The current socio-economic situation has brought a need to look for alternative ways to get energy that allow reducing the high dependence on fossil fuel sources while deflect from the climate change arising from the result of the use of these energy resources. Renewable sources of energy, low and medium temperature appear as high potential of energy resources, which have a major influence on the way of life of the people to enable decentralized energy production. In Brazil, in particular, have also the need to decentralize the energy grid, currently focused on energy from water source. The current water crisis, exemplifies the urgency of betting on other energy sources, as a way to help in emergency situations such as the current one. Therefore, this study evaluates the possibility of using biomass as a heat source in a Rankine Cycle Organic where instead of water; it uses thermal fluid as working fluid, was compared the urban areas of the city of Guaratinguetá with the urban area of the metropolitan region of São Paulo. Thus, it was established two scenarios, so it was possible to establish the cycle to be used

Background: Percutaneous coronary intervention (PCI) has increased as the initial revascularization strategy in chronic coronary artery disease. Consequently, more patients undergoing coronary artery bypass grafting (CABG) have history of coronary stent. Objective: Evaluate the impact of previous PCI on in-hospital mortality after CABG in patients with multivessel coronary artery disease. Methods: Between May/2007 and June/2009, 1099 consecutive patients underwent CABG on cardiopulmonary bypass. Patients with no PCI (n=938, 85.3%) were compared with patients with previous PCI (n=161, 14.6%). Logistic regression models and propensity score matching analysis were used to assess the risk-adjusted impact of previous PCI on in-hospital mortality. Results: Both groups were similar, except for the fact that patients with previous PCI were more likely to have unstable angina (16.1% x 9.9%, p=0.019). In-hospital mortality after CABG was higher in patients with previous PCI (9.3% x 5.1%, p=0.034) and it was comparable with EuroSCORE and 2000 Bernstein-Parsonnet risk score. Using multivariate logistic regression analysis, previous PCI emerged as an independent predictor of postoperative in-hospital mortality (odds ratio 1.94, 95% CI 1.02-3.68, p=0.044) as strong as diabetes (odds ratio 1.86, 95% CI 1.07-3.24, p=0.028). After computed propensity score matching based on preoperative risk factors, in-hospital mortality remained higher among patients with previous PCI (odds ratio 3.46, 95% CI 1.10-10.93, p=0.034). Conclusions: Previous PCI in patients with multivessel coronary artery disease is an independent risk factor for in-hospital mortality after CABG. This fact must be considered when PCI is indicated as initial alternative in patients with more severe coronary artery disease. (Arq Bras Cardiol 2012;99(1):586-595)

Background: Although the release of cardiac biomarkers after percutaneous (PCI) or surgical revascularization (CABG) is common, its prognostic significance is not known. Questions remain about the mechanisms and degree of correlation between the release, the volume of myocardial tissue loss, and the long-term significance. Delayed-enhancement of cardiac magnetic resonance (CMR) consistently quantifies areas of irreversible myocardial injury. To investigate the quantitative relationship between irreversible injury and cardiac biomarkers, we will evaluate the extent of irreversible injury in patients undergoing PCI and CABG and relate it to postprocedural modifications in cardiac biomarkers and long-term prognosis. Methods/Design: The study will include 150 patients with multivessel coronary artery disease (CAD) with left ventricle ejection fraction (LVEF) and a formal indication for CABG; 50 patients will undergo CABG with cardiopulmonary bypass (CPB); 50 patients with the same arterial and ventricular condition indicated for myocardial revascularization will undergo CABG without CPB; and another 50 patients with CAD and preserved ventricular function will undergo PCI using stents. All patients will undergo CMR before and after surgery or PCI. We will also evaluate the release of cardiac markers of necrosis immediately before and after each procedure. Primary outcome considered is overall death in a 5-year follow-up. Secondary outcomes are levels of CK-MB isoenzyme and I-Troponin in association with presence of myocardial fibrosis and systolic left ventricle dysfunction assessed by CMR. Discussion: The MASS-V Trial aims to establish reliable values for parameters of enzyme markers of myocardial necrosis in the absence of manifest myocardial infarction after mechanical interventions. The establishments of these indices have diagnostic value and clinical prognosis and therefore require relevant and different therapeutic measures. In daily practice, the inappropriate use of these necrosis markers has led to misdiagnosis and therefore wrong treatment. The appearance of a more sensitive tool such as CMR provides an unprecedented diagnostic accuracy of myocardial damage when correlated with necrosis enzyme markers. We aim to correlate laboratory data with imaging, thereby establishing more refined data on the presence or absence of irreversible myocardial injury after the procedure, either percutaneous or surgical, and this, with or without the use of cardiopulmonary bypass.

Background The optimal revascularization strategy for diabetic patients with multivessel coronary artery disease (MVD) remains uncertain for lack of an adequately powered, randomized trial. The FREEDOM trial was designed to compare contemporary coronary artery bypass grafting (CABG) to percutaneous coronary intervention (PCI) with drug-eluting stents in diabetic patients with MVD against a background of optimal medical therapy. Methods A total of 1,900 diabetic participants with MVD were randomized to PCI or CABG worldwide from April 2005 to March 2010. FREEDOM is a superiority trial with a mean follow-up of 4.37 years (minimum 2 years) and 80% power to detect a 27.0% relative reduction. We present the baseline characteristics of patients screened and randomized, and provide a comparison with other MVD trials involving diabetic patients. Results The randomized cohort was 63.1 +/- 9.1 years old and 29% female, with a median diabetes duration of 10.2 +/- 8.9 years. Most (83%) had 3-vessel disease and on average took 5.5 +/- 1.7 vascular medications, with 32% on insulin therapy. Nearly all had hypertension and/or dyslipidemia, and 26% had a prior myocardial infarction. Mean hemoglobin A1c was 7.8 +/- 1.7 mg/dL, 29% had low-density lipoprotein <70 mg/dL, and mean systolic blood pressure was 134 +/- 20 mm Hg. The mean SYNTAX score was 26.2 with a symmetric distribution. FREEDOM trial participants have baseline characteristics similar to those of contemporary multivessel and diabetes trial cohorts. Conclusions The FREEDOM trial has successfully recruited a high-risk diabetic MVD cohort. Follow-up efforts include aggressive monitoring to optimize background risk factor control. Background-The importance of complete revascularization remains unclear and contradictory. This current investigation compares the effect of complete revascularization on 10-year survival of patients with stable multivessel coronary artery disease (CAD) who were randomly assigned to percutaneous coronary intervention (PCI) or coronary artery bypass graft (CABG). Methods and Results-This is a post hoc analysis of the Second Medicine, Angioplasty, or Surgery Study (MASS II), which is a randomized trial comparing treatments in patients with stable multivessel CAD, and preserved systolic ventricular function. We analyzed patients who underwent surgery (CABG) or stent angioplasty (PCI). The survival free of overall mortality of patients who underwent complete (CR) or incomplete revascularization (IR) was compared. Of the 408 patients randomly assigned to mechanical revascularization, 390 patients (95.6%) underwent the assigned treatment; complete revascularization was achieved in 224 patients (57.4%), 63.8% of those in the CABG group and 36.2% in the PCI group (P = 0.001). The IR group had more prior myocardial infarction than the CR group (56.2% X 39.2%, P = 0.01). During a 10-year follow-up, the survival free of cardiovascular mortality was significantly different among patients in the 2 groups (CR, 90.6% versus IR, 84.4%; P = 0.04). This was mainly driven by an increased cardiovascular specific mortality in individuals with incomplete revascularization submitted to PCI (P = 0.05). Conclusions-Our study suggests that in 10-year follow-up, CR compared with IR was associated with reduced cardiovascular mortality, especially due to a higher increase in cardiovascular-specific mortality in individuals submitted to PCI.

Background-The Second Medicine, Angioplasty, or Surgery Study (MASS II) included patients with multivessel coronary artery disease and normal systolic ventricular function. Patients underwent coronary artery bypass graft surgery (CABG, n = 203), percutaneous coronary intervention (PCI, n = 205), or medical treatment alone (MT, n = 203). This investigation compares the economic outcome at 5-year follow-up of the 3 therapeutic strategies. Methods and Results-We analyzed cumulative costs during a 5-year follow-up period. To analyze the cost-effectiveness, adjustment was made on the cumulative costs for average event-free time and angina-free proportion. Respectively, for event-free survival and event plus angina-free survival, MT presented 3.79 quality-adjusted life-years and 2.07 quality-adjusted life-years; PCI presented 3.59 and 2.77 quality-adjusted life-years; and CABG demonstrated 4.4 and 2.81 quality-adjusted life-years. The event-free costs were $9071.00 for MT; $19 967.00 for PCI; and $18 263.00 for CABG. The paired comparison of the event-free costs showed that there was a significant difference favoring MT versus PCI (P<0.01) and versus CABG (P<0.01) and CABG versus PCI (P<0.01). The event-free plus angina-free costs were $16 553.00, $25 831.00, and $24 614.00, respectively. The paired comparison of the event-free plus angina-free costs showed that there was a significant difference favoring MT versus PCI (P=0.04), and versus CABG (P<0.001); there was no difference between CABG and PCI (P>0.05). Conclusions-In the long-term economic analysis, for the prevention of a composite primary end point, MT was more cost effective than CABG, and CABG was more cost-effective than PCI.

BACKGROUND In some randomized trials comparing revascularization strategies for patients with diabetes, coronary-artery bypass grafting (CABG) has had a better outcome than percutaneous coronary intervention (PCI). We sought to discover whether aggressive medical therapy and the use of drug-eluting stents could alter the revascularization approach for patients with diabetes and multivessel coronary artery disease. METHODS In this randomized trial, we assigned patients with diabetes and multivessel coronary artery disease to undergo either PCI with drug-eluting stents or CABG. The patients were followed for a minimum of 2 years (median among survivors, 3.8 years). All patients were prescribed currently recommended medical therapies for the control of low-density lipoprotein cholesterol, systolic blood pressure, and glycated hemoglobin. The primary outcome measure was a composite of death from any cause, nonfatal myocardial infarction, or nonfatal stroke. RESULTS From 2005 through 2010, we enrolled 1900 patients at 140 international centers. The patients' mean age was 63.1 +/- 9.1 years, 29% were women, and 83% had three-vessel disease. The primary outcome occurred more frequently in the PCI group (P=0.005), with 5-year rates of 26.6% in the PCI group and 18.7% in the CABG group. The benefit of CABG was driven by differences in rates of both myocardial infarction (P<0.001) and death from any cause (P=0.049). Stroke was more frequent in the CABG group, with 5-year rates of 2.4% in the PCI group and 5.2% in the CABG group (P=0.03). CONCLUSIONS For patients with diabetes and advanced coronary artery disease, CABG was superior to PCI in that it significantly reduced rates of death and myocardial infarction, with a higher rate of stroke. Objectives Predictors of adverse outcomes following myocardial infarction (MI) are well established; however, little is known about what predicts enzymatically estimated infarct size in patients with acute ST-elevation MI. The Complement And Reduction of INfarct size after Angioplasty or Lytics trials of pexelizumab used creatine kinase (CK)-MB area under the curve to determine infarct size in patients treated with primary percutaneous coronary intervention (PCI) or fibrinolysis. Methods Prediction of infarct size was carried out by measuring CK-MB area under the curve in patients with ST-segment elevation MI treated with reperfusion therapy from January 2000 to April 2002. Infarct size was calculated in 1622 patients (PCI=817; fibrinolysis=805). Logistic regression was used to examine the relationship between baseline demographics, total ST-segment elevation, index angiographic findings (PCI group), and binary outcome of CK-MB area under the curve greater than 3000 ng/ml. Results Large infarcts occurred in 63% (515) of the PCI group and 69% (554) of the fibrinolysis group. Independent predictors of large infarcts differed depending on mode of reperfusion. In PCI, male sex, no prior coronary revascularization and diabetes, decreased systolic blood pressure, sum of ST-segment elevation, total (angiographic) occlusion, and nonright coronary artery culprit artery were independent predictors of larger infarcts (C index=0.73). In fibrinolysis, younger age, decreased heart rate, white race, no history of arrhythmia, increased time to fibrinolytic therapy in patients treated up to 2 h after symptom onset, and sum of ST-segment elevation were independently associated with a larger infarct size (C index=0.68). Conclusion Clinical and patient data can be used to predict larger infarcts on the basis of CK-MB quantification. These models may be helpful in designing future trials and in guiding the use of novel pharmacotherapies aimed at limiting infarct size in clinical practice. Objetivo: avaliar os efeitos de precondicionamento isquêmico remoto (PCI-R) no modelo de transplante de intestino delgado fetal. Métodos: foram constituídos dois grupos: transplante isogênico (Iso, camundongos C57BL/6, n=24) e transplante alogênico (Alo, camundongos BALB/c, n=24). Em cada grupo, distribuíram-se os animais com e sem PCI-R, que foi realizado por oclusão da artéria femoral esquerda da fêmea prenhe durante 10 minutos, seguida por tempo igual de reperfusão. O imunossupressor utilizado foi Tacrolimo (Fk, 5 mg/kg/dia v.o.). Ao final obteve-se os seguintes subgrupos: Alo-Tx, Alo-Pci, Alo-Fk, Alo-Pci-Fk, Iso-Tx, Iso-Pci, Iso-Fk e Iso-Pci-Fk. O enxerto foi transplantado no espaço entre o músculo reto-abdominal e pré- peritoneal dos receptores a meio centímetro do apêndice xifóide, à esquerda da linha mediana. Após o sétimo dia de seguimento, o enxerto foi removido, fixado e embebido em parafina para avaliação histomorfológica (desenvolvimento e rejeição) e análise imunohistoquímica (anti-PCNA e anti-caspase-3 clivada). Os dados foram analisados usando ANOVA e testes complementares e foi considerado significante quando p <0.05. Resultados: A avaliação do desenvolvimento do enxerto no grupo de Iso mostrou que o PCI-R reduziu o desenvolvimento comparado com Iso-Tx (5,2±0,4 vs 9,0±0,8), o Fk e sua associação com PCI-R aumentaram o desenvolvimento do enxerto comparado com PCI-R (11,2±0,7 e 10,2±0,8, respectivamente). No grupo Alo, o Fk e/ou sua associação com PCI-R aumentaram o desenvolvimento comparado com Alo-Tx e Alo com PCI-R (6,0±0,8, 9,0±1,2, 0,0±0,0, 0,5±0,3, respectivamente). A expressão de PCNA foi maior no grupo ISO em animais tratados com Fk e PCI-R comparados a outros grupos (12,2±0,8 vs Tx: 8,8±0,9, PCI-R: 8,0±0,4 e Fk: 9,0±0,6). No grupo Alo, a expressão de PCNA não diferiu entre grupos. A rejeição do enxerto foi menor nos grupos tratados com PCI-R (-18%), Fk (- 68%) ou ambos (-61%) comparados com Alo-Tx. A expressão de caspase-3 clivada foi menor no grupo Iso em animais tratados com associação de PCI-R e Fk (6,2 ±0,9 vs Tx: 8,6±0,5; PCI-R: 5,8 ±0,9 e Fk: 6,0 ±0,3). Conclusão: O PCIR mostrou efeito benéfico sobre a lesão de isquemia e reperfusão do enxerto intestinal fetal nos transplantes isogênico e alogênico, aumentando o número de células caliciformes e a proliferação celular. No transplante alogênico, aumentou o desenvolvimento do enxerto, diminuiu o grau de rejeição aguda na ausência de imunossupressão, porém não apresentou efeito sinérgico com o imunossupressor. No transplante isogênico houve diminuição do grau de desenvolvimento do enxerto, porém foi efetivo na redução da apoptose.

O objetivo foi estudar os efeitos do precondicionamento isquêmico e da N-acetilcisteína no clampeamento da tríade portal comparado com o clampeamento vascular excluindo a via biliar. Foram utilizados oitenta ratos EPM-1 Wistar distribuídos aleatoriamente em 2 grupos de 40 animais. Estes se distinguiram pela inclusão (CVB=1) ou não (SVB=2) do ducto biliar no clampeamento e foram redistribuídos em subgrupos de 10. IR1: 20 minutos após a celiotomia, o pedículo contendo os elementos vasculares e o ducto biliar para os lobos mediano e lateral esquerdo do fígado foi clampeado por 40 minutos, seguido de 30 minutos de reperfusão; PCI1: 10 minutos de isquemia e 10 minutos de reperfusão. Após o PCI, seguiu-se o mesmo procedimento usado para IR1; NAC1: (150mg.Kg-1 ), administrada 15 minutos antes do período isquêmico e 5 minutos antes da reperfusão; S1: dissecção e clampeamento exclusivo do ducto biliar durante o período de isquemia. Nos subgrupos IR2, PCI2 e NAC2, o clampeamento excluiu a via biliar; S2: dissecção e observação por 90min. Foram colhidas amostras de sangue para a dosagem dos níveis enzimáticos e fragmento do fígado isquêmico para coloração HE. Na análise estatística dos resultados foram utilizados testes não paramétricos e o nível de rejeição da hipótese de nulidade foi fixado em 5%. A lesão de IR hepática foi menos grave nos animais do grupo de clampeamento seletivo incluindo o ducto biliar, com AST (766 vs 1380) e ALT (840 vs 1576); PCI protegeu o fígado da IR nos animais com clampeamento seletivo da tríade portal com relação à AST (421 vs 1131) e ALT (315 vs 1085). Na avaliação morfológica, o PCI inibiu a ocorrência de esteatose microvesicular e núcleos picnóticos (0% de lesão moderada ou intensa) e a NAC protegeu parcialmente 17% e 50% de lesão moderada ou intensa para CVB e SVB, respectivamente. O clampeamento seletivo da tríade portal resulta em lesão de IR do fígado menos grave, quando comparado à exclusão do ducto biliar. O PCI protege o fígado da lesão de IR. A NAC mostra um efeito de proteção parcial, reduzindo as alterações parenquimatosas, mas não os níveis de aminotransferases.

[ES] Este artículo estudia la evolución de la historiografía obrera italiana, que surge del marxismo y vinculada al PCI. Esta historiografía militante se caracterizó por la recopilación de fuentes documentales y el estudio del movimiento organizado, al tiempo que se debatía sobre cuestiones socio-económicas. En una segunda fase, a partir de 1956, la historiografía obrera italiana avanzó en pos de un mayor rigor científico, propósito que se concretó en una mayor apertura de los archivos y en la ampliación de la investigación a temáticas políticamente sensibles. Posteriormente, al calor de los cambios experimentados por el país en los años sesenta y setenta, y favorecida por los contactos con historiografías foráneas, la historiografía obrera italiana perdió progresivamente su carácter militante y se renovó con nuevas interrogantes y metodologías. [EN] This article looks at the evolution of labour historiography in Italy, a movement that emerged from Marxism and was linked to the PCI. This militant historiography was characterized by the compilation of documentary sources and the study of the organised movement, while general socio-economic issues were under discussion. The second phase, from 1956 onward, saw Italian labour historiography making advances in achieving greater thoroughness, involving the opening of more archives and expanding the research to politically sensitive issues. Subsequently, in the light of changes experienced by the country in the 1960s and 1970s, and thanks to foreign historiographies, the Italian labour historiography lost its militant nature in a progressive way and developed new queries and methodologies.

La tesi è una ricerca di storia politica che affianca due diverse “storie” di centro-sinistra, quella nazionale e quella che vide protagonista la Democrazia Cristiana del Trentino. Lo studio analizza i fatti attraverso il filtro delle DC come se quello trentino e quello nazionale fossero due partiti, per poi tentare di capire ciò che accadeva alla loro sinistra alla ricerca dei diversi pesi e dei differenti equilibri che al centro e alla periferia si manifestavano nei rapporti con il PSI e con il PCI, e per osservare le reazioni della Chiesa così da valutare se le gerarchie romane e quelle trentine interagirono in modo differente sugli sviluppi delle rispettive esperienze politiche di quegli anni. Il testo è organizzato in quattro capitoli. Il primo e il secondo (speculari e dedicati allo stesso lustro: 1955-1960) rappresentano un confronto tra i differenti iter d’avvicinamento al centro-sinistra che la politica nazionale e quella trentina sperimentarono nella seconda metà degli anni Cinquanta. Nel terzo capitolo (1960-1964) e nel quarto (1964-1968) le vicende nazionali e quelle locali sono invece raccontate in modo intrecciato, ripercorrendo le diverse fasi dell’alleanza tra Democrazia Cristiana e Partito Socialista, e nel contempo dando conto della trasformazione del Trentino da una realtà di tipo agricolo ad una di tipo industriale, del passaggio da una comunità di tipo cattolico tradizionale ad una che si accinge a vivere in un contesto secolarizzato, e da una società che si autopercepisce come periferica ad una che ospita una delle contestazioni studentesche più peculiari, incisive e note d’Italia.

La ricerca affronta il rapporto tra il Partito comunista italiano e le organizzazioni della sinistra extraparlamentare nate nel biennio 1968-1969. Sulla base di documentazione d’archivio e fonti a stampa, vengono ricostruite ed analizzate le relazioni tra questi due soggetti nel periodo compreso tra la fine degli anni Sessanta e la metà del decennio successivo, quando i principali gruppi politici della sinistra extraparlamentare si dotarono di una struttura organizzativa più stabile che segnava una discontinuità con l’esperienza precedente. Nel corso della prima metà degli anni Settanta, i rapporti tra il PCI e queste organizzazioni furono complessi e talvolta contraddittori. Il conflitto si consumò prevalentemente sulla reciproca pretesa di possedere l’esclusiva rappresentanza politica del fermento sociale che attraversava il paese in quegli anni: il PCI rappresentando se stesso come l’unica forza politica capace di mediare tra movimenti sociali e istituzioni; i gruppi della sinistra parlamentare come «avanguardie» di un irrealizzabile progetto «rivoluzionario».

Anche se la politica editoriale comunista rappresenta un campo di indagine fondamentale nella ricerca sul Pci, la sua attività editoriale è caduta in un oblio storico. Assumendo il libro come supporto materiale e veicolo della cultura politica comunista, e la casa editrice come canale di socializzazione, questa ricerca s’interroga sui suoi processi di costruzione e di diffusione. La ricerca si muove in due direzioni. Nel primo capitolo si è tentato di dare conto delle ragioni metodologiche dell’indagine e della messa a punto delle ipotesi di ricerca sul “partito editore”, raccogliendo alcune sfide poste alla storia politica da altri ambiti disciplinari, come la sociologia e la scienza politica, che rappresentano una vena feconda per la nostra indagine. La seconda direzione, empirica, ha riguardato la ricognizione delle fonti e degli strumenti di analisi per ricostruire le vicende del “partito editore” dal 1944 al 1956. La suddivisione della ricerca in due parti – 1944-1947 e 1947-1956 – segue a grandi linee la periodizzazione classica individuata dalla storiografia sulla politica culturale del Pci, ed è costruita su quattro fratture storiche – il 1944, con la “svolta di Salerno”; il 1947, con la “svolta cominformista”; il 1953, con la morte di Stalin e il disgelo; il 1956, con il XX Congresso e i fatti d’Ungheria – che sono risultate significative anche per la nostra ricerca sull’editoria comunista. Infine, il presente lavoro si basa su tre livelli di analisi: l’individuazione dei meccanismi di decisione politica e dell’organizzazione assunta dall’editoria comunista, esaminando gli scopi e i mutamenti organizzativi interni al partito per capire come i mutamenti strategici e tattici si sono riflessi sull’attività editoriale; la ricostruzione della produzione editoriale comunista; infine, l’identificazione dei processi di distribuzione e delle politiche per la lettura promosse dal Pci.
