This thesis is an analytical analysis of consumption in Brazil, based on data from the Consumer Expenditure Survey, years 2008 to 2009, collected by the Brazilian Institute of Geography and Statistics. The main aim of the thesis was to identify differences and similarities in consumption among Brazilian households, and estimate the importance of demographic and geographic characteristics. Initially, households belonging to different social classes and geographical regions were compared based on their consumption. For further insights, two cluster analyses were conducted. Firstly, households were grouped according to the absolute values of expenditures. Five clusters were discovered; cluster membership showed larger spending in all of the expense categories for households having higher income, and a substantial association with particular demographic variables, including as region, neighborhood, race and education. Secondly, cluster analysis was performed on proportionate distribution of total spending by every household. Five groups of households were revealed: Basic Consumers, the largest group that spends only on fundamental goods, Limited Spenders, which additionally purchase alcohol, tobacco, literature and telecommunication technologies, Mainstream Buyers, characterized by spending on clothing, personal care, entertainment and transport, Advanced Consumers, which have high relative expenses on financial and legal services, healthcare and education, and Exclusive Spenders, households distinguished by spending on vehicles, real estate and travelling.

Objective: To analyze the panorama of breastfeeding in Brazil through an integrative literature review highlighting its advances and challenges. Methods: We carried out an integrative literature review in the SciELO and PubMed databases and in booklets published on the websites of the Ministry of Health and the International Baby Food Action Network (IFBAN) using the following Portuguese and English descriptors: aleitamento materno (breastfeeding), autoeficácia (self-efficacy), promoção da saúde (health promotion) and desmame (weaning) in the period from 2002 to 2015. Results: We identified at first 43 articles, 33 booklets, 1 thesis and 3 dissertations, including in the study 17 articles, 3 dissertations and 19 booklets due to information saturation. It was verified that breastfeeding rates have reduced significantly over time with direct implications in infant mortality rates, being associated with early weaning and the lack of promotion of maternal self-efficacy in the prenatal and postpartum. To change this situation, Brazil imposed a number of public policies aimed at breastfeeding success, which advocated to raise rates. Conclusion: Despite the advances, the Brazilian panorama of breastfeeding shows that the country remains below the recommendations of international organizations, and overcoming the obstacles to successful breastfeeding constitutes a major challenge for the Brazilian public health.

The collection and conservation of the Cucurbita genus in Brazil happened in so dispersed institutions, an assessment of the storage conditions and the genetic diversity is needed, making it possible to identify new priorities for the genus. This work constitutes diagnosis of geographical distribution, storage conditions in situ and ex situ and on genetic diversity of the Cucurbita genus in Brazil. Research was done in herbariums, databases, literature and in situ (expeditions to rural areas and (markets) to map the areas of occurrence of the species. During these expeditions, questionnaires were applied to obtain information about the property and genus Cucurbita. Questionnaires were sent to 173 Brazilian institutions regarding the preservation conditions ex situ. A genetic variability of the Cucurbita genus was found in traditional Brazilian agriculture. Collections must be prioritized in the northern and southern regions (all states); the southeastern region, all states, except Minas Gerais; central-west, in Mato Grosso do Sul and Mato Grosso; the northeastern region, the states of Alagoas, Maranhão, Paraíba, Pernambuco, Piauí and Sergipe. Currently 5.545 entries are being conserved in the Germplasm banks, however, C. pepo, C. ficifolia, C. argyrosperma and wild species are poorly represented. The characterization level of conserved entries is low in the ex situcollections. Participative research projects must be financed as a way to stimulate the farmers to continue planting their local varieties. A coleta e conservação do gênero Cucurbita no Brasil aconteceu de forma dispersa pelas instituições, sendo necessário um diagnóstico sobre as condições de conservação e sobre a diversidade genética, tornando possível identificar novas prioridades para o gênero. Este trabalho realizou diagnóstico sobre distribuição geográfica, condições de conservação in situ e ex situ e sobre diversidade genética do gênro Cucurbita no Brasil. Para mapear as áreas de ocorrência das espécies, foram realizados levantamentos de informações em herbários, banco de dados, literatura e levantamentos in situ (expedições para áreas rurais, feiras livres e CEASAs). Nessas expedições foram aplicados questionários, buscando informações sobre a propriedade e o gênero Cucurbita. Em relação as condições de conservação ex situ, foram enviados questionários para 173 instituições brasileiras. Foi constatado que existe variabilidade genética do gênero Cucurbita na agricultura tradicional brasileira. Devem ser priorizadas coletas nas regiões Norte e Sul (todos os estados); região Sudeste, todos os estados, exceto Minas Gerais; região Centro-Oeste, no Mato Grosso do Sul e Mato Grosso; região Nordeste, os estados de Alagoas, Maranhão, Paraíba, Pernambuco, Piauí e Sergipe. Atualmente estão sendo conservados nos Bancos de Germoplasma 5.545 acessos, no entanto C. pepo, C ficifolia, C. argyrosperma e espécies silvestres estão pouco representadas. É baixa a taxa de caracterização dos acessos conservados nas coleções ex situ. Devem ser financiados projetos de pesquisa participativa como uma forma de estimular e dar condições aos agricultores de continuarem cultivado suas variedades locais.

Objective: While in many Western affluent countries there is widespread awareness of chronic fatigue syndrome (CFS), also known as myalgic encephalomyelitis (ME), little is known about the awareness of CFS/ME in low- and middle-income countries. We compared the awareness of CFS in Brazil and the United Kingdom. Methods: Recognition and knowledge of CFS were assessed among 120 Brazilian specialist doctors in two major university hospitals using a typical case vignette of CFS. We also surveyed 3914 and 2435 consecutive attenders in Brazilian and British primary care clinics, respectively, concerning their awareness of CFS. Results: When given a typical case vignette of CFS, only 30.8% [95% confidence interval (CI), 22.7-39.9%] of Brazilian specialist doctors mentioned chronic fatigue or CFS as a possible diagnosis, a proportion substantially lower than that observed in Western affluent countries. Similarly, only 16.2% (95% CI, 15.1-17.4%) of Brazilian primary care attenders were aware of CFS, in contrast to 55.1% (95% CI, 53.1-57.1%) of their British counterparts (P <.001). This difference remained highly significant after controlling for patients` sociodemographic and socioeconomic characteristics (P <.001). Conclusions: The awareness of CFS was substantially lower in Brazil than the United Kingdom. The observed difference may influence patients` help-seeking behavior and both doctors` and patients` beliefs and attitudes in relation to fatigue-related syndromes. Attempts to promote the awareness of CFS should be considered in Brazil, but careful plans are required to ensure the delivery of sound evidence-based information. (c) 2008 Elsevier Inc. Objective. The objective of this study was to conduct a cost-effectiveness analysis of a universal rotavirus vaccination program among children : 5 years of age in Brazil. Methods. Considering a hypothetical annual cohort of approximately 3 300 000 newborns followed over 5 years, a decision-tree model was constructed to examine the possible clinical and economic effects of rotavirus infection with and without routine vaccination of children. Probabilities and unit costs were derived from published research and national administrative data. The impact of different estimates for key parameters was studied using sensitivity analysis. The analysis was conducted from both healthcare system and societal perspectives. Results. The vaccination program was estimated to prevent approximately 1735 351 (54%) of the 3 210 361 cases of rotavirus gastroenteritis and 703 (75%) of 933 rotavirus-associated deaths during the 5-year period. At a vaccine price of 18.6 Brazilian reais (R$) per dose, this program would cost R$121 673 966 and would save R$38 536 514 in direct costs to the public healthcare system and R$71 778 377 in direct and indirect costs to society. The program was estimated to cost R$1 028 and R$1 713 per life-years saved (LYS)from the societal and healthcare system perspectives, respectively. Conclusions. Universal rotavirus vaccination was a cost-effective strategy for both perspectives. However, these findings are highly sensitive to diarrhea incidence rate, proportion of severe cases, vaccine coverage, and vaccine price.
