This study analyzes the level of disclosure on derivatives, in accordance with the recommendations issued by the Basel Committee on Banking Supervision and the Organization of Securities Commission, by Portuguese banks. We have analyzed individual annual reports related to 2009, using a disclosure index based on those recommendations. We concluded that the number of companies that provides information about derivative instruments is quite high, but the average value of disclosure is very low. The information disclosed more frequently is that related to general aspects, followed by information related to accounting and valuation methods.

OBJETIVO: Estimar a prevalência e fatores associados ao sentimento de discriminação entre estudantes. MÉTODOS: Estudo transversal com amostra representativa de 1.170 escolares de um total de 2.282 matriculados na sétima série do ensino fundamental em escolas municipais de Gravataí (RS), em 2005. Os participantes foram selecionados por amostragem aleatória por conglomerado. Os dados foram obtidos com instrumentos auto-aplicados (Global School-based Student Health Survey, body shape questionnaire, classificação socioeconômica) preenchidos em sala de aula. Foi utilizada a regressão de Cox modificada para estudos transversais, segundo modelo hierarquizado em quatro etapas. RESULTADOS: A prevalência de sentimento de discriminação foi de 21,0%, mais prevalente entre: as meninas (RP=1,93, IC 95% 1,51;2,46); os que apresentaram absenteísmo escolar (RP=1,54, IC 95% 1,21;1,97); os que fizeram uso na vida de tabaco (RP=1,53, IC 95% 1,18;1,98); os preocupados com sua imagem corporal (RP=1,42, IC 95% 1,07;1,88); os com sentimento de solidão (RP=2,50, IC 95% 1,80;3,46) e tristeza (RP=1,29, IC 95% 1,02;1,62); os com dificuldade para dormir (RP:1,41, IC 95% 1,08;1,83); os com ideação suicida (RP=1,45, IC 95% 1,13;1,85) e os que sofreram algum tipo de injúria acidental (RP=1,56, IC 95% 1,23;1,97) ou intencional (RP=2,04, IC 95% 1,51;2,76). CONCLUSÕES: O sentimento de discriminação esteve associado ao sexo e à experiência com tabaco. Sua associação com fatores psicossociais indica a coexistência de situações adversas, como a insatisfação com a imagem corporal, sintomas depressivos e presença de injúrias. Esses resultados mostram a importância da atuação conjunta de professores e profissionais de saúde na identificação precoce desse sentimento, orientação e acompanhamento de jovens enfrentando essas situações.

The behavior of mechanical manipulators with backlash is analyzed. In order to acquire and study the signals an experimental setup is implemented. The signal processing capabilities of the wavelets are used for de-noising the experimental signals and the energy of the obtained components is analyzed. To evaluate the backlash effect upon the robotic system, it is proposed an index based on the pseudo phase plane representation. Several tests are developed that demonstrate the coherence of the results.

A pesquisa sobre resiliência sugere que a criança que se desenvolve em contexto adverso, poderá usufruir de atributos relevantes, pessoais e do ambiente. Neste sentido pretendeu-se estudar, até que ponto, as competências de modulação sensorial da criança e a qualidade das interacções mãe-filho, influenciavam as trajectórias de risco e podiam promover as oportunidades de resiliência da criança. Participaram no estudo 136 crianças, 67 do sexo feminino e 69 do sexo masculino, com idades entre os 7 e os 36 meses. Analisámos a sensibilidade materna em situação de jogo livre recorrendo à escala CARE-Index e o processamento sensorial através do de entrevista baseado no protocolo de Dunn (1997) assente nos quatro padrões de processamento sensorial: baixo registo; sensibilidade sensorial; procura sensorial; evitamento sensorial, construto anteriormente validado. Constituímos, com base nas premissas do modelo de avaliação autêntica, um índex de capacidades, que nos serviu como referencial para a avaliação do risco e da resiliência. Os resultados indicaram que a resiliência infantil em ambiente de pobrezaestava associada a indicadores de elevada sensibilidade materna e a índices adequados de processamento sensorial. A discussão dos resultados enquadrou-se nos modelos actuais e emergentes das influências neurobiológicas e ambientais nos processos de risco e de resiliência.

The present study describes the experience of dental caries in Indians communities of the Xingu, in order to supply parameters for further analysis of trends of the disease in Indians. We performed oral health examination in 288 Indians from four communities (Yawalapiti, Aweti, Mehinaku and Kamaiura) living in the southern part of the Xingu National Park, using international criteria defined by the World Health Organization. The outcome measures were the DMFT and dmft scores, and the care index. Indians of the Upper Xingu presented high levels of caries, in all age groups. The average DMFT for 11 to 13-year-old children - 5.93 - was lower than the index measured in 1993 for 12-year-old schoolchildren in nearby cities - 8.23 -, whose United Nations' human development index ranked medium. However, Indians presented a much lower care index, per age group, than these cities, and a high ratio of missing teeth for persons above 20 years old. These observations indicate low incorporation of dental care services. The irregularity of the services programmed for these communities, and the changing dietary and cultural patterns, mainly derived from their contact with the non-indigenous population of Brazil, reinforce the pressing need for health promotion initiatives aimed at these groups.

RESUMO: O relatório anual de 2001 da Organização Mundial de Saúde (OMS), anunciou o interesse social dos problemas crescentes em matéria de saúde mental e relativo à necessidade urgente de uma extensão, como também, para uma melhor organização da oferta de tratamento. Sob proposta da Bélgica, esta observação foi traduzida em uma resolução « Mental health :responding to the call for action » (déclaração de intenção do 18 de maio de 2002). Nós notamos que certos países, como os Estados Unidos ou a Grã Bretanha transformam sistematicamente o tratamento residencial em um tratamento mais dirigido para a comunidade. Eles também se concentram na descoberta prematura dos problemas psiquiátricos. Este fenômeno de désinstitutionalisação obteve resultados concretos e traz um ganho certo, tanto para os atores do campo da saúde mental, como também para os candidatos a um tratamento. O tratamento das pessoas dentro do seu meio de vida é muito claramente reconhecido. As evoluções que marcaram os tratamentos em saúde mental na Bélgica durante estes utimos 40 anos mostram que vários paços importantes já foram feitos. A reforma tem como objetivo converter a oferta hospitalar e montar tratamentos dirigidos para e dentro da comunidade pela criação de equipas móveis que se inscrevem no dispositivo comunitário, historicamente muito activo bem que sub-financiado. A reorganização dos sistemas de tratamento está baseada na criação de redes, para construir um dispositivo flexível e contínuo que considera as necessidades dos pacientes. Esta reforma é, ao mesmo tempo, ambiciosa e complexa,. É uma visão nova, uma mudança de cultura, não só, para todos os atores da saúde mental, mas também para os pacientes e suas famílias. A reforma está baseada numa visão global e integrada que associa no mesmo movimento todas as autoridades competentes relativamente a saúde que elas sejam federais, regionais ou comunitárias.-----------ABSTRACT: The World Health Organization’s (WHO) annual report of 2001 identified a social interest for the increasing problems related to mental health and the urgent need of an extension and a better organization of mental health care. On a proposal of Belgium this statement was transformed into a resolution « Mental health: responding to the call for action» (Declaration of Intent, May 18th, 2002). Some countries such as the USA or the UK systematically dismantled in-patient residential care in favor of more community-based care and a focus on early detection of psychiatric problems. This de -institutionalization has clearly and concretely paid off and the value of bringing mental health care to claimants and treating them in their own residence was acknowledged. In Belgium, the evolutions which have shaped mental health care in the last forty years indicate that a number of important steps are already taken. The Reform aims to convert the supply of hospital care into community-based mental health care services through the creation of mobile teams which offer services in everyday life of the user. These teams take place in the community, historically highly active howbeit underfunded. The reorganization of health care system relies upon the creation of networks in order to build a flexible and continuous device which take into account user’s needs. This Reform can be seen as both ambitious and complex. It is a completely new vision, a major cultural shift for all mental health care stakeholders, but also for users and their relatives. The Reform is based on a global and integrated approach which links I, a same movement all relevant health authorities whether they be federal, regional or community. de-institutionalization – community mental health – recovery - users and families involvement - networking.

RESUMO: Resumo Uma prestação de serviços de saúde mental para crianças e adolescentes (CAMHS) oportuna, eficaz e baseada na evidência pode evitar incapacidade a longo prazo. No entanto, estes serviços são criticamente sub-financiados em todo o mundo. É um imperativo garantir que este precioso recurso não seja desperdiçado. Os governos e outras partes interessadas relevantes na área da saúde mental precisam de conhecer o estado de saúde mental da população, quais os recursos disponíveis e como melhor utilizar os recursos disponíveis para orientar uma política e decisões efectivas sobre os níveis de serviços. Objetivo: O objetivo deste artigo é explorar o papel da acuidade, gravidade e complexidade na determinação dos cuidados em saúde mental especializados recebidos por crianças e adolescentes que sofrem perturbações mentais. Métodos: Este estudo é exploratório envolvendo uma revisão sistemática da literatura. Foi realizada uma pesquisa com palavras-chave utilizando bases de dados PsychINFO, EMBASE, PubMed e MEDLINE. A literatura cinzenta também foi investigada com um enfoque nas abordagens sistémicas, organizacionais e políticas para a organização e comissionamento de CAMHS. Foram selecionados apenas documentos escritos em Inglês. Três países, Bélgica, Reino Unido e Estados Unidos, todos eles com modelos muito diferentes de organização de CAMHS, foram revistos para investigar de que forma os conceitos de acuidade, gravidade e complexidade foram utilizados na sua concepção de serviços para determinar o nível da assistência prestada. Resultados: Nem a Bélgica, nem o modelo norte-americano de CAMHS organização de serviço parecem estar alinhados com os principais conceitos na determinação do nível de prestação de serviços. O sistema de serviços do Reino Unido de CAMHS está mais estreitamente alinhado com esses conceitos e tem uma alocação de recursos mais equilibrada entre o hospital e a comunidade. O seu ponto fraco está na falta de flexibilidade entre os níveis de serviço e na falta de apoio para com o sector dos cuidados de saúde primários. Conclusões: A variabilidade na alocação de recursos a diferentes níveis especializados de CAMHS (em regime de internamento, ambulatório, e na comunidade) e o modelo diferente de estrutura de serviços entre os países estudados indica uma inconsistência na forma como as crianças e adolescentes que apresentam aos CAMHS são referenciados para os cuidados que recebem. Isto põe em questão se as crianças e adolescentes com perturbações mentais estão a receber o nível e tipo de cuidados concordantes com as suas necessidades. A concepção e o comissionamento de sistemas de CAMHS levam-nos à discussão de uma série de princípios fundamentais que devem ser considerados. O sistema perfeito no entanto, ainda está para ser encontrado. -------------------------------- ABSTRACT: Provision of timely, effective, evidence based mental health services to children and adolescents can prevent long term impairment, but they are critically underfunded across the globe. There is an imperative to ensure this precious resource is not wasted. Governments and other relevant mental health stakeholders need to know the mental health status of the population, what resources are available and how best to use the resources available to guide effective policy and decisions about service levels. Aim: The aim of this paper is to explore the role of acuity, severity and complexity in determining the specialist mental health care that children and adolescents experiencing mental disorders receive. Methods: This study is exploratory involving a systematic scan of the literature. A key word search was conducted using databases PSYCHINFO, EMBASE, PUBMED and MEDLINE. Grey literature was also searched to focus on systemic, organisational and policy approaches to the organisation and commissioning of CAMHS. Only documents written in English were selected. Three countries Belgium, UK and the US all with very different models of service organisation for CAMHS were reviewed to investigate how well the concepts of acuity, severity and complexity were used to determine the level of care delivered in their service design. Findings: Neither the Belgium nor the US model of CAMHS service organisation appear to align with the key concepts driving intensity of level of service provision. The UK CAMHS service system most closely aligns with the concepts. It has a more balanced resource allocation between hospital and community. Its downfall is in its lack of flexibility between service levels and its lack of support for the primary care sector. Conclusions: The variability in resource allocation to different service levels (inpatient, outpatient, community) within specialist CAMHS and the differing model of service structure across countries indicates an inconsistency in how children and adolescents presenting to CAMHS are allocated to the care they receive. This puts into question whether children and adolescent with mental disorders are receiving a level and type of care commensurate with their needs. In commissioning and designing CAMHS systems a number of key principles that should be considered are discussed. The perfect system however, is yet to be found.

The present paper focuses on a damage identification method based on the use of the second order spectral properties of the nodal response processes. The explicit dependence on the frequency content of the outputs power spectral densities makes them suitable for damage detection and localization. The well-known case study of the Z24 Bridge in Switzerland is chosen to apply and further investigate this technique with the aim of validating its reliability. Numerical simulations of the dynamic response of the structure subjected to different types of excitation are carried out to assess the variability of the spectrum-driven method with respect to both type and position of the excitation sources. The simulated data obtained from random vibrations, impulse, ramp and shaking forces, allowed to build the power spectrum matrix from which the main eigenparameters of reference and damage scenarios are extracted. Afterwards, complex eigenvectors and real eigenvalues are properly weighed and combined and a damage index based on the difference between spectral modes is computed to pinpoint the damage. Finally, a group of vibration-based damage identification methods are selected from the literature to compare the results obtained and to evaluate the performance of the spectral index.

This paper has three contributions. First, it shows how field work within small firms in PR Chinese has provided new evidence which enables us to measure and calibrate Entrepreneurial Orientation (EO), as ‘spirit’, and Intangible Assets (IA), as ‘material’, for use in models of small firm growth. Second, it uses inter-item correlation analysis and both exploratory and confirmatory factor analysis to provide new measures of EO and IA, in index and in vector form, for use in econometric models of firm growth. Third, it estimates two new econometric models of small firm employment growth in PR China, under the null hypothesis of Gibrat’s Law, using our two new index-based and vector-based measures of EO and IA. Estimation is by OLS with adjustment for heteroscedasticity, and for sample selectivity. Broadly, it finds that EO attributes have had little significant impact on small firm growth, and indeed innovativeness and pro-activity paradoxically may even dampen growth. However, IA attributes have had a positive and significant impact on growth, with networking, and technological knowledge being of prime importance, and intellectual property and human capital being of lesser but still significant importance. In the light of these results, Gibrat’s Law is generalized, and Jovanovic’s learning theory is extended, to emphasise the importance of IA to growth. These findings cast new empirical light on the oft-quoted national slogan in PR China of “spirit and material”. So far as small firms are concerned, this paper suggests that their contribution to PR China’s remarkable economic growth is not so much attributable to the ‘spirit’ of enterprise (as suggested by propaganda) as, more prosaically, to the pursuit of the ‘material’.

Making the links between Obesity & Well-being. A briefing paper developed for Care Services Improvement Partnership (CSIP) North West. The paper covers the epidemiology and evidence of links between health and obesity. Mental well-being is a key factor of obesity and weight management. Good mental health is a protective factor for good physical health and against physical illness and is essential for making healthy lifestyle choices and behaviour changes. Poor mental health can lead to unhealthy lifestyle choices and unhealthy weight management. Obesity and physical illness can also lead to poor mental health. People with mental health problems, especially severe, are also at increased risk of obesity and related poor health. In order to ensure that strategies and programmes effectively address the relevant mental well-being factors, a mental well-being impact assessment (MWIA) could be undertaken. This process is based on health impact assessment methodology and a set of evidence based mental well-being determinants and factors, grouped under the four themes of enhancing control, increasing resilience and community assets, facilitating participation and promoting inclusion. The process also involves identifying indicators to measure progress. Evaluation shows it is effective in engaging stakeholders in service development and evaluation and it increases understanding of mental well-being and its determinants.

ABSTRACT: BACKGROUND: The prevalence of obesity has increased in societies of all socio-cultural backgrounds. To date, guidelines set forward to prevent obesity have universally emphasized optimal levels of physical activity. However there are few empirical data to support the assertion that low levels of energy expenditure in activity is a causal factor in the current obesity epidemic are very limited. METHODS: The Modeling the Epidemiologic Transition Study (METS) is a cohort study designed to assess the association between physical activity levels and relative weight, weight gain and diabetes and cardiovascular disease risk in five population-based samples at different stages of economic development. Twenty-five hundred young adults, ages 25-45, were enrolled in the study; 500 from sites in Ghana, South Africa, Seychelles, Jamaica and the United States. At baseline, physical activity levels were assessed using accelerometry and a questionnaire in all participants and by doubly labeled water in a subsample of 75 per site. We assessed dietary intake using two separate 24-h recalls, body composition using bioelectrical impedance analysis, and health history, social and economic indicators by questionnaire. Blood pressure was measured and blood samples collected for measurement of lipids, glucose, insulin and adipokines. Full examination including physical activity using accelerometry, anthropometric data and fasting glucose will take place at 12 and 24 months. The distribution of the main variables and the associations between physical activity, independent of energy intake, glucose metabolism and anthropometric measures will be assessed using cross-section and longitudinal analysis within and between sites. DISCUSSION: METS will provide insight on the relative contribution of physical activity and diet to excess weight, age-related weight gain and incident glucose impairment in five populations' samples of young adults at different stages of economic development. These data should be useful for the development of empirically-based public health policy aimed at the prevention of obesity and associated chronic diseases.

Of the approximately 25,000 bridges in Iowa, 28% are classified as structurally deficient, functionally obsolete, or both. Because many Iowa bridges require repair or replacement with a relatively limited funding base, there is a need to develop new bridge materials that may lead to longer life spans and reduced life-cycle costs. In addition, new and effective methods for determining the condition of structures are needed to identify when the useful life has expired or other maintenance is needed. Due to its unique alloy blend, high-performance steel (HPS) has been shown to have improved weldability, weathering capabilities, and fracture toughness than conventional structural steels. Since the development of HPS in the mid-1990s, numerous bridges using HPS girders have been constructed, and many have been economically built. The East 12th Street Bridge, which replaced a deteriorated box girder bridge, is Iowa’s first bridge constructed using HPS girders. The new structure is a two-span bridge that crosses I-235 in Des Moines, Iowa, providing one lane of traffic in each direction. A remote, continuous, fiber-optic based structural health monitoring (SHM) system for the bridge was developed using off-the-shelf technologies. In the system, sensors strategically located on the bridge collect raw strain data and then transfer the data via wireless communication to a gateway system at a nearby secure facility. The data are integrated and converted to text files before being uploaded automatically to a website that provides live strain data and a live video stream. A data storage/processing system at the Bridge Engineering Center in Ames, Iowa, permanently stores and processes the data files. Several processes are performed to check the overall system’s operation, eliminate temperature effects from the complete strain record, compute the global behavior of the bridge, and count strain cycles at the various sensor locations.

Evidence-based medicine has enabled to approach disease in a more rational and scientific way. Clinical research has identified behaviours and risk factors that could cause disease often "silent" at the beginning, such as diabetes. Despite the clear impact of these evidences on public health, it seems that the individual risk perception level remains weak. To mention as well, the health professionals very often have a different views, which makes it difficult to communicate the risk with patients. In this article we describe the principles of risk perception, the diabetes related risk perception concerning cardiovascular complications, and suggest some practical strategies and tools which could improve risk communication in the everyday practice.
