A monitorização ambiental é essencial para a tomada de decisões tanto na ciência como na indústria. Em particular, uma vez que a água é essencial à vida e a superfície da Terra é composta principalmente por água, a monitorização do clima e dos parâmetros relacionados com a água em ecossistemas sensíveis, tais como oceanos, lagoas, rios e lagos, é de extrema importância. Um dos métodos mais comuns para monitorar a água é implantar bóias. O presente trabalho está integrado num projeto mais amplo, com o objetivo de projectar e desenvolver uma bóia autónoma para a investigação científica com dois modos de funcionamento: (i) monitorização ambiental ; e (ii) baliza ativa de regata. Assim, a bóia tem duas aplicações principais: a coleta e armazenamento de dados e a assistência a regatas de veleiros autónomos. O projeto arrancou há dois anos com um grupo de quatro estudantes internacionais. Eles projetaram e construíram a estrutura física, compraram e montaram o sistema de ancoragem da bóia e escolherem a maioria dos componentes electrónicos para o sistema geral de controlo e medição. Este ano, durante o primeiro semestre, dois estudantes belgas - Jeroen Vervenne e Hendrick Verschelde – trabalharam nos subsistemas de recolha e armazenamento de dados (unidade de controlo escrava) e de telemetria e configuração (unidade de controlo mestre) assim como definiram o protocolo de comunicação da aplicação. O trabalho desta tese continua o desenvolvimento do subsistema de telemetria e configuração. Este subsistema _e responsável pela configuração do modo de funcionamento e dos sensores assim como pela comunicação com a estacão de base (controlo ambiental), barcos (baliza ativa de regata) e com o subsistema de recolha e armazenamento de dados. O desenvolvimento do subsistema de recolha e armazenamento de dados, que coleta e armazena num cartão SD os dados dos sensores selecionados, prossegue com outro estudante belga - Mathias van Flieberge. O objetivo desta tese é, por um lado, implementar o subsistema de telemetria e de configuração na unidade de controle mestre e, por outro lado, refinar e implementar, conjuntamente com Mathias van Flieberge, o protocolo de nível de aplicação projetado. Em particular, a unidade de controlo mestre deve processar e atribuir prioridades às mensagens recebidas da estacão base, solicitar dados à unidade de controlo escrava e difundir mensagens com informação de posição e condições de vento e água no modo de regata. Enquanto que a comunicação entre a unidade de controlo mestre e a estacão base e a unidade de controlo mestre e os barcos é sem fios, a unidade de controlo mestre e a unidade de controlo escrava comunicam através de uma ligação série. A bóia tem atualmente duas limitações: (i) a carga máxima é de 40 kg; e (ii) apenas pode ser utilizada em rios ou próximo da costa dada à limitação de distância imposta pela técnica de comunicação sem fios escolhida.

Para encarar a detecção crescente de distúrbios do equilíbrio ácido-base e hidro-electrolítico nos nossos serviços hospitalares, descrevemos e exemplificamos um programa informático auxiliar no diagnóstico e terapêutica destes distúrbios. Partindo dos resultados dum ionograma e dos gases no sangue são apontados os síndromas presentes, e na sequência de um curto diálogo máquina/utilizador os diagnósticos etiológicos mais prováveis. 0 programa propõe também uma terapêutica concreta para esse doente e fornece referências bibliográficas. São discutidas criticamente as possíveis aplicações deste trabalho na clínica e no ensino médico.

Currently, the quality of the Indonesian national road network is inadequate due to several constraints, including overcapacity and overloaded trucks. The high deterioration rate of the road infrastructure in developing countries along with major budgetary restrictions and high growth in traffic have led to an emerging need for improving the performance of the highway maintenance system. However, the high number of intervening factors and their complex effects require advanced tools to successfully solve this problem. The high learning capabilities of Data Mining (DM) are a powerful solution to this problem. In the past, these tools have been successfully applied to solve complex and multi-dimensional problems in various scientific fields. Therefore, it is expected that DM can be used to analyze the large amount of data regarding the pavement and traffic, identify the relationship between variables, and provide information regarding the prediction of the data. In this paper, we present a new approach to predict the International Roughness Index (IRI) of pavement based on DM techniques. DM was used to analyze the initial IRI data, including age, Equivalent Single Axle Load (ESAL), crack, potholes, rutting, and long cracks. This model was developed and verified using data from an Integrated Indonesia Road Management System (IIRMS) that was measured with the National Association of Australian State Road Authorities (NAASRA) roughness meter. The results of the proposed approach are compared with the IIRMS analytical model adapted to the IRI, and the advantages of the new approach are highlighted. We show that the novel data-driven model is able to learn (with high accuracy) the complex relationships between the IRI and the contributing factors of overloaded trucks

Formation of whey protein isolate protein aggregates under the influence of moderate electric fields upon ohmic heating (OH) has been monitored through evaluation of molecular protein unfolding, loss of its solubility, and aggregation. To shed more light on the microstructure of the protein aggregates produced by OH, samples were assayed by transmission electron microscopy (TEM). Results show that during early steps of an OH thermal treatment, aggregation of whey proteins can be reduced with a concomitant reduction of the heating chargeby reducing the come-up time (CUT) needed to reach a target temperatureand increase of the electric field applied (from 6 to 12 V cm1). Exposure of reactive free thiol groups involved in molecular unfolding of -lactoglobulin (-lg) can be reduced from 10 to 20 %, when a CUT of 10 s is combined with an electric field of 12 V cm1. Kinetic and multivariate analysis evidenced that the presence of an electric field during heating contributes to a change in the amplitude of aggregation, as well as in the shape of the produced aggregates. TEM discloses the appearance of small fibrillar aggregates upon the influence of OH, which have recognized potential in the functionalization of food protein networks. This study demonstrated that OH technology can be used to tailor denaturation and aggregation behavior of whey proteins due to the presence of a constant electric field together with the ability to provide a very fast heating, thus overcoming heat transfer limitations that naturally occur during conventional thermal treatments.

The publication, Approved Drug Products with Therapeutic Equivalence Evaluations (the List, commonly known as the Orange Book), identifies drug products approved on the basis of safety and effectiveness by the Food and Drug Administration (FDA) under the Federal Food, Drug, and Cosmetic Act (the Act). Drugs on the market approved only on the basis of safety (covered by the ongoing Drug Efficacy Study Implementation [DESI] review [e.g., Donnatal® Tablets and Librax® Capsules] or pre-1938 drugs [e.g., Phenobarbital Tablets]) are not included in this publication. The main criterion for the inclusion of any product is that the product is the subject of an application with an effective approval that has not been withdrawn for safety or efficacy reasons. Inclusion of products on the List is independent of any current regulatory action through administrative or judicial means against a drug product. In addition, the List contains therapeutic equivalence evaluations for approved multisource prescription drug products. These evaluations have been prepared to serve as public information and advice to state health agencies, prescribers, and pharmacists to promote public education in the area of drug product selection and to foster containment of health care costs. Therapeutic equivalence evaluations in this publication are not official FDA actions affecting the legal status of products under the Act.

Therapeutic drug monitoring (TDM) aims to optimize treatments by individualizing dosage regimens based on the measurement of blood concentrations. Dosage individualization to maintain concentrations within a target range requires pharmacokinetic and clinical capabilities. Bayesian calculations currently represent the gold standard TDM approach but require computation assistance. In recent decades computer programs have been developed to assist clinicians in this assignment. The aim of this survey was to assess and compare computer tools designed to support TDM clinical activities. The literature and the Internet were searched to identify software. All programs were tested on personal computers. Each program was scored against a standardized grid covering pharmacokinetic relevance, user friendliness, computing aspects, interfacing and storage. A weighting factor was applied to each criterion of the grid to account for its relative importance. To assess the robustness of the software, six representative clinical vignettes were processed through each of them. Altogether, 12 software tools were identified, tested and ranked, representing a comprehensive review of the available software. Numbers of drugs handled by the software vary widely (from two to 180), and eight programs offer users the possibility of adding new drug models based on population pharmacokinetic analyses. Bayesian computation to predict dosage adaptation from blood concentration (a posteriori adjustment) is performed by ten tools, while nine are also able to propose a priori dosage regimens, based only on individual patient covariates such as age, sex and bodyweight. Among those applying Bayesian calculation, MM-USC*PACK© uses the non-parametric approach. The top two programs emerging from this benchmark were MwPharm© and TCIWorks. Most other programs evaluated had good potential while being less sophisticated or less user friendly. Programs vary in complexity and might not fit all healthcare settings. Each software tool must therefore be regarded with respect to the individual needs of hospitals or clinicians. Programs should be easy and fast for routine activities, including for non-experienced users. Computer-assisted TDM is gaining growing interest and should further improve, especially in terms of information system interfacing, user friendliness, data storage capability and report generation.

En aquest estudi es pretenia assolir la pràctica d'un sistema que només s'ha plantejat teòricament, doncs l'experiència acumulada, ja abans de la reforma orgànica de 2003, parteix de la figura dels serveis comuns, amb un criteri d'eficiència, racionalitat i economia per invertir en l'Administració de justícia catalana. Assumir i donar resposta jurídica concreta a tots els reptes tècnics pot fer-se des d'una perspectiva dogmàtica, tot i que el temps transcorregut també permet advertir exigències pràctiques a l'àmbit de la dogmàtica processal, el context socio-cultural i, naturalment, les necessitats laborals. En aquest sentit, els objectius s'introdueixen cap a la garantia d'un apropament material, conceptual i quotidià de l'Administració de justícia al ciutadà, establert com a eix inextricable del sistema, tenint present la de vegades despesa incomprensible en una realitat històricament menystinguda i deficitària, malgrat l'esforç pressupostari fet els darrers anys que, malgrat tot, no ha evitat discordances greus per manca de racionalitat i eficiència en el consum diari dels operadors implicats. Es pretén aconseguir, per mitjà del nou sistema d'oficina judicial estructurat, un avenç efectiu i econòmic a l'estat de la Justícia al país, especialment envers la dilació dels tràmits, a més de reclamar reformes legals, de caire processal especialment, que no es prestin a mers paràmetres d'ajust formal, sinó que incideix substantivament en millores per altre part reclamades fa temps per la doctrina científica. S'han aconseguit les fites definides des del punt de vista teòric, així com s'ha afrontat tots el problemes conceptuals i hipòtesis pràctiques més significatives, establint pautes de resposta sota la prèvia determinació de les qüestions debatudes i els conflictes habituals que correspon enfrontar. D'aquesta manera, s'ha estudiat la normativa en presència i la jurisprudència que hi dona actualitat pràctica, sense oblidar la doctrina d'autors. S'ha repassat el funcionament existent a l'oficina judicial i les previsions de la mateixa en un futur immediat, remarcant les noves tecnologies en ús i projectades, de igual manera que les bondats i crítiques de tots els operador jurídics actuants.

To start off, this document describes the Catalan model for emergencies response and its reference frame in terms of geography, location population…In addition, describes the main actors involved in emergencies response such as: police, the Fire and Rescue Emergency Service, the Emergency Medical System, Civil Protection, Reception and Management of Emergency Calls, Rural Agents, ADF’s and UME. Civil Protection, Firefighters and Police are includes in the training model developed by the Institute for Public Safety of Catalonia which at the same time does research in both security and safety matters. Research activities are performed by the Area for Research, Knowledge and International Cooperation at the ISPC and an example of these activities are European Research Projects such as COIM-Best (Coordination Improvement by Best Practices) and BESECU (cross-cultural differences of human behaviour in fire disasters and other crisis situations) among others.

North South Survey of Children’s Oral Health 2002 This report presents the results of the first contemporaneous North South study of the oral health of children and adolescents on the island of Ireland. The last National Survey of Childrenâ?Ts Oral Health in the Republic of Ireland was carried out in 1984. The present study describes the oral health experience of children and adolescents in Ireland today. The results are of interest to the public, to policymakers and to service providers and will be of assistance in the evaluation of current oral health services and the planning of services for the next decade. BACKGROUND: Cardiopulmonary bypass (CPB) with aortic cross-clamping and cardioplegic arrest remains the method of choice for patients requiring standard myocardial revascularization. Therefore, very high-risk patients presenting with acute coronary syndrome, unstable angina, onset of cardiac decompensation and requiring emergency multiple myocardial revascularization, can have a poor outcome. The on-pump beating heart technique can reduce the mortality and the morbidity in such a selected group of patients and this report describes our clinical experience. METHODS: Out of 290 patients operated for CABG from January 2005 to January 2006, 25 (8.6%) selected high-risk patients suffering from life threatening coronary syndrome (mean age 69 +/- 7 years) and requiring emergency multiple myocardial revascularization, underwent on-pump beating heart surgery. The mean pre-operative left ventricle ejection fraction (LVEF) was 27 +/- 8%. The majority of them (88%) suffered of tri-vessel coronary disease and 6 (24%) had a left main stump disease. Nine patients (35%) were on severe cardiac failure and seven among them (28%) received a pre-operative intra-aortic balloon pump. The pre-operative EuroScore rate was equal or above 8 in 18 patients (73%). RESULTS: All patients underwent on-pump-beating heart coronary revascularization. The mean number of graft/patient was 2.9 +/- 0.6 and the internal mammary artery was used in 23 patients (92%). The mean CPB time was 84 +/- 19 minutes. Two patients died during the recovery stay in the intensive care unit, and there were no postoperative myocardial infarctions between the survivors. Eight patients suffered of transitorily renal failure and 1 patient developed a sternal wound infection. The mean hospital stay was 12 +/- 7 days. The follow-up was complete for all 23 patients survived at surgery and the mean follow-up time was 14 +/- 5 months. One patient died during the follow-up for cardiac arrest and 2 patients required an implantable cardiac defibrillator. One year after surgery they all had a standard trans-thoracic echocardiogram showing a mean LVEF rate of 36 +/- 11.8%. CONCLUSION: Standard on-pump arrested heart coronary surgery has higher mortality and morbidity in emergencies. The on-pump beating heart myocardial revascularization seems to be a valid alternative for the restricted and selected cohort of patients suffering from life threatening coronary syndrome and requiring multiple emergency CABG.

Women who smoke underestimate the risks of smoking on their health, especially the impact of the anti-estrogenic and toxic effects of tobacco at the different stages of their life. The risk of female infertility related to tobacco is now well-proven, as is the risk of arterial and venous thrombotic event when tobacco is associated with oral contraception. Many foetal and maternal pathologies are linked to maternal smoking. Regarding post-menopausal women, smoking is associated with an increased osteoporotic and cardio-vascular risk. Doctors are at the first line to advise women and propose them help and assistance in their quit smoking process in a way adapted to their situation.

Anaphylactic reactions to hymenoptera venoms are common and, in our latitudes, mainly concern wasps and bees. Recently, molecular biology techniques have contributed to identifying and to sequencing the major allergens of insect venoms and led to the production of recombinant allergens. Assays for specific IgE directed against these recombinant allergens have recently been made available in clinical practice. They provide considerable assistance in identifying the insect responsible for an anaphylactic reaction, in particular when standard tests are positive for both wasp and bee. This article focuses on these new laboratory tests and also reviews the management of patients experiencing an anaphylactic reaction after hymenoptera sting.

The aim of this document is to provide general information about mIBG scintigraphy in cancer patients. The guidelines describe the mIBG scintigraphy protocol currently used in clinical routine, but do not include all existing procedures for neuroendocrine tumours. The guidelines should therefore not be taken as exclusive of other nuclear medicine modalities that can be used to obtain comparable results. It is important to remember that the resources and facilities available for patient care may vary from one country to another and from one medical institution to another. The present guidelines have been prepared for nuclear medicine physicians and intend to offer assistance in optimizing the diagnostic information that can currently be obtained from mIBG scintigraphy. The corresponding guidelines of the Society of Nuclear Medicine (SNM) and the Dosimetry, Therapy and Paediatric Committee of the EANM have been taken into consideration, and partially integrated into this text. The same has been done with the most relevant literature on this topic, and the final result has been discussed within a group of distinguished experts.

We analyzed a one-year case series and performed a longitudinal (4 month) cohort analysis of urgent requests made to home care agencies by and for their > or = 65 years old clients in order to estimate the frequency of unscheduled services delivered by home care agencies and to identify risk factors. All 40 home care agencies located in a Swiss region were included in the study and we registered 3,816 urgent requests (75/1,000 > or = 65 years residents per year). Among home care users, the presence of a urinary catheter, incontinence and the need for assistance in bathing were predictors of unscheduled services. Resources should be planned in order to help home care teams to handle unexpected, disruptive clusters of urgent requests that may compromise their scheduled activities.
