Objectives: To evaluate the situation regarding gender sensitivity in national health plans in Latin America and the European Union for the decade 2000–2010. Methods: A systematic search and content analysis of national health plans were carried out within 37 countries. Gender sensitivity, defined as the extent to which a health plan considers gender as a central category and develops measures to reduce any gender-related inequalities, was analysed through an ad hoc checklist. Results: The description of health problems by sex was more frequent than intervention proposals aimed at reducing gender health disparities. The greatest number of specific intervention proposals targeted at overcoming gender-based health inequalities were associated with sexual and/or reproductive health, gender based violence, the working environment and human resources training. Compared to the European Union member states, Latin American health plans were found to be generally more gender sensitive. Conclusions: National health plans are still generally lacking in gender sensitivity. Disparities exist in health policy formulation in favour of men, whilst women's health continues to be identified mainly with reproductive health. If gender sensitivity is not taken into account, efforts to improve the quality of clinical care will be insufficient as gender inequalities will persist.

Background: It has been shown that gender equity has a positive impact on the everyday activities of people (decision making, income allocation, application and observance of norms/rules) which affect their health. Gender equity is also a crucial determinant of health inequalities at national level; thus, monitoring is important for surveillance of women’s and men’s health as well as for future health policy initiatives. The Gender Equity Index (GEI) was designed to show inequity solely towards women. Given that the value under scrutiny is equity, in this paper a modified version of the GEI is proposed, the MGEI, which highlights the inequities affecting both sexes. Methods: Rather than calculating gender gaps by means of a quotient of proportions, gaps in the MGEI are expressed in absolute terms (differences in proportions). The Spearman’s rank coefficient, calculated from country rankings obtained according to both indexes, was used to evaluate the level of concordance between both classifications. To compare the degree of sensitivity and obtain the inequity by the two methods, the variation coefficient of the GEI and MGEI values was calculated. Results: Country rankings according to GEI and MGEI values showed a high correlation (rank coef. = 0.95). The MGEI presented greater dispersion (43.8%) than the GEI (19.27%). Inequity towards men was identified in the education gap (rank coef. = 0.36) when using the MGEI. According to this method, many countries shared the same absolute value for education but with opposite signs, for example Azerbaijan (−0.022) and Belgium (0.022), reflecting inequity towards women and men, respectively. This also occurred in the empowerment gap with the technical and professional job component (Brunei:-0.120 vs. Australia, Canada Iceland and the U.S.A.: 0.120). Conclusion: The MGEI identifies and highlights the different areas of inequities between gender groups. It thus overcomes the shortcomings of the GEI related to the aim for which this latter was created, namely measuring gender equity, and is therefore of great use to policy makers who wish to understand and monitor the results of specific equity policies and to determine the length of time for which these policies should be maintained in order to correct long-standing structural discrimination against women.

Aims: To describe gender- and social class-related inequalities in sexual satisfaction and analyze their relationship with self-perceived health status. Methods: This population-based, cross-sectional study included 7384 sexually active people aged 16 years and over residing in Spain in 2009 (3951 men and 3433 women). The explanatory variables were gender, age, social class, share in performing domestic tasks, spend time looking after oneself, collaborate economically in supporting the family, caring for children, self-perceived health status, and the desire to increase or decrease frequency of having sexual relations. Bivariate and multivariate logistic regression models were fitted. Results: Among women, sexual satisfaction declines progressively after age 45. Sexual satisfaction is 1.7 times higher among women who look after themselves and who feel good compared with those who do not. The odds of wanting to increase sex is 3.3 times higher for women who are satisfied compared with women who desire a lower frequency of sexual intercourses; and good perceived health was associated with sexual satisfaction. In satisfied men, the corresponding odds is 1.9 times that of men desiring to reduce their frequency of sex. Conclusions: Gender and social class inequalities are found in sexual satisfaction. This is associated with perceived health status, adding evidence in support of the World Health Organization definition of sexual health.

Background: The immigrant population living in Spain grew exponentially in the early 2000s but has been particularly affected by the economic crisis. This study aims to analyse health inequalities between immigrants born in middle- or low-income countries and natives in Spain, in 2006 and 2012, taking into account gender, year of arrival and socioeconomic exposures. Methods: Study of trends using two cross-sections, the 2006 and 2012 editions of the Spanish National Health Survey, including residents in Spain aged 15–64 years (20 810 natives and 2950 immigrants in 2006, 14 291 natives and 2448 immigrants in 2012). Fair/poor self-rated health, poor mental health (GHQ-12 > 2), chronic activity limitation and use of psychotropic drugs were compared between natives and immigrants who arrived in Spain before 2006, adjusting robust Poisson regression models for age and socioeconomic variables to obtain prevalence ratios (PR) and 95% confidence interval (CI). Results: Inequalities in poor self-rated health between immigrants and natives tend to increase among women (age-adjusted PR2006 = 1.39; 95% CI: 1.24–1.56, PR2012 = 1.56; 95% CI: 1.33–1.82). Among men, there is a new onset of inequalities in poor mental health (PR2006 = 1.10; 95% CI: 0.86–1.40, PR2012 = 1.34; 95% CI: 1.06–1.69) and an equalization of the previously lower use of psychotropic drugs (PR2006 = 0.22; 95% CI: 0.11–0.43, PR2012 = 1.20; 95% CI: 0.73–2.01). Conclusions: Between 2006 and 2012, immigrants who arrived in Spain before 2006 appeared to worsen their health status when compared with natives. The loss of the healthy immigrant effect in the context of a worse impact of the economic crisis on immigrants appears as potential explanation. Employment, social protection and re-universalization of healthcare would prevent further deterioration of immigrants’ health status.

Objective. Lower socioeconomic status (SES) is associated in industrialized countries with unhealthy lifestyle characteristics, such as smoking, physical inactivity and being overweight or obese. This paper examines changes over time in the association between SES and smoking status, physical activity and being overweight or obese in Australia. Methods. Data were taken from three successive national health surveys in Australia carried out in 1989-90 (n = 54 576), 1995 (n = 53 828) and 2001 (n = 26 863). Participants in these surveys were selected using a national probability sampling strategy, and aggregated data for geographical areas are used to determine the changing association between SES and lifestyle over time. Findings. Overall, men had less healthy lifestyles, In 2001 inverse SES trends for both men and women showed that those living in lower SES areas were more likely to smoke and to be sedentary and obese, There were some important socioeconomic changes over the period 1989-90 to 2001. The least socioeconomically disadvantaged areas had the largest decrease in the percentage of people smoking tobacco (24% decrease for men and 12% for women) and the largest decrease in the percentage of people reporting sedentary activity levels (25% decrease for men and 22% for women). While there has been a general increase in the percentage over time of those who are overweight or obese, there is a modest trend for being overweight to have increased (by about 16% only among females) among those living in areas of higher SES. Conclusion. Socioeconomic inequalities have been increasing for several key risk behaviours related to health; this suggests that T specific population-based prevention strategies intended to reduce health inequalities are needed.

This article analyzes the relationship between employment status (ES), on one hand, and self-rated health and psychological distress, on the other, in the context of the Great Recession beginning in 2008. For this purpose, it is necessary to move beyond the employment/unemployment dichotomy characteristics of previous theories and research concerning the relationship between the labor market, recession, and health. The authors use data from the Spanish National Health Surveys in 2006 (n = 15,128), before the crisis, and in 2012 (n = 11,124), when its consequences had taken effect. The results of the regression analysis indicate a structural change in the relationship between ES and health. Health inequality patterns changed during the crisis, with increased deterioration in the health of unemployed, especially the long-term unemployed, and self-employed workers. Health inequalities were reduced for temporary workers. The results support the idea that the structure of the association between ES and health varies according to the economic cycle. The association between recession, ES, and health would be directly related to the specific characteristics of the economic and employment contexts under study. In the Spanish case, labor market segmentation processes based on numerical flexibility—a key feature of the Mediterranean Variety of Capitalism—may explain the results obtained.

Two types of health reforms in Latin America are analysed: one based on insurance and service commodification and the one referred to the unified public systems of progressive governments. Health insurance with explicit service packages has not fulfilled their purposes of universal coverage, equal access to necessary health services and improvement of health conditions but has opened health as a field of profit making for insurance companies and private health providers. The national health services as a state obligation have developed territorialized health services and widened substantially timely access to the majority of the population. The adoption of an integrated and wide social policy has an impact on population well fare. It faces some problems derived from the old health systems and the power of the insurance and medical complex.

The aim of this thesis was to investigate, using the real-time test case of the 2014 Commonwealth Games, whether the realist synthesis methodology could contribute to the making of health policy in a meaningful way. This was done by looking at two distinct research questions: first, whether realist synthesis could contribute new insights to the health policymaking process, and second, whether the 2014 Commonwealth Games volunteer programme was likely to have any significant, measurable, impact on health inequalities experienced by large sections of the host population. The 2014 Commonwealth Games legacy laid out ambitious plans for the event, in which it was anticipated that it would provide explicit opportunities to impact positively on health inequalities. By using realist synthesis to unpick the theories underpinning the volunteer programme, the review identifies the population subgroups for whom the programme was likely to be successful, how this could be achieved and in what contexts. In answer to the first research question, the review found that while realist methods were able to provide a more nuanced exposition of the impacts of the Games volunteer programme on health inequalities than previous traditional reviews had been able to provide, there were several drawbacks to using the method. It was found to be resource-intensive and complex, encouraging the exploration of a much wider set of literatures at the expense of an in-depth grasp of the complexities of those literatures. In answer to the second research question, the review found that the Games were, if anything, likely to exacerbate health inequalities because the programme was designed in such a way that individuals recruited to it were most likely to be those in least need of the additional mental and physical health benefits that Games volunteering was designed to provide. The following thesis details the approach taken to investigate both the realist approach to evidence synthesis and the likelihood that the 2014 Games volunteer programme would yield the expected results.

Recent health policy in England has demanded greater involvement of patients and the public in the commissioning of health and social care services. Public involvement is seen as a means of driving up service quality, reducing health inequalities and achieving value in commissioning decisions. This paper presents a summary and analysis of the forms that public involvement in commissioning are to take, along with empirical analysis from a qualitative study of service-user involvement. It is argued that the diversity of constituencies covered by the notion of ‘public involvement’, and the breadth of aims that public involvement is expected to achieve, require careful disaggregation. Public involvement in commissioning may encompass a variety of interest groups, whose inputs may include population needs assessment, evaluation of service quality, advocacy of the interests of a particular patient group or service, or a combination of all of these. Each of these roles may be legitimate, but there are significant tensions between them. The extent to which the structures for public involvement proposed recognize these possible tensions is arguably limited. Notably, new Local Involvement Networks (LINks), which will feed into commissioning decisions, are set as the arbiters of these different interests, a demanding role which will require considerable skill, tenacity and robustness if it is to be fulfilled effectively.

El concepto de actividad física es concebido de diferentes formas. Mostrando que existen varios factores que afectan de manera directa e indirecta la percepción que los sujetos construyen entorno a él, generando así una aproximación a diferentes definiciones de la actividad física desde varias perspectivas y dimensiones, donde predomina una noción netamente biológica. Este estudio pretende analizar, como desde las clases sociales se concibe la actividad física en sus conceptos y prácticas considerando los modelos de determinantes y determinación social para la salud. Con fin de comprender como los autores de la literatura científica conciben la actividad física y la relación con las clases sociales, desde una perspectiva teórica de los determinantes sociales de la salud y la teoría de la determinación social, se realizó una revisión documental y análisis de contenido de los conceptos y prácticas de la actividad física que se han considerado en los últimos 10 años. Para ello se seleccionaron las bases de datos PubMed y BVS (Biblioteca Virtual de Salud) por sus énfasis en publicaciones de salud mundialmente. Mostrando que la actividad física es concebida dominantemente desde una perspectiva biológica que ejerce una mirada reduccionista. Las relaciones entre actividad física y las clases sociales están claramente establecidas, sin embargo, estas relaciones pueden discrepar teniendo en cuenta el concepto de clase social, el contexto y la orientación de los autores y las poblaciones objetos de estudio. Obteniendo como resultado que los estudios documentados, revisados y analizados muestran una clara tendencia al modelo de determinantes; no obstante, algunos estudios en sus análisis se orientan hacia el modelo de determinación social. En cuanto al concepto de clases sociales los autores consideran una combinación de factores culturales y económicos sin atreverse a adoptar un concepto específico.

Os objetivos do estudo foram: estimar as prevalências de doenças crônicas na população brasileira em 2008, comparando-as com as de 2003; avaliar o impacto da doença crônica no uso de serviços e nas restrições das atividades; e, analisar os diferenciais nas prevalências de doenças crônicas específicas, segundo nível de escolaridade e filiação a plano privado de saúde. Os dados foram obtidos do suplemento saúde das PNAD-2008 e 2003. As análises (prevalências e razões de prevalências brutas e ajustadas) foram feitas com o aplicativo Stata 11. A prevalência de ter ao menos uma doença crônica foi mais elevada em: idosos, mulheres, cor/raça preta ou indígena, menor escolaridade, migrantes, moradores em áreas urbanas e na região Sul do país. As condições crônicas mais prevalentes foram: hipertensão, doença de coluna, artrite e depressão. Houve, entre 2003 e 2008, aumento da prevalência de diabetes, hipertensão, câncer e cirrose, e redução de insuficiência renal crônica e tuberculose. A maioria das doenças estudadas foram mais prevalentes nos segmentos de menor escolaridade e sem plano de saúde. As maiores diferenças entre os segmentos sociais foram observadas nas prevalências de cirrose, insuficiência renal crônica, tuberculose e artrite/reumatismo.

OBJETIVO: Descrever a distribuição do coeficiente de mortalidade infantil e seus componentes no município do Embu, São Paulo, no período de 1995 a 1998, segundo os estratos de condições de vida. MÉTODOS: Estudo descritivo com análise por conglomerados,dos 135 setores censitários do município de Embu, agrupados em quatro estratos de condições de vida: estrato 1, com melhores condições de moradia, renda e escolaridade; estratos 2 e 3, intermediários; estrato 4, no qual todas as moradias eram aglomerados subnormais ou favelas. Foram calculados os coeficientes de mortalidade infantil, neonatal e pós-neonatal, a proporção entre óbitos neonatais e pós-neonatais, o risco atribuível populacional e mortalidade proporcional por causas, para os anos de 1995 a 1998, segundo os quatro estratos de condições de vida estabelecidos. RESULTADOS: O estrato 4 apresentou maiores coeficientes de mortalidade infantil e risco atribuível populacional em relação aos estratos intermediários, em todos os anos do estudo. Esse estrato apresentou, também, as menores proporções entre mortalidade neonatal e pós-neonatal. O risco atribuível populacional no estrato 4 foi maior que os demais estratos para as afecções perinatais (159,4), doenças respiratórias (271,4) e doenças infecciosas (415,6). Identificaram-se dados demográficos semelhantes em áreas próximas aos limites geograficamente constituídos pelo estudo e heterogeneidade de eventos num mesmo território. CONCLUSÕES: Identificou-se uma relação entre desigualdades sociais e mortalidade infantil, segundo os critérios de condições de vida estabelecidos para este estudo, entretanto, não houve distribuição homogênea nos quatro estratos populacionais, revelando dificuldades em utilizá-los como parâmetros para desigualdades sociais em grandes centros urbanos.

A participação social no Brasil evoluiu de movimento operário e de sindicatos, culminando na institucionalização através de Conselhos. Na área da saúde, foi legalizada pela Lei 8142/90. O objetivo deste estudo é conhecer a prática do controle social exercida em Conselhos de Unidades e sua influência nas políticas de saúde do município de Campo Grande, MS. Foram feitos cinco estudos de caso, tendo como fonte principal as atas de reuniões e como referencial de análise um documento do Ilpes/Claps (1975). Os Conselhos organizam-se em plenário, com coordenador, secretário, composição hoje paritária, representatividade reduzida e periodicidade mensal. O processo decisório contempla principalmente elementos técnico-administrativos e técnico-operacionais. No período 1998-2002, o controle social fortaleceu-se por encaminhamentos mais concretos, mas a capacidade de deliberação precisa ser fortalecida por uma capacitação que inclua elementos técnicos, políticos e administrativos, representatividade, fortalecimento da cidadania, divulgação intensa das atividades dos Conselhos, inclusive na mídia, maior mobilização social e articulação entre os vários Conselhos e instâncias municipais que fazem interface com o setor de saúde.

Este artigo versa sobre contribuições da teoria social a um estudo sobre o risco à saúde humana e ao ambiente, desenvolvido entre 2003 e 2005 em área urbana contaminada, localizada no bairro Vila Carioca, no sudeste do município de São Paulo, Brasil. Resíduos perigosos provenientes de processo produtivo do setor químico, dispostos inadequadamente na localidade ao longo do tempo, resultaram em contaminação ambiental, cujos efeitos representam riscos à saúde da população local. A investigação foi realizada com o objetivo de identificar interpretações sociais sobre o conceito de situação de risco, condizentes com concepções incorporadoras da dimensão social do risco e voltadas à melhoria das condições de saúde ambiental. Utilizou-se metodologia qualitativa de pesquisa, alicerçada na teoria social, e instrumentos variados de coleta de dados. Os resultados apontaram interpretações sociais diferenciadas sobre o conceito de situação de risco, sugerindo diversidade de concepções entre a população pesquisada a respeito dos problemas ambientais e de saúde que os atingiam. Neste artigo, apresentam-se fundamentos do enfoque do risco, na teoria social e na obra de Ulrick Beck sobre sociedade de risco, a fim de conferir suporte teórico à interpretação dos dados coletados em campo. Tais contribuições da teoria social, em contraponto com abordagens multidisciplinares e ecossistêmicas em saúde e ambiente, são discutidas como forma de incorporar a diversidade de interpretações sociais expressadas pela população, no sentido de favorecer a inter-relação entre portadores de risco e decisões políticas locais sobre a questão do risco, ampliar o escopo dos fenômenos observados e propiciar a busca de melhores condições de saúde dos indivíduos e da qualidade do seu ambiente
