OBJECTIVE: We studied the angiographic findings in patients with small epidural hematomas and cranial fractures crossing over the trajectory of the middle meningeal artery and its branches. Additionally, the Occurrence of traumatic vascular lesions and their clinical relevance and treatment are discussed. METHODS: A consecutive analysis was performed for 24 patients who harbored small epidural hematomas in middle meningeal artery topography associated with cranial fractures. Computed tomographic scans and plain x-ray studies were used to diagnose linear cranial fractures. Patients with large epidural hematomas or associated traumatic lesions were excluded from the study. Selective ipsilateral external carotid angiograms were obtained, and an endovascular procedure was performed if any vascular injury was evidenced. RESULTS: In all patients with cranial fractures crossing over the middle meningeal artery and its branches, some kind of vascular lesion was seen. Two types of findings were noted: active extravasation of the contrast medium (71%) and pseudoaneurysms (29%). Early filling of diploic vessels was found in 8.3% of fractures concomitantly with active extravasation. Embolization was performed in all patients. No additional enlargement of the epidural hematoma was observed, and the postoperative period was uneventful. CONCLUSION: This study shows that pseudoaneurysms and active extravasation of contrast are common findings in this subset of patients. Although the natural history of these lesions is still poorly understood, additional investigation with ipsilateral external carotid angiography may be recommended, considering the potentially catastrophic consequences of late rupture.

Background: A 150 cm(3) pear-shaped gastric balloon with a 30 cm-long duodenal stem and a 7 g metallic weight at its distal end was designed and developed to facilitate weight loss by (a) delaying gastric emptying thus enhancing interprandial satiety, and (b) stimulating antral and duodenal receptors of satiation. Methods: Twenty-six patients (body mass index of 29 to 40 kg/m(2)) who failed to lose weight despite dietary intervention underwent endoscopic implantation of the balloon device. Patients were monitored for tolerance to the balloon, complications, weight loss, and compliance with a restricted caloric intake. Results: Six men and 20 women with a median body weight of 93.0kg (range, 73.5 to 119.9), median body mass index 34.3 kg/m(2) (range, 28.8 to 39.5) underwent balloon implantation for a median period of 4.0 months (range, 0.75 to 6.0). Twenty-two patients successfully complied with a 1250 to 1500 kcal daily diet restriction during the study period. Median weight reduction was 6.5 kg (range, 3.7 to 19.9). Patients with initial body weight of > 90 kg tended to loose more weight (8.1 kg) than patients weighing < 90 kg (4.5 kg) (P = 0.14). Nine patients with dwell times of 6 months lost 11.5 +/- 4.6 kg. The balloon malfunctioned in 4 patients (in I patient, the balloon leaked spontaneously but remained in the stomach and in 3 patients, the balloon migrated distally). Conclusions: Our novel balloon device may be effective in inducing weight loss by promoting compliance with a restricted caloric intake and is well tolerated due to its small size. Complications resulted from balloon rupture, which can be easily prevented by enhancements in design and use of alternative materials.

Background Recent physiological knowledge allows the design of bariatric procedures that aim at neuroendocrine changes instead of at restriction and malabsorption. Digestive adaptation is a surgical technique for obesity based in this rationale. Methods The technique includes a sleeve gastrectomy, an omentectomy and a jejunectomy that leaves initial jejunum and small bowel totaling at least 3 m (still within normal variation of adult human bowel length). Fasting ghrelin and resistin and fasting and postprandial GLP-1 and PYY were measured pre- and postoperatively. Results Patients: 228 patients with initial body mass index (BMI) varying from 35 to 51 kg/m(2); follow-up: I to 5 years; average EBMIL% was 79.7% in the first year; 77.7% in the second year; 71.6% in the third year; 68.9% in the fourth year. Patients present early satiety and major improvement in presurgical comorbidities, especially diabetes. Fasting ghrelin and resistin were significantly reduced (P<0.05); GLP-1 and PYY response to food ingestion was enhanced (P<0.05). Surgical complications (4.4%) were resolved without sequela and without mortality. There was neither diarrhea nor detected malabsorption. Conclusions Based on physiological and supported by evolutionary data, this procedure creates a proportionally reduced gastrointestinal (GI) tract that amplifies postprandial neuroendocrine responses. It leaves basic GI functions unharmed. It reduces production of ghrelin and resistin and takes more nutrients to be absorbed distally enhancing GLP-1 and PYY secretion. Diabetes was improved significantly without duodenal exclusion. The patients do not present symptoms nor need nutritional support or drug medication because of the procedure, which is safe to perform.

Aim To evaluate gastrointestinal motility during 5-fluorouracil (5-FU)-induced intestinal mucositis. Materials and methods Wistar rats received 5-FU (150 mg kg(-1), i.p.) or saline. After the 1st, 3rd, 5th, 15th and 30th day, sections of duodenum, jejunum and ileum were removed for assessment of epithelial damage, apoptotic and mitotic indexes, MPO activity and GSH concentration. In order to study gastrointestinal motility, on the 3rd or 15th day after 5-FU treatment, gastric emptying in vivo was measured by scintilographic method, and stomach or duodenal smooth muscle contractions induced by CCh were evaluated in vitro. Results On the third day of treatment, 5-FU induced a significant villi shortening, an increase in crypt depth and intestinal MPO activity and a decrease in villus/crypt ratio and GSH concentration. On the first day after 5-FU there was an increase in the apoptosis index and a decrease in the mitosis index in all intestinal segments. After the 15th day of 5-FU treatment, a complete reversion of all these parameters was observed. There was a delay in gastric emptying in vivo and a significant increase in gastric fundus and duodenum smooth muscle contraction, after both the 3rd and 15th day. Conclusions 5-FU-induced gastrointestinal dysmotility outlasts intestinal mucositis.

Infection by Helicobacter pylori is associated with the development of several gastroduodenal diseases, including gastritis, peptic ulcer disease (gastric ulcers and duodenal ulcers), and gastric adenocarcinoma. Although a number of putative virulence factors have been reported for H. pylori, there are conflicting results regarding their association with specific H. pylori-related diseases. In this work, we investigated the presence of virB11 and cagT, located in the left half of the cag pathogenicity island (cagPAI), and the jhp917-jhp918 sequences, components of the dupA gene located in the plasticity zone of H. pylori, in Brazilian isolates of H. pylori. We also examined the association between these genes and H. pylori-related gastritis, peptic ulcer disease, and gastric and duodenal ulcers in an attempt to identify a gene marker for clinical outcomes related to infection by H. pylori. The cagT gene was associated with peptic ulcer disease and gastric ulcers, whereas the virB11 gene was detected in nearly all of the samples. The dupA gene was not associated with duodenal ulcers or any gastroduodenal disease here analyzed. These results suggest that cagT could be a useful prognostic marker for the development of peptic ulcer disease in the state of Sao Paulo, Brazil. They also indicate that cagT is associated with greater virulence and peptic ulceration, and that this gene is an essential component of the type IV secretion system of H. pylori.

The essential oil of Eucalyptus tereticornis (EOET) has pharmacological activities but their effects on the gastrointestinal tract are yet unknown. It possesses alpha- and beta-pinene as minor constituents, isomers largely used as food or drink additives. In this work, we studied their actions on gut motility. After feeding with a liquid test meal, conscious rats received perorally EOET, alpha-, or beta-pinene, and the fractional dye retention was determined. EOET and its constituents decreased the gastric retention. In anesthetized rats, pinenes increased gastric tonus, while enhancing the meal progression in the small intestine of conscious rats. Both alpha- and beta-pinene contracted gastric strips in vitro but relaxed the duodenum. Conversely, EOET relaxed both the gastric and duodenal strips. In conclusion, EOET accelerates the gastric emptying of liquid, and part of its action is attributed to the contrasting effects induced by alpha- and beta-pinene on the gut.

Background: Obstructive sleep apnea (OSA) is related to increased systemic inflammation and arterial hypertension. We hypothesize that OSA is frequent in patients with acute hypertensive intracerebral hemorrhage (ICH) and is related to the perihematoma edema. Methods: Thirty-two non-comatose patients with a hypertensive ICH underwent polysomnography in the acute phase. Perihematoma edema volume was measured on CT scans at admission, after 24 h (early control) and after 4-5 days (late control). The Spearman coefficient (r(s)) was used for correlations. Results: OSA occurred in 19 (59.4%) patients. The apnea-hypopnea index was correlated with relative edema at admission CT (r(s) = 0.40; p = 0.031), early CT (r(s) = 0.46; p = 0.011) and at late CT (r(s) = 0.59; p = 0.006). Conclusions: OSA is highly frequent during the acute phase of hypertensive ICH and is related to perihematoma edema. Copyright (C) 2009 S. Karger AG, Basel

Gastrointestinal mucositis is a common side effect of cancer chemotherapy. Platelet-activating factor (PAF) is produced during gut inflammation. There is no evidence that PAF participates in antineoplastic-induced intestinal mucositis. This study evaluated the role of PAF in 5-fluorouracil (5-FU)-induced intestinal mucositis using a pharmacological approach and PAF receptor knockout mice (PAFR(-/-)). Wild-type mice or PAFR(-/-) mice were treated with 5-FU (450 mg/kg, i.p.). Other mice were treated with saline or BN52021 (20 mg/kg, s.c.), an antagonist of the PAF receptor, once daily followed by 5-FU administration. After the third day of treatment, animals were sacrificed and tissue samples from the duodenum were removed for morphologic evaluation. In addition, myeloperoxidase activity and the cytokine concentration were measured. 5-FU treatment decreased the duodenal villus height/crypt depth ratio, increased MPO activity, and increased the concentration of TNF-alpha, IL-1 beta and KC in comparison with saline-treated animals. In PAFR(-/-) mice and PAFR antagonist-treated mice, 5-FU-dependent intestinal damage was reduced and a decrease in duodenal villus height/crypt depth ratio was attenuated. However, the 5-FU-dependent increase in duodenum MPO activity was not affected. Without PAFR activation, 5-FU treatment did not increase the TNF-alpha, IL-1 beta and KC concentration. In conclusion, our study establishes the role of PAFR activation in 5-FU-induced intestinal mucositis. This study implicates treatment with PAFR antagonists as novel therapeutic strategy for this condition.

The objectives of this investigation were to understand transplacental transport of iron by secreted uteroferrin (UF) and haemophagous areas of water buffalo placenta and clarify the role(s) of blood extravasation at the placental-maternal interface. Placentomes and interplacentomal region of 51 placentae at various stages of gestation were fixed, processed for light and transmission electron microscopy, histochemistry and immunohistochemistry. Haemophagous areas were present in placentomes collected between 4 and 10 months of pregnancy. Perl`s reaction for ferric iron was negative in placentomes, but positive in endometrial glands. Positive staining for UF indicated areas in which it was being taken up by phagocytosis and/or fluid phase pinocytosis in areolae of the interplacentomal mesenchyme, with little staining in endometrial stroma. Imunohistochemistry detected UF in trophectoderm of haemophagous regions of placentomes and in other parts of the foetal villous tree, but the strongest immunostaining was in the epithelial cells and lumen of uterine glands. Ultrastructural analyses indicated that erythrophagocytosis was occurring and that erythrocytes were present inside cells of the chorion that also contained endocytic vesicles and caveolae. Results of this study indicate that both the haemophagous areas of placentomes and the areolae at the interface between chorion and endometrial glands are important sites for iron transfer from mother to foetal-placental tissues in buffalo throughout pregnancy.

Enzymes are crucial for the metabolism of macromolecular substrates. In the great majority of cells, most enzymes are constitutive. Nevertheless, inducible enzymes can predominate, determining specialized cell functions. Within this context, histochemistry/immunohistochemistry and biochemistry were used to investigate expression of peroxidase and reduced nicotinamide-adenine dinucleotide phosphate (NADPH)-oxidase, as well as the expression and activity of cathepsin D and acid phosphatase, in trophoblast cells within the endotheliochorial labyrinth and marginal hematoma of the term cat placenta. In the marginal hematoma, elevated Cathepsin D expression and activity was accompanied by erythrophagocytosis. In contrast, acid phosphatase activity was much more intense in the labyrinth, where metabolic exchanges occur. Peroxidase and NAD(P)H-oxidase were predominantly active in trophoblast cells within endosomal vesicles of different placental compartments, indicating that, although reactive oxygen species might participate in endosomal/lysosomal processes, they are not territorially specific or functional markers. These findings highlight differential characteristics of cathepsin D and acid phosphatase activity within each placental compartment, thereby contributing to the comprehension of the territorial role played by the placenta and facilitating future metabolic studies. O uso de antibióticos em cirurgias nasais é prática rotineira entre os otorrinolaringologistas. A maioria dos membros da Sociedade Americana de Rinologia utiliza rotineiramente antibiótico no pós-operatório de septoplastias, conduta esta considerada desnecessária por muitos autores. OBJETIVO: Nosso objetivo é avaliar a necessidade do uso de antibióticos em septoplastias, e as principais complicações pós-operatórias descritas na literatura. FORMA DE ESTUDO: clínico prospectivo com coorte transversal. MATERIAL E MÉTODOS: Avaliamos prospectivamente 35 pacientes submetidos à septoplastia e turbinectomia no Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, divididos em 3 grupos: o primeiro grupo não recebeu antibioticoterapia, o segundo grupo recebeu apenas cefazolina no momento na indução anestésica, e o terceiro grupo recebeu cefazolina no momento da indução anestésica e cefalexina durante 7 dias no pós-operatório. Um protocolo foi aplicado no pós-operatório imediato, após uma semana e um mês, pesquisando sangramentos, febre, náuseas, vômitos e dor, associado à endoscopia nasal, avaliando a presença de hematoma ou abscessos, além de procurar quantificar secreção purulenta. RESULTADOS: Não observamos diferença significativa com relação à dor, febre, náuseas, vômitos e sangramentos pós-operatórios entre os grupos. Nenhum paciente desenvolveu hematoma ou abscesso septal. Não houve também diferença com relação à quantidade de secreção purulenta nas fossas nasais através da endoscopia nasal. CONCLUSÃO: Septoplastias são consideradas potencialmente contaminadas, e não necessitam de antibioticoprofilaxia, pelo baixo risco de infecção pós-operatória.

A possibilidade de ser necessário um acesso combinado, com uma incisão cervical e outra torácica, torna o tratamento do bócio mergulhante um desafio tanto no pré quanto no intra-operatório. Discutimos uma padronização da técnica cirúrgica para minimizar a necessidade da abordagem torácica, tornando o bócio mergulhante uma patologia tratável cirurgicamente, por uma única incisão cervical, e com baixos índices de complicações. OBJETIVO: Avaliar a abordagem cirúrgica do bócio mergulhante por cervicotomia e analisar as complicações cirúrgicas. MATERIAL E MÉTODOS: Foi realizada uma coorte histórica com corte transversal por análise retrospectiva dos prontuários de pacientes submetidos à tireoidectomia no período de maio de 2002 a julho de 2007. Um total de 316 pacientes foi submetido à tireoidectomia sendo 33 (10,4%) por bócio mergulhante. RESULTADOS: Todos os 33 pacientes foram tratados cirurgicamente por via cervical sem necessidade de esternotomia. Não foram observadas lesões definitivas de nervo laríngeo inferior ou hipoparatireoidismo definitivo. Apenas 2 pacientes apresentaram paresia de nervo recorrente e 2 pacientes foram reabordados por hematoma cervical. CONCLUSÃO: Pacientes com bócio mergulhante podem ser tratados cirurgicamente por uma única incisão cervical com segurança e baixos índices de complicação.

Foram estudados 17 casos de ancilostomose e determinados alguns parâmetros hematológicos como: dosagem de hemoglobina, do ferro sérico e da transferrina, contagem de hemácias, hematócrito, volume corpuscular médio (VCM) e hemoglobina corpuscular média (HCM). O estudo incluiu também a obtenção de alguns dados eritrocinéticos, como a determinação da velocidade de decaimento do ferro plasmático (T1V2 do 59Fe) e da incorporação do ferro à hemoglobina. O estudo radioisotópico permitiu ainda determinar o volume de sangue e a quantidade de hemoglobina perdida nas fezes, bem como o teor de ferro reabsorvido dessa hemoglobina. Estabeleceram também o grau de infecção através da contagem de ovos e de vermes nas fezes. Os doentes não apresentaram evidente alteração nutricional. A carência de ferro foi o fator comum a todos os casos que exibiram anemia, constituindo a base fisiopatológica da anemia ancilostomótica. O verme fixado à mucosa duodenal suga o sangue do hospedeiro e esta espoliação de sangue a longo prazo provoca a anemia. O volume de sangue perdido é geralmente proporcional ao grau de infecção, mas a quantidade de hemoglobina perdida mostrou ser independente do volume de sangue espoliado. A reabsorção de grande parte do ferro da hemoglobina perdida na luz intestinal concorre para que a anemia se estabeleça mais tardiamente que em outras hemorragias como a vaginal. Os indivíduos anêmicos foram submetidos a transfusões de sangue e com isso apresentaram melhora clínica e laboratorial, imediata mas temporária. A cura clínica foi estabelecida somente após tratamento adequado com vermífugos.

Introdução – A reconstrução mamária, no processo pós-cancro da mama, poderá ser levada a cabo através da lipoaspiração cujas consequências poderão ser ultrapassadas através da fisioterapia. Objetivo – Verificar, num estudo de caso, os resultados da drenagem linfática manual (DLM) e das bandas neuromusculares (BNM) na dor, edema, hematoma e pigmentação do abdómen e mamas após lipoaspiração para reconstrução mamária. Metodologia – Uma paciente foi sujeita a 10 sessões de fisioterapia com aplicação de DLM e BNM. Em cada sessão foram avaliados, nas mamas e no abdómen, a dor, o edema, o hematoma e a pigmentação. Resultados – Eliminação da dor, desaparecimento do hematoma abdominal, normalização da pigmentação mamária e diminuição do edema. Conclusão – Neste estudo de caso, a DLM e as BNM melhoram as consequências no pós-cirúrgico de lipoaspiração para reconstrução mamária. ABSTRACT - Introduction – Breast reconstruction, post breast cancer, can be carried out through liposuction which consequences may be overcomed through physiotherapy. Objective – Check, in a study case, the results of manual lymphatic drainage (MLD) and neuromuscular bandages (NMB) in pain, edema, bruising and pigmentation on abdomen and breasts, after liposuction for breast reconstruction. Methodology – A patient underwent 10 sessions of physiotherapy with application of MLD and NMB. In each session were evaluated breast and abdomen pain, edema, bruising and pigmentation. Results – Elimination of pain, disappearance of abdominal hematoma, normalization of breast pigmentation and reduction of edema. Conclusion – In this study case, MLD and NMB improve the consequences of post-surgical liposuction for breast reconstruction.

O ferro é encontrado em praticamente todos os seres vivos, sendo um cofator para proteínas que desempenham funções essenciais à vida. Nos mamíferos, a maioria do ferro está incorporada na hemoglobina ou armazenado no fígado, ligado à ferritina. É absorvido pelos enterócitos, sendo a principal forma de controlo dos seus níveis. A sobrecarga de ferro pode levar a hemocromatose, podendo ser tóxica para vários órgãos. O fator de transcrição Nrf2 é importante na ativação de genes citoprotetores em situações de stress oxidativo/eletrofílico, colocando-se a hipótese de que poderá estar envolvido na resposta à progressão de doença devido à sobrecarga de ferro. Com o objetivo de determinar se a via do Nrf2 representa uma proteção contra a toxicidade do ferro a nível hepático, foram realizadas duas experiências nas quais murganhos C57BL/6 (B6) e Nrf2-/- machos foram alimentados com dieta standard ou com dieta enriquecida em ferro carbonilo (FeC) (0,5% ou 2,0%). Os resultados demonstram sobrecarga de ferro nos animais que receberam dieta enriquecida, sendo que os que receberam FeC 2,0% apresentaram níveis mais elevados de ferro hepático e sérico, bem como da saturação da transferrina. Os murganhos Nrf2-/- são mais suscetíveis a esta acumulação, mostrando evidências patológicas mais graves, nomeadamente necrose hepatocítica e infiltração de células inflamatórias. A deleção do Nrf2 associado a uma dieta suplementada com FeC 2,0% parece não ser suficiente para o desenvolvimento de fibrose hepática. O estudo da expressão de genes e proteínas do metabolismo do ferro mostrou que os animais B6 e Nrf2-/- são igualmente capazes de responder à sobrecarga de ferro, sugerindo que a sua diferente suscetibilidade à toxicidade do ferro não se deverá a uma regulação ineficiente da homeostasia do Fe. A dieta com FeC 2,0% aumentou a expressão de dois genes alvo do Nrf2, Nqo1 e Gsta1, o que não se verificou com os genes e proteínas GCLC e GCLM. A expressão de genes pró-inflamatórios não mostrou evidências de inflamação nestes animais. Foi demonstrado que os animais Nrf2-/- são mais suscetíveis à toxicidade do ferro, concluindo-se que a via do Nrf2 é ativada em resposta a uma dieta contendo quantidades excessivas de FeC e que confere proteção contra a acumulação de ferro em murganhos B6.
