The rising success rate of solid organ (SOT) and haematopoietic stem cell transplantation (HSCT) and modern immunosuppression make transplants the first therapeutic option for many diseases affecting a considerable number of people worldwide. Consequently, developing countries have also grown their transplant programs and have started to face the impact of neglected tropical diseases (NTDs) in transplant recipients. We reviewed the literature data on the epidemiology of NTDs with greatest disease burden, which have affected transplant recipients in developing countries or may represent a threat to transplant recipients living in other regions. Tuberculosis, Leprosy, Chagas disease, Malaria, Leishmaniasis, Dengue, Yellow fever and Measles are the topics included in this review. In addition, we retrospectively revised the experience concerning the management of NTDs at the HSCT program of Amaral Carvalho Foundation, a public transplant program of the state of São Paulo, Brazil.

RESUMO: O envelhecimento demográfico é uma realidade dos nossos dias. A preparação da alta de enfermagem é um elemento da prestação de cuidados que pode ser explorado na fase de envelhecimento para dotar os idosos de conhecimentos, capacidades e responsabilidade na gestão da sua condição de saúde. Deste modo delineou-se um estudo de tipo descritivo, transversal e exploratório com o objectivo de analisar e compreender o processo de preparação da alta hospitalar de enfermagem ao idoso internado por agudização de doença, que se apresente autónomo no momento da alta, de modo a capacitá-lo para a gestão da sua condição de saúde. O estudo decorreu num Serviço de Medicina Interna. Definiu-se como fontes de informação e métodos de colheita de dados 25 entrevistas a idosos, 16 questionários abertos a enfermeiros prestadores de cuidados e análise dos registos do processo de internamento dos idosos. Como principais resultados destacamos que os idosos quando internados apresentam preocupações relacionadas com a sua condição de saúde e o motivo de internamento, os quais são pouco valorizadas na preparação da alta; os cuidados com a saúde que os idosos têm antes do internamento são valorizados nos registos de enfermagem; um terço dos idosos não teve conhecimento do seu diagnóstico médico; os diagnósticos de enfermagem activos no momento da alta não são contemplados na preparação da alta; os idosos com a aproximação do regresso a casa manifestam preocupações sobre os cuidados a ter com a sua saúde e manifestam interesse em ser informados sobre os mesmos; os idosos sentem pouca disponibilidade por parte dos enfermeiros para a preparação da alta; as intervenções de preparação da alta centram-se em intervenções do tipo ensinar, instruir e treinar, tendo-se verificado discrepâncias sobre os cuidados prestados e o seu conteúdo sob as várias fontes de informação; o diálogo é a estratégia de preparação da alta que prevalece; mais informação e informação escrita são aspectos que podem melhorar a preparação da alta segundo os idosos. Podemos assim concluir que não existe uma estrutura de preparação da alta a idosos com capacidade para gerir a sua condição de saúde e que muito pode ser melhorado em termos de preparação da alta a idosos no sentido de tomar o máximo partido deste elemento dos cuidados e assim obter ganhos em saúde. ----------- ABSTRACT: The demographic aging is a reality nowadays. The nursing discharge planning is an element of care that should be studied on aging to provide older people with knowledge, abilities and responsibility in order to deal their health condition. Thus, it was outlined a descriptive study, transversal and exploratory in order to analyze and understand how the process nursing discharge occurs, by planning the elderly hospitalized with acute disease, in order to support patients for better management of health conditions. The study was carried in an Internal Medicine Service of a Central Hospital. As information sources and methods of data collection, we defined 25 interviews with elderly, 16 open-ended questionnaires to nurses and analysis of records from the process of the elderly hospitalized. In the main results we observe the elderly hospitalized have concerns about their health condition and the reason for their hospitalization, which are undervalued in the discharge planning process; the health care reported by elderly related to time before hospitalization were registered in nursing records; one-third of the elderly don’t know the medical diagnosis; the active nursing diagnosis at discharge assets are not included in the discharge planning process; in the discharge, the elderly had concerns over the care of their health and expressed interest in being informed of it; the elderly don’t feel availability from the nurses in the discharge planning; the discharge interventions focus on interventions like teaching, instructing and training, and there have been different views about the care provided and its contents under the various sources of information; the high prevailing strategy to discharge planning is the dialogue; more information and written information are aspects that can improve the discharge planning process. We conclude that there is no structure in discharge planning for the elderly with high ability to manage his health condition. There are several things that can be improved to the discharge planning for the elderly in order to take full advantage of this element of care.

Botulism is a rare and potentially lethal illness caused by Clostridium botulinum neurotoxin. We describe the findings of a laboratorial investigation of 117 suspected cases of botulism reported to the surveillance system in Brazil from January 2000 to October 2008. Data on the number and type of samples analyzed, type of toxins identified, reporting of the number of botulism cases and transmission sources are discussed. A total of 193 clinical samples and 81 food samples were analyzed for detection and identification of the botulism neurotoxin. Among the clinical samples, 22 (11.4%) presented the toxin (nine type A, five type AB and eight with an unidentified type); in food samples, eight (9.9%) were positive for the toxin (five type A, one type AB and two with an unidentified type). Of the 38 cases of suspected botulism in Brazil, 27 were confirmed by a mouse bioassay. Laboratorial botulism diagnosis is an important procedure to elucidate cases, especially food-borne botulism, to confirm clinical diagnosis and to identify toxins in food, helping sanitary control measures.

Irrigation schemes and dams have posed a great concern on public health systems of several countries, mainly in the tropics. The focus of the present review is to elucidate the different ways how these human interventions may have an effect on population dynamics of anopheline mosquitoes and hence, how local malaria transmission patterns may be changed. We discuss different studies within the three main tropical and sub-tropical regions (namely Africa, Asia and the Pacific and the Americas). Factors such as pre-human impact malaria epidemiological patterns, control measures, demographic movements, human behaviour and local Anopheles bionomics would determine if the implementation of an irrigation scheme or a dam will have negative effects on human health. Some examples of successful implementation of control measures in such settings are presented. The use of Geographic Information System as a powerful tool to assist on the study and control of malaria in these scenarios is also highlighted.

SUMMARY Ophidic accidents are an important public health problem due to their incidence, morbidity and mortality. An increasing number of cases have been registered in Brazil in the last few years. Several studies point to the importance of knowing the clinical complications and adequate approach in these accidents. However, knowledge about the risk factors is not enough and there are an increasing number of deaths due to these accidents in Brazil. In this context, acute kidney injury (AKI) appears as one of the main causes of death and consequences for these victims, which are mainly young males working in rural areas. Snakes of the Bothrops and Crotalus genera are the main responsible for renal involvement in ophidic accidents in South America. The present study is a literature review of AKI caused by Bothrops and Crotalus snake venom regarding diverse characteristics, emphasizing the most appropriate therapeutic approach for these cases. Recent studies have been carried out searching for complementary therapies for the treatment of ophidic accidents, including the use of lipoic acid, simvastatin and allopurinol. Some plants, such as Apocynaceae, Lamiaceae and Rubiaceae seem to have a beneficial role in the treatment of this type of envenomation. Future studies will certainly find new therapeutic measures for ophidic accidents.

Asthma in schoolchildren became a priority both for developing and developed countries. Two large multi-centre epidemiological studies(Portuguese Study of Allergic Diseases in Childhood and the International Study of Asthma and Allergies in Childhood) were implemented in the last decade. One of the main objectives was to compare prevalence of asthma and evaluate cultural, ethnic and environmental variables in schoolchildren of different continents. These studies can contribute to a global strategy (with national specificity’s) including self management programs in the control of asthma to reduce the morbidity and mortality, and promote better quality of life with better allocation of resources.

The application of the same epidemiological methods in different countries allows important comparisons between different races and cultures. During the last decade, two large multi-centres epidemiological studies, the Portuguese Study of Allergic Diseases in Childhood (PAC study) and the International Study of Asthma and Allergies in Childhood (ISAAC study), were implemented in Portuguese speaking regions. The main objectives were to assess and compare allergic diseases prevalence. The authors stress out the significant differences observed in schoolchildren from the three continents, with different genetic and environmental background. It was found an increase trend in the prevalence of all allergic diseases, mainly rhinitis, in last decade. Rhinitis has been identified as an independent risk factor for asthma in Caucasian population.

SUMMARYThe epidemiological characteristics of paracoccidioidomycosis were reviewed and updated. The new endemic areas in Brazil were discussed in the section regarding the geographic distribution of the mycosis. Subclinical infection with Paracoccidioides brasiliensis was discussed on the basis of skin test surveys with antigens of the fungus, seroepidemiological studies, and disease cases outside Latin America. Large case series permitted a comparison of the prevalence of the mycosis in different regions, its estimated incidence and risk factors for the development of the disease. Aspects modulating the expression of the clinical forms of paracoccidioidomycosis are also presented. This review also deals with diseases associated with the mycosis, opportunistic paracoccidioidomycosis, lethality, mortality and infection and disease in animals.

This report is a retrospective study of the epidemiology of scorpion sting cases recorded from 2007 to 2013 in the State of Ceará, Northeastern Brazil. Data were collected from the Injury Notification Information System database of the Health Department of Ceará. A total of 11,134 cases were studied and distributed across all the months of the studied period and they occurred mainly in urban areas. Victims were predominantly 20-29 years-old women. Most victims were bitten on the hand; and received medical assistance within 1-3 hours after being bitten. Cases were mostly classified as mild and progressed to cure. Scorpion envenomation in Ceará is an environmental public health problem that needs to be monitored and controlled throughout the year.

In the American continent, honeybee envenomation is a public health problem due to the high incidence and severity of the cases. Despite its medical importance, there is a lack of epidemiological studies on this topic in Brazil, especially referring to the Northeastern states. The present study has aimed to describe the epidemiological features of honeybee envenomation cases in the state of the Ceará, Northeastern Brazil, from 2007 to 2013. Data were collected from the Injury Notification Information System database of the Health Department of Ceará. A total of 1,307 cases were analyzed. Cases were shown to be distributed in all the months of the studied years, reaching higher frequencies in August. The majority of cases occurred in urban areas and involved men aged between 20 and 29 years. Victims were mainly stung on the head and torso, and they received medical assistance predominantly within 3 hours after being stung. Local manifestations were more frequent than systemic ones. Most cases were classified as mild and progressed to cure. The high number of honeybee sting cases shows that Ceará may be an important risk area for such injuries. Moreover, the current study provides data for the development of strategies to promote control and prevention of bee stings in this area.

An area believed to be an autochthonous focus for Chagas' disease was investigated in the municipality of Caxias, Rio de Janeiro State. The study included search for domestic triatomine bugs, serological test (IFT and CFT) in persons in whose house infested bugs were discovered, detailed clinicai examination and xenodiagnosis test of ali serologicall/ yy positive persons, and xenodiagnosis test on dogs from households in which infected triatomine bugs have been found. Only in one of the locatities (Piranema) domestic Triatoma infestans have been discovered. some of which were infected with T. cruzi. A small number of persons (mostly children) had a positive serologicál test for Chagas’ disease, but in all of them the infection was clinically asymptomatic. From two dogs, belonging to a household in which serologically positive children and infected T. infestans were discovered, T. cruzi was isolated by xenodiagnosis. The importunt epidemiological information obtained from this investigation was the discovery of domestic adaptation of T. infestans in an area with dense population .and with very low social and sanitary conditions, in a locality considered as non-endemic for the infection.

RESUMO - Objectivos: Este estudo pretende caracterizar os casos de Tuberculose notificados em Portugal Continental, em função das suas características sociodemográficas e clínicas, aferir a sua evolução através da comparação dos biénios 2000/2001 e 2008/2009 e a possibilidade de existência de diferenças entre os casos de Tuberculose na população imigrante e não imigrante. Métodos: Foi desenvolvido um estudo descritivo de casos notificados nos biénios 2000/2001 (n= 9182) e 2008/2009 (n= 6087). A fonte de dados foi o Sistema De Vigilância Intrínseco ao Programa Nacional de Luta contra a Tuberculose (SVIG-TB). Para calcular as incidências foram utilizados os dados disponibilizados pelo Instituto Nacional de Estatística e pelo Serviço de Estrangeiros e Fronteiras referentes à população não imigrante e imigrante residente em Portugal. Foi utilizado o teste do Qui-quadrado e o teste exacto de Fisher para analisar possíveis relações entre variáveis qualitativas. O teste de Mann-Whitney foi utilizado para comparação da distribuição entre variáveis quantitativas nos dois grupos. A odds ratio foi determinada para aferir a força de associação entre as variáveis e a condição de imigrante. Resultados: No biénio 2000/2001, 10,2% dos casos notificados eram de imigrantes e no biénio 2008/2009 aumentou para 13,3%. Relativamente às características sociodemográficas dos pacientes, observaram-se diferenças estatisticamente significativas apenas na idade. Nas características clínicas observaram-se diferenças significativas em algumas patologias associadas, nomeadamente na infecção VIH (p <0,001: OR=1,696).Quanto aos factores de risco, observaram-se diferenças no consumo do tabaco (p <0,001: OR= 0,499) e drogas (p <0,001; OR= 0,582). Relativamente à situação final de tratamento existem diferenças significativas nas categorias em tratamento (p <0,001;OR=0,661), tratamento completo (p <0,001;OR =1,293) e morte (p <0,001; OR=1,806). Conclusões: O número de casos de Tuberculose em Portugal, bem como a taxa de incidência, diminuíram do biénio 2000/2001 para o biénio 2008/2009 em ambos os grupos. Os imigrantes são mais jovens, têm maior risco de infecção VIH e menor risco de patologias crónicas.
