Abstract Objective: To propose an algorithm to determine the necessity for ultrasonography-guided fine-needle aspiration (US-FNA) in preoperative axillary lymph node staging of patients with invasive breast cancer. Materials and Methods: Prospective study developed at National Cancer Institute. The study sample included 100 female patients with breast cancer referred for axillary staging by US-FNA. Results: The overall US-FNA sensitivity was set at 79.4%. The positive predictive value was calculated to be 100%, and the negative predictive value, 69.5%. The US-FNA sensitivity for lymph nodes with normal sonographic features was 0%, while for indeterminate lymph nodes it was 80% and, for suspicious lymph nodes, 90.5%. In the assessment of invasive breast tumors stages T1, T2 and T3, the sensitivity was respectively 69.6%, 83.7% and 100%. US-FNA could avoid sentinel node biopsy in 54% of cases. Conclusion: Axillary ultrasonography should be included in the preoperative staging of all patients with invasive breast cancer. The addition of US-FNA in cases of lymph nodes suspicious for malignancy may prevent more than 50% of sentinel lymphadenectomies, significantly shortening the time interval to definitive therapy.

Modeling ecological niches of species is a promising approach for predicting the geographic potential of invasive species in new environments. Argentine ants (Linepithema humile) rank among the most successful invasive species: native to South America, they have invaded broad areas worldwide. Despite their widespread success, little is known about what makes an area susceptible - or not - to invasion. Here, we use a genetic algorithm approach to ecological niche modeling based on high-resolution remote-sensing data to examine the roles of niche similarity and difference in predicting invasions by this species. Our comparisons support a picture of general conservatism of the species' ecological characteristics, in spite of distinct geographic and community contexts

Morphological and spectroscopic studies of Sr2CeO4 blue phosphor in the form of fine particles prepared from a powdered multi-component precursor, via a combustion method, are reported. Samples were also prepared through a solid-state reaction and from a polymeric precursor for comparison. Citric acid or glycine as fuels in the combustion method lead to a mixture which is heated at 950 ºC for 4 h, resulting in spheroidal particles with a diameter between 250-550 nm. Samples from the polymeric precursor result in spheroidal particles (350-550 nm) and from the solid-state reaction in irregular particles (~ 5 mum). Therefore, the combustion method is adequate for preparation of Sr2CeO4 in the form of spherical fine particles.

MOTOR IMPAIRMENTS ARE COMMON AFTER STROKE but efficacious therapies for these dysfunctions are scarce. Extending an earlier study on the effects of music-supported training (MST), behavioral indices of motor function were obtained before and after a series of training sessions to assess whether this new treatment leads to improved motor functions. Furthermore, music-supported training was contrasted to functional motor training according to the principles of constraint-induced therapy (CIT). In addition to conventional physiotherapy, 32 stroke patients with moderately impaired motor function and no previous musical experience received 15 sessions of MST over a period of three weeks, using a manualized, step-bystep approach. A control group consisting of 15 patients received 15 sessions of CIT in addition to conventional physiotherapy. A third group of 30 patients received exclusively conventional physiotherapy and served as a control group for the other three groups. Fine as well as gross motor skills were trained by using either a MIDI-piano or electronic drum pads programmed to emit piano tones. Motor functions were assessed by an extensive test battery. MST yielded significant improvement in fine as well as gross motor skills with respect to speed, precision, and smoothness of movements. These improvements were greater than after CIT or conventional physiotherapy. In conclusion, with equal treatment intensity, MST leads to more pronounced improvements of motor functions after stroke than CIT.

Understanding the factors controlling fine root respiration (FRR) at different temporal scales will help to improve our knowledge about the spatial and temporal variability of soil respiration (SR) and to improve future predictions of CO2 effluxes to the atmosphere. Here we present a comparative study of how FRR respond to variability in soil temperature and moisture in two widely spread species, Scots pines (Pinus sylvestris L.) and Holm-oaks (HO; Quercus ilex L.). Those two species show contrasting water use strategies during the extreme summer-drought conditions that characterize the Mediterranean climate. The study was carried out on a mixed Mediterranean forest where Scots pines affected by drought induced die-back are slowly being replaced by the more drought resistant HO. FRR was measured in spring and early fall 2013 in excised roots freshly removed from the soil and collected under HO and under Scots pines at three different health stages: dead (D), defoliated (DP) and non-defoliated (NDP). Variations in soil temperature, soil water content and daily mean assimilation per tree were also recorded to evaluate FRR sensibility to abiotic and biotic environmental variations. Our results show that values of FRR were substantially lower under HO (1.26 ± 0.16 microgram CO2 /groot·min) than under living pines (1.89 ± 0.19 microgram CO2 /groot·min) which disagrees with the similar rates of soil respiration previously observed under both canopies and suggest that FRR contribution to total SR varies under different tree species. The similarity of FRR rates under HO and DP furthermore confirms other previous studies suggesting a recent Holm-oak root colonization of the gaps under dead trees. A linear mixed effect model approach indicated that seasonal variations in FRR were best explained by soil temperature (p<0.05) while soil moisture was not exerting any direct control over FRR, despite the low soil moisture values during the summer sampling. Plant assimilation rates were positively related to FRR explaining part of the observed variability (p<0.01). However the positive relations of FRR with plant assimilation occurred mainly during spring, when both soil moisture and plant assimilation rates were higher. Our results finally suggest that plants might be able to maintain relatively high rates of FRR during the sub-optimal abiotic and biotic summer conditions probably thanks to their capacity to re-mobilize carbon reserves and their capacity to passively move water from moister layers to upper layers with lower water potentials (where the FR were collected) by hydraulic lift.

Den snart 200 år gamla vetenskapsgrenen organisk synteskemi har starkt bidragit till moderna samhällens välfärd. Ett av flaggskeppen för den organiska synteskemin är utvecklingen och produktionen av nya läkemedel och speciellt de aktiva substanserna däri. Därmed är det viktigt att utveckla nya syntesmetoder, som kan tillämpas vid framställningen av farmaceutiskt relevanta målstrukturer. I detta sammanhang är den ultimata målsättningen dock inte endast en lyckad syntes av målmolekylen, utan det är allt viktigare att utveckla syntesrutter som uppfyller kriterierna för den hållbara utvecklingen. Ett av de centralaste verktygen som en organisk kemist har till förfogande i detta sammanhang är katalys, eller mera specifikt möjligheten att tillämpa olika katalytiska reaktioner vid framställning av komplexa målstrukturer. De motsvarande industriella processerna karakteriseras av hög effektivitet och minimerad avfallsproduktion, vilket naturligtvis gynnar den kemiska industrin samtidigt som de negativa miljöeffekterna minskas avsevärt. I denna doktorsavhandling har nya syntesrutter för produktion av finkemikalier med farmaceutisk relevans utvecklats genom att kombinera förhållandevis enkla transformationer till nya reaktionssekvenser. Alla reaktionssekvenser som diskuteras i denna avhandling påbörjades med en metallförmedlad allylering av utvalda aldehyder eller aldiminer. De erhållna produkterna innehållende en kol-koldubbelbindning med en närliggande hydroxyl- eller aminogrupp modifierades sedan vidare genom att tillämpa välkända katalytiska reaktioner. Alla syntetiserade molekyler som presenteras i denna avhandling karakteriseras som finkemikalier med hög potential vid farmaceutiska tillämpningar. Utöver detta tillämpades en mängd olika katalytiska reaktioner framgångsrikt vid syntes av dessa molekyler, vilket i sin tur förstärker betydelsen för de katalytiska verktygen i organiska kemins verktygslåda.

Tutkimuksella tarkasteltiin yläkoululaisten kouluruokailun ajankohtaisia ilmiöitä sekä selvitettiin kouluruokailutottumusten ja oppilaiden taustamuuttujien (sukupuoli, painoindeksi, fyysinen kunto ja aktiivisuus, uni ja väsyneisyys sekä koulumenestys) mahdollista yhteyttä. Tutkimuksen kohderyhmä muodostui Loimaan kaupungissa sijaitsevista Puistokadun ja Opintien koulun kahdeksas- ja yhdeksäsluokkalaisista oppilaista. Tutkimus suoritettiin kyselyyn osallistuneilla kouluilla marraskuun alussa 2011, jolloin tutkimukseen osallistui molemmista kouluista 69 oppilasta kokonaisotannan ollessa 138 oppilasta. Tutkimusjoukko koostui 65 tytöstä ja 72 pojasta. Tämä tutkimus on tutkimusotteeltaan pääosin kvantitatiivinen, mutta siinä esiintyy myös kvalitatiivisia piirteitä. Tutkimusmenetelmänä käytettiin strukturoituja ja avoimia kysymyksiä sisältävää kyselylomaketta, joka oli muodostettu tätä tutkimusta varten aiempien tutkimuksien pohjalta. Tutkimuksen tulokset olivat myönteisiä kouluruokailun osalta. Oppilaiden antama keskiarvo kouluruoalle oli 7,5 arvosteluasteikon ollessa neljästä kymmeneen. Oppilaista 86 prosenttia osallistui kouluruokailuun päivittäin ja 83 prosenttia viihtyi kouluruokalassa yli kymmenen minuuttia. Oppilaat söivät ruoka-aineiden välisiä suhteita kuvaavan lautasmallin eri osia hyvin. Oppilaat toivoivat kouluruokailuunsa enemmän jälkiruokia ja pehmeää leipää sekä paremmanmakuista pääruokaa. Oppilaiden taustamuuttujia tarkastellessa havaittiin niillä olevan yhteyksiä oppilaiden kouluruokailutottumuksiin, mutta tilastollisesti tarkasteltuna yhteydet eivät olleet merkitseviä. Ainoastaan yöunien keston ja väsyneisyyden yhteys kouluruokailutottumuksiin oli melkein merkitsevää. Tutkimukseen osallistuneet oppilaat eivät noudattaneet vähähiilihydraattista ruokavaliota, joten ”karppauksella” ei ollut vaikutuksia kouluruokailuun. Tutkimuksen tuloksia verrattaessa laajempiin tutkimuksiin (Terveyden ja hyvinvoinnin laitos 2011; Hoppu ym. 2008; Urho & Hasunen 2004), voidaan todeta tutkimukseen osallistuneiden oppilaiden pitävän kouluruoasta ja kouluruokailusta enemmän kuin ikätoverinsa. Oppilaat osallistuivat aktiivisemmin kouluruokailuun, söivät lautasmallin osia paremmin ja viettivät enemmän aikaa kouluruokailussa.
