The aim of this study was to contribute to the assessment of exposure levels of ultrafine particles in the urban environment of Lisbon, Portugal, due to automobile traffic, by monitoring lung deposited alveolar surface area (resulting from exposure to ultrafine particles) in a major avenue leading to the town center during late spring, as well as in indoor buildings facing it. Data revealed differentiated patterns for week days and weekends, consistent with PM2.5 and PM10 patterns currently monitored by air quality stations in Lisbon. The observed ultrafine particulate levels may be directly correlated with fluxes in automobile traffic. During a typical week, amounts of ultrafine particles per alveolar deposited surface area varied between 35 and 89.2 mu m2/cm3, which are comparable with levels reported for other towns in Germany and the United States. The measured values allowed for determination of the number of ultrafine particles per cubic centimeter, which are comparable to levels reported for Madrid and Brisbane. In what concerns outdoor/indoor levels, we observed higher levels (32 to 63%) outdoors, which is somewhat lower than levels observed in houses in Ontario.

A sample of 1,288 inhabitants of Salvador, Bahia, Brazil, were submitted to the determination of anti-HBs using radioimmunoassay procedure, and analysed according to age, sex and income. Overall prevalence of anti-HBs was 11,8%. ranging from 6,7% among children aged less than three years old to 26,1% among those aged 30 years and older. Males presented prevalence of anti-HBs similar to female individuals, and those with a higher income showed frequencies of anti-HBs greater than those with a lower income level. The following conclusions were drawn: The high prevalence of anti-HBs observed among children suggests early contact with hepatitis B virus, possibly due to vertical transmission and intrafamiliar dissemination of the disease; the frequency of anti-HBs increases with age; the lower prevalence of anti-HBs among those with low income suggests that this group may present higher prevalence of carriers of the hepatitis B virus surface antigen.

Hepatitis B markers were determined in 397 individuals from Niterói and 680 from Nova Iguaçu and prevalences of 9.1% (1.0% of HBsAg and 8.1% of anti HBs) and 11.1% (1.8% of HBsAg and 9.3% of antiHBs) were found, respectively. The comparative prevalence of both markers in relation to age showed a higher prevalence of HBsAg in the group 21-50 years old. Considering the antiHBs antibody, it was demostrated a gradual increase with age, reaching 14.9% in Niterói and 29.1% in Nova Iguaçu in individuals over 51 years old. For hepatitis A, in 259 samples from Niterói, equally distributed by age groups, an overall prevalence of 74.5% of anti-HAV antibodies was found. This prevalence increases gradually reaching 90.0% at age over thirty. In 254 samples from Nova Iguaçu analysed, a prevalence of 90.5% of antibodies was encountered when the same criteria of distribution of samples were used. This level of prevalence reached 90.0% already in the age over ten years old. The tests were performed by enzyme immunoassay with reagents prepared in our laboratory.

The aim of this work was to assess the influence of meteorological conditions on the dispersion of particulate matter from an industrial zone into urban and suburban areas. The particulate matter concentration was related to the most important meteorological variables such as wind direction, velocity and frequency. A coal-fired power plant was considered to be the main emission source with two stacks of 225 m height. A middle point between the two stacks was taken as the centre of two concentric circles with 6 and 20 km radius delimiting the sampling area. About 40 sampling collectors were placed within this area. Meteorological data was obtained from a portable meteorological station placed at approximately 1.7 km to SE from the stacks. Additional data was obtained from the electrical company that runs the coal power plant. These data covers the years from 2006 to the present. A detailed statistical analysis was performed to identify the most frequent meteorological conditions concerning mainly wind speed and direction. This analysis revealed that the most frequent wind blows from Northwest and North and the strongest winds blow from Northwest. Particulate matter deposition was obtained in two sampling campaigns carried out in summer and in spring. For the first campaign the monthly average flux deposition was 1.90 g/m2 and for the second campaign this value was 0.79 g/m2. Wind dispersion occurred predominantly from North to South, away from the nearest residential area, located at about 6 km to Northwest from the stacks. Nevertheless, the higher deposition fluxes occurred in the NW/N and NE/E quadrants. This study was conducted considering only the contribution of particulate matter from coal combustion, however, others sources may be present as well, such as road traffic. Additional chemical analyses and microanalysis are needed to identify the source linkage to flux deposition levels.

The Tagus estuary is bordered by the largest metropolitan area in Portugal, the Lisbon capital city council. It has suffered the impact of several major tsunamis in the past, as shown by a recent revision of the catalogue of tsunamis that struck the Portuguese coast over the past two millennia. Hence, the exposure of populations and infrastructure established along the riverfront comprises a critical concern for the civil protection services. The main objectives of this work are to determine critical inundation areas in Lisbon and to quantify the associated severity through a simple index derived from the local maximum of momentum flux per unit mass and width. The employed methodology is based on the mathematical modelling of a tsunami propagating along the estuary, resembling the one occurred on the 1 November of 1755 that followed the 8.5 M-w Great Lisbon Earthquake. The employed simulation tool was STAV-2D, a shallow-flow solver coupled with conservation equations for fine solid phases, and now featuring the novelty of discrete Lagrangian tracking of large debris. Different sets of initial conditions were studied, combining distinct tidal, atmospheric and fluvial scenarios, so that the civil protection services were provided with comprehensive information to devise public warning and alert systems and post-event mitigation intervention. For the most severe scenario, the obtained results have shown a maximum inundation extent of 1.29 km at the AlcA cent ntara valley and water depths reaching nearly 10 m across Lisbon's riverfront.

Due to their detrimental effects on human health, the scientific interest in ultrafine particles (UFP) has been increasing, but available information is far from comprehensive. Compared to the remaining population, the elderly are potentially highly susceptible to the effects of outdoor air pollution. Thus, this study aimed to (1) determine the levels of outdoor pollutants in an urban area with emphasis on UFP concentrations and (2) estimate the respective dose rates of exposure for elderly populations. UFP were continuously measured over 3 weeks at 3 sites in north Portugal: 2 urban (U1 and U2) and 1 rural used as reference (R1). Meteorological parameters and outdoor pollutants including particulate matter (PM10), ozone (O3), nitric oxide (NO), and nitrogen dioxide (NO2) were also measured. The dose rates of inhalation exposure to UFP were estimated for three different elderly age categories: 64–70, 71–80, and >81 years. Over the sampling period levels of PM10, O3 and NO2 were in compliance with European legislation. Mean UFP were 1.7 × 104 and 1.2 × 104 particles/cm3 at U1 and U2, respectively, whereas at rural site levels were 20–70% lower (mean of 1 ×104 particles/cm3). Vehicular traffic and local emissions were the predominant identified sources of UFP at urban sites. In addition, results of correlation analysis showed that UFP were meteorologically dependent. Exposure dose rates were 1.2- to 1.4-fold higher at urban than reference sites with the highest levels noted for adults at 71–80 yr, attributed mainly to higher inhalation rates.

The concerns on metals in urban wastewater treatment plants (WWTPs) are mainly related to its contents in discharges to environment, namely in the final effluent and in the sludge produced. In the near future, more restrictive limits will be imposed to final effluents, due to the recent guidelines of the European Water Framework Directive (EUWFD). Concerning the sludge, at least seven metals (Cd, Cr, Cu, Hg, Ni, Pb and Zn) have been regulated in different countries, four of which were classified by EUWFD as priority substances and two of which were also classified as hazardous substances. Although WWTPs are not designed to remove metals, the study of metals behaviour in these systems is a crucial issue to develop predictive models that can help more effectively the regulation of pre-treatment requirements and contribute to optimize the systems to get more acceptable metal concentrations in its discharges. Relevant data have been published in the literature in recent decades concerning the occurrence/fate/behaviour of metals in WWTPs. However, the information is dispersed and not standardized in terms of parameters for comparing results. This work provides a critical review on this issue through a careful systematization, in tables and graphs, of the results reported in the literature, which allows its comparison and so its analysis, in order to conclude about the state of the art in this field. A summary of the main consensus, divergences and constraints found, as well as some recommendations, is presented as conclusions, aiming to contribute to a more concerted action of future research. © 2015, Islamic Azad University (IAU).

To obtain base line data on incidence, duration, clinical characteristics and etiology of acute respiratory infections (ARI), 276 children from deprived families living in Montevideo were followed during 32 months. The target population was divided into two groups for the analysis of the results: children aged less than 12 months and those older than this age. During the follow-up period 1.056 ARI episodes were recorded. ARI incidence was 5.2 per child/year. It was 87% higher in infants than in the older group, as was the duration of the episodes. Most of the diseases were mild. Tachypnea and retractions were seldom observed, but 12 children were refered to the hospital, and 2 infants died. Viral etiology was identified in 15.3% of the episodes. RSV was the predominant agent producing annual outbreaks. Moderate to heavy colonization of the upper respiratory tract by Streptococcus pneumoniae (32.3%) and Hemophilus sp. (18.9%) was recorded during ARI episodes. This community-based study furnish original data on ARI in Uruguay. It enabled to asses the impact of these infections on childhood.

The prevalence of rubella antibodies was evaluated through a ramdom Seroepidemiological survey in 1400 blood samples of 2-14 year old children and in 329 samples of umbilical cord serum. Rubella IgG antibodies were detected by ELISA, and the sera were collected in 1987, five years before the mass vaccination campaign with measles-mumps-rubella vaccine carried out in the city of São Paulo in 1992. A significant increase in prevalence of rubella infection was observed after 6 years of age, and 77% of the individuals aged from 15 to 19 years had detectable rubella antibodies. However, the seroprevalence rose to 90.5% (171/189) in cord serum samples from children whose mothers were 20 to 29 years old, and reached 95.6% in newborns of mothers who were 30 to 34 years old, indicating that a large number of women are infected during childbearing years. This study confirms that rubella infection represents an important Public Health problem in São Paulo city. The data on the seroprevalence of rubella antibodies before the mass vaccination campaign reflects the baseline immunological status of this population before any intervention and should be used to design an adequate vaccination strategy and to assess the Seroepidemiological impact of this intervention.

A set of radiation measurements were carried out in several public and private institutions. These were selected with basis on the people affluence and passage to these sites. These measurements were registration formed either indoor, outdoor or underground and were compiled in three Case Studies. Radiation doses measurements were also made, surface and underground locations, and compiled in other two Case Studies. There were sampled, at the same time, humidity, temperature, atmospheric pressure and relevant construction materials at sampling locations. They were collected and registration formed to analyse if there is any relation or contribution for the measured value in each specific place. Geostatistical models were used to elaborate maps of the results both for radiation values and for doses. Preliminary relations were established among the measured parameters.

Bancroftian filariasis is spreading in towns of endemic areas as in Recife, northeastern Brazil, where it is a major public health problem. This paper deals with the prevalence of microfilaraemia and filarial disease in two urban areas of Recife, studying their association with individual characteristics and variables related to the exposure to the vectors. The parasitologic survey was performed through a "door-to-door" census and microfilaraemia was examined by the thick-drop technique using 45µl of peripheral blood collected between 20:00 and 24:00 o' clock. 2,863 individuals aged between 5 and 65 years were interviewed and submitted to clinical examination. Males aged between 15 and 44 years old presented the greatest risk of being microfilaraemic. Microfilaraemia was also significantly associated with no use of bednet to sleep. The risk of being microfilaraemic was greater among those who had lived in the studied areas for more than 5 years. The overall disease prevalence was 6.3%. Males presented the greatest risk of developing acute disease. The risk of developing chronic manifestations was also greater among males and increased with age. We found no association between time of residence, bednet use, microfilaraemia and acute and chronic disease. We may conclude that in endemic areas there are subgroups of individuals who has a higher risk of being microfilariae carriers due to different behaviours in relation to vector contact.

RESUMO: Desde 1640 até data extrema de 1834, os Irmãos Hospitaleiros de S. João de Deus foram os responsáveis, directa e indirectamente, pela administração e corpo de enfermagem dos Reais Hospitais Militares em Portugal, actividades que analisamos ao longo dos séculos, desenvolvendo pressupostos temáticos relativamente a sua actuação no tempo e no espaço. É durante o séc. XVII até ao séc. XIX, que vemos os cuidados da corte para com a assistência aos soldados enfermos e doentes, ao publicar inúmera legislação relativamente à complexidade assistencial na área militar, a qual foi por nós compilada para melhor contextualização da importância dos Hospitais Militares em Portugal. Os Regimentos, os Alvarás, os Regulamentos e as Ordens do Dia, constituem um objecto fundamental de pesquisa e análise para caracterizar o quotidiano nesses mesmos locais. Os Hospitais Militares desde a sua fundação, dos primórdios das Guerras da Aclamação em 1640, até ao advento do liberalismo em 1834, eram centros de conhecimento técnico e científico com um corpo assistencial especializado, onde um conjunto pluridisciplinar de profissionais zelava qualitativamente pelos assistidos, e onde os Irmãos Hospitaleiros de S. João de Deus desempenhavam funções de administradores, enfermeiros e capelães. Nesse sentido elaboramos uma listagem cronológica para inter relacionar os Irmãos Hospitaleiros e os Hospitais Militares, pois é impossível separar a Ordem de S. João de Deus da componente assistencial aos enfermos e doentes militares em Portugal. A importância urbana e arquitectónica, que os Reais Hospitais Militares tiveram no contexto orgânico e defensivo nas Praças de Guerra, é realçado pela forma como estes se encontravam implantados e construídos, demarcando-se esteticamente da globalidade edificada, pois constituíam parte integrante dos equipamentos militares, como era teorizado pelos técnicos militares. Assim analisamos a localização dos imóveis, para além do próprio edifício hospitalar, com o meio, ou seja com a urbanidade das Praças de guerra. A sobriedade arquitectónica dos Hospitais Militares, integrada nos grandes ciclos das correntes culturais europeia e nacional, associada à riqueza decorativa e iconoclasta desenvolvida nesses locais, dá-nos uma dimensão da importância científica que esses núcleos assistenciais tiveram, contribuindo para a difusão do culto e circulação da imaginária de S. João de Deus em Portugal e dos Santos venerados nos Hospitais Militares. Desta forma compreendemos o alicerçar devocional que o reino tinha por este Santo, como o fundador do conceito assistencial do hospital moderno. Estando intrinsecamente ligado a este facto vemos o proliferar do culto e da imaginária de S. João de Deus em Portugal, centrando-se a iconografia artística do Santo em torno das localidades onde se enraizaram os Hospitais Militares. Hoje, nos imóveis hospitalares, não é difícil analisar uma lenta evolução da funcionalidade dos seus espaços, gravitando o desenvolvimento estrutural assistencial em torno das enfermarias e salas de cirurgia, mantendo-se perene este arquétipo arquitectónico desde o séc. XVII até meados do séc. XIX, as quais foram levantadas, comparadas e analisadas. Foi com a exclaustração das Ordens Religiosas, pelo Decreto de 29 de Maio de 1834, que acabou a extraordinária e valorosa acção administrativa, tutelar e corpo de enfermagem dos Irmãos Hospitaleiros de S. João de Deus, na área específica da assistência militar em Portugal, extinguindo-se, nalguns casos, os Hospitais Militares, pois o reino não estava preparado para substituir esses profissionais de saúde. O nosso estudo desenvolve-se por cerca de 295 anos, espaço temporal em que os Hospitais Militares foram administrados e fundados pelos Irmãos de S. João de Deus em Portugal.---------ABSTRACT: Since 1640 until 1834 the Hospitaller Brothers of S. John of God were the responsibles, direct and indirectly, for the administration and nursing body of the Royal Military Hospitals in Portugal, activities that we analyse throughout the centuries, developing thematic presuppositions regarding its performance in time and in space. It is during the 17th century until the 19th century, that we see the court’s care with the assistance of the wounded and sick by the publishing of much legislation regarding the assistance complexity in the military area, which was compiled by us in order to achieve a better comprehension of the importance of the Military Hospitals in Portugal. The Regiments, Charters, Regulations and Orders of Day constitute a fundamental object of research and analysis to characterise the quotidian of these locations. The Military Hospitals, since its foundation, in the beginning of the Wars of Acclamation in 1640, until the advent of liberalism in 1834, were centres of technical and scientific knowledge with a specialized assistance body, were a multidisciplinary set of professionals took qualitatively care of the attended, and where the Hospitaller Brothers of S. John of God performed the tasks of administrators, nurses and chaplains. In this perspective, we created a chronological listing in order to relate the Hospitaller Brothers with the Military Hospitals, since it is impossible to separate the Hospitaller Order of S. John of God from the component of assistance to the military sick and wounded in Portugal. The urban and architectural importance that the Royal Military Hospitals had in the organic and defensive context of the War Fortifications is emphasized by the way these were implanted and built and by its architectural demarcation of the edified whole, since they constituted an integrant part of the military equipments, as it was theorized for the military architecture. Therefore we analyse the location of the real estate, analysing not only the hospital building itself, but also its relation with the environment, i. e. with the urbanism of the war fortifications. The architectural sobriety of Military Hospitals, integrated in the big cycles of cultural streams in Europe and Portugal, associated to the decorative and iconoclastic wealth developed in these locations, give us a dimension of the scientific importance that these hospitals had, contributing to the diffusion of the cult and circulation of sculptures and paintings of S. John of God in Portugal and of the Saints revered in the Hospitals. In this way, we understand the consolidation of the devotion that the kingdom had for this Saint, the founder of the assistance concept of the modern hospital. The proliferation of the cult and iconography of S. John of God is intrinsically connected to this fact, the artistic iconography concentrating itself around the localities were the Military Hospitals were built. Today, in the assistance buildings, it is not difficult to analyse a slow evolution of the functionality of its spaces, gravitating the structural assistance development around the infirmaries and surgery rooms, this architectural archetype being perennial from the 17th century until the middle of the 19th century. These infirmaries were pointed out, compared and analysed. It was the expulsion of the Religious Orders, by the Decree of May 29th 1834, that ended with the extraordinary and valorous administrative and tutelary action and nursing body of the Hospitaller Brothers of S. John of God, in the specific area of military assistance in Portugal, extinguishing, in some cases, the Military Hospitals, since the kingdom wasn’t prepared to substitute these health professionals. Our study is developed in a timeframe of 295 years, period in which the Military Hospitals were administrated and founded by the Brothers of S. John of God in Portugal.
