O presente estudo teve como objetivo compreender a forma como cuidadores, pais e/ou responsáveis, compreendem o desenvolvimento emocional no contexto de circulação de crianças. Para tanto, embasou-se na teoria psicodinâmica sobre o desenvolvimento emocional, cuidador (es) e circulação de crianças.Utilizou-se o método qualitativo, no intuito de capturar dados e informações sobre o fenômeno estudado em sua singularidade (MINAYO, 2010). A dimensão interdisciplinar do presente estudo, calcado na união de conceitos antropológicos e psicológicos, levantou a necessidade de utilizar mais de uma estratégia metodológica, para responder o problema da pesquisa e alcançar o seu objetivo. Desta forma, utilizou-se para coleta dos dados a entrevista semi-estruturada com uma dupla de pais e/ou responsáveis por cinco crianças regularmente matriculadas no primeiro ano do ensino fundamental, além do professor responsável pela referida classe. Fez-se o grupo focal com pais e/ou responsáveis, além da observação de um dia inteiro de cada criança. A análise de conteúdo foi utilizada para avaliar qualitativamente as respostas das entrevistas, ou seja, analisar o material verbal (FRANCO, 2007). Utilizou-se, ainda, do ecomapa e de um viés da perspectiva proposta por Mauss (2001), os quais auxiliaram a montagem da rede social (BOTT, 1976) e a observação participante com registro em diário de campo e posterior análise, ao permitir identificar e interpretar os fenômenos em estudo. Conclui-se que o desenvolvimento emocional na atualidade está ancorado em uma circulação de crianças vigiada, com redes de malha estreita, com poucas relações possíveis sem a escolha dos responsáveis. O ser em cuidado então se sente sufocado, por conseguinte os cuidadores passam a exigir cuidados redobrados de si que os tornam carentes e necessitados de escuta, pois a interdição e o medo gera angústia, ansiedade e futura agressividade. Assim, deve-se ampliar e investir em pesquisas que apreendam a circulação de crianças como forma de investigar como ela está se desenvolvendo emocionalmente na atualidade, para diminuir transtornos psiquiátricos futuros e auxiliar os cuidadores de crianças.

Os transtornos de ansiedade apresentam a maior incidência na população mundial dentre os transtornos psiquiátricos, e a eficácia clínica das drogas ansiolíticas é baixa, em parte devido ao desconhecimento acerca das bases neuroquímicas desses transtornos. Para uma compreensão mais ampla e evolutivamente substanciada desses fenômenos, a utilização de espécies filogeneticamente mais antigas pode ser uma aproximação interessante no campo da modelagem comportamental; assim, sugerimos o uso do paulistinha (Danio rerio Hamilton 1822) na tentativa de compreender a modulação de comportamentos tipo-ansiedade pelo sistema serotonérgico. Demonstramos que os níveis extracelulares de serotonina no encéfalo de paulistinhas adultos expostos ao teste de preferência claro/escuro [PCE] (mas não ao teste de distribuição vertical eliciada pela novidade [DVN]) apresentam-se elevados em relação a animais manipulados mas não expostos aos aparatos. Além disso, os níveis teciduais de serotonina no rombencéfalo e no prosencéfalo são elevados pela exposição ao PCE, enquanto no mesencéfalo são elevados pela exposição ao DVN. Os níveis extracelulares de serotonina estão correlacionados negativamente com a geotaxia no DVN, e positivamente com a escototaxia, tigmotaxia e a avaliação de risco no PCE. O tratamento agudo com uma dose baixa de fluoxetina (2,5 mg/kg) aumenta a escototaxia, a tigmotaxia e a avaliação de risco no PCE, diminui a geotaxia e o congelamento e facilita a habituação no DVN. O tratamento com buspirona diminui a escototaxia, a tigmotaxia e o congelamento nas doses de 25 e 50 mg/kg no PCE, e diminui a avaliação de risco na dose de 50 mg/kg; no DVN, ambas as doses diminuem a geotaxia, enquanto somente a maior dose diminui o congelamento e facilita a habituação. O tratamento com WAY 100635 diminui a escototaxia nas doses de 0,003 e 0,03 mg/kg, enquanto somente a dose de 0,03 mg/kg diminui a tigmotaxia e a avaliação de risco no PCE. No DVN, ambas as doses diminuem a geotaxia, enquanto somente a menor dose facilita a habituação e aumenta o tempo em uma “base” (“homebase”). O tratamento com SB 224289 não produziu efeitos sobre a escototaxia, mas aumentou a avaliação de risco na dose de 2,5 mg/kg; no DVN, essa droga diminuiu a geotaxia e o nado errático nas doses de 2,5 e 5 mg/kg, enquanto a dose de 2,5 mg/kg aumentou a formação de “bases”. O tratamento com DL-para-clorofenilalanina (2 injeções de 300 mg/kg, separadas por 24 horas) diminuiu a escototaxia, a tigmotaxia e a avaliação de risco no PCE, aumentou a geotaxia e a formação de bases e diminuiu a habituação no DVN. Quando os animais são pré-expostos a uma “substância de alarme” co-específica, observa-se um aumento nos níveis extracelulares de serotonina associados a um aumento na escototaxia, congelamento e nado errático no PCE; os efeitos comportamentais e neuroquímicos foram bloqueados pelo pré tratamento com fluoxetina (2,5 mg/kg), mas não pelo pré-tratamento com WAY 100,635 (0,003 mg/kg). Animais da linhagem leopard apresentam maior escototaxia e avaliação de risco no PCE, assim como níveis teciduais elevados de serotonina no encéfalo; o fenótipo comportamental é resgatado pelo tratamento com fluoxetina (5 mg/kg). Esses dados sugerem que o sistema serotonérgico dessa espécie modula o comportamento no DVN e no PCE de forma oposta; que a resposta de medo produzida pela substância de alarme também parece aumentar a atividade do sistema serotonérgico, um efeito possivelmente mediado pelos transportadores de serotonina, e ao menos um fenótipo mutante de alta ansiedade também está associado a esses transportadores. Sugere-se que, de um ponto de vista funcional, a serotonina aumenta a ansiedade e diminui o medo em paulistinhas.

Background: There are no reported cases of factitious or simulated obsessive compulsive disorder (OCD). However, over the last years, our clinic has come across a number of individuals that seem to exaggerate, mislabel or even intentionally produce obsessive and/or compulsive symptoms in order to be diagnosed with OCD.Methods: In this study, experienced clinicians working on a university-based OCD clinic were requested to provide clinical vignettes of patients who, despite having a formal diagnosis of OCD, were felt to display non-genuine forms of this condition.Results: Ten non-consecutive patients with a self-proclaimed diagnosis of OCD were identified and described. Although patients were diagnosed with OCD according to various structured interviews, they exhibited diverse combinations of the following features: (i) overly technical and/or doctrinaire description of their symptoms, (ii) mounting irritability, as the interviewer attempts to unveil the underlying nature of these descriptions; (iii) marked shifts in symptom patterns and disease course; (iv) an affirmative yes pattern of response to interview questions; (v) multiple Axis I psychiatric disorders; (vi) cluster B features; (vii) an erratic pattern of treatment response; and (viii) excessive or contradictory drug-related side effects.Conclusions: In sum, reliance on overly structured assessments conducted by insufficiently trained or naive personnel may result in invalid OCD diagnoses, particularly those that leave no room for clinical judgment. Background: Depression and anxiety are psychiatric disorders that are related to tobacco use and associated with dependence, the process of cessation, lapses and relapses after quitting smoking. Objective: To analyze the association of nicotine dependence with the level of anxiety and depression in patients who are in the process of smoking cessation. Methods: Cross-sectional study conducted with patients who sought the smoking cessation program in Cuiabá/MT. All the smokers enrolled from May to August 2012, participated in this study. Four instruments were applied: Socio-demographic Questionnaire, Fagerstrom test, Beck Anxiety Inventory and Beck Depression Inventory. Following bivariate analysis, using the crude prevalence ratio, with level of significance lower than 5% (p < 0.05), having as variable outcome the nicotine dependence >= 5 (Fagerstrom). The associations with p < 0.20 were selected for robust Multiple Poisson Regression (RPa). Results: Associations of Fagerstrom >= 5 with the male sex (RP = 1.15 CI 95% 1.03-1.28); number of cigarettes/day (RP = 1,33; CI 95% 1.19-1.48); and moderate/severe level of depression (RP = 1.15; CI 95% 1.04-1.28) were found. In the final model (RPa), the following variables remained associated: male gender (RPa 1.12; CI 95% 1.01-1.24), number of cigarettes/day (RPa 1.28; CI 95% 1.15-1.43) and high level of depression (RPa 1.12; CI 95% 1.01-1.23). Discussion: High level of nicotine dependence was associated significantly with the level of depression, emphasizing the association between smoking and psychiatric comorbidities.

Studies have shown that aversive conditions at the university may have great influence on academic achievement and the development of psychiatric disorders such as depression. In university context, social relations have been investigated over the years, but are scarce publications relating mental health and social skills. Thus, this paper compared a group of college students with a clinical depression with a group without clinical depression for the consequences and feelings that differents responses of social skills may have in social interactions with various interlocutors. A total of 128 students participated, 64 for clinical depression and 64 non-clinical. Questionário de Avaliação de Comportamentos e Contextos para Universitários – QHC – Universitários, Inventário de Fobia Social (Mini-Spin), Inventário de Depressão de Beck (BDI) and Entrevista clínica estruturada para o DSM-IV (SCID-I) were applied. The data were analyzed by test t of Student. The results show that students with depression have a great difficulty about the social skills regardless of which require more or less assertion and general way for all social interactions, either family, friends, roommate and partner.

The neural control of the cardiovascular system is a complex process that involves many structures at different levels of nervous system. Several cortical areas are involved in the control of systemic blood pressure, such as the sensorimotor cortex, the medial prefrontal cortex and the insular cortex. Non-invasive brain stimulation techniques - repetitive transcranial magnetic stimulation (rTMS) and transcranial direct current stimulation (tDCS) - induce sustained and prolonged functional changes of the human cerebral cortex. rTMS and tDCS has led to positive results in the treatment of some neurological and psychiatric disorders. Because experiments in animals show that cortical modulation can be an effective method to regulate the cardiovascular system, non-invasive brain stimulation might be a novel tool in the therapeutics of human arterial hypertension. We here review the experimental evidence that non-invasive brain stimulation can influence the autonomic nervous system and discuss the hypothesis that focal modulation of cortical excitability by rTMS or tDCS can influence sympathetic outflow and, eventually, blood pressure, thus providing a novel therapeutic tool for human arterial hypertension. Aims To describe, in the context of DSM-V, how a focus on addiction and compulsion is emerging in the consideration of pathological gambling (PG). Methods A systematic literature review of evidence for the proposed re-classification of PG as an addiction. Results Findings include: (i) phenomenological models of addiction highlighting a motivational shift from impulsivity to compulsivity associated with a protracted withdrawal syndrome and blurring of the ego-syntonic/ego-dystonic dichotomy; (ii) common neurotransmitter (dopamine, serotonin) contributions to PG and substance use disorders (SUDs); (iii) neuroimaging support for shared neurocircuitries between behavioural and substance addictions and differences between obsessivecompulsive disorder (OCD), impulse control disorders (ICDs) and SUDs; (iv) genetic findings more closely related to endophenotypic constructs such as compulsivity and impulsivity than to psychiatric disorders; (v) psychological measures such as harm avoidance identifying a closer association between SUDs and PG than with OCD; (vi) community and pharmacotherapeutic trials data supporting a closer association between SUDs and PG than with OCD. Adapted behavioural therapies, such as exposure therapy, appear applicable to OCD, PG or SUDs, suggesting some commonalities across disorders. Conclusions PG shares more similarities with SUDs than with OCD. Similar to the investigation of impulsivity, studies of compulsivity hold promising insights concerning the course, differential diagnosis and treatment of PG, SUDs, and OCD.

Although traditionally obsessive-compulsive disorder (OCD) and impulse control disorders (ICD) have represented opposing ends of a continuum, recent research has demonstrated a frequent co-occurrence of impulsive and compulsive behaviours, which may contribute to a worse clinical picture of some psychiatric disorders. We hypothesize that individuals with 'impulsive' OCD as characterized by poor insight, low resistance, and reduced control towards their compulsions will have a deteriorative course, greater severity of hoarding and/or symmetry/ordering symptoms, and comorbid ICD and/or substance use disorders (SUD). The sample consisted of 869 individuals with a minimum score of 16 on the Yale-Brown Obsessive Compulsive Scale (Y-BOCS). Of these, 65 had poor insight, low resistance, and reduced control towards compulsions ('poor IRC') and 444 had preserved insight, greater resistance and better control over compulsions ('good IRC'). These two groups were compared on a number of clinical and demographic variables. Individuals with poor IRC were significantly more likely to have a deteriorative course (p < 0.001), longer duration of obsessions (p = 0.017), greater severity of symmetry/ordering (p < 0.001), contamination/cleaning (p < 0.001) and hoarding (p = 0.002) symptoms, and comorbid intermittent explosive disorder (p = 0.026), trichotillomania (p = 0.014) and compulsive buying (p = 0.040). Regression analysis revealed that duration of obsessions (p = 0.037) and hoarding severity (p = 0.005) were significant predictors of poor IRC. In the absence of specific measures for impulsivity in OCD, the study highlights the utility of simple measures such as insight, resistance and control over compulsions as a phenotypic marker of a subgroup of OCD with impulsive features demonstrating poor clinical outcome. OBJECTIVES: The objectives of this study were to verify the degree of anxiety, respiratory distress, and health-related quality of life in a group of asthmatic patients who have experienced previous panic attacks. Additionally, we evaluated if a respiratory physiotherapy program (breathing retraining) improved both asthma and panic disorder symptoms, resulting in an improvement in the health-related quality of life of asthmatics. METHODS: Asthmatic individuals were assigned to a chest physiotherapy group that included a breathing retraining program held once a week for three months or a paired control group that included a Subtle Touch program. All patients were assessed using the Diagnostic and Statistical Manual of Mental Disorders IV, the Sheehan Anxiety Scale, the Quality of Life Questionnaire, and spirometry parameter measurements. RESULTS: Both groups had high marks for panic disorder and agoraphobia, which limited their quality of life. The Breathing Retraining Group program improved the clinical control of asthma, reduced panic symptoms and agoraphobia, decreased patient scores on the Sheehan Anxiety Scale, and improved their quality of life. Spirometry parameters were unchanged. CONCLUSION: Breathing retraining improves the clinical control of asthma and anxiety symptoms and the health-related quality of life in asthmatic patients.
