Two analytical methods for the quality control of dihydrocodeine in commercial pharmaceutical formulations have been developed and compared with reference methods: a square wave voltammetric (SWV) method and a flow injection analysis system with electrochemical detection (FIA-EC). The electrochemical methods proposed were successfully applied to the determination of dihydrocodeine in pharmaceutical tablets and in oral solutions. These methods do not require any pretreatment of the samples, the formulation only being dissolved in a suitable electrolyte. Validation of the methods showed it to be precise, accurate and linear over the concentration range of analysis. The automatic procedure based on a flow injection analysis manifold allows a sampling rate of 115 determinations per hour.

Introdução: A velocidade da marcha quando analisada, conjuntamente com outras variáveis, permite-nos uma melhor compreensão acerca da natureza dos défices e como direcionar o tratamento. Objectivo(s): avaliar a associação da velocidade média da marcha com as variáveis espaciotemporais, angulares e de distribuição das pressões plantares em indivíduos com lesão meniscal há 4 anos, utilizando o membro não lesado como controlo. Métodos: estudo realizado em dez participantes com lesão meniscal (idade 35,3 ± 10,63 anos, altura 170,0 ± 0,09 cm, massa 67,5 ± 7,22 kg) avaliados em 3 ciclos de marcha a uma velocidade auto-selecionada. A quantificação das variáveis foi calculada através do programa Ariel Performance Analysis System e pelo Pedar System. As imagens foram editadas, digitalizadas, transformadas e suavizadas com um filtro digital com uma frequência de corte de 6HZ. Para estimar a associação das variáveis foram efetuados modelos de regressão linear e apresentados os coeficiente de regressão (β) e os respetivos intervalos de confiança (IC95%). Resultados: Foi possível verificar que a velocidade está significativa e positivamente associada aos parâmetros temporais das fases oscilantes (βML=0,044; IC95%:0,015;0,073; βMNL=0,061; IC95%:0,037;0,086), oscilação inicial (βML=0,055; IC95%:0,006;0,105; βMNL=0,091; IC95%:0,011;0,170) e cadência (βML=0,016; IC95%:0,009;0,023; βMNL=0,011; IC95%:0,006;0,017), em ambos os membros, e aos parâmetros temporais das fases de apoio unilateral (βML=0,046; IC95%:0,019;0,07), oscilação terminal (βML=0,081; IC95%:0,003;0,159) e apoio médio (βML=0,046; IC95%:0,008;0,085), apenas no membro lesado. Foi também observada, em ambos os membros, a existência de associações negativas significativas com os parâmetros temporais das fases de duplo apoio (βML=-0,024; IC95%:-0,037;-0,011; βMNL=-0,032; IC95%:-0,048;-0,015), apoio (βML=-0,044; IC95%:-0,073;-0,015; βMNL=-0,061; IC95%:-0,086;-0,037), resposta de carga (βML=-0,029; IC95%:-0,055;-0,004; βMNL=-0,047; IC95%:-0,081;-0,013), pré-oscilação (βML=-0,047; IC95%:-0,082;-0,013; βMNL=-0,060; IC95%:-0,098;-0,023) e tempo do ciclo de marcha (βML=-1,435; IC95%:-2,090;-0,781; βMNL=-0,941; IC95%:-1,431;-0,451). Foi ainda identificada, no membro lesado, uma associação positiva com a pressão plantar máxima normalizada durante o contacto do calcanhar terminal (βML=0,612; IC95%:0,077;1,147) e com o passo normalizado (βML=2,413; IC95%:0,264;4,561) e uma associação negativa limítrofe com a amplitude de flexão do joelho durante a elevação da ponta dos dedos (βML=-0,031; IC95%:-0,061;0,000). Conclusão: A velocidade média da marcha parece influenciar bastante os parâmetros temporais, sem provocar grandes alterações nos parâmetros espaciais, angulares e de pressão plantar, sendo esta associação semelhante em ambos os membros e independente do local da lesão.

Intelligent wheelchairs (IW) are technologies that can increase the autonomy and independence of elderly people and patients suffering from some kind of disability. Nowadays the intelligent wheelchairs and the human-machine studies are very active research areas. This paper presents a methodology and a Data Analysis System (DAS) that provides an adapted command language to an user of the IW. This command language is a set of input sequences that can be created using inputs from an input device or a combination of the inputs available in a multimodal interface. The results show that there are statistical evidences to affirm that the mean of the evaluation of the DAS generated command language is higher than the mean of the evaluation of the command language recommended by the health specialist (p value = 0.002) with a sample of 11 cerebral palsy users. This work demonstrates that it is possible to adapt an intelligent wheelchair interface to the user even when the users present heterogeneous and severe physical constraints.

Nos últimos anos, tem-se assistido a uma maior preocupação com o meio ambiente, a atual conjuntura mundial está cada vez mais direcionada para a eficiência energética e para a utilização de fontes de energias renováveis. Os principais governos mundiais, incluindo o português, já perceberam a necessidade de enveredar por esse caminho e nesse sentido aplicam medidas que direcionam e consciencializam a população para a eficiência energética e para as energias renováveis. Em Portugal, o setor das energias renováveis assume atualmente uma posição de extrema importância, resultante da expressão que governo português tem vindo a implementar no panorama energético nacional, da qual resulta uma importante contribuição para o desenvolvimento económico, na criação de riqueza e geração de emprego. Neste contexto, e no caso particular da energia fotovoltaica têm sido implementadas medidas que incentivam a aposta nesta tecnologia, prova disso é o Decreto-Lei n.º 153/2014 aprovado em conselho de ministros em Setembro de 2014, que promove essencialmente o autoconsumo. O autoconsumo consiste na utilização de painéis fotovoltaicos para produção de energia elétrica para consumo próprio com ou sem recurso a equipamentos de acumulação. Em termos práticos, este sistema permite que os consumidores produzam a sua própria energia através de uma fonte renovável ao invés de adquirir essa energia na rede elétrica de serviço público. As políticas de incentivo ao autoconsumo proporcionam uma oportunidade para os consumidores interessados em investir na produção da própria energia elétrica, neste sentido e de forma a ajudar no dimensionamento de unidades de produção de autoconsumo foi desenvolvida, no âmbito desta tese, uma ferramenta de apoio ao dimensionamento de sistemas de autoconsumo fotovoltaico sem acumulação em ambiente doméstico, com o objetivo de estimar as necessidades de potência fotovoltaica a instalar em habitações de baixa tensão normal. Na base da construção desta ferramenta estiveram essencialmente os perfis de consumo, aprovados pela Entidade Reguladora dos Serviços Energéticos, de todos os clientes finais que não dispõem de equipamento de medição com registo de consumos e também a estimativa de produção fotovoltaica desenvolvida pelo Centro Comum de Investigação da Comissão Europeia. A aplicação desenvolvida tem como principal funcionalidade proporcionar ao utilizador o dimensionamento de unidades de produção de autoconsumo fotovoltaico, mediante a introdução de alguns dados tais como o distrito, a potência contratada, a tarifa e o consumo energético anual. Esta aplicação apresenta resultados relativos ao dimensionamento do sistema, como é o caso da potência a instalar e da estimativa de produção fotovoltaica anual, e resultados relativos à análise económica do sistema como é o caso do valor atual líquido, da taxa interna de rentabilidade e do payback do investimento.

In this thesis a semi-automated cell analysis system is described through image processing. To achieve this, an image processing algorithm was studied in order to segment cells in a semi-automatic way. The main goal of this analysis is to increase the performance of cell image segmentation process, without affecting the results in a significant way. Even though, a totally manual system has the ability of producing the best results, it has the disadvantage of taking too long and being repetitive, when a large number of images need to be processed. An active contour algorithm was tested in a sequence of images taken by a microscope. This algorithm, more commonly known as snakes, allowed the user to define an initial region in which the cell was incorporated. Then, the algorithm would run several times, making the initial region contours to converge to the cell boundaries. With the final contour, it was possible to extract region properties and produce statistical data. This data allowed to say that this algorithm produces similar results to a purely manual system but at a faster rate. On the other hand, it is slower than a purely automatic way but it allows the user to adjust the contour, making it more versatile and tolerant to image variations.

High throughput genome (HTG) and expressed sequence tag (EST) sequences are currently the most abundant nucleotide sequence classes in the public database. The large volume, high degree of fragmentation and lack of gene structure annotations prevent efficient and effective searches of HTG and EST data for protein sequence homologies by standard search methods. Here, we briefly describe three newly developed resources that should make discovery of interesting genes in these sequence classes easier in the future, especially to biologists not having access to a powerful local bioinformatics environment. trEST and trGEN are regularly regenerated databases of hypothetical protein sequences predicted from EST and HTG sequences, respectively. Hits is a web-based data retrieval and analysis system providing access to precomputed matches between protein sequences (including sequences from trEST and trGEN) and patterns and profiles from Prosite and Pfam. The three resources can be accessed via the Hits home page (http://hits. isb-sib.ch).

The mechanical behaviour of ectodermal cells in the area opaca and the supracellular organization of fibronectin in the adjacent extracellular matrix were studied in whole chick blastoderms developing in vitro. The pattern of spontaneous mechanical activity and its modification by immunoglobulins against fibronectin were determined using a real-time image-analysis system. The pattern of fibronectin was studied using immunocytochemical techniques. It was found that the ectodermal cells in the area opaca actively develop a radially oriented contraction, which leads to a distension of the area pellucida from which the embryo develops. Abnormally increased tension resulted in perturbations of gastrulation and neurulation. An optimized mechanical equilibrium within the blastoderm seems to be necessary for normal development. Anti-fibronectin antibodies applied to the basal side of the blastoderm led rapidly and reversibly to an increase of tension in the contracted cells. This observation indicates that modifications of the extracellular matrix can be transmitted to cytoskeletal elements within adjacent cells. The extracellular matrix of the area opaca contains fibronectin arranged in radially oriented fibrils. This orientation corresponds to the direction of migration of the mesodermal cells. Interestingly, the radial pattern of fibronectin is found in the regions where the ectodermal cells are contracted and develop radially oriented forces. This observation suggests that the supracellular assembly of the extracellular materials could be influenced by the mechanical activity of adjacent cells. Possible modulations of the supracellular organization of extracellular matrix by other factors, e.g. diffusible metabolites, is also discussed. The presence of characteristically organized extracellular matrix components, of spatially differentiated cell activities and of reciprocal interactions between them makes the young chick blastoderm an excellent system for physiological studies of the coordinated cellular activities that lead to changes in form, complexity and function.

This study assessed medical students' perception of individual vs. group training in breaking bad news (BBN) and explored training needs in BBN. Master-level students (N = 124) were randomised to group training (GT)-where only one or two students per group conducted a simulated patient (SP) interview, which was discussed collectively with the faculty-or individual training (IT)-where each student conducted an SP interview, which was discussed during individual supervision. Training evaluation was based on questionnaires, and the videotaped interviews were rated using the Roter Interaction Analysis System. Students were globally satisfied with the training. Still, there were noticeable differences between students performing an interview (GT/IT) and students observing interviews (GT). The analysis of the interviews showed significant differences according to scenarios and to gender. Active involvement through SP interviews seems required for students to feel able to reach training objectives. The evaluation of communication skills, revealing a baseline heterogeneity, supports individualised training.

Background: ln Switzerland no HIV test is performed without the patient's consent based on a Voluntary Counseling and Testing policy (VCT). We hypothesized that a substantial proportion of patients going through an elective surgery falsely believed that an HIV test was performed on a routine basis and that the lack of transmission of result was interpreted as being HIV negative. Method: All patients with elective orthopedic surgery during 2007 were contacted by phone in 2008. A structured questionnaire assessed their belief about routine preoperative blood analysis (diabetes, coagulation function, HIV test and cholesterol level) as well as result awareness and interpretation. Variables included age and gender. Analysis were conducted using the software JMP 6.0.3. Results: 1123 patients were included. 130 (12 %) were excluded (Le. unreachable, unable to communicate on the phone, not operated). 993 completed the survey (89 %). Median age was 51 (16-79). 50 % were female. 376 (38 %) patients thought they had an HIV test performed before surgery but none of them had one. 298 (79 %) interpreted the absence of result as a negative HIV test. A predictive factor to believe an HIV test had been done was an age below 50 years old (45 % vs 33 % for 16-49 years old and 50-79 years old respectively, p < 0.001). No difference was observed between genders. Conclusion: ln Switzerland, nearly 40 % of the patients falsely thought an HIV test had been performed on a routine basis before surgery and were erroneously reassured about their HIV status. These results should either improve the information given to the patient regarding preoperative exams, or motivate public health policy to consider HIV opt-out screening instead of VCT strategy.

The presence of Gm(1,2,4,10,21) and Km(1) factors in vitreous humor taken from human corpses was investigated. The results revealed a good agreement between the factors detected in this biological material and in blood. Their presence in vitreous humor is independent of the secretor type.

BACKGROUND Fragile X syndrome (FXS) is an inherited neurodevelopmental condition characterised by behavioural, learning disabilities, physical and neurological symptoms. In addition, an important degree of comorbidity with autism is also present. Considered a rare disorder affecting both genders, it first becomes apparent during childhood with displays of language delay and behavioural symptoms.Main aim: To show whether the combination of 10 mg/kg/day of ascorbic acid (vitamin C) and 10 mg/kg/day of α-tocopherol (vitamin E) reduces FXS symptoms among male patients ages 6 to 18 years compared to placebo treatment, as measured on the standardized rating scales at baseline, and after 12 and 24 weeks of treatment.Secondary aims: To assess the safety of the treatment. To describe behavioural and cognitive changes revealed by the Developmental Behaviour Checklist Short Form (DBC-P24) and the Wechsler Intelligence Scale for Children-Revised. To describe metabolic changes revealed by blood analysis. To measure treatment impact at home and in an academic environment. METHODS/DESIGN A phase II randomized, double-blind pilot clinical trial. SCOPE male children and adolescents diagnosed with FXS, in accordance with a standardized molecular biology test, who met all the inclusion criteria and none of the exclusion criteria. INSTRUMENTATION clinical data, blood analysis, Wechsler Intelligence Scale for Children-Revised, Conners parent and teacher rating scale scores and the DBC-P24 results will be obtained at the baseline (t0). Follow up examinations will take place at 12 weeks (t1) and 24 weeks (t2) of treatment. DISCUSSION A limited number of clinical trials have been carried out on children with FXS, but more are necessary as current treatment possibilities are insufficient and often provoke side effects. In the present study, we sought to overcome possible methodological problems by conducting a phase II pilot study in order to calculate the relevant statistical parameters and determine the safety of the proposed treatment. The results will provide evidence to improve hyperactivity control and reduce behavioural and learning problems using ascorbic acid (vitamin C) and α-tocopherol (vitamin E). The study protocol was approved by the Regional Government Committee for Clinical Trials in Andalusia and the Spanish agency for drugs and health products. BACKGROUND: Polymorphism of the Duffy Antigen Receptor for Chemokines (DARC) is associated with susceptibility to and the severity of Plasmodium vivax malaria in humans. P. vivax uses DARC to invade erythrocytes. Individuals lacking DARC are 'resistant' to P. vivax erythrocytic infection. However, susceptibility to P. vivax in DARC+ individuals is reported to vary between specific DARC genotypes. We hypothesized that the natural acquisition of antibodies to P. vivax blood stages may vary with the host genotype and the level of DARC expression. Furthermore, high parasitemia has been reported to effect the acquisition of immunity against pre-erythrocytic parasites. We investigated the correlation between host DARC genotypes and the frequency and magnitude of antibodies against P. vivax erythrocytic stage antigens. METHODOLOGY/FINDINGS: We assessed the frequencies and magnitudes of antibody responses against P. vivax and P. falciparum sporozoite and erythrocytic antigens in Colombian donors from malaria-endemic regions. The frequency and level of naturally-acquired antibodies against the P. vivax erythrocytic antigens merozoite surface protein 1 (PvMSP1) and Duffy binding protein (PvDBP) varied with the host DARC genotypes. Donors with one negative allele (FY*B/FY*Bnull and FY*A/FY*Bnull) were more likely to have anti-PvMSP1 and anti-PvDBP antibodies than those with two positive alleles (FY*B/FY*B and FY*A/FY*B). The lower IgG3 and IgG1 components of the total IgG response may account for the decreased responses to P. vivax erythrocytic antigens with FY*A/FY*B and FY*B/FY*B genotypes. No such association was detected with P. falciparum erythrocytic antigens, which does not use DARC for erythrocyte invasion. CONCLUSION/SIGNIFICANCE: Individuals with higher DARC expression, which is associated with higher susceptibility to P. vivax infection, exhibited low frequencies and magnitudes of P. vivax blood-stage specific antibody responses. This may indicate that one of the primary mechanisms by which P. vivax evades host immunity is through DARC indirectly down-regulating humoral responses against erythrocytic invasion and development.

The distribution of parvalbumin (PV), calretinin (CR), and calbindin (CB) immunoreactive neurons was studied with the help of an image analysis system (Vidas/Zeiss) in the primary visual area 17 and associative area 18 (Brodmann) of Alzheimer and control brains. In neither of these areas was there a significant difference between Alzheimer and control groups in the mean number of PV, CR, or CB immunoreactive neuronal profiles, counted in a cortical column going from pia to white matter. Significant differences in the mean densities (numbers per square millimeter of cortex) of PV, CR, and CB immunoreactive neuronal profiles were not observed either between groups or areas, but only between superficial, middle, and deep layers within areas 17 and 18. The optical density of the immunoreactive neuropil was also similar in Alzheimer and controls, correlating with the numerical density of immunoreactive profiles in superficial, middle, and deep layers. The frequency distribution of neuronal areas indicated significant differences between PV, CR, and CB immunoreactive neuronal profiles in both areas 17 and 18, with more large PV than CR and CB positive profiles. There were also significantly more small and less large PV and CR immunoreactive neuronal profiles in Alzheimer than in controls. Our data show that, although the brain pathology is moderate to severe, there is no prominent decrease of PV, CR and CB positive neurons in the visual cortex of Alzheimer brains, but only selective changes in neuronal perikarya.

Fibriinimonomeerejä syntyy fibrinogeenistä veren hyytyessä. Fibriinimonomeerit muodostavat liukoisia komplekseja, joiden olemassaolo voidaan osoittaa plasmasta silloin, kun niitä esiintyy veressä suuria määriä, kuten disseminoituneen intravaskulaarisen koagulaation (DIK) yhteydessä. HUSLABin Meilahden sairaalan laboratoriossa fibriinimonomeerien olemassaolo todetaan tällä hetkellä hemagglutinaatioon perustuvalla kvalitatiivisella testillä, jossa tulos tulkitaan silmämääräisesti. Diagnostica Stago on tuonut markkinoille uuden kvantitatiivisen STA® - Liatest® FM immunoturbidometrisen testin, joka perustuu monoklonaaliseen fibriinimonomeerivasta-aineeseen. Uusi testi on automatisoitavissa, mikä helpottaa testin suoritusta. Työssäni testasin immunoturbidometrisen menetelmän analyyttisiä suorituskykyominaisuuksia määrittämällä Blood Coagulation System (BCS) -analysaattorilla sarjan sisäisen ja sarjojen välisen toistuvuuden sekä lineaarisuuden. Tulosten perusteella menetelmä toimii toistettavasti, mutta lineaarisuus ei ole optimaalinen. Vertasin myös nykyisen ja uuden menetelmän tuloksia 40:stä potilasnäytteestä. Kaikki näytteet olivat agglutinaatiotestissä negatiivisia. Osa näistä antoi kuitenkin positiivisia tuloksia immunoturbidometrisellä testillä. Tulosten perusteella immunoturbidometrinen menetelmä ei mittaa ainoastaan fibriinimonomeerejä. Lisäksi tarkastelin fibriinimonomeeritulosten korrelaatiota saman näytteen fibrinogeeni- ja D-dimeerituloksiin. Fibriinimonomeeritulokset eivät korreloineet fibrinogeenitulosten kanssa, mutta D-dimeeritulosten kanssa korrelaatio oli voimakasta ja tilastollisesi merkitsevää. D-dimeeri mahdollisesti ristireagoi menetelmässä. Jos menetelmä halutaan ottaa käyttöön, se edellyttää lisätutkimuksia.

While the influence of HLA-AB and -DRB1 matching on the outcome of bone marrow transplantation (BMT) with unrelated donors is clear, the evaluation of HLA-C has been hampered by its poor serological definition. Because the low resolution of standard HLA-C typing could explain the significant number of positive cytotoxic T lymphocyte precursor frequency (CTLpf) tests found among HLA-AB-subtype, DRB1/B3/B5-subtype matched patient/donor pairs, we have identified by sequencing the incompatibilities recognized by CD8+ CTL clones obtained from such positive CTLpf tests. In most cases the target molecules were HLA-C antigens that had escaped detection by serology (e.g. Cw*1601, 1502 or 0702). Direct recognition of HLA-C by a CTL clone was demonstrated by lysis of the HLA class I-negative 721.221 cell line transfected with Cw*1601 cDNA. Because of the functional importance of Cw polymorphism, a PCR-SSO oligotyping procedure was set up allowing the resolution of 29 Cw alleles. Oligotyping of a panel of 382 individuals (including 101 patients and their 272 potential unrelated donors, 5 related donors and 4 platelet donors) allowed to determine HLA-C and HLA A-B-Cw-DRB1 allelic frequencies, as well as a number of A-Cw, B-Cw, and DRB1-Cw associations. Two new HLA-Cw alleles (Cw*02023 and Cw*0707) were identified by DNA sequencing of PCR-amplified exon 2-intron 2-exon 3 amplicons. Furthermore, we determined the degree of HLA-C compatibility in 287 matched pairs that could be formed from 73 patients and their 184 potential unrelated donors compatible for HLA-AB by serology and for HLA-DRB1/ B3/B5 by oligotyping. Cw mismatches were identified in 42.1% of these pairs, and AB-subtype oligotyping showed that 30% of these Cw-incompatible pairs were also mismatched for A or B-locus subtype. The degree of HLA-C incompatibility was strongly influenced by the linkage with B alleles and by the ABDR haplotypes. Cw alleles linked with B*4403, B*5101, B18, and B62 haplotypes were frequently mismatched. Apparently high resolution DNA typing for HLA-AB does not result in full matching at locus C. Since HLA-C polymorphism is recognized by alloreactive CTLs, such incompatibilities might be as relevant as AB-subtype mismatches in clinical transplantation.
