The study of anatomy has changed enormously in the last few decades. No longer do medical students have to spend long hours in the dissecting room searching fruitlessly for the otic ganglion or tracing the small arteries that form the anastomosis round the elbow joint. They now need to know only the basic essentials of anatomy with particular emphasis on their clinical relevance and this is a change that is long overdue. However, students still have examinations to pass and in this book the authors, a surgeon and an anatomist, have tried to provide a means of rapid revision without any frills. To this end, the book follows the standard format of the at a Glance series and is arranged in short, easily digested chapters, written largely in note form, with the appropriate illustrations on the facing page. Where necessary, clinical applications are included in italics and there are a number of clinical illustrations. We thus hope that this book will be helpful in revising and consolidating the knowledge that has been gained from the dissecting room and from more detailed and explanatory textbooks.

RESUMO Padrões de riqueza de espécies, abundância, diversidade, equitabilidade e dominância, além da organização em guildas tróficas e comportamentais da fauna de Scarabaeinae foram analisados em três diferentes fitofisionomias (floresta, campo natural e lavoura) na região noroeste do estado do Rio Grande do Sul, Brasil, entre setembro e novembro de 2012, através da utilização de armadilhas de queda iscadas com distintos recursos (fezes, carne em decomposição e banana fermentada). Foram capturados 9.325 espécimes, distribuídos em nove gêneros e 32 espécies. As espécies mais abundantes foram Onthophagus aff.tristis Harold, 1873, Canthidium aff.trinodosum (Boheman, 1858), Canthon aff.fallax Harold, 1868, Canthon lividusBlanchard, 1845 e Deltochilum morbillosum Burmeister, 1848 que representaram 65,54% do total de indivíduos amostrados. A floresta apresentou os valores mais elevados de número de indivíduos e de espécies. A menor riqueza foi observada no campo, enquanto a lavoura apresentou a menor abundância de indivíduos. Scarabaeinae apresentou diferenças quali- e quantitativas em suas assembleias pelas fitofisionomias amostradas. A floresta abrigou a maior riqueza observada de espécies e uma fração destas é exclusiva deste ambiente, e dificilmente ocorre em outros tipos de ecossistemas. A floresta apresentou uma proporção maior de espécies generalistas ou copro-necrófagas em sua composição. Outra parte desta fauna, representada por espécies coprófagas, está adaptada ao ambiente aberto indicando uma modificação na guilda alimentar causada pela substituição da floresta por pastagem. Dessa forma, em um contexto mais amplo a paisagem pode desempenhar um papel importante na diversidade de Scarabaeinae.

A - NORTE - A floresta em tôrno de Belém parece já bastante alterada, sendo poucas as árvores de grande porte. O solo é sempre arenoso, com uma camada de húmus muito fina a qual, quando é removida, deixa ver uma areia branca. Em Utinga era de notar o grande número de árvores caídas, justamente as maiores, o que sugere um solo pobre e raso, originando um enraizamento superficial e conseqüente queda das mesmas pelos ventos. Na floresta de àgua Preta é muito maior o número de árvores finas, o que sugere um solo ainda mais pobre. Trata-se de floresta típica da planície (Quartenário antigo), onde o solo foi formado por deposição do arenitos pobres do Centro-oeste capturados pela bacia do Tocatins. B - Nordeste - 1. Caatinga: O tipo de vegetação que predomina no Nordeste é o da caatinga, vegetação de arbustiva a arbórea, espinhosa e decidual. A parte vista, de Terezina até Paulo Afonso, via Ubajara, Fortaleza, Crato e Floresta, apresenta várias nuances fisiográficas de densidade e porte. Essas são devidas, aparentemente ao menos, não só à intervenção humana, como também à grande variação na constituição física do solo. Assim é que observamos: a) Caatinga arbórea - dominada pela carnaúba: Copernicia cerifera (A. Cam.) Mart. nos lugares encharcados, pela oiticica: Licania rigida Benth. na linha demarcatória das maiores enchentes, e pelo juàzeiro: Zizyphus joazeiro Mart., fora da influência das àguas dos rios. b) Caatinga aberta - nos solos pedregosos, com dominância da imburana-de-espinho: Bursera leptophloeos Engl. e do umbu: Spondias tuberosa A. Cam. Ocorre nas enconstas dessecadas dos serrotes de quartzito. c) Caatinga arbustiva densa - existente nos largos vales dos maiores rios, provável fruto da intervenção...

The Brazilian Institute for Agrarian Reform is promoting researches on soil fertility in southern Mato Grosso, complemented by a survey of the main types of vegetation. The data obtained were also to be used to build up the legends for the map of vegetation. Photos were taken by the authors and botanical material collected in order to permit the identification of important species. Observations made led to the above table of main physiognomies.

Although medicine is practised in a secular setting, religious and spiritual issues have an impact on patient perspectives regarding their health and the management of any disorders that may afflict them. This is especially true in psychiatry, as feelings of spirituality and religiousness are very prevalent among the mentally ill. Clinicians are rarely aware of the importance of religion and understand little of its value as a mediating force for coping with mental illness. This book addresses various issues concerning mental illness in psychiatry: the relation of religious issues to mental health; the tension between a theoretical approach to problems and psychiatric approaches; the importance of addressing these varying approaches in patient care and how to do so; and differing ways to approach Christian, Muslim, and Buddhist patients. This is the first book to specifically cover the impact of religion and spirituality on mental illness.

Pregnancy reduces maternal risk of breast cancer in the long term, but the biological determinants of the protection are unknown. Animal experiments suggest that estrogens and progesterone could be involved, but direct human evidence is scant. A case-control study (536 cases and 1,049 controls) was nested within the Finnish Maternity Cohort. Eligible were primiparous women who delivered at term a singleton offspring before age 40. For each case, two individually matched controls by age (±6 months) and date of sampling (±3 months) were selected. Estradiol, estrone and progesterone in first-trimester serum were measured by high-performance liquid chromatography tandem mass spectrometry and sex-hormone binding globulin (SHBG) by immunoassay. Odds ratios (OR) and 95% confidence intervals (CI) were estimated through conditional logistic regression. In the whole study population there was no association of breast cancer with any of the studied hormones. In analyses stratified by age at diagnosis, however, estradiol concentrations were positively associated with risk of breast cancer before age 40 (upper quartile OR, 1.81; CI, 1.08-3.06), but inversely associated with risk in women who were diagnosed ≥age 40 (upper quartile OR, 0.64; CI, 0.40-1.04), p(interaction) 0.004. Risk estimates for estrone mirrored those for estradiol but were less pronounced. Progesterone was not associated with risk of subsequent breast cancer. Our results provide initial evidence that concentrations of estrogens during the early parts of a primiparous pregnancy are associated with maternal risk of breast cancer and suggest that the effect may differ for tumors diagnosed before and after age 40.

A new trypanosomatid species, Blastocrithidia cyrtomeni, is herein described using morphological and molecular data. It was found parasitising the alimentary tract of the insect host Cyrtomenus bergi, a polyphagous pest. The morphology of B. cyrtomeni was investigated using light and transmission microscopy and molecular phylogeny was inferred from the sequences of spliced leader RNA (SL rRNA) - 5S rRNA gene repeats and the 18S small subunit (SSU) rRNA gene. Epimastigotes of variable size with straphanger cysts adhering to the middle of the flagellum were observed in the intestinal tract, hemolymph and Malpighian tubules. Kinetoplasts were always observed anterior to the nucleus. The ultrastructure of longitudinal sections of epimastigotes showed the flagellum arising laterally from a relatively shallow flagellar pocket near the kinetoplast. SL RNA and 5S rRNA gene repeats were positive in all cases, producing a 0.8-kb band. The amplicons were 797-803 bp long with > 98.5% identity, indicating that they originated from the same organism. According to the sequence analysis of the SL-5S rRNA gene repeats and the 18S SSU rRNA gene, B. cyrtomeni is different from all other known species or isolates of Trypanosomatidae. Both analyses indicate that among known species, it is most closely related to Blastocrithidia triatomae.

BACKGROUND: Insulin-like growth factor-I (IGF-I) and C-reactive protein (CRP) may be positively associated with the risk of epithelial ovarian cancer (EOC) but no previous studies have investigated their associations with non-epithelial ovarian cancers (NEOC). METHODS: A case-control study was nested within the Finnish Maternity Cohort. Case subjects were 58 women diagnosed with sex cord-stromal tumors (SCST) and 30 with germ cell tumors (GCT) after recruitment. Control subjects (144 for SCST and 74 for GCT) were matched for age, parity, and date of blood donation of the index case. RESULTS: Doubling of IGF-I concentration was not related to maternal risk of either SCST (OR 0.97, 95% CI 0.58-1.62) or GCT (OR 1.13, 95% CI 0.51-2.51). Similarly, doubling of CRP concentrations was not related to maternal risk of either SCST (OR 1.10, 95% CI 0.85-1.43) or GCT (OR 0.93, 95% CI 0.68-1.28). CONCLUSIONS: Pre-diagnostic IGF-I and CRP concentrations during the first trimester of pregnancy were not associated with increased risk of NEOC in the mother. Risk factors for NEOC may differ from those of EOC.

Background: Although combination antiretroviral therapy (cART) dramatically reduces rates of AIDS and death, a minority of patients experience clinical disease progression during treatment. <p>Objective: To investigate whether detection of CXCR4(X4)-specific strains or quantification of X4-specific HIV-1 load predict clinical outcome. Methods: From the Swiss HIV Cohort Study, 96 participants who initiated cART yet subsequently progressed to AIDS or death were compared with 84 contemporaneous, treated nonprogressors. A sensitive heteroduplex tracking assay was developed to quantify plasma X4 and CCR5 variants and resolve HIV-1 load into coreceptor-specific components. Measurements were analyzed as cofactors of progression in multivariable Cox models adjusted for concurrent CD4 cell count and total viral load, applying inverse probability weights to adjust for sampling bias. Results: Patients with X4 variants at baseline displayed reduced CD4 cell responses compared with those without X4 strains (40 versus 82 cells/mu l; P= 0.012). The adjusted multivariable hazard ratio (HR) for clinical progression was 4.8 [95% confidence interval (Cl) 2.3-10.0] for those demonstrating X4 strains at baseline. The X4-specific HIV-1 load was a similarly independent predictor, with HR values of 3.7(95%Cl, 1.2-11.3) and 5.9 (95% Cl, 2.2-15.0) for baseline loads of 2.2-4.3 and > 4.3 log(10)copies/ml, respectively, compared with < 2.2 log(10)copies/ml. Conclusions: HIV-1 coreceptor usage and X4-specific viral loads strongly predicted disease progression during cART, independent of and in addition to CD4 cell count or total viral load. Detection and quantification of X4 strains promise to be clinically useful biomarkers to guide patient management and study HIV-1 pathogenesis.

BACKGROUND: Elevated serum concentrations of insulin-like growth factor (IGF)-1 have been associated with increased risk of breast cancer. Previously, we reported a similar association in samples obtained during pregnancy. The current study was conducted to further characterize the association of IGF-1 during pregnancy with maternal breast cancer risk. METHODS: A case-control study was nested within the Finnish Maternity Cohort. The study was limited to primiparous women less than 40 years of age, who donated blood samples during early (median, 12 weeks) pregnancy and delivered a single child at term. Seven hundred and nineteen women with invasive breast cancer were eligible. Two controls (n = 1,434) were matched to each case on age and date at blood donation. Serum IGF-1 concentration was measured using an Immulite 2000 analyzer. Conditional logistic regression was used to estimate odds ratios (OR) and 95% confidence intervals (CI).RESULTS: No significant associations were observed between serum IGF-1 concentrations and breast cancer risk in both the overall analysis (OR 1.08 (95% CI 0.80-1.47) and in analyses stratified by histological subtype, lag-time to cancer diagnosis, age at pregnancy or age at diagnosis.CONCLUSIONS: There was no association between IGF-1 and maternal breast cancer risk during early pregnancy in this large nested case-control study.Impact:Serum IGF-1 concentrations during early pregnancy may not be related to maternal risk of breast cancer.
