Factor V Leiden (FV Leiden) is the most common inherited thrombophilia in Caucasians increasing the risk for venous thrombosis. Its prevalence in Finland is 2-3%. FV Leiden has also been associated with several pregnancy complications. However, the importance of FV Leiden as their risk factor is unclear. The aim of the study was to assess FV Leiden as a risk factor for pregnancy complications in which prothrombotic mechanisms may play a part. Specifically, the study aimed to assess the magnitude of the risk, if any, associated with FV Leiden for pregnancy-associated venous thrombosis, pre-eclampsia, unexplained stillbirth, and preterm birth. The study was conducted as a nested case-control study within a fixed cohort of 100,000 consecutive pregnant women in Finland. The study was approved by the ethics committee of the Finnish Red Cross Blood Service and by the Ministry of Social Affairs and Health. All participants gave written informed consent. Cases and controls were identified by using national registers. The diagnoses of the 100,000 women identified from the National Register of Blood Group and Blood Group Antibodies of Pregnant Women were obtained from the National Hospital Discharge Register. Participants gave blood samples for DNA tests and filled in questionnaires. The medical records of the participants were reviewed in 49 maternity hospitals in Finland. Genotyping was performed in the Finnish Genome Center. When evaluating pregnancy-associated venous thrombosis (34 cases, 641 controls), FV Leiden was associated with 11-fold risk (OR 11.6, 95% CI 3.6-33.6). When only analyzing women with first venous thrombosis, the risk was 6-fold (OR 5.8, 95% CI 1.6-21.8). The risk was increased by common risk factors, the risk being highest in women with FV Leiden and pre-pregnancy BMI over 30 kg/m2 (75-fold), and in women with FV Leiden and age over 35 years (60-fold). When evaluating pre-eclampsia (248 cases, 679 controls), FV Leiden was associated with a trend of increased risk (OR 1.7, 95% CI 0.8-3.9), but the association was not statistically significant. When evaluating unexplained stillbirth (44 cases, 776 controls), FV Leiden was associated with over 3-fold risk (OR 3.8, 95% CI 1.2-11.6). When evaluating preterm birth (324 cases, 752 controls), FV Leiden was associated with over 2-fold risk (OR 2.4, 95% CI 1.3-4.6). FV Leiden was especially associated with late preterm birth (32-36 weeks of gestation), but not with early preterm birth (< 32 weeks of gestation). The results of this large population-based study can be generalized to Finnish women with pregnancies continuing beyond first trimester, and may be applied to Caucasian women in populations with similar prevalence of FV Leiden and high standard prenatal care.

Symptomless nasopharyngeal carriage of Streptococcus pneumoniae (pneumococcus) is very common in young children. Occasionally the carriage proceeds into mild mucosal diseases, such as sinusitis or acute otitis media, or into serious life-threatening diseases, such as pneumonia, sepsis or meningitis. Each year, up to one million children less than five years of age worldwide die of invasive pneumococcal diseases (IPD). Especially in the low-income countries IPD is a leading health problem in infants; 75% of all IPD cases occur before one year of age. This stresses the need of increased protection against pneumococcus in infancy. Anti-pneumococcal antibodies form an important component in the defence against pneumococcal infection. Maternal immunisation and early infant immunisation are two possible ways by which potentially protective antibody concentrations against pneumococci could be achieved in early infancy. The aim of this thesis is to increase the knowledge of antibody mediated protection against pneumococcal disease in infants and young children. We investigated the transfer of maternal anti-pneumococcal antibodies from Filipino mothers to their infants, the persistence of the transferred antibodies in the infants, the immunogenicity of the 23-valent pneumococcal polysaccharide vaccine (PPV) in infants and the response of the children to a second dose of PPV at three years of age. We also investigated the development of antibodies to pneumococcal protein antigens in relation to culture-confirmed pneumococcal carriage in infants. Serum samples were collected from the mothers, the umbilical cords and from the infants at young age as well as at three years of age. The samples were used to determine the antibody concentrations to pneumococcal serotypes 1, 5, 6B, 14, 18C and 19F, as well as to the pneumococcal proteins PspA, PsaA, Ply, PspC, PhtD, PhtDC and LytC by the enzyme immunoassay. The findings of the present study confirm previously obtained results and add to the global knowledge of responses to PPV in young children. Immunising pregnant women with PPV provides the infants with increased concentrations of pneumococcal polysaccharide antibodies. Of the six serotypes examined, serotypes 1 and 5 were immunogenic already in infants. At three years of age, the children responded well to the second dose of PPV suggesting that maternal and early infant immunisations might not induce hyporesponsiveness to polysaccharide antigens after subsequent immunisations. The anti-protein antibody findings provide useful information for the development of pneumococcal protein vaccines. All six proteins studied were immunogenic in infancy and the development of anti-protein antibodies started early in life in relation to pneumococcal carriage.

``The goal of this study was to examine the effect of maternal iron deficiency on the developing hippocampus in order to define a developmental window for this effect, and to see whether iron deficiency causes changes in glucocorticoid levels. The study was carried out using pre-natal, post-natal, and pre + post-natal iron deficiency paradigm. Iron deficient pregnant dams and their pups displayed elevated corticosterone which, in turn, differentially affected glucocorticoid receptor (GR) expression in the CA1 and the dentate gyrus. Brain Derived Neurotrophic Factor (BDNF) was reduced in the hippocampi of pups following elevated corticosterone levels. Reduced neurogenesis at P7 was seen in pups born to iron deficient mothers, and these pups had reduced numbers of hippocampal pyramidal and granule cells as adults. Hippocampal subdivision volumes also were altered. The structural and molecular defects in the pups were correlated with radial arm maze performance; reference memory function was especially affected. Pups from dams that were iron deficient throughout pregnancy and lactation displayed the complete spectrum of defects, while pups from dams that were iron deficient only during pregnancy or during lactation displayed subsets of defects. These findings show that maternal iron deficiency is associated with altered levels of corticosterone and GR expression, and with spatial memory deficits in their pups.'' (C) 2013 Elsevier Inc. As rapid brain development occurs during the neonatal period, environmental manipulation during this period may have a significant impact on sleep and memory functions. Moreover, rapid eye movement (REM) sleep plays an important role in integrating new information with the previously stored emotional experience. Hence, the impact of early maternal separation and isolation stress (MS) during the stress hyporesponsive period (SHRP) on fear memory retention and sleep in rats were studied. The neonatal rats were subjected to maternal separation and isolation stress during postnatal days 5-7 (6 h daily/3 d). Polysomnographic recordings and differential fear conditioning was carried out in two different sets of rats aged 2 months. The neuronal replay during REM sleep was analyzed using different parameters. MS rats showed increased time in REM stage and total sleep period also increased. MS rats showed fear generalization with increased fear memory retention than normal control (NC). The detailed analysis of the local field potentials across different time periods of REM sleep showed increased theta oscillations in the hippocampus, amygdala and cortical circuits. Our findings suggest that stress during SHRP has sensitized the hippocampus amygdala cortical loops which could be due to increased release of corticosterone that generally occurs during REM sleep. These rats when subjected to fear conditioning exhibit increased fear memory and increased, fear generalization. The development of helplessness, anxiety and sleep changes in human patients, thus, could be related to the reduced thermal, tactile and social stimulation during SHRP on brain plasticity and fear memory functions. Background: Type 2 diabetes mellitus is associated with a diverse range of pathologies. The aim of the study was to determine the incidence of diabetes-related complications, the prevalence of coexistent chronic conditions and to report multimorbidity in people with type 2 diabetes living in the Basque Country. Methods: Administrative databases, in four cross sections (annually from 2007 to 2011) were consulted to analyse 149,015 individual records from patients aged >= 35 years with type 2 diabetes mellitus. The data observed were: age, sex, diabetes-related complications (annual rates of acute myocardial infarction, major amputations and avoidable hospitalisations), diabetes-related pathologies (prevalence of ischaemic heart disease, renal failure, stroke, heart failure, peripheral neuropathy, foot ulcers and diabetic retinopathy) and other unrelated pathologies (44 diseases). Results: The annual incidence for each condition progressively decreased during the four-year period: acute myocardial infarction (0.47 to 0.40%), major amputations (0.10 to 0.08%), and avoidable hospitalisations (5.85 to 5.5%). The prevalence for diabetes-related chronic pathologies was: ischaemic heart disease (11.5%), renal failure (8.4%), stroke (7.0%), heart failure (4.3%), peripheral neuropathy (1.3%), foot ulcers (2.0%) and diabetic retinopathy (7.2%). The prevalence of multimorbidity was 90.4%. The highest prevalence for other chronic conditions was 73.7% for hypertension, 13.8% for dyspepsia and 12.7% for anxiety. Conclusions: In the type 2 diabetes mellitus population living in the Basque Country, incidence rates of diabetes complications are not as high as in other places. However, they present a high prevalence of diabetes related and unrelated diseases. Multimorbidity is very common in this group, and is a factor to be taken into account to ensure correct clinical management.

Eventos ou estímulos no início da vida podem afetar o desenvolvimento do indivíduo; dentre esses o tabagismo materno. A exposição materna isolada à nicotina, principal componente do cigarro, causa na prole alterações metabólicas, em curto e longo prazo, como aumento da adiposidade, resistência à leptina, e disfunção tireoideana e adrenal. Entretanto é sabido que na fumaça de cigarro estão presentes outros componentes com potenciais efeitos tóxicos. Assim propomos comparar o efeito de duas formas de exposição neonatal à fumaça do cigarro sobre o perfil endócrino-metabólico da prole em curto e longo prazo. Para isso, no 3 dia após o nascimento, ratos lactentes foram submetidos a dois modelos: Modelo I (exposição pelo leite materno), ninhadas separadas em: exposição à fumaça (EF; n=8) lactantes expostas à fumaça de cigarros 2R1F (1,7 mg de nicotina/cigarro por 1h, 4 vezes ao dia), separadas de suas proles e grupo controle (C; n=8), onde as mães foram separadas de suas proles e expostas ao ar filtrado; Modelo II (exposição direta à fumaça), ninhadas separadas em: exposição à fumaça (EF; n=8) mães e proles expostas à fumaça de cigarros 2R1F e controle (C; n=8) mães e proles expostas ao ar filtrado. A exposição ao tabaco ocorreu até o desmame. Mães sacrificadas aos desmame e proles aos desmame e aos 180 dias de idade. As mães lactantes expostas à fumaça (EF) apresentaram hipoleptinemia (-46%), hiperprolactinemia (+50%), hipoinsulinemia (-40%) e diminuição de triglicérides (-53%). Quanto a composição bioquímica do leite, as lactantes EF mostraram aumento de lactose (+52%) e triglicérides (+78%). No modelo I, as proles EF apresentaram ao desmame: diminuição da gordura corporal total (-24%), aumento de proteína corporal total (+17%), diminuição da glicemia (-11%), hiperinsulinemia (+28%), hipocorticosteronemia (-40%) e aumento de triglicérides (+34%). Quando adultas, as proles EF apresentaram somente alteração da função adrenal onde observou-se menor conteúdo de catecolaminas (-50%) e da expressão de tirosina hidroxilase na medula adrenal (-56%). No modelo II, as proles EF aos 21 dias exibiram diminuição da MC (-7%), do comprimento nasoanal (-5%), da gordura retroperitoneal (-59%), da área dos adipócitos viscerais (-60%) com maior área dos adipócitos subcutâneos (+95%), aumento do T4 (+59), da corticosterona (+60%), do conteúdo adrenal de catecolaminas (+58%) e hipoinsulinemia (-29%). Aos 180 dias, as proles EF apresentaram aumento da ingestão alimentar (+10%), dos depósitos de gordura visceral (~60%) e conteúdo de gordura corporal total (+50%), menor área dos adipócitos subcutâneos (-24%), aumento da leptina (+85%), glicemia (+11%), adiponectina (+1.4x), T3 (+71%), T4 (+57%) e TSH (+36,5%) com menor corticosterona (-41%) e catecolaminas adrenais (-57%) e aumento dos triglicerídeos (+65%). Somados nossos dados evidenciam o impacto negativo que a exposição à fumaça do cigarro tem sobre o desenvolvimento do neonato com o surgimento de desordens endócrinas futuras, que ocorrem pelo menos em parte devido às alterações observadas nas mães. Portanto, independente da forma de exposição, seja via aleitamento materno ou por inalação direta, é de grande importância alertar a sociedade sobre as possíveis complicações metabólicas em longo prazo decorrente da exposição involuntária do neonato ao tabagismo.

Maternal effects on the quality of progeny can have direct impacts on population productivity. Rockfish are viviparous and the oil globule size of larvae at parturition has been shown to have direct effects on time until starvation and growth rate. We sampled embryos and preparturition larvae opportunistically from 89 gravid quillback rockfish (Sebastes maliger) in Southeast Alaska. Because the developmental stage and sampling period were correlated with oil globule size, they were treated as covariates in an analysis of maternal age, length, and weight effects on oil globule size. Maternal factors were related to developmental timing for almost all sampling periods, indicating that older, longer, and heavier females develop embryos earlier than younger, shorter, or lighter ones. Oil globule diameter and maternal length and weight were statistically linked, but the relationships may not be biologically significant. Weight-specific fecundity did not increase with maternal size or age, suggesting that reproductive output does not increase more quickly as fish age and grow. Age or size truncation of a rockfish population, in which timing of parturition is related to age and size, could result in a shorter parturition season. This shortening of the parturition season could make the population vulnerable to fluctuating environmental conditions.

A Diabetes Mellitus Gestacional (DMG) pode ser definida como intolerância a carboidrato durante a gravidez e estima-se que pode afetar 10-22% de todas as pacientes grávidas. Durante a gravidez podem surgir diversas complicações para o feto como risco elevado de aborto espontâneo, anormalidades congênitas e morbidade e mortalidade neonatal. Entretanto, podem surgir também alterações morfofuncionais em diversos órgãos da mãe diabética, porém isso não é bem estabelecido. Investigar se haverá ou não alterações bioquímicas e histopatológicas em diversos órgãos, como hipófise, útero, placenta e pâncreas de ratas grávidas com diabetes mellitus durante e no final da gravidez e compará-las . Além disso, investigar se há alteração na matriz extracelular (MEC) da hipófise desses animais. No 5 dia de vida, ratas Wistar foram divididas em dois grupos: um tratado com estreptozotocina (Grupo Diabético / DIAB), na dose de 90 mg/kg, subcutâneo e outro grupo, que foi tratado com veículo (tampão citrato/CTR). Aos 90 dias de vidas, foram submetidas ao cruzamento. Após isso, foram sacrificadas no 11 e 21 dia de gravidez. Foram avaliados glicemia e bioquímica maternal e número de implantes .O pâncreas, útero, placenta e hipófises foram coradas com Hematoxilina e Eosina e somente as hipófises foram coradas com Massom e Picrosirius, para avaliação da MEC.Os animais diabéticos tanto do 11 quanto do 21 dia apresentaram uma redução no número de implantes, menor peso e maior glicemia e colesterol total, em relação aos animais controle independente do dia da gravidez. Não foi verificada diferença dos níveis de triglicerídeos entre os grupos não diabéticos e diabéticos, independente dos dias. Entretanto, os animais diabéticos que finalizaram o período de gestação apresentaram uma maior glicemia maternal em relação ao grupo diabético do 11 dia. Pâncreas de ratas diabéticas do 21 dia apresentaram vacuolização intracitoplasmática das ilhotas, insulite,migração de células inflamatórias, espessamento da parede do vaso e fibrose periductal e vascular. Essas alterações foram verificadas com bem menor intensidade nos animais diabéticos do 11 dia. Foi verificado que a placenta de animais diabéticos apresenta congestão na interface materno-fetal, migração celular, maior concentração de vasos maternos e fetais, mas em forma irregular , necrose e vacuolização. A hipófise de animais diabéticos mostraram células cromófobas agregadas, aumento da espessura de fibras de colágeno vermelhas da MEC, em contraste com o controle, que foi visualizado fibras em verde e em formato de feixe. A diabetes desempenhou um total remodelamento da hipófise. Gravidez de animais diabeticos mostraram maior dano ao pâncreas e placenta, especialmente no final da gravidez. Em consequência dessa alterações, esses animais diabéticos apresentaram hiperglicemia, maior colesterol total, porém menor peso materno, número de implantes e sem alterações nos triglicerídeos. Esse é o primeiro estudo a demonstrar remodelamento tecidual em alguns elementos da MEC na hipófise, como espessamento da camada da MEC e fibras de colágeno em verde. Alterações da MEC da hipófise são provavelmente devido ao processo de diabetes na gestação.

The influences of age, size, and condition of spawning females on fecundity and oocyte quality were analyzed for the Patagonian stock of Argentine Hake (Merluccius hubbsi). Samples of mature females were collected in the spawning area as part of 2 research surveys conducted in January 2010 and 2011, during the peak of the reproductive season. Batch fecundity (BF) ranged between 40,500 (29 cm total length [TL]) and 2,550,000 (95 cm TL) hydrated oocytes, and was positively correlated with TL, gutted weight, age, hepatosomatic index (HSI), and the relative condition factor (Kn). Relative fecundity ranged between 85 and 1040 hydrated oocytes g–1 and showed significant positive relationships with gutted weight, HSI, and Kn; however, coefficients of determination were low for all regressions. Dry weights of samples of 100 hydrated oocytes ranged between 1.8 and 3.95 mg and were positively correlated with all variables analyzed, including batch and relative fecundity. Multiple regression models created with data of the morphophysiological characteristics of females supported maternal influences on fecundity and egg weights. Within the studied size range (29–95 cm TL), larger individuals had better somatic and egg condition, mainly revealed by higher HSI and hydrated oocytes with larger oil droplets (275.71μm [standard error 1.49]). These results were associated with the higher feeding activity of larger females during the spawning season in comparison with the feeding activity of young individuals (<5 years old); the better nutritional state of larger females, assumed to result from more feeding, was conducive to greater production of high-quality eggs.
