Objective: To assess understanding of, and actual and potential roles in management of attention-deficit/hyperactivity disorder (ADHD) among GPs. Methods: A cross-sectional questionnaire survey of Queensland GPs selected randomly from the Royal Australian College of General Practitioners directory of members was carried out. Main outcome measures were knowledge levels of ADHD, current management practices, referral patterns and self-perceived information and training needs. Results: Three hundred and ninety-nine GPs returned a completed questionnaire (response rate 76%). Roles identified by GPs were: the provisional diagnosis of ADHD and referral to specialist services for confirmation of the diagnosis and initiation of management; assistance with monitoring progress once a management plan was in place; education of the child and their family regarding the disorder; and liaison with the school where necessary. Perceived barriers to increased involvement of GPs were: time and resource constraints of general practice; concerns regarding abuse and addiction liability of prescription stimulants; complex diagnostic issues associated with childhood behavioural problems; and lack of training and education regarding ADHD. Conclusions: General practitioners identify a role for themselves in ADHD care that is largely supportive in nature and involves close liaison with specialist services.

Leaf water relations responses to limited water supply were determined in 7-month-old plants of a dry inland provenance of Eucalyptus argophloia Blakely and in a humid coastal provenance (Gympie) and a dry inland provenance (Hungry Hills) of Eucalyptus cloeziana F. Muell. Each provenance of E. cloeziana exhibited a lower relative water content at the turgor loss point, a lower apoplastic water content, a smaller ratio of dry mass to turgid mass and a lower bulk modulus of elasticity than the single provenance of E. argophloia. Osmotic potential at full turgor and water potential at the turgor loss point were significantly lower in E. argophloia and the inland provenance of E. cloeziana than in the coastal provenance of E. cloeziana. There was limited osmotic adjustment in response to soil drying in E. cloeziana, but not in E. argophloia. Between-species differences in water relations parameters were larger than those between the E. cloeziana provenances. Both E. cloeziana provenances maintained turgor under moderate water stress through a combination of osmotic and elastic adjustments. Eucalyptus argophloia had more rigid cell walls and reached lower water potentials with less reduction in relative water content than either of the E. cloeziana provenances, thereby enabling it to extract water from dryer soils.

The aim of this study was to compare accumulated oxygen deficit data derived using two different exercise protocols with the aim of producing a less time-consuming test specifically for use with athletes. Six road and four track male endurance cyclists performed two series of cycle ergometer tests. The first series involved five 10 min sub-maximal cycle exercise bouts, a (V) over dotO(2peak) test and a 115% (V) over dotO(2peak) test. Data from these tests were used to estimate the accumulated oxygen deficit according to the calculations of Medbo et al. (1988). In the second series of tests, participants performed a 15 min incremental cycle ergometer test followed, 2 min later, by a 2 min variable resistance test in which they completed as much work as possible while pedalling at a constant rate. Analysis revealed that the accumulated oxygen deficit calculated from the first series of tests was higher (P< 0.02) than that calculated from the second series: 52.3 +/- 11.7 and 43.9 +/- 6.4 ml . kg(-1), respectively (mean +/- s). Esse estudo objetivou investigar, em regime de casa de vegetação, o desempenho fisiológico de duas cultivares jovens e enxertadas de seringueira (Hevea brasiliensis), submetidas à deficiência hídrica e posterior recuperação. Os parâmetros analisados foram o potêncial hídrico foliar (ψw), taxa de sobrevivência e crescimento após reidratação e a fluorescência transiente da clorofila a. Trinta dias após a aclimatação, o déficit hídrico foi iniciado pela supressão total da água até o potencial hídrico (ψw) atingir valores críticos (38 dias), seguido por 30 dias de reidratação. Os dois genótipos (RRIM600 e FX3864) apresentaram um comportamentos semelhantes de redução do ψw com o avanço da supressão hídrica. Entretanto, o genótipo FX3864 mostrou-se mais susceptível ao déficit hídrico comprovado por valores que demonstram deficiência no transporte de elétrons no etapa fotoquímica da fotossíntese e por uma menor taxa de sobrevivência após desidratação e menor crescimento e desenvolvimento após a reidratação. RRIM600 apresentou uma maior tolerância à imposição da supressão hídrica, confirmada pelos valores da maioria dos parâmetros analisados e pelo menor tempo necessário para sua recuperação.

Partiendo del principio de la imposibilidad de separar la salud mental de la salud en general, podemos afirmar que más del 75% de los psicólogos españoles se declaran enmarcados en el llamado modelo cognitivo-conductual de evaluación e intervención en el campo de la psicología clínica Sin embargo, a la luz de la expansión en la aplicación de la psicología y a los nuevos retos que ello plantea, en estas líneas nos ha parecido más interesante definir las funciones y enmarcar los ámbitos de actuación de los psicólogos que trabajan en el campo de la salud mental en particular y de la salud en general.

El Ministerio de Salud en Chile se ha planteado como uno de sus principales objetivos, proveer servicios acorde a las expectativas de la población. Para lograrlo, se requiere necesariamente, conocer cuál es la calidad del servicio ofrecida por los centros de salud. En Chile, los centros de atención secundaria, no han desarrollado escalas de calidad del servicio que tengan un buen grado de validez, fiabilidad y dimensionalidad. Este artículo, propone una escala que cumpla estas condiciones, para medir de manera óptima la calidad del servicio que los Centros de Atención Secundaria ofrecen a sus pacientes. Para lograr este objetivo, se aplicó una encuesta estructurada y posteriormente se realizó un exhaustivo análisis psicométrico de los datos, a través de una serie de análisis exploratorios y confirmatorios que incluyó ecuaciones estructurales. Este estudio permitió construir y proponer una escala fiable, válida y con buen grado de dimensionalidad, compuesta por cinco subescalas claramente relacionadas e integradas en un único constructo. Estas son: confiabilidad, empatía, capacidad de respuesta, accesibilidad y tangibles.

Dada la magnitud del problema de salud en los paises de America Latina y el Caribe, el déficit de recursos financieros necesarios para proveer atención médica no se puede cubrir a través de aumentos en los precios, o adoptando medidas como pago por servicio o cobro por los costos de los servicios. Es necesario recurrir a la política fiscal. Se demuestra que es posible aumentar los recursos disponibles para la atención médica, tanto por el aumento en la recaudación de impuestos, como por la toma de decisión política de aumentar los ingresos del gobierno destinados a financiar la atención médica. Se revisa la experiencia de los Fondos de Emergencia Social y de las operaciones de cambio de deuda por salud como importantes fuentes adicionales de recursos especialmente para financiar la inversión.

Since the number and proportion of old people increases worldwide, health professionals and systems should be made aware and prepared to deal with their problems. Cognitive deficit and symptoms of depression are commom among the elderly, and may occur in relation to various risk factors such as health conditions and psychosocial variables. In order to study cognitive deficit and the presence of signs and symptoms of depression, 62 elderly community subjects enrolled at a Community Health Unit in Porto Alegre, southern Brazil, were interviewed. They were evaluated by means of the Mini Mental State Exam, the Montgomery-Asberg Depression rating scale, and a questionnaire on health conditions, living arrangements and social variables. Higher levels of symptoms of depression were observed among subjects exposed to major risk factors for cerebrovascular diseases (diabetes and coronary disease), while impaired cognitive performance was seen among individuals who could not count on the presence of a confidant (social network variable). The results suggest that the early identification of major risk groups among old people can help to prevent institutionalization and keep individuals in the community.
