The aim of this study was to assess patterns and correlates of family variables in 31 adolescents treated for their first episode of a schizophrenia spectrum disorder (early-onset schizophrenia [EOS]). Expressed emotion, perceived criticism, and rearing style were assessed. Potential correlates were patient psychopathology, premorbid adjustment, illness duration, quality of life (QoL), sociodemographic variables, patient and caregiver "illness concept," and caregiver personality traits and support. Families were rated as critical more frequently by patients than raters (55% vs. 13%). Perceived criticism was associated with worse QoL in relationship with parents and peers. An adverse rearing style was associated with a negative illness concept in patients, particularly with less trust in their physician. Future research should examine perceived criticism as a predictor of relapse and indicator of adolescents with EOS who need extended support and treatment. Rearing style should be carefully observed because of its link with patients' illness concept and, potentially, to service engagement and medication adherence

There have been numerous attempts to reveal the neurobiological basis of schizophrenia spectrum disorders. Results however, remain as heterogeneous as the schizophrenia spectrum disorders itself. Therefore, one aim of this thesis was to divide patients affected by this disorder into subgroups in order to homogenize the results of future studies. In a first study it is suggested that psychopathological rating scales should focus on symptoms-clusters that may have a common neurophysiological background. The here presented Bern Psychopathology Scale (BPS) proposes that alterations in three wellknown brain systems (motor, language, and affective) are largely leading to the communication failures observable on a behavioral level, but also - as repeatedly hypothesized - to dysconnectivity within and between brain systems in schizophrenia spectrum disorders. The external validity of the motor domain in the BPS was tested against the objective measure of 24 hours wrist actigraphy, in a second study. The subjective, the quantitative, as well as the global rating of the degree of motor disorders in this patient group showed significant correlations to the acquired motor activity. This result confirmed in a first step the practicability of the motor domain of the BPS, but needs further validation regarding pathological brain alterations. Finally, in a third study (independent from the two other studies), two cerebral Resting State Networks frequently altered in schizophrenia were investigated for the first time using simultaneous EEG/fMRI: The well-known default mode network and the left working memory network. Besides the changes in these fMRI-based networks, there are well-documented findings that patients exhibit alterations in EEG spectra compared to healthy controls. However, only through the multimodal approach it was possible to discover that patients with schizophrenia spectrum disorders have a slower driving frequency of the Resting State Networks compared to the matched healthy controls. Such a dysfunctional coupling between neuronal frequency and functional brain organization could explain in a uni- or multifactorial way (dysfunctional cross-frequency coupling, maturational effects, vigilance fluctuations, task-related suppression), how the typical psychotic symptoms might occur. To conclude, the major contributions presented in this thesis were on one hand the development of a psychopathology rating scale that is based on the assumption of dysfunctional brain networks, as well as the new evidence of a dysfunctional triggering frequency of Resting State Networks from the simultaneous EEG/fMRI study in patients affected by a schizophrenia spectrum disorder.

El Trastorno de Espectro Autista (TEA) es un trastorno que impide el correcto desarrollo de funciones cognitivas, habilidades sociales y comunicativas en las personas. Un porcentaje significativo de personas con autismo presentan además dificultades en la comprensión lectora. El proyecto europeo FIRST está orientado a desarrollar una herramienta multilingüe llamada Open Book que utiliza Tecnologías del Lenguaje Humano para identificar obstáculos que dificultan la comprensión lectora de un documento. La herramienta ayuda a cuidadores y personas con autismo transformando documentos escritos a un formato más sencillo mediante la eliminación de dichos obstáculos identificados en el texto. En este artículo se presenta el proyecto FIRST así como la herramienta desarrollada Open Book.

Investigation into the earliest signs of autism in infants has become a significant sub-field of autism research. This work invokes specific ethical concerns such as use of 'at-risk' language, communicating study findings to parents and the future perspective of enrolled infants when they reach adulthood. This study aimed to ground this research field in an understanding of the perspectives of members of the autism community. Following focus groups to identify topics, an online survey was distributed to autistic adults, parents of children with autism and practitioners in health and education settings across 11 European countries. Survey respondents (n = 2317) were positively disposed towards early autism research, and there was significant overlap in their priorities for the field and preferred language to describe infant research participants. However, there were also differences including overall less favourable endorsement of early autism research by autistic adults relative to other groups and a dislike of the phrase 'at-risk' to describe infant participants, in all groups except healthcare practitioners. The findings overall indicate that the autism community in Europe is supportive of early autism research. Researchers should endeavour to maintain this by continuing to take community perspectives into account.

Autism is a pervasive developmental disorder and Asperger’s syndrome is part of the spectrum of autism disorders. This thesis aims to: • Review and investigate current theories concerning visual function in individuals with Asperger’s syndrome and high functioning autism spectrum disorder and to translate the findings into clinical practice by developing a specific protocol for the eye examination of individuals of this population. • Investigate whether those with Asperger’s syndrome are more likely to suffer from Meares-Irlen syndrome and/or dyslexia. • Assess the integrity of the M-cell pathway in Asperger’s syndrome using perimetric tests available in optometric practice to investigate and also to describe the nature of any defects. • Evaluate eye movement strategies in Asperger’s whilst viewing both text and images. Also to evaluate the most appropriate methodology for investigating eye movements; namely optical digital eye tracking and electrophysiology methodologies. Findings of the investigations include • Eye examinations for individuals with Asperger’s syndrome should contain the same testing methods as for the general population, with special consideration for clear communication. • There is a depression of M-pathway visual field sensitivity in 57% (8/14) of people with Asperger’s syndrome, supporting previous evidence for an M-cell deficit in some individuals. • There is a raised prevalence of dyslexia in Asperger’s syndrome (26% of a sample of 31) but not necessarily of Meares-Irlen syndrome. • Gaze strategies are abnormal in Asperger’s syndrome, for both reading and viewing of images. With increased saccadic movement and decreased viewing of faces in comparison to background detail.

Ce mémoire présente et discute d’une problématique importante qui s’inscrit dans un contexte actuel, autant sur le plan international que local, puisqu’elle touche 1 enfant sur 68 (CDC, 2010). On parle ici du trouble du spectre de l’autisme (TSA). Plus spécifiquement, cette recherche se concentre sur les jeunes adultes autistes, qui du jour au lendemain, se retrouvent face à une interruption de services. Elle suggère qu’une adaptation adéquate de l’environnement soit davantage explorée en vue d’offrir des centres de jour adaptés à cette clientèle. La question de recherche est la suivante : Quels sont les éléments du design intérieur qui peuvent améliorer la qualité de vie des adultes autistes? Il est essentiel de s’y attarder, puisque les personnes TSA sont entre autres caractérisées par leur hypersensibilité et leur hyposensibilité face à l’environnement. Bien que le sujet de l’autisme soit richement documenté par le domaine des sciences médicales, il est apparu qu’il l’est moins par celui de l’aménagement. La stratégie méthodologique de cette recherche repose sur des analyses de précédents, des observations participantes ainsi que la tenue d’entretiens semi-dirigés auprès de parents d’enfants autistes. Cette étude vise simultanément l’amélioration de la compréhension de la pratique émergente du design comme moyen d’intervention ainsi que l’identification des facteurs (éclairages, couleurs, matériaux, division de l’espace, etc.) pouvant améliorer le bien-être des adultes autistes. Sur la base des données collectées, il a été remarqué qu’effectivement, plusieurs moyens peuvent être entrepris pour concevoir des environnements améliorant le bien-être de ces adultes. Les résultats extraits du terrain mènent à des propositions d’aménagement claires : rendre l’abstrait le plus concret possible, aménager des zones de retrait social, offrir une variété d’ambiances, et finalement, offrir un encadrement sécuritaire.

O autismo é uma condição que faz parte de um grupo de perturbações do desenvolvimento global das funções cerebrais e que, por regra, é diagnosticada no início da infância. A dificuldade na linguagem e comunicação, o défice na interação social, as esteriotipias e os interesses específicos e comportamentos repetitivos caracterizam os indivíduos portadores desta patologia. A saúde oral das crianças autistas é geralmente precária e as necessidades de tratamento dentário elevadas. No entanto, os problemas comportamentais destes doentes fazem com que os pais não recorram às consultas de medicina dentária, outras vezes quando recorrem à consulta as dificuldades de colaboração impedem a prestação de cuidados de saúde oral adequados a estas crianças. Pretendeu-se assim, com este trabalho, realizar uma revisão sistemática de literatura científica, publicada nos últimos 15 anos, acerca dos problemas orais que atingem os pacientes autistas, e simultaneamente, compilar diretrizes de atuação clínica para orientar o médico dentista no atendimento destes doentes. Para isto, durante os meses de Outubro de 2014 a Outubro de 2015, procedeu-se a uma pesquisa bibliográfica nas bases de dados PubMed e B-on, sendo consultados também outros bancos de dados como LILACS – BIREME, SciELO, utilizando as seguintes palavras-chave: “autism”, “pediatric dentistry”, “Asperger Syndrome”, “Rett Syndrome”, “Childhood Disintegrative Disorder”, “prevalence”, “neurobiology”, “etiology”, “diagnosis”, “diagnostic criteria”, “comorbidity”, “oral health”, “dental caries”, “periodontal disease”, “oral habits”, “bruxism”, “self-injury”, “dental trauma”, “dental injury”, “malocclusion”, “behavior management techniques” separadas ou associadas pelo operador de pesquisa booleano AND. Na pesquisa foram empregues os seguintes limites: artigos publicados nos últimos 15 anos, abstract disponível, estudos em humanos e artigos e língua inglesa, francesa, portuguesa e espanhola. Desta pesquisa resultou um total de 150 artigos que foram selecionados primeiramente pelos títulos, seguidamente pela leitura dos abstracts e, finalmente, do artigo por inteiro, obtendo-se assim 95 artigos, para revisão. Foram ainda considerados artigos de referência publicados em anos anteriores, livros de texto médicos e publicações portuguesas com dados epidemiológicos sobre as Perturbações do Espetro do Autismo em Portugal. As doenças orais encontradas nas crianças autistas são semelhantes às das crianças sem qualquer perturbação mental, contudo a preferência por alimentos cariogénicos, a diminuição do fluxo salivar induzida pelos fármacos, associadas a uma pobre higiene oral, justificam uma maior prevalência de cárie. As doenças periodontais, também muito prevalentes neste grupo, desenvolvem-se em virtude da combinação da falta de hábitos de higiene oral, com os efeitos secundários de fármacos administrados a estes doentes, como os anticonvulsivantes. No seu atendimento na consulta dentária recorre-se às mesmas estratégias de orientação de comportamento aplicadas nas crianças saudáveis, para contornar os sentimentos de medo, ansiedade, desconfiança e a incapacidade de interação social, e assim evitar comportamentos de recusa durante a consulta dentária. É no entanto de salientar que os distúrbios comportamentais, o défice da comunicação e a falta de capacidades de interação social, caraterísticas do autismo impossibilitam a eficácia das técnicas de controlo do comportamento comunicativas, obrigando, muitas vezes, ao uso de técnicas de controlo de comportamento avançadas para prestação de cuidados de saúde oral com eficácia e em segurança. É importante uma grande motivação de pais/responsáveis para a saúde e higiene oral das crianças com Perturbações do Espetro do Autismo, e que todos os profissionais de saúde envolvidos no cuidado destes doentes contribuam para a aprendizagem de comportamentos que promovam a saúde oral destes doentes.

Deficits in social communication and interaction have been identified as distinguishing impairments for individuals with an autism spectrum disorder (ASD). As a pivotal skill, the successful development of social communication and interaction in individuals with ASD is a lifelong objective. Point-of-view video modeling has the potential to address these deficits. This type of video involves filming the completion of a targeted skill or behavior from a first-person perspective. By presenting only what a person might see from his or her viewpoint, it has been identified to be more effective in limiting irrelevant stimuli by providing a clear frame of reference to facilitate imitation. The current study investigated the use of point-of-view video modeling in teaching social initiations (e.g., greetings). Using a multiple baseline across participants design, five kindergarten participants were taught social initiations using point-of-view video modeling and video priming. Immediately before and after viewing the entire point-of-view video model, the participants were evaluated on their social initiations with a trained, typically developing peer serving as a communication partner. Specifically, the social initiations involved participants’ abilities to shift their attention toward the peer who entered the classroom, maintain attention toward the peer, and engage in an appropriate social initiation (e.g., hi, hello). Both generalization and maintenance were tested. Overall, the data suggest point-of-view video modeling is an effective intervention for increasing social initiations in young students with ASD. However, retraining was necessary for acquisition of skills in the classroom environment. Generalization in novel environments and with a novel communication partner, and generalization to other social initiation skills was limited. Additionally, maintenance of gained social initiation skills only occurred in the intervention room. Despite the limitations of the study and variable results, there are a number of implications moving forward for both practitioners and future researchers examining point-of-view modeling and its potential impact on the social initiation skills of individuals with ASD.

O Transtorno do Espectro do Autismo (TEA) caracteriza-se por uma série de distúrbios cognitivos e neurocomportamentais e sua prevalência mundial é estimada em 1 criança com TEA a cada 160 crianças com típico desenvolvimento (TD). Indivíduos com TEA apresentam dificuldade em interpretar as emoções alheias e em expressar sentimentos. As emoções podem ser associadas à manifestação de sinais fisiológicos, e, dentre eles, os sinais cerebrais têm sido muito abordados. A detecção dos sinais cerebrais de crianças com TEA pode ser benéfica para o esclarecimento de suas emoções e expressões. Atualmente, muitas pesquisas integram a robótica ao tratamento pedagógico do TEA, através da interação com crianças com esse transtorno, estimulando habilidades sociais, como a imitação e a comunicação. A avaliação dos estados mentais de crianças com TEA durante a sua interação com um robô móvel é promissora e assume um aspecto inovador. Assim, os objetivos deste trabalho foram captar sinais cerebrais de crianças com TEA e de crianças com TD, como grupo controle, para o estudo de seus estados emocionais e para avaliar seus estados mentais durante a interação com um robô móvel, e avaliar também a interação dessas crianças com o robô, através de escalas quantitativas. A técnica de registro dos sinais cerebrais escolhida foi a eletroencefalografia (EEG), a qual utiliza eletrodos colocados de forma não invasiva e não dolorosa sobre o couro cabeludo da criança. Os métodos para avaliar a eficiência do uso da robótica nessa interação foram baseados em duas escalas internacionais quantitativas: Escala de Alcance de Metas (do inglês Goal Attainment Scaling - GAS) e Escala de Usabilidade de Sistemas (do inglês System Usability Scale - SUS). Os resultados obtidos mostraram que, pela técnica de EEG, foi possível classificar os estados emocionais de crianças com TD e com TEA e analisar a atividade cerebral durante o início da interação com o robô, através dos ritmos alfa e beta. Com as avaliações GAS e SUS, verificou-se que o robô móvel pode ser considerado uma potencial ferramenta terapêutica para crianças com TEA.
