This article begins from the assumption (which may seem controversial to many) that anyone who thinks that our current economic crisis is a temporary blip until ‘normal service’ (i.e. a return to ‘business as usual’) is resumed, profoundly misunderstands the severity and significance of what’s happening to the global economy and its impacts on the future prosperity of the island of Ireland. The economic recession represents nothing short of a re-structuring of the global economy and the creation of a new dispensation between governments, markets and citizens. The full implications of the re-regulation of the market, with the state bailing out and part nationalising the financial sector in both jurisdictions on the island (as in other parts of the world) have yet to be seen, but what we are witnessing is the emergence of a new economic model. Those who think we can, or even ought to, return to the pre-2008 economic model, are gravely mistaken. The current economic downturn marks the end of the ‘neo-liberal’ model and the beginnings of the transition (an inevitable transition, this article will argue) towards a new low carbon, renewable, green and sustainable economy and society.

Birds are capable of true navigation, the ability to return to a known goal from a place they have never visited before. This is demonstrated most spectacularly during the vast migratory journeys made by these animals year after year, often between continents and occasionally global in nature. However, it remains one of the great unanswered questions in science, despite more than 50 years of research in this field. Nevertheless, the study of true navigation in birds has made significant advances in the previous 20 years, in part thanks to the integration of many disciplines outside its root in behavioural biology, to address questions of neurobiology, molecular aspects, and the physics of sensory systems and environmental cues involved in bird navigation, often involving quantum physics. However, true navigation remains a controversial field, with many conflicting and confusing results making interpretation difficult, particularly for those outside or new to the field. Unlike many general texts on migration, which avoid discussion of these issues, this review will present these conflicting findings and assess the state of the field of true navigation during bird migration.

This paper contributes to a debate on what constitutes rehabilitation. Current criminal justice practice tends to focus on lowering recidivism by utilising strategies geared towards cognitive behavioural modification and educational/vocational skill development. The paper focuses on the perspectives of custodial educators in a Juvenile Justice Centre in Northern Ireland. Their definition of rehabilitation is less concerned about lowering recidivism and instead focuses more on meeting the needs of the young people entering custody, more so than preparing them for their return to the community. Education staff present a model of rehabilitation that is fundamentally about improving the lives of young people. Despite expecting young people to return to custody Education staff contend that young people’s lives improved because they were exposed to a welcoming, caring and pro‐social environment which has helped the young people transform inttody.

As Américas constituem a mais relevante fonte inter-regional de turistas vindos para a Europa, sendo os EUA de longe o maior mercado de long haul para a Europa. Mas, apesar da relevância deste mercado nas estatísticas, há uma carência de estudos sobre o mesmo. A maioria dos Norte-Americanos que visitam a Europa mantéem-se em áreas urbanas, limitando o potencial efeito do turismo para equilibrar o crescimento económico na Europa, principalmente em áreas rurais que têm vindo cada vez mais a atrair turistas nas últimas décadas, contando sobretudo da EU. Para o turismo ser realmente uma força positiva de desenvolvimento equilibrado na Europa, os pesquisadores do comportamento do consumidor devem tentar entender por que os viajantes Norte-Americanos não arriscam ir para zonas rurais que supostamente têm o capital tradicional e cultural autêntico que sobretudo os turistas nostálgicos procuram. Esta dissertação pretende contribuir para o conhecimento do mercado Norte-Americano que viaja para a Europa. O objectivo deste estudo centra-se na examinação do papel dos constrangimentos, da nostalgia, e da experiência de viagem para as intenções dos turistas Norte-Americanos de revisitar em destinos rurais Europeus. Apesar de existirem referências à nostalgia em pesquisas de turismo, estas centram-se essencialmente em estudos de carácter qualitativo e tem sido dada pouca relevância à construção de ferramentas quantitativas para o estudo da nostalgia a nível do turismo. Neste estudo, é desenvolvida uma escala de nostalgia para o turismo (NOSTOUR). Os resultados mostram que a nostalgia, relacionada com as viagens pode ser concebida e operacionalizada em quatro dimensões: individual, interpessoal, cultural e virtual. A nostalgia, por si só, não tem um efeito mediador nos constrangimentos para viajar a nível da intenção de regressar e visitar destinos rurais na Europa mas com a adição, do determinante experiência de viagem esse efeito existe.

Describes the innovative approach adopted in a UK business school to improve the number of student placements. A module was designed to provide students with the skills to search, apply for and gain a year-long placement as part of a degree ‘sandwich’ course. A blend of workshops, recorded lectures, online formative feedback exercises and one-to-one career coaching created a tailored, practical approach to skill enhancement. This session provides the presentation of the adopted methodology the results of the evaluative research, a live demonstration of the coaching approach and a discussion with the audience of ideas for development of the approach.

Resilience is the intrinsic capacity which allows individuals to adapt to adverse situations. Among unemployed, resilience obtains a particular importance as a must-required skill to face unemployment and make it possible to return to the labour market. The present work aims at discover which social and individual aspects are most responsible to increase resilience levels among the unemployed. In order to find those aspects, a questionnaire was applied to a sample of Portuguese unemployed. The results were then analysed and interpreted, and some of the possible solutions able to increase resilience levels among the Portuguese unemployed were listed and justified.

BACKGROUND: Poorly controlled cardiovascular risk factors are common. Evaluating whether physicians respond appropriately to poor risk factor control in patients may better reflect quality of care than measuring proportions of patients whose conditions are controlled. OBJECTIVES: To evaluate therapy modifications in response to poor control of hypertension, dyslipidemia, or diabetes in a large clinical population. DESIGN: Retrospective cohort study within an 18-month period in 2002 to 2003. SETTING: Kaiser Permanente of Northern California. PATIENTS: 253,238 adult members with poor control of 1 or more of these conditions. MEASUREMENTS: The authors assessed the proportion of patients with poor control who experienced a change in pharmacotherapy within 6 months, and they defined "appropriate care" as a therapy modification or return to control without therapy modification within 6 months. RESULTS: A total of 64% of patients experienced modifications in therapy for poorly controlled systolic blood pressure, 71% for poorly controlled diastolic blood pressure, 56% for poorly controlled low-density lipoprotein cholesterol level, and 66% for poorly controlled hemoglobin A1c level. Most frequent modifications were increases in number of drug classes (from 70% to 84%) and increased dosage (from 15% to 40%). An additional 7% to 11% of those with poorly controlled blood pressure, but only 3% to 4% of those with elevated low-density lipoprotein cholesterol level or hemoglobin A1c level, returned to control without therapy modification. Patients with more than 1 of the 3 conditions, higher baseline values, and target organ damage were more likely to receive "appropriate care." LIMITATIONS: Patient preferences and suboptimal adherence to therapy were not measured and may explain some failures to act. CONCLUSIONS: As an additional measure of the quality of care, measuring therapy modifications in response to poor control in a large population is feasible. Many patients with poorly controlled hypertension, dyslipidemia, or diabetes had their therapy modified and, thus, seemed to receive clinically "appropriate care" with this new quality measure.

BACKGROUND: The treatment of status epilepticus (SE) is based on relatively little evidence although several guidelines have been published. A recent study reported a worse SE prognosis in a large urban setting as compared to a peripheral hospital, postulating better management in the latter. The aim of this study was to analyse SE episodes occurring in different settings and address possible explanatory variables regarding outcome, including treatment quality. METHODS: Over six months we prospectively recorded consecutive adults with SE (fit lasting five or more minutes) at the Centre Hospitalier Universitaire Vaudois (CHUV) and in six peripheral hospitals (PH) in the same region. Demographical, historical and clinical variables were collected, including SE severity estimation (STESS score) and adherence to Swiss SE treatment guidelines. Outcome at discharge was categorised as "good" (return to baseline), or "poor" (persistent neurological sequelae or death). RESULTS: Of 54 patients (CHUV: 36; PH 18), 33% had a poor outcome. Whilst age, SE severity, percentage of SE episodes lasting less than 30 minutes and total SE duration were similar, fewer patients had a good outcome at the CHUV (61% vs 83%; OR 3.57; 95% CI 0.8-22.1). Mortality was 14% at the CHUV and 5% at the PH. Most treatments were in agreement with national guidelines, although less often in PH (78% vs 97%, P = 0.04). CONCLUSION: Although not statistically significant, we observed a slightly worse SE prognosis in a large academic centre as compared to smaller hospitals. Since SE severity was similar in the two settings but adherence to national treatment guidelines was higher in the academic centre, further investigation on the prognostic role of SE treatment and outcome determinants is required.

This is a mixed methodology study that uses an autoethnographic approach to combine both an autobiography and a survey of practitioners who work in children’s mental health. It is largely about the implementation of Evidence-Based Practice (EBP), and the questions, concerns, experiences that I have had, and compared them with those of my fellow practitioners. In addition, it is about my journey as a mental health professional, and how I have come to recognize that in order to achieve the goals I wanted to achieve, I needed to return to university to pursue a Master’s degree. Within the research, I identify and discuss different definitions of EBP and identify several themes. I deconstruct the implementation of EBPs through the lens of Foucault and his notions of governmentality. I offer policy and practice recommendations to improve the implementation of EBP and the services received by children facing mental health issues.

This study investigated parent and child perspectives of childhood cancer survivors' return to school. Four specific areas were examined including cognitive and academic concerns, social issues, perceived support, and the impact on siblings. Participants consisted of parents and childhood cancer survivors who were recruited through a regional parent support group. Data was collected during a focus group and interviews. Using a descriptive content analysis, results indicated that participants generally received the necessary resources, however issues such as consistency and having to advocate in order to attain these resources served as barriers for the families.

This study examines and describes athletes’ felt sense of readiness returning to play following a concussion. Analyses of the interviews yielded a description of each participant’s experiences with concussions. Descriptions of this phenomenon generated by informants provide a detailed account of the unique issues athletes face when returning to play following a concussion. Participants’ descriptions highlight that in order to play, an athlete knows that he/she ought to be emotionally and physically ready to play. However, the athletes in this study believe that there is not an actual test that can “prove” this and that they can choose to lie and/or cheat the tests to return to play while they are still symptomatic. Athletes, parents, coaches, and trainers will benefit from learning to be better educated on the severity of concussions, concussion detection, assessment and the serious health consequences that can result from playing with a concussion.
