natl. Acad. Sci. USA, 81: 3356-3360). We have isoleted two clones from a genomic library of T. cruzi (Y strain) that contain, in one clone a family of at least seven copies of a repetitive sequence of approximately 600 base pairs, and in the other an independent copy of the same sequence. One copy of the repetition (HSP) and the independent clone (HCR) were sequenced by the Sanger procedure (Fig.). This sequence hybridized to four strains of T. cruzi tested and did not hybridize to eleven species of trypanosotids from five different Genera, being a good candidate for diagnostic assays. Radiotherapy is a widely used treatment option in cancer. However, recent evidence suggests that doses of ionizing radiation (IR) delivered inside the tumor target volume, during fractionated radiotherapy, can promote tumor invasion and metastasis. Furthermore, the tissues that surround the tumor area are also exposed to low doses of IR that are lower than those delivered inside the tumor mass, because external radiotherapy is delivered to the tumor through multiple radiation beams, in order to prevent damage of organs at risk. The biological effects of these low doses of IR on the healthy tissue surrounding the tumor area, and in particular on the vasculature remain largely to be determined. We found that doses of IR lower or equal to 0.8 Gy enhance endothelial cell migration without impinging on cell proliferation or survival. Moreover, we show that low-dose IR induces a rapid phosphorylation of several endothelial cell proteins, including the Vascular Endothelial Growth Factor (VEGF) Receptor-2 and induces VEGF production in hypoxia mimicking conditions. By activating the VEGF Receptor-2, low-dose IR enhances endothelial cell migration and prevents endothelial cell death promoted by an anti-angiogenic drug, bevacizumab. In addition, we observed that low-dose IR accelerates embryonic angiogenic sprouting during zebrafish development and promotes adult angiogenesis during zebrafish fin regeneration and in the murine Matrigel assay. Using murine experimental models of leukemia and orthotopic breast cancer, we show that low-dose IR promotes tumor growth and metastasis and that these effects were prevented by the administration of a VEGF receptor-tyrosine kinase inhibitor immediately before IR exposure. These findings demonstrate a new mechanism to the understanding of the potential pro-metastatic effect of IR and may provide a new rationale basis to the improvement of current radiotherapy protocols.

The effect of the human immunodeficiency virus (HIV) infection on IgG production against purified protein derivative (PPD) and 2,3-diacil-trehalose (SL-IV) was investigated by an enzyme-linked immunosorbent assay (ELISA) test. Comparison between the antigens showed that immunocompetent patients produce preferentially antibodies to SL-IV than to PPD (73.3% versus 63.3%). Combination of these results showed an increase of the sensitivity to 80%, which decreased over the spectrum of immunodepression caused by HIV. In the tuberculous HIV seropositive group the sensitivities of SL-IV and PPD were 36.4% versus 40% and 0% versus 22.2% in the tuberculosis/acquired immunodeficiency syndrome (TB/AIDS) group. Combination of these results gave respectively 54.5% and 20%, showing that serological tests have limited value for diagnosis of tuberculosis in HIV infected patients. High antibody levels were observed in HIV seropositive asymptomatic group, but only two individuals were positive for both antigens. In the follow up, one of them developed tuberculous lymphadenitis, indicating that further work is needed to access the value of serological tests in predicting tuberculosis in HIV infected individuals.

Six hundred and eleven random-source dogs (338 male, 273 female) one year of age or older, from six sections of the city of Recife, Pernambuco, were examined antemortem for circulating microfilariae Dirofilaria immitis and Dipetalonema reconditum adult heartworm (D. immitis) antigen, and examined postmortem for adult heartworms. The prevalence of heartworm infection was 2.3% (14/611), as determined by necropsy for adult worms, and 1% (6/611) had circulating microfilariae of D. immitis; thus, 57.1% of the heartworm-infected dogs had occult infections. The results of serological testing indicated that 1.3% (8/611) of the dogs were positive for adult heartworm antigen. A total of 42 (6.9%) of the dogs had microfilariae of D. reconditum; 40 of these had only D. reconditum and two additional dogs had microfilariae of both species, D. immitis and D. reconditum.

In order to evaluate the predictive value of acid fast bacilii (AFB) smear for the diagnosis of Mycobacterium tuberculosis in respiratory specimens in a setting with a high prevalence of Aids and an unknown prevalence of nontuberculous mycobacteria (NTM), we retrospectively examined specimens cultured for mycobacteria between 1 September 1993 and 30 September 1994 and medical records of patients with positive culture in a General Hospital, Aids reference in Rio de Janeiro, Brazil. Seventy three per cent (1517/2077) of samples were respiratory specimens and mycobacteria were recovered from 20.6% (313/1517) of these. M. tuberculosis was identified in 94.2% (295/313) and NTM in 5.8% (18/313). The yield of positive AFB smear and of positive culture was 6.1% (93/1517) and 20.6% (313/1517), respectively. The positive predictive value (PPV) of AFB for M. tuberculosis was 98.4% in expectorated sputum and 96.4% in bronchoalveolar lavage. Forty four percent (130/295) of specimens with positive culture for M. tuberculosis and 66.7% (12/18) for NTM were from patients HIV positive. The conclusion was that in our study population, the PPV of AFB for M. tuberculosis in respiratory specimens was high and the prevalence of NTM was low despite the high prevalence of HIV positive.

In a previous work we demonstrated that diarylheptanoids extracted from Centrolobium sclerophyllum are very active against Leishmania amazonensis promastigotes. In order to continue our studies with these class of compounds, we decided to evaluate the activity of several diarylheptanoids derived from curcumin (diferuloyl methane) against the extracellular form (promastigotes) of L. amazonensis. Furthermore, an experiment against the intracellular form of the parasite (amastigotes) was carried out, comparing the most active compound among the curcumin derivatives (the methylcurcumin) with des-O-methylcentrolobine, the most active diarylheptanoid derived from C. sclerophyllum.
