Most of the critical studies of Baroque novels written in Spanish America during the 1960s and 1970s are characterized by a limited examination of their formal and stylistic representations. This dissertation explored the way in which certain writers developed a new Baroque tendency, the so-called Neobarroco, that presented a particular vision of history. José Lezama Lima, Reinaldo Arenas and Severo Sarduy developed innovative fictional and historiographic perceptions as alternative discourses to understand and perceive the cultural intricacies of Cuba and the New World. Their novels posited an elaborated poetic theory of history that can be summarized by the principle of supratemporal analogies, interweaved by a "metaphoric subject" that makes possible the conception of "imaginary eras". Since this poetry arises from a network of metaphoric correspondences, the image is conceived as a cultural creation that acts upon reality. ^ Although this study traced the trajectory of their writings from the point of view of their own essays, our focus was on the act of recovering the past as reshaped forms that are present in the memory. Paradiso, El mundo alucinante and De donde son los cantantes exemplified the attempt to place Americanness within the realm of poetics and history as one single discourse constructed by a combination of self-consciousness and historiographic meditation. ^ Basing my thesis on postmodernist theory (Ihab Hassan, Brian McHale, Linda Hutcheon) and philosophies of history (Michel Foucault, Hayden White, Keith Jenkins, Dominick LaCapra), I argued that the antagonistic paradoxes faced by postmodernism were reconcilable tendencies of the Neobarroco prior to the actual debate on the postmodern condition. The aesthetic trend initiated by these writers and their reading of history confronted the official historiographic discourse, thus empowering a contemporary voice in the current debate on historical skepticism. ^

This study focuses on the works of Cuban writer Reinaldo Arenas, one of the most prolific and controversial Latin American authors in the second half of the twentieth century. First, I propose Arenas as the creator of the Cuban revolutionary novel (a term coined by critics when referring to the narrative written after the revolution), within the scope of postmodern historiographic metafiction and against the trend of the official revolutionary novel promoted by the political establishment. Through the analysis of the five novels of the pentagony and other texts, my study follows the tragic journey of the antihero protagonist, from adolescence into adulthood, registering the correlation between his existential crisis and the narrative historical discourse. Contemporary Cuba from 1959 onwards—the historical-political circumstances that afflicted and overwhelmed him the most—becomes the point of reference to deconstruct reality and reaffirm the existence of a “self” threatened by the violence of a totalitarian discourse. Out of the fragments of this reality, Arenas undertook a radical reconstruction in which he inverted and questioned every inherited cultural value, as well as the power structures. Within this context, Arenas projects what I call “the Cuban hideous unreal”, an ontological and literary vision antagonistic to the carpentirean concept of the American “marvelous real”. ^ Despite the ostracism Reinaldo Arenas suffered for ten years, this study shows how he established through his work a meditative dialogue with himself and the common man. This perspective formulates a permanent literary and philosophic reflection with thinkers and writers of his country and the West, as the basis for a rejection of the Cuban reality. The resultant interdisciplinary and postmodern dialogue constitutes one of the most significant and distinctive contributions of his work. ^

Segundo a Organização Mundial de Saúde denomina-se gravidez na adolescência a gestação ocorrida em jovens de 10 a 20 anos (exclusive) que se encontram, portanto, em pleno desenvolvimento dessa fase da vida - a adolescência. Esse tipo de gravidez em geral não foi planejada, nem desejada e acontece em meio a relacionamentos sem estabilidade. A gravidez precoce é uma das ocorrências mais preocupantes relacionadas à sexualidade da adolescência, com sérias conseqüências para a vida dos adolescentes envolvidos, de seus filhos que nascerão ou não e de suas famílias. Este estudo tem o objetivo de elaborar um plano de ação para prevenção da ocorrência de gravidez na adolescência na área da ESF Wandy de Moraes Silva-Itaguara/MG o qual compreende os seguintes passos: definição do problema; priorização do problema; descrição do problema priorizado; explicação do problema; selecionação dos nós críticos; desenho das operações; identificação dos recursos críticos; análise de viabilidade do plano de ação; elaboração do plano operativo e gestão do plano. O plano de ação realizado pela ESF Wandy de Moraes Silva é aplicável, pois, possui todos os recursos necessários para sua realização e superação dos nós críticos que foram identificados. Isso será possível por meio da execução de projetos intersetoriais.

Este trabalho foi realizado no município de Campo Largo-PR como parte das atividades do curso de especialização em Saúde da Família modalidade a distância, através da Universidade Aberta do SUS com a Universidade Federal de Pelotas, visando melhorar a saúde bucal dos escolares dos 6 aos 12 anos da Escola Municipal Nicolau Moraes de Castro. O projeto de intervenção foi desenvolvido através de ações em quatro eixos pedagógicos: monitoramento e avaliação, organização e gestão do serviço, engajamento público e qualificação da prática clínica. Elaboramos uma ficha-espelho para os registros de cada aluno, transcrevendo as informações na planilha de coleta de dados semanalmente. Foram capacitados os funcionários da escola e profissionais da equipe de saúde bucal com informações e orientações sobre o programa de intervenção para serem transmitidos para a comunidade. A equipe de enfermagem junto com a equipe de saúde bucal fez palestras sobre orientações de cárie, escovação e nutrição. A equipe de saúde bucal além das palestras realizou procedimentos clínicos e procedimentos coletivos de escovação supervisionada, aplicação de gel fluoretado na escova dental. Foram realizados exames iniciais e planejamento clínico com todos os escolares dessa faixa etária da escola, totalizando 147 no final da intervenção. Além disso, todos receberam escovação dental supervisionada com creme dental e participaram das atividades de promoção de saúde. Os alunos de maior risco de cárie dental foram priorizados no atendimento clínico, alcançando no quarto mês 68% de tratamento concluído dos escolares. O trabalho foi de grande motivação para ampliarmos a integração entre a escola e a Unidade Básica de Saúde e para aumentar a integração de toda a equipe de saúde num trabalho voltado para a odontologia. Com certeza, este trabalho está trazendo e trará bons frutos com um futuro melhor para a saúde bucal e geral da população.

As doenças crônicas não transmissíveis, diabetes e hipertensão arterialrepresentam um dos principais desafios de saúde para o desenvolvimento global nas próximas décadas. Ameaçam a qualidade de vida de milhões de pessoas, representam o maior custo para os sistemas de saúde de todo o mundocom grande impacto econômico para os portadores, suas famílias e a sociedade em geral dos paísesespecialmente os de baixa e média renda. Torna-se, portanto, imperativo a implementação de modelos de atenção à saúde que incorporem estratégias diversas-individuais e coletivas a fim de melhorar a qualidade da atenção e alcançar o controle adequado destas doenças crônicas. O objetivo deste trabalho foi melhorar a atenção à saúde dos usuárioscom hipertensão arterial sistêmica e/ou diabetes mellitus na UBS/ESF 02 Mascarenhas de Moraes em Uruguaiana, Rio Grande do Sul. Para tal, uma intervenção foi realizada na unidade de saúde durante 12 semanas (abril a junho) em 2015. Os protocolos preconizados pelo Ministério da Saúde foram adotados para guiarem as ações desenvolvidas em quatro eixos pedagógicos: monitoramento e avaliação, gestão e organização do serviço, engajamento público e qualificação da prática clínica. Ao final dos três meses de intervenção, 361 hipertensos e 124diabéticos foram assistidos, o que permitiu o alcance de um percentual de cobertura de 98,9% para usuários hipertensos e de 100% para usuários diabéticos. A maioria dos indicadores qualitativos de avaliação das ações programáticas se mantiveram em 100% durante os três meses de intervenção. As principais ações desenvolvidas durante a intervenção foram: capacitação e treinamento da equipe, reuniões periódicas com a equipe, mapeamento e cadastramento dos usuários, monitoramento e avaliação das ações semanalmente, atendimento clínico, busca ativa dos faltosos, visitas domiciliares e atividades coletivas de prevenção e promoção de saúde. Os Agentes Comunitários de Saúde tiveram participação fundamental nas buscas ativas mesmo em número reduzido, e ajudaram a determinar os casos mais vulneráveis. Para o serviço, houve estreitamento e fortalecimento das relações interpessoais entre a equipe e entre equipe e comunidade. A intervenção qualificou a assistência aos diabéticos e hipertensos da Unidade de Saúde 02 Mascarenhas no município Uruguaiana/Rio Grande do Sul e foi inserida com êxito na rotina de serviço da referida unidade de saúde.

Films of silk fibroin (SF) and sodium alginate (SA) blends were prepared by solution casting technique. The miscibility of SF and SA in those blends was evaluated and scanning electron microscopy (SEM) revealed that SF/SA 25/75 wt.% blends underwent microscopic phase separation, resulting in globular structures composed mainly of SF. X-ray diffraction indicated the amorphous nature of these blends, even after a treatment with ethanol that turned them insoluble in water. Thermal analyses of blends showed the peaks of degradation of pristine SF and SA shifted to intermediate temperatures. Water vapor permeability, swelling capacity and tensile strength of SF films could be enhanced by blending with SA. Cell viability remained between 90 and 100%, as indicated by in vitro cytotoxicity test. The SF/SA blend with self-assembled SF globules can be used to modulate structural and mechanical properties of the final material and may be used in designing high performance wound dressing.

The aim of this investigation was to compare the skeletal stability of three different rigid fixation methods after mandibular advancement. Fifty-five class II malocclusion patients treated with the use of bilateral sagittal split ramus osteotomy and mandibular advancement were selected for this retrospective study. Group 1 (n = 17) had miniplates with monocortical screws, Group 2 (n = 16) had bicortical screws and Group 3 (n = 22) had the osteotomy fixed by means of the hybrid technique. Cephalograms were taken preoperatively, 1 week within the postoperative care period, and 6 months after the orthognathic surgery. Linear and angular changes of the cephalometric landmarks of the chin region were measured at each period, and the changes at each cephalometric landmark were determined for the time gaps. Postoperative changes in the mandibular shape were analyzed to determine the stability of fixation methods. There was minimum difference in the relapse of the mandibular advancement among the three groups. Statistical analysis showed no significant difference in postoperative stability. However, a positive correlation between the amount of advancement and the amount of postoperative relapse was demonstrated by the linear multiple regression test (p < 0.05). It can be concluded that all techniques can be used to obtain stable postoperative results in mandibular advancement after 6 months.

Bleeding complications in dengue may occur irrespective of the presence of plasma leakage. We compared plasma levels of modulators of the endothelial barrier among three dengue groups: bleedings without plasma leakage, dengue hemorrhagic fever, and non-complicated dengue. The aim was to evaluate whether the presence of subtle alterations in microvascular permeability could be detected in bleeding patients. Plasma levels of VEGF-A and its soluble receptors were not associated with the occurrence of bleeding in patients without plasma leakage. These results provide additional rationale for considering bleeding as a complication independent of endothelial barrier breakdown, as proposed by the 2009 WHO classification.

Cryosurgery is an efficient therapeutic technique used to treat benign and malignant cutaneous diseases. The primary active mechanism of cryosurgery is related to vascular effects on treated tissue. After a cryosurgical procedure, exuberant granulation tissue is formed at the injection site, probably as a result of angiogenic stimulation of the cryogen and inflammatory response, particularly in endothelial cells. To evaluate the angiogenic effects of freezing, as part of the phenomenon of healing rat skin subjected to previous injury. Two incisions were made in each of the twenty rats, which were divided randomly into two groups of ten. After 3 days, cryosurgery with liquid nitrogen was performed in one of incisions. The rats' samples were then collected, cut and stained to conduct histopathological examination, to assess the local angiogenesis in differing moments and situations. It was possible to demonstrate that cryosurgery, in spite of promoting cell death and accentuated local inflammation soon after its application, induces quicker cell proliferation in the affected tissue and maintenance of this rate in a second phase, than in tissue healing without this procedure. These findings, together with the knowledge that there is a direct relationship between mononuclear cells and neovascularization (the development of a rich system of new vessels in injury caused by cold), suggest that cryosurgery possesses angiogenic stimulus, even though complete healing takes longer to occur. The significance level for statistical tests was 5% (p<0,05).

Hypothalamic inflammation is a common feature of experimental obesity. Dietary fats are important triggers of this process, inducing the activation of toll-like receptor-4 (TLR4) signaling and endoplasmic reticulum stress. Microglia cells, which are the cellular components of the innate immune system in the brain, are expected to play a role in the early activation of diet-induced hypothalamic inflammation. Here, we use bone marrow transplants to generate mice chimeras that express a functional TLR4 in the entire body except in bone marrow-derived cells or only in bone marrow-derived cells. We show that a functional TLR4 in bone marrow-derived cells is required for the complete expression of the diet-induced obese phenotype and for the perpetuation of inflammation in the hypothalamus. In an obesity-prone mouse strain, the chemokine CX3CL1 (fractalkine) is rapidly induced in the neurons of the hypothalamus after the introduction of a high-fat diet. The inhibition of hypothalamic fractalkine reduces diet-induced hypothalamic inflammation and the recruitment of bone marrow-derived monocytic cells to the hypothalamus; in addition, this inhibition reduces obesity and protects against diet-induced glucose intolerance. Thus, fractalkine is an important player in the early induction of diet-induced hypothalamic inflammation, and its inhibition impairs the induction of the obese and glucose intolerance phenotypes.

Otorhinolaryngological manifestations of rheumatologic diseases represent a great challenge not only to the generalistphysician but also to the ENT doctor andrheumatologist. They often represent early manifestations of an autoimmune disorder which requires prompt and aggressive immunosuppressive treatment. Auditory, nasal, laryngeal and eye symptoms can be the first manifestation of rheumatic diseases and their proper assessment helps the doctor to identify signs of disease activity. The objective of this study is to identify the ENT manifestations in patients with rheumatic diseases in a high complexity hospital, regarding facilitating an early diagnosis and treatment. We performed clinical and complete otorhinolaryngological evaluations in patients selected from the outpatient rheumatology in a standardized manner by the use of a standardized form filling during the secondhalf of 2010. In the study group, systemic lupus erythematosus (SLE) patients had predominantly laryngeal manifestations, while patients with Sjögren's syndrome showed a higher prevalence of otologic manifestations. Changes in audiometric tests were found in 53% of Wegener's granulomatosis (WG) patients, 80% of relapsing polychondritis (RP), 33% of systemic lupus erythematosus (SLE) and 50% of Churg-Strauss syndrome (SCS). Regarding nasal alterations, these were found so prevalent in all conditions, especially Churg-Strauss syndrome. This study demonstrated that most patients treated in our hospital has the ENT signs and symptoms commonly associated in previous studies on rheumatic diseases, but further studies with a larger number of patients must be made to establish such relations.

To assess quality of care of women with severe maternal morbidity and to identify associated factors. This is a national multicenter cross-sectional study performing surveillance for severe maternal morbidity, using the World Health Organization criteria. The expected number of maternal deaths was calculated with the maternal severity index (MSI) based on the severity of complication, and the standardized mortality ratio (SMR) for each center was estimated. Analyses on the adequacy of care were performed. 17 hospitals were classified as providing adequate and 10 as nonadequate care. Besides almost twofold increase in maternal mortality ratio, the main factors associated with nonadequate performance were geographic difficulty in accessing health services (P < 0.001), delays related to quality of medical care (P = 0.012), absence of blood derivatives (P = 0.013), difficulties of communication between health services (P = 0.004), and any delay during the whole process (P = 0.039). This is an example of how evaluation of the performance of health services is possible, using a benchmarking tool specific to Obstetrics. In this study the MSI was a useful tool for identifying differences in maternal mortality ratios and factors associated with nonadequate performance of care.

Chronic telogen effluvium (CTE), a poorly understood condition, can be confused with or may be a prodrome to female pattern hair loss (FPHL). The pathogenesis of both is related to follicle cycle shortening and possibly to blood supply changes. To analyze a number of histomorphometric and immunohistochemical findings through vascular endothelial growth factor (VEGF), Ki-67, and CD31 immunostaining in scalp biopsies of 20 patients with CTE, 17 patients with mild FPHL and 9 controls. Ki-67 index and VEGF optical density were analyzed at the follicular outer sheath using ImageJ software. CD31 microvessel density was assessed by a Chalkley grid. Significant follicle miniaturization and higher density of nonanagen follicles were found in FPHL, compared with patients with CTE and controls. Ki-67+ index correlated positively with FPHL histological features. The FPHL group showed the highest VEGF optical density, followed by the CTE and control groups. No differences were found in CD31 microvessel density between the three groups. Histomorphometric results establish CTE as a distinct disorder, separate from FPHL from its outset. Its pathogenic mechanisms are also distinct. These findings support the proposed mechanism of 'immediate telogen release' for CTE, leading to cycle synchronization. For FPHL, accelerated anagen follicular mitotic rates and, thus, higher Ki-67 and VEGF values, would leave less time for differentiation, resulting in hair miniaturization.
