Syfte: Syftet med denna studie var att beskriva uppfattningen hos sjuksköterskor med tjänst på särskilda boenden gällande deras arbete med tillämpningen av munhälsobedömningar och deras uppfattning om hur munvård utförs. Metod: Studien genomfördes med kvalitativ deskriptiv design. Data samlades in hjälp av intervjuer med sjuksköterskor (n= 6). Intervjuerna analyserades med hjälp av innehållsanalys och fyra olika teman identifierades: Sjuksköterskors uppfattning om munvård, sjuksköterskors uppfattning om munhälsobedömning, sjuksköterskors uppfattning om munvår och munhälsobedömning i samband med palliativ vård samt sjuksköterskor uppfattning om samarbete med folktandvården. Resultat: Resultatet visade sjuksköterskorna uppfattade att munhälsobedömningar sällan utförs av sjuksköterskorna. Dock ansåg de att munhälsobedömning och munvård utförs regelbundet av såväl sjuksköterskor samt omvårdnadspersonal när vårdtagarna befinner sig i livets slutskede. De eftersöker instruktioner och utbildning av munhälsobedömning enligt ROAG innan detta införs i verksamheten. Munhälsobedömning har utförts av folktandvården en gång om året och de har även kommit till boendena om det finns problem med någon vårdtagares munhälsa. Folktandvården är ansvarig för att skriva vårdtagarnas munvårdskort som är ett stöd för vårdpersonalen vid utförande av daglig munvård. Resultatet har även visat att daglig munvård utförs av undersköterskor och vårdbiträden. Slutsats: Resultatet visar att munhälsobedömning utförs bristfälligt av sjuksköterskor i den kommunala äldreomsorgen. Vidare anser de tillfrågade sjuksköterskorna att utbildning i munhälsobedömningsinstrumentet ROAG är nödvändig innan införandet.

Syftet Att undersöka vilken effekt reminiscens- och musikterapi har på äldre deprimerade personer på äldreboende. Metod: Litteratursökning utfördes i databaserna CHINAL, Medline, Psycinfo samt Scopus. Till grund för resultatet valdes n=13 artiklar med kvantitativ design. Urvalet av artiklarna till resultatet har utförts med hjälp av inklusionskriterier, relevant abstract och publicerade mellan 2000-2013. Artiklarna skulle motsvara syfte och frågeställningar samt uppfylla hög eller medelpoäng i kvalitetsgranskningen. Resultat: Reminiscensterapi kunde reducera och minska depressiva symtom hos äldre personer på äldreboende. Musikterapi kunde också vara en effektiv åtgärd för att minska depression hos äldre personer på äldreboende. Ett annat resultat som framkom var att sömnen kan förbättras av musikterapi. Slutsats: Reminiscens- och musikterapi kunde minska depression hos äldre personer som bor på äldreboende, genom att musik stimulerar limbiska systemet och reminiscensterapin låter de äldre att återberätta och leva på nytt genom sina minnen. Det kan vara värdefullt för de äldre att vårdgivare på äldreboende lär sig grunder för reminiscens och musikterapi för att kunna använda det i den dagliga vården av de äldre för att minska depression men även som en förebyggande åtgärd.

Socialstyrelsens brukarundersökning visar att äldre som bor på särskilt boende är mest missnöjda med de aktiviteter och den sociala samvaron som verksamheten erbjuder. Äldreomsorgen bygger på den nationella värdegrunden som innefattar att den äldre ska få leva ett värdigt liv och känna välbefinnande, men det är inte klart hur denna värdegrund påverkar personalens arbetssätt eller de sociala aktiviteter som erbjuds. Vår studie angriper denna fråga. Fem särskilda boenden var representerade både från kommunala och privata vårdgivare i två olika kommuner. Semistrukturerade intervjuer genomfördes med de särskilda boendes enhetschefer, som sedan transkriberades och tematiskt analyserades. Vårt resultat tyder på att den nationella värdegrunden inom äldreomsorgen tolkas olika på alla de särskilda boendena. Alla enhetschefer la stor vikt på personcentrerad vård, men trots det så erbjöd man liknade sociala aktiviteter ofta då som gruppaktivitet. Resultaten diskuterar vi med hänvisning till Aktivitetsteorin och Gerotranscendenceteorin.

Background: Animal-Assisted Therapy using dogs have been described as having a calming effect, decrease sundowning and blood-pressure in persons with Alzheimer’s disease. The aim was to investigate how continuous and scheduled visits by a prescribed therapy dog affected daytime and night-time sleep for persons with Alzheimer’s disease. Methods: In this case study, registration of activity and sleep curves was conducted from five persons with moderate to severe Alzheimer’s disease living at a nursing home, over a period of 16 weeks using an Actiwatch. Data was analysed with descriptive statistics. Result: The study shows no clear pattern of effect on individual persons daytime activity and sleep when encounter with a therapy dog, but instead points to a great variety of possible different effects that brings an increased activity at different time points, for example during night-time sleep. Conclusions: Effects from the use of a Animal-Assisted Therapy with a dog in the care of persons with Alzheimer’s disease needs to be further investigated and analysed from a personcentred view including both daytime and nightime activities.

A artrose do quadril é uma doença articular degenerativa que atinge principalmente idosos, faixa etária que vem gradativamente aumentando nos últimos tempos. Assim, medidas de racionalização do uso de leitos e recursos hospitalares são necessárias para melhor viabilizar os procedimentos cirúrgicos e reduzir gastos para o sistema de saúde, uma vez que quase dois terços destes gastos decorrem do período de internação. Com esse objetivo, foi desenvolvido um protocolo assistencial de artroplastia total de quadril (PAATQ), multidisciplinar, com consultas ambulatoriais e visitas domiciliares programadas para pacientes a serem submetidos à artroplastia total de quadril (ATQ), visando uma melhor abordagem por parte da equipe médica e de enfermagem e uma melhor orientação dos pacientes e familiares quanto ao procedimento cirúrgico. Este estudo transversal (piloto), com controle histórico aninhado a um ensaio clínico randomizado, tem por objetivo avaliar a factibilidade de implantação e a adesão, por parte das equipes médica e de enfermagem, à utilização deste protocolo. Secundariamente, visa determinar o impacto desta rotina no tempo de internação, na independência funcional do paciente e nos eventos clínicos da fase hospitalar, bem como avaliar a factibilidade das visitas domiciliares de enfermagem. Um total de 22 pacientes (9 homens e 13 mulheres) submetidos à ATQ no Hospital de Clínicas de Porto Alegre (Brasil), com média (dp) de idade de 58,86 (16,87), variando de 21 a 86 anos, foram incluídos no estudo. Os pacientes foram divididos em dois grupos grupo 1 (n=12) e grupo 2 (n=10) de acordo com aplicação ou não do PAATQ. No grupo 1, as principais comorbidades foram a hipertensão arterial sistêmica (HAS) (dois casos), o diabete mellitus (um caso); e o alcoolismo (um caso).No grupo 2, houve um caso de HAS. O tempo de permanência médio (dp) pós-operatório (DPO) foi de 5,2 e 7,5 dias para os grupos 1 e 2, respectivamente (p=0.0055). A adesão ao protocolo foi de 90% e 100% por parte das equipes médica e de enfermagem, respectivamente. Sete pacientes do grupo 1 conseguiram sentar fora do leito no 2o dia pós-operatório (PO), dois sentaram no 3o e nenhum foi incapaz de sair do leito. Todos deambularam com muletas um dia após sentarem. No grupo 2, todos os pacientes saíram do leito no 4o DPO. Em conclusão, o presente protocolo mostrou-se factível, tendo obtido ótima adesão por parte da equipe e propiciado redução do tempo de internação. O seguimento do programa determinará mais detalhadamente a sua eficácia e a factibilidade das visitas domiciliares.

The aim of this study was to follow the daily routine of elderly individuals residing at the Vila Vicentina Júlia Freire (VVJF) nursing home in João Pessoa, Brazil, observing and analyzing as much as possible, the way in which they live in a group and how they deal, through personal experiences, with their own body image and with that of others. The results were obtained from information considered relevant, according to the perspectives of the residents themselves, contained in narratives of their history and related to their quality of life. Our interest lays in how they coped with changes in their bodies at old age and adapted to the challenges of this new phase. To obtain this information, with significant expressions, we used the Narrative Interview (NI) as a research tool in order to redeem their life experience and develop an understanding of their impressions about the life experience at the current nursing home. We selected, among the housed, ten subjects - actors - to appear as a sample of the group together. The design of our sample had a hybrid model, combining two types: random sample (probability) and an intentional sample (not probability). After recording transcripts and ensuring that all the subjects showed common interests that were relevant to the group, we chose three categories for analysis as follows: a. the elderly as seen by themselves and by others; b. religiosity as a moral factor; c. intimacy and relationships. Our choice was based on the importance of the theme as a multidisciplinary topic in the quality of life of populations. The subjects, chosen as spokespersons for the VVJF, spoke, among other things, about the way in which they viewed the world, their own body and how they experienced the environment and their fellow residents, in an attitude of reflection and criticism that confirms the lucidity of an old age conditioned to an elderly institution. The method allowed us to investigate our subjects from a multidisciplinary point of view. We suggest other studies in assorted areas of the human sciences, having as principle, man at the center of all our scientific questionings. The paradigm that the elder is, necessarily, unhappy in his sheltered has been, in this job, broken through the testimonies collected, establishing multiple perspectives of the analyzed subjects by their respective narrative records. Involving the participation of researchers with major in Administration, Communication and Law, this study filled, in this aspect, the requirements of the multidisciplinarity of the Program of Pos-Graduation in Health Science

This study aimed to validate the contents of an Instrument for Nursing Consultation in the Home Visit of people with Spinal Cord Injury (INCEVDOP-LM), based on the Self-Care Deficit Theory. The methodological development study was conducted with spinal cord injured (SCI) people ascribed in the Family Health Units the city of Natal/RN/Brazil, and with the nurses of these institutions. The study was conducted from Januray 2012 to January 2013 in two phases: the first aimed to identify the need for self-care of persons with SCI, and the second to develop and validate the INCEVDOP-LM. The first phase consisted of a census study of people with SCI living in Natal/RN. In the second phase, a non-probabilistic convencience sample of subjects was selected to form two groups: First stage - Group 1 of the first stage was comprised by 73 adults with SCI diagnosed with paraplegia or tetraplegia, with cognitive function preserved and that were registered to some family health unit; Group 2 of the Second phase was composed of six experts that were nurses with doctoral formation, scientific experience in the area of technology development or assistance to persons with SCI, and with publications in periodicals Qualis A2. Data collection of the first phase was conducted through home visits of people with SCI that responded three instruments: Questionnaire I (comprised of demographic and socioeconomic variables), The Competency Rating Scale for Self-care (ASA) and the Barthel Index (an instrument for evaluation of functional capacity). The research for the second phase was conducted in two stages: I-construction of the INCEVDOP LM; II-validation of the INCEVDOP-LM. The instrument and an evaluation form were forwarded to the experts for the validation. The correlations between the responses were analyzed by the Kappa test, with accepting values of>0.75. The evaluation criteria were: organization, clarity, simplicity, readability, appropriateness of vocabulary, objectivity, accuracy, reliability and suitability and the positive responses with frequency values of≥90% were considered excellent. The chi-square test was used to investigate the differences between proportions. The study attended to the principles of Human Rights CNS Resolution 196/96. Results were reported by means of four articles derived from the study. The findings indicate that the items that showed disagreement among experts (k=0.02) were diagnoses, interventions and evaluation of the nursing features pertaining to the domains of Nutrition, Hygiene, Elimination, Physical, Social and Psychological, and of the Ability to perform work activities feature. Agreement among the experts were reported for the other items, with kappa ranging from 0.72 to 1. After removing items with disagreement, all criteria achieved excellent rates and no significant differences were observed between the proportions of responses of evaluation of experts (p>0.05). We conclude that the instrument shows validity to serve as a guide for nurses to conduct a systematic consultation during the home visit to people with spinal cord injury, with emphasis on self-care. The instrument must go through other levels of validation when applied in the clinical setting

This study assessed the oral health of individuals 60 years or older in the city of Araraquara, Sao Paulo, Brazil, in 1998. Of the 194 people who participated in the study, 91 of them were institutionalized and had an average age of 73.6 years, and 103 were not institutionalized and had an average age of 69.3 years. The study participants were examined by a previously trained oral surgeon who determined the prevalence of the most common oral health problems. The results revealed a large number of edentulous individuals (72% of those institutionalized and 60% of the noninstitutionalized participants) and many persons with extracted teeth (93% and 90%, respectively), as well as a high frequency of periodontal pockets (57% and 75%, respectively) and of inadequate dentures (80% and 61%, respectively). Our results show reduced quality of life for a large proportion of these older individuals, and also indicate that public health services should pay greater attention to this population group.

Depression is a highly prevalent illness among institutionalized aged and assumes peculiar characteristics such as the risk for progressing to dementia. The aims of this study was to assess the cognitive functions of institutionalized elderly with clinical diagnosis of depression and compare the severity of depressive symptoms with cognitive performance. From 120 residents at a nursing home in Rio Claro, Brazil, we study 23 individuals (mean age: 74.3 years; mean schooling: 4.0 years) with diagnosis of depression. At first, a clinical diagnosis of depression and measurement of its symptoms using the Geriatric Depression Scale were performed. The patient then underwent a neuropsychological assessment based on the following tests: Mini-Mental Examination, Verbal Fluency, Visual Perception, Immediate Memory, Recent Memory, Recognition, Clock Drawing Test. The patients were divided into two groups: those with less severe depression symptoms (Group 1: N=9) and more severe symptoms (Group 2: N=14). The significant difference between symptom severity of the two groups was p=0.0001. Patients with more severe symptoms revealed a slightly inferior cognitive performance in most of the tests when compared to those with less severe symptoms (p>0.05). In relation to Verbal Fluency, patients with more severe depression symptoms presented a significantly inferior cognitive performance when compared to those with less severe symptoms (p=0.0082). Verbal Fluency revealed to be a more sensitive test for measuring early cognitive alterations in institutionalized aged with depression, and appears to be a useful resource in monitoring the cognitive functions of patients faced with the risk of dementia. © Copyright Moreira Jr. Editora.

Objective: The aim of this study was to analyse denture users' oral care habits with regard to the use of their prostheses. Background: Rehabilitative treatment is only successful when patients are motivated and aware of correct prosthesis use and hygiene. Materials and methods: Questionnaires were distributed to 150 complete denture users at the Federal University of Bahia School of Dentistry, the Esmeralda Natividade Health Center, the Bahian Science Development Foundation and a Salvador nursing home. The questionnaire included information on gender, age, length of prosthesis use, cleaning methods and materials, etc. The data were analysed using EpiInfo version 6 software. The chi-squared test was used for statistical analysis, with a significance level of 5%. Results: Questionnaire results showed that 78% of the subjects, with an average age of 67.3 years, had used the same complete denture for over 5 years. 64% slept with their prostheses and 44% removed them from the mouth only for cleaning. None of the patients interviewed knew anything about brushes designed specifically for complete dentures. 37.3% had a restricted diet and 44% believed that a complete denture would last for more than 10 years. Conclusion: Within the limitations of this study, it was concluded that the edentulous patients surveyed had limited awareness of prosthetic hygiene and long-term oral care despite extended periods of denture use. © 2008 The Authors.

The economic burden associated with osteoporosis is considerable. As such, cost-effectiveness analyses are important contributors to the diagnostic and therapeutic decision-making process. The aim of this study was to review the cost effectiveness of treating post-menopausal osteoporosis with bisphosphonates and identify the key factors that influence the cost effectiveness of such treatment in the Swiss setting. A systematic search of databases (MEDLINE, EMBASE and the Cochrane Library) was conducted to identify published literature on the cost effectiveness of bisphosphonates in post-menopausal osteoporosis in the Swiss setting. Outcomes were compared with similar studies in Western European countries. Three cost-effectiveness studies of bisphosphonates in this patient population were identified; all were from a healthcare payer perspective. Outcomes showed that, relative to no treatment, treatment with oral bisphosphonates was predicted to be cost saving for most women aged ≥70 years with osteoporosis or at least one risk factor for fracture, and cost effective for women aged ≥75 years without prior fracture when used as a component of a population-based screen-and-treat programme. Results were most sensitive to changes in fracture risk, cost of fractures, cost of treatment, nursing home admissions and adherence with treatment. Swiss results were generally comparable to those in other European settings. Assuming similar clinical efficacy, lowering treatment cost (through the use of price-reduced brand-name or generic drugs) and/or improving adherence should both contribute to further improving the cost effectiveness of bisphosphonates in women with post-menopausal osteoporosis. Published evidence indicates that bisphosphonates are estimated to be similarly cost effective or cost saving in most treatment scenarios of post-menopausal osteoporosis in Switzerland and in neighbouring European countries.

Trends in mental health services for older adults during the past decade were used to predict salient issues for the current decade. These include overreliance on inpatient treatment, increased use of general hospitals as treatment sites, inadequate integration with the nursing-home industry, and insufficient mental health referrals from general medical providers. In the decade ahead, the mental health needs of older adults are unlikely to be an identified focus; rather the issues will overlap with other priorities (e.g., biomedical research on brain functioning, alternative treatment programs for the chronically mentally ill, and containing health care costs). Advocates for the elderly will be successful to the extent that they cast aging services within the context of these other concerns.

Data from 50 residents of a long-term care facility were used to examine the extent to which performance on a brief, objective inventory could predict a clinical psychologist's evaluation of competence to participate in decisions about medical care. Results indicate that the competence to participate in medical decisions of two-thirds of the residents could be accurately assessed using scores on a mental status instrument and two vignette-based measures of medical decision-making. These procedures could enable nursing home staff to objectively assess the competence of residents to participate in important decisions about their medical care.
