The case of a 41-year-old alcoholic patient who presented with massive upper gastrointestinal bleeding is reported. The diagnosis was established rapidly by endoscopy which revealed bleeding through the papilla of Vater. Selective angiography of the superior mesenteric artery opacified the pancreatic duct as well as the duodenum. CT scan showed signs of chronic pancreatitis. Because of the recurrence of bleeding and shock, an urgent operation was necessary and a Whipple procedure was performed. Recovery was complete. The clinical presentation, the etiology, the diagnostic modalities and the treatment of this particular condition are discussed. Usually, hemorrhage through the pancreatic duct presents as repeated episodes of upper gastrointestinal bleeding with no source found at endoscopy. The presence of chronic pancreatitis or of epigastric pain during bleeding should suggest the diagnosis. Upper gastrointestinal endoscopy and angiography are the principal diagnostic tools. Definitive treatment requires surgery, and resection in most of the cases.

Aims:We searched for very high energy (VHE) γ-ray emission from the supernova remnant Cassiopeia A Methods: The shell-type supernova remnant Cassiopeia A was observed with the 17 m MAGIC telescope between July 2006 and January 2007 for a total time of 47 h. Results: The source was detected above an energy of 250 GeV with a significance of 5.2σ and a photon flux above 1 TeV of (7.3 ± 0.7_stat ± 2.2_sys) × 10-13 cm-2s-1. The photon spectrum is compatible with a power law dN/dE ∝ E-Γ with a photon index Γ = 2.3 ± 0.2_stat ± 0.2_sys. The source is point-like within the angular resolution of the telescope.

L'absence d'une définition claire de la dangerosité et de la violence en psychiatrie, ainsi que le constat d'échec général des outils de prédiction ne doivent pas interrompre le développement du management des risques dans l'institution. Nous proposons l' utilisation d' une échelle d'observation des comportements agressifs qui s'inspire largement du modèle de l'OAS (Overt Agression Scale ) développée par Yudofsky et ses collaborateurs. Il s'agit d'améliorer la qualité de l'observation infirmière et, ce faisant, d'utiliser l'OAS comme un médium pluridisciplinaire d'une part, et comme outil de communication avec le patient concerné, d'autre part. Entre banalisation et stigmatisation, nous restons attentifs au besoin de sécurité du personnel soignant et aux risques d'exclusion des patients "dérangeants".
