Body fat distribution is a heritable trait and a well-established predictor of adverse metabolic outcomes, independent of overall adiposity. To increase our understanding of the genetic basis of body fat distribution and its molecular links to cardiometabolic traits, here we conduct genome-wide association meta-analyses of traits related to waist and hip circumferences in up to 224,459 individuals. We identify 49 loci (33 new) associated with waist-to-hip ratio adjusted for body mass index (BMI), and an additional 19 loci newly associated with related waist and hip circumference measures (P < 5 × 10−8). In total, 20 of the 49 waist-to-hip ratio adjusted for BMI loci show significant sexual dimorphism, 19 of which display a stronger effect in women. The identified loci were enriched for genes expressed in adipose tissue and for putative regulatory elements in adipocytes. Pathway analyses implicated adipogenesis, angiogenesis, transcriptional regulation and insulin resistance as processes affecting fat distribution, providing insight into potential pathophysiological mechanisms.

The leading cause of death in the Western world continues to be coronary heart disease (CHD). At the root of the disease process is dyslipidemia an aberration in the relevant amounts of circulating blood lipids. Cholesterol builds up in the arterial wall and following rupture of these plaques, myocardial infarction or stroke can occur. Heart disease runs in families and a number of hereditary forms are known. The leading cause of adult dyslipidemia presently however is overweight and obesity. This thesis work presents an investigation of the molecular genetics of common, hereditary dyslipidemia and the tightly related condition of obesity. Familial combined hyperlipidemia (FCHL) is the most common hereditary dyslipidemia in man with an estimated population prevalence of 1-6%. This complex disease is characterized by elevated levels of serum total cholesterol, triglycerides or both and is observed in about 20% of individuals with premature CHD. Our group identified the disease to be associated with genetic variation in the USF1 transcription factor gene. USF1 has a key role in regulating other genes that control lipid and glucose metabolism as well as the inflammatory response all central processes in the progression of atherosclerosis and CHD. The first two works of this thesis aimed at understanding how these USF1 variants result in increased disease risk. Among the many, non-coding single-nucleotide polymorphisms (SNPs) that associated with the disease, one was found to have a functional effect. The risk-enhancing allele of this SNP seems to eradicate the ability of the important hormone insulin to induce the expression of USF1 in peripheral tissues. The resultant changes in the expression of numerous USF1 target genes over time probably enhance and accelerate the atherogenic processes. Dyslipidemias often represent an outcome of obesity and in the final work of this thesis we wanted to address the metabolic pathways related to acquired obesity. It is recognized that active processes in adipose tissue play an important role in the development of dyslipidemia, insulin resistance and other pathological conditions associated with obesity. To minimize the confounding effects of genetic differences present in most human studies, we investigated a rare collection of identical twins that differed significantly in the amount of body fat. In the obese, but otherwise healthy young adults, several notable changes were observed. In addition to chronic inflammation, the adipose tissue of the obese co-twins was characterized by a marked (47%) decrease in amount of mitochondrial DNA (mtDNA) a change associated with mitochondrial dysfunction. The catabolism of branched chain amino acids (BCAAs) was identified as the most down-regulated process in the obese co-twins. A concordant increase in the serum level of these insulin secretagogues was identified. This hyperaminoacidemia may provide the feed-back signal from insulin resistant adipose tissue to the pancreas to ensure an appropriately augmented secretory response. The down regulation of BCAA catabolism correlated closely with liver fat accumulation and insulin. The single most up-regulated gene (5.9 fold) in the obese co-twins was osteopontin (SPP1) a cytokine involved in macrophage recruitment to adipose tissue. SPP1 is here implicated as an important player in the development of insulin resistance. These studies of exceptional study samples provide better understanding of the underlying pathology in common dyslipidemias and other obesity associated diseases important for future improvement of intervention strategies and treatments to combat atherosclerosis and coronary heart disease.

A prevalência de obesidade infantil vem crescendo em todo o mundo e está associada com aumento da morbimortalidade por doenças cardiovasculares na vida adulta. A obesidade na infância, somada às alterações no metabolismo glicêmico e lipídico e ao aumento do estresse oxidativo e estado inflamatório contribuem para o aumento da espessura do complexo médio-intimal da carótida (carotid artery intima-medial thickness - cIMT) em tenra idade, possibilitando o desenvolvimento precoce do processo aterosclerótico. O objetivo deste estudo foi avaliar e comparar a cIMT, os indicadores do metabolismo glicídico e lipídico, o estado oxidativo e antioxidante, a composição corporal e o consumo alimentar em crianças pré-púberes obesas e eutróficas e determinar as inter-relações entre as variáveis. Foram medidos massa corporal total (MCT), estatura (E), circunferência da cintura (CC); glicemia, insulina, colesterol total (CT), lipoproteína de baixa densidade (LDL-colesterol), lipoproteína de alta densidade (HDL-colesterol), ácido úrico, proteína C-reativa ultra-sensível (PCR-us) e capacidade antioxidante (DPPH) sanguíneos; cIMT (USG, General Eletric); consumo alimentar (3 recordatórios de 24 h) para análise de macronutrientes e ácidos graxos. Foram, ainda, calculados o índice de massa corporal por idade (IMC/I) e HOMA-IR. O grupo de crianças obesas (n = 30) apresentava IMC/I acima do p97 (WHO, 2007) cujos dados foram comparados com os de um grupo controle (n = 25), composto por crianças eutróficas, da mesma faixa etária. As análises estatísticas acompanharam as características da amostra para dados não-paramétricos, com graus de significância de p < 0,05. A idade das crianças, em média, foi de 7,8 1,3 anos. A comparação dos indicadores entre os grupos mostrou valores significativamente maiores de MCT, IMC/I, CC, consumo calórico e de carboidratos, CT, LDL-colesterol, insulina, HOMA-IR, ácido úrico, PCR-us e cIMT no grupo de crianças obesas. Foram encontradas associações positivas da cIMT com MCT, IMC/I e CC. Essa última associou-se positivamente com ácido úrico, insulina e HOMA-IR. A PCR-us mostrou associação positiva com MCT, IMC/I, CC, ácido úrico, insulina e HOMA-IR. Os resultados analisados nos permitem concluir que as crianças obesas apresentaram maior massa adiposa abdominal, maior consumo energético, proveniente de carboidratos e valores maiores dos fatores de risco para doenças cardiovasculares do que seus pares eutróficos. Nossos resultados analisados em conjunto, mostram que a obesidade infantil acarreta danos cardiometabólicos que poderão causar prejuízos a saúde na vida adulta. O processo de aterosclerose precoce sofre influência da massa de gordura total e abdominal, a qual está diretamente relacionada à resistência à insulina, ao estado inflamatório e antioxidante. O conhecimento dos fatores de risco desta população deverá embasar estratégias de tratamento com o objetivo de reduzir a morbimortalidade por doenças cardiovasculares na idade adulta.

A exposição precoce a fatores de risco cardiovascular gera estado inflamatório crônico, podendo causar dano da função endotelial, seguido de espessamento da íntima-média carotídea. O objetivo desta pesquisa foi estudar a espessura íntima-média carotídea e seu comportamento em relação aos fatores e biomarcadores de risco cardiovascular em crianças com excesso de peso pré-púberes. Realizou-se estudo transversal com 80 obesos, 18 com sobrepeso e 31 eutróficos do Ambulatório de Pediatria do Hospital Universitário Pedro Ernesto da Universidade do Estado do Rio de Janeiro. Avaliou-se, através de comparação de médias, medianas e frequências, o comportamento dos fatores de risco e da espessura íntima-média carotídea entre os sexos; entre obesos, com sobrepeso e eutróficos; entre resistentes e não resistentes à insulina. Avaliou-se, através de análise de regressão logística bivariada e multivariada, associação entre os fatores de risco e espessamento de íntima-média carotídea. Houve diferença estatisticamente significativa das médias e medianas de escore Z de índice de massa corpórea (p-valor=0,02), pressão arterial sistólica (p-valor=0,04) e adiponectina (p-valor=0,02) entre sexos; de circunferência da cintura (p-valor=0,0001), pressão arterial sistólica (p-valor=0,0001), diastólica (p-valor=0,001), homeostaticmodelacessment for insulinresitance (p-valor=0,0001), colesterol total (p-valor=0,02), HDL (p-valor=0,01), LDL (p-valor=0,03), triglicerídeos (p-valor=0,01), proteína C reativa (p-valor=0,0001), interleucina 6 (p-valor=0,02), leptina (p-valor=0,0001), espessura da íntima-média carotídea esquerda (p-valor=0,03) entre obesos, com sobrepeso e eutróficos; de escore Z de índice de massa corpórea (p-valor=0,0009), circunferência da cintura (p-valor=0,0001), pressão arterial sistólica (p-valor=0,0001), diastólica (p-valor=0,0006), colesterol total (p-valor=0,0004), triglicerídeos (p-valor=0,0002), leptina (p-valor=0,004) entre resistentes e não resistentes à insulina. Na regressão logística bivariada, escore Z de índice de massa corpórea, circunferência da cintura e pressão arterial sistólica associaram-se positivamente (p-valor<0,05) com o espessamento das carótidas direita, esquerda e com média dos valores de ambas. Na regressão logística multivariada, escore Z de índice de massa corpórea (p-valor=0,02) e pressão arterial sistólica (p-valor=0,04), associaram-se positivamente com íntima-média carotídea espessada à esquerda; níveis tensionais sistólicos (p-valor=0,01) se associaram com a média dos valores da íntima média carotídea de ambos os lados.Os achados mostram nas crianças pré-púberes com excesso de peso: que os fatores e biomarcadores de risco cardiovascular já se encontram presentes; influência de escore Z de índice de massa corpórea e níveis tensionais sistólicos sobre espessura íntima-média carotídea. A prevenção de aterosclerose deve iniciar precocemente, identificando-se e controlando-se fatores de risco cardiovascular. O pediatra deve procurar promover saúde cardiovascular da criança, prevenindo e/ou controlando obesidade, orientado prática regular de exercícios físicos e hábitos alimentares saudáveis.

Introdução: a apneia obstrutiva do sono (AOS) é considerada um fator de risco para as doenças cardiovasculares. Os mecanismos responsáveis pelo desenvolvimento da aterosclerose potencializados pela AOS não são completamente conhecidos. Entretanto, existem evidências de que a AOS está associada com aumento no estresse oxidativo, elevação nos mediadores inflamatórios, resistência à insulina, ativação do sistema nervoso simpático, elevação da pressão arterial (PA) e a disfunção endotelial. Objetivo: avaliar a relação da AOS com a função endotelial, o estresse oxidativo, os biomarcadores inflamatórios, o perfil metabólico, a adiposidade corporal, a atividade simpática e a PA em indivíduos obesos. Métodos: estudo transversal envolvendo 53 pacientes obesos, com índice de massa corporal (IMC) ≥ 30 e < 40 Kg/m2, sem distinção de raça e gênero, apresentando idade entre 20 e 55 anos. O estudo do sono foi realizado com o equipamento Watch-PAT 200, sendo feito o diagnóstico de AOS quando índice apneia-hipopneia (IAH) ≥ 5 eventos/h. Todos os participantes foram submetidos à avaliação do (a): adiposidade corporal (peso, % gordura corporal e circunferências da cintura, quadril e pescoço); PA; atividade do sistema nervoso simpático (concentrações plasmáticas de catecolaminas); biomarcadores inflamatórios (proteína C reativa ultrassensível (PCR-us) e adiponectina); estresse oxidativo (malondialdeído); metabolismo glicídico (glicose, insulina e HOMA-IR) e lipídico (colesterol total e frações e triglicerídeos); e função endotelial (índice de hiperemia reativa (RHI) avaliado com o equipamento Endo-PAT 2000 e moléculas de adesão celular). A análise estatística foi realizada com o software STATA versão 10. Resultados: dos 53 pacientes avaliados 20 foram alocados no grupo sem AOS (grupo controle; GC) (IAH: 2,550,35 eventos/h) e 33 no grupo com AOS (GAOS) (IAH: 20,163,57 eventos/h). A faixa etária (39,61,48 vs. 32,52,09 anos) e o percentual de participantes do gênero masculino (61% vs. 25%) foram significativamente maiores no GAOS do que no GC (p=0,01). O GAOS em comparação o GC apresentou valores significativamente mais elevados de circunferência do pescoço (CP) (40,980,63 vs. 38,650,75 cm; p=0,02), glicemia (92,541,97 vs. 80,21,92 mg/dL; p=0,0001), PA sistólica (126,051,61 vs.118,16 1,86 mmHg; p=0,003) e noradrenalina (0,160,02 vs. 0,120,03 ng/mL; p=0,02). Após ajustes para fatores de confundimento, a glicose e a PCR-us foram significativamente mais elevadas no GAOS. Os 2 grupos apresentaram valores semelhantes de IMC, insulina, HOMA-IR, perfil lipídico, adiponectina, PA diastólica, adrenalina, dopamina, moléculas de adesão celular e malondialdeído. A função endotelial avaliada pelo RHI também foi semelhante nos 2 grupos (GAOS:1,850,2 vs. GC:1,980,1; p=0,31). Nas análises de correlação, considerando todos os participantes do estudo, o IAH apresentou associação positiva e significativa com CP e PCR-us após ajustes para fatores de confundimento. A saturação mínima de O2 se associou de forma negativa e significativa com a CP, os níveis séricos de insulina e o HOMA-IR, mesmo após ajustes para fatores de confundimento. Conclusões: o presente estudo sugere que em obesos a AOS está associada com valores mais elevados de glicemia e inflamação; o aumento do IAH apresenta associação significativa com a obesidade central e com a inflamação; e a queda na saturação de oxigênio se associa com resistência à insulina.

beta-arrestin-1 is an adaptor protein that mediates agonist-dependent internalization and desensitization of G-protein-coupled receptors (GPCRs) and also participates in the process of heterologous desensitization between receptor tyrosine kinases and GPCR signaling. In the present study, we determined whether beta-arrestin-1 is involved in insulin-induced insulin receptor substrate 1 (IRS-1) degradation. Overexpression of wild-type (WT) beta-arrestin-1 attenuated insulin-induced degradation of IRS-1, leading to increased insulin signaling downstream of IRS-1. When endogenous beta-arrestin-1 was knocked down by transfection of beta-arrestin-1 small interfering RNA, insulin-induced IRS-1 degradation was enhanced. Insulin stimulated the association of IRS-1 and Mdm2, an E3 ubiquitin ligase, and this association was inhibited to overexpression of WT beta-arrestin-1, which led by decreased ubiquitin content of IRS-1, suggesting that both beta-arrestin-1 and IRS-1 competitively bind to Mdm2. In summary, we have found the following: (i) beta-arrestin-1 can alter insulin signaling by inhibiting insulin-induced proteasomal degradation of IRS-1; (ii) beta-arrestin-1 decreases the rate of ubiquitination of IRS-1 by competitively binding to endogenous Mdm2, an E3 ligase that can ubiquitinate IRS-1; (iii) dephosphorylation of S412 on beta-arrestin and the amino terminus of beta-arrestin-1 are required for this effect of beta-arrestin on IRS-1 degradation; and (iv) inhibition of beta-arrestin-1 leads to enhanced IRS-1 degradation and accentuated cellular insulin resistance.

Obesity is a low grade inflammatory state associated with premature cardiovascular morbidity and mortality. Along with traditional risk factors the measurement of endothelial function, insulin resistance, inflammation and arterial stiffness may contribute to the assessment of cardiovascular risk. We conducted a randomised placebo controlled trial to assess the effects of 12 weeks treatment with a PPAR-alpha agonist (fenofibrate) and a PPAR-gamma agonist (pioglitazone) on these parameters in obese glucose tolerant men. Arterial stiffness was measured using augmentation index and pulse wave velocity (PWV). E-selectin, VCAM-1 and ICAM-1 were used as markers of endothelial function. Glucose-dependent insulinotropic polypeptide (GIP) has significant potential in diabetes therapy due to its ability to serve as a glucose-dependent activator of insulin secretion. However, its biological activity is severely compromised by the ubiquitous enzyme dipeptidylpeptidase IV (DPP IV), which removes the N-terminal Tyr(1)-Ala(2) dipeptide from GIP. Therefore, 2 novel N-terminal Ala(2)-substituted analogs of GIP, with Ala substituted by 2-aminobutyric acid (Abu) or sarcosine (Sar), were synthesized and tested for metabolic stability and biological activity both in vitro and in vivo. Incubation with DPP IV gave half-lives for degradation of native GIP, (Abu(2))GIP, and (Sar(2))GIP to be 2.3, 1.9, and 1.6 hours, respectively, while in human plasma, the half-lives were 6.2, 7.6, and 5.4 hours, respectively. In Chinese hamster lung (CHL) cells expressing the cloned human GIP receptor, native GIP, (Abu(2))GIP, and (Sar(2))GIP dose-dependently stimulated cyclic adenosine monophosphate (camp) production with EC50 values of 18.2, 38.5, and 54.6 nmol/L, respectively. In BRIN-BD11 cells, both (Abu(2))GIP and (Sar(2))GIP (10(-13) to 10(-8) mol/L) dose-dependently stimulated insulin secretion with significantly enhanced effects at 16.7 mmol/L compared with 5.6 mmol/L glucose. In obese diabetic (ob/ob) mice, GIP and (Sar(2))GIP significantly increased (1.4-fold to 1.5-fold; P <.05) plasma insulin concentrations, whereas (Abu(2))GIP exerted only minor effects. Changes in plasma glucose were small reflecting the severe insulin resistance of this mutant. The present data show that substitution of the penultimate N-terminal Ala(2) in GIP by Abu or Sar results in analogs with moderately reduced metabolic stability and biological activity in vitro, but with preserved biological activity in vivo. Insulin resistance and diabetes might promote neurodegenerative disease, but a molecular link between these disorders is unknown. Many factors are responsible for brain growth, patterning, and survival, including the insulin-insulin-like growth factor (IGF)-signaling cascades that are mediated by tyrosine phosphorylation of insulin receptor substrate (IRS) proteins. Irs2 signaling mediates peripheral insulin action and pancreatic beta-cell function, and its failure causes diabetes in mice. In this study, we reveal two important roles for Irs2 signaling in the mouse brain. First, disruption of the Irs2 gene reduced neuronal proliferation during development by 50%, which dissociated brain growth from Irs1-dependent body growth. Second, neurofibrillary tangles containing phosphorylated tau accumulated in the hippocampus of old Irs2 knock-out mice, suggesting that Irs2 signaling is neuroprotective. Thus, dysregulation of the Irs2 branch of the insulin-Igf-signaling cascade reveals a molecular link between diabetes and neurodegenerative disease.

Aims: Glycation of insulin has been demonstrated within pancreatic beta-cells and the resulting impaired bioactivity may contribute to insulin resistance in diabetes. We used a novel radioimmunoassay to evaluate the effect of nateglinide on plasma concentrations of glycated insulin and glucose tolerance in type 2 diabetes. Methods. Ten patients (5 M/5 F, age 57.8 +/- 1.9 years, HbA(1c), 7.6 +/- 0.5%,, fasting plasma glucose 9.4 +/- 1.2 mmol/l, creatinine 81.6 +/- 4.5 mumol/l) received oral nateglinide 120 mg or placebo, 10 min prior to 75 g oral glucose in a random, single blind, crossover design, 1 week apart. Blood samples were taken for glycated insulin, glucose, insulin and C-peptide over 225 min. Results: Plasma glucose and glycated insulin responses were reduced by 9% (P = 0.005) and 38% (P = 0.047), respectively, following nateglinide compared with placebo. Corresponding AUC measures for insulin and C-peptide were enhanced by 36% (P = 0.005) and 25% (P = 0.007) by nateglinide. Conclusions: Glycated insulin in type 2 diabetes is reduced in response to the insulin secretagogue nateglinide, resulting in preferential release of native insulin. Since glycated insulin exhibits impaired biological activity, reduced glycated insulin release may contribute to the anti hyperglycaemic action of nateglinide. (C) 2003 Elsevier Ireland Ltd. Emerging science supports therapeutic roles of strawberries, blueberries, and cranberries in metabolic syndrome, a prediabetic state characterized by several cardiovascular risk factors. Interventional studies reported by our group and others have demonstrated the following effects: strawberries lowering total and LDL-cholesterol, but not triglycerides, and decreasing surrogate biomarkers of atherosclerosis (malondialdehyde and adhesion molecules); blueberries lowering systolic and diastolic blood pressure and lipid oxidation and improving insulin resistance; and low-calorie cranberry juice selectively decreasing biomarkers of lipid oxidation (oxidized LDL) and inflammation (adhesion molecules) in metabolic syndrome. Mechanistic studies further explain these observations as up-regulation of endothelial nitric oxide synthase activity, reduction in renal oxidative damage, and inhibition of the activity of carbohydrate digestive enzymes or angiotensin-converting enzyme by these berries. These findings need confirmation in future studies with a focus on the effects of strawberry, blueberry, or cranberry intervention in clinical biomarkers and molecular mechanisms underlying the metabolic syndrome.

AIMS: The effect of dietary sucrose on insulin resistance and the pathogenesis of diabetes and vascular disease is unclear. We assessed the effect of 5% versus 15% sucrose intakes as part of a weight maintaining, eucaloric diet in overweight/obese subjects.

METHODS: Thirteen subjects took part in a randomised controlled crossover study (M:F 9:4, median age 46 years, range 37-56 years, BMI 31.7±0.9 kg/m(2)). Subjects completed two 6 week dietary periods separated by 4 week washout. Diets were designed to have identical macronutrient profile. Insulin action was assessed using a two-step hyperinsulinaemic euglycaemic clamp; glucose tolerance, vascular compliance, body composition and lipid profiles were also assessed.

RESULTS: There was no change in weight or body composition between diets. There was no difference in peripheral glucose utilization or suppression of endogenous glucose production. Fasting glucose was significantly lower after the 5% diet. There was no demonstrated effect on lipid profiles, blood pressure or vascular compliance.

CONCLUSION: A low-sucrose diet had no beneficial effect on insulin resistance as measured by the euglycaemic glucose clamp. However, reductions in fasting glucose, one hour insulin and insulin area under the curve with the low sucrose diet on glucose tolerance testing may indicate a beneficial effect and further work is required to determine if this is the case. BACKGROUND: Dietary cocoa is an important source of flavonoids and is associated with favorable cardiovascular disease effects, such as improvements in vascular function and lipid profiles, in nondiabetic adults. Type 2 diabetes (T2D) is associated with adverse effects on postprandial serum glucose, lipids, inflammation, and vascular function.

OBJECTIVE: We examined the hypothesis that cocoa reduces metabolic stress in obese T2D adults after a high-fat fast-food-style meal.

METHODS: Adults with T2D [n = 18; age (means ± SEs): 56 ± 3 y; BMI (in kg/m(2)): 35.3 ± 2.0; 14 women; 4 men) were randomly assigned to receive cocoa beverage (960 mg total polyphenols; 480 mg flavanols) or flavanol-free placebo (110 mg total polyphenols; <0.1 mg flavanols) with a high-fat fast-food-style breakfast [766 kcal, 50 g fat (59% energy)] in a crossover trial. After an overnight fast (10-12 h), participants consumed the breakfast with cocoa or placebo, and blood sample collection [glucose, insulin, lipids, and high-sensitivity C-reactive protein (hsCRP)] and vascular measurements were conducted at 0.5, 1, 2, 4, and 6 h postprandially on each study day. Insulin resistance was evaluated by homeostasis model assessment.

RESULTS: Over the 6-h study, and specifically at 1 and 4 h, cocoa increased HDL cholesterol vs. placebo (overall Δ: 1.5 ± 0.8 mg/dL; P ≤ 0.01) but had no effect on total and LDL cholesterol, triglycerides, glucose, and hsCRP. Cocoa increased serum insulin concentrations overall (Δ: 5.2 ± 3.2 mU/L; P < 0.05) and specifically at 4 h but had no overall effects on insulin resistance (except at 4 h, P < 0.05), systolic or diastolic blood pressure, or small artery elasticity. However, large artery elasticity was overall lower after cocoa vs. placebo (Δ: -1.6 ± 0.7 mL/mm Hg; P < 0.05), with the difference significant only at 2 h.

CONCLUSION: Acute cocoa supplementation showed no clear overall benefit in T2D patients after a high-fat fast-food-style meal challenge. Although HDL cholesterol and insulin remained higher throughout the 6-h postprandial period, an overall decrease in large artery elasticity was found after cocoa consumption. Insulin signaling to the glomerular podocyte is important for normal kidney function and is implicated in the pathogenesis of diabetic nephropathy (DN). This study determined the role of the insulin receptor substrate 2 (IRS2) in this system. Conditionally immortalized murine podocytes were generated from wild-type (WT) and insulin receptor substrate 2-deficient mice (Irs2−/−). Insulin signaling, glucose transport, cellular motility and cytoskeleton rearrangement were then analyzed. Within the glomerulus IRS2 is enriched in the podocyte and is preferentially phosphorylated by insulin in comparison to IRS1. Irs2−/− podocytes are significantly insulin resistant in respect to AKT signaling, insulin-stimulated GLUT4-mediated glucose uptake, filamentous actin (F-actin) cytoskeleton remodeling and cell motility. Mechanistically, we discovered that Irs2 deficiency causes insulin resistance through up-regulation of the phosphatase and tensin homolog (PTEN). Importantly, suppressing PTEN in Irs2−/− podocytes rescued insulin sensitivity. In conclusion, this study has identified for the first time IRS2 as a critical molecule for sensitizing the podocyte to insulin actions through its ability to modulate PTEN expression. This finding reveals two potential molecular targets in the podocyte for modulating insulin sensitivity and treating DN.
