Background Several studies link the seamless fit of implant-supported prosthesis with the accuracy of the dental impression technique obtained during acquisition. In addition, factors such as implant angulation and coping shape contribute to implant misfit. Purpose To identify the most accurate impression technique and factors affecting the impression accuracy. Material and Methods A systematic review of peer-reviewed literature was conducted analyzing articles published between 2009 and 2013. The following search terms were used: implant impression, impression accuracy, and implant misfit. A total of 417 articles was identified, 32 were selected for review. Results All 32 selected studies refer to in vitro studies. Fourteen articles compare open and closed impression technique, 8 advocate the open technique and 6 report similar results. Other 14 articles evaluate splinted and non-splinted techniques; all advocating the splinted technique. Polyether material usage was reported in 9; 6 studies tested vinyl polysiloane and 1 study used irreversible hydrocolloid. Eight studies evaluated different copings designs. Intra-oral optical devices were compared in 4 studies. Conclusions The most accurate results were achieved with two configurations: (1) the optical intra-oral system with powder; and (2) the open technique with splinted squared transfer copings, using polyether as impression material.

Poly(dimethylsiloxane) (PDMS) is an organosilicon polymer widely used in the fabrication of microfluidic systems to integrate biochips. In this study, we propose the use of an adapted PDMS mould for the creation of a miniaturized, reusable, reference electrode for in-chip electrochemical measurements. Through its integrated microfluidic system it is possible to replenish internal buffer solutions, unclog critical junctions and treat the electrode’s surface, assuring a long term reuse of the same device. Planar Ag/AgCl reference electrodes were microfabricated over a passivated p-type Silicon Wafer. The PDMS mould, containing an integrated microfluidic system, was fabricated based on patterned SU-8 mould, which includes a lateral horizontal inlet access point. Surface oxidation was used for irreversible permanent bondage between flat surfaces. The final result was planar Ag/AgCl reference electrode with integrated microfluidic that allows for electrochemical analysis in biochips

[Excerto] El V Informe del IPCC insiste, de forma más contundente, sobre la existencia y riesgo irreversible del cambio climático. Las declaraciones de los científicos son cada vez más concluyentes, alertando que, para evitar interferencias peligrosas en el sistema climático, no podemos seguir con el statu quo y es preciso un giro copernicano a nivel tecnológico, institucional y humano de inicio inmediato, implicación global y sustanciosas inversiones (comunicado de prensa del IPCC, 13/04/2014). Ante la extralimitación en la emisión de gases de efecto invernadero, se impone una reacción de la especie humana íntimamente relacionada con los límites. Límites que también están vinculados al previsible fin de la etapa del petróleo fácil y al agotamiento de recursos. A la par de las advertencias científicas, están surgiendo numerosos movimientos sociales y redes, iniciativas económicas y políticas que proponen y experimentan alternativas que a escala micro se muestran eficaces ante un cambio drástico hacia un futuro bajo en carbono(...).

We report a case of chronic nonspecific constrictive pericarditis with myocardial involvement in a 19-month-old infant. The patient underwent total pericardiectomy and had irreversible hemodynamic instability. Constrictive pericarditis is rare in childhood. It may follow several processes, most frequently an infectious disease. The natural course of the disease consists of progressive myocardial dysfunction with atrioventricular valvular involvement. When diagnosis is established early in the course of the disease and treatment started immediately, the evolution is favorable in most instances.

Chloroquine has been widely used in rheumatological treatment, but potential severe side effects require careful follow-up. Cardiac damage is not a common consequence, but its clinical relevance has not yet been described. We report the case of a 58-year-old woman with rheumatoid arthritis, in whom chronic chloroquine use resulted in major irreversible cardiac damage. She presented with syncopal episodes due to complete atrioventricular block confirmed by electrophysiological study whose changes were concluded to be irreversible and a permanent pacemaker was indicated. Endomyocardial biopsy was also performed to search for histopathological and ultrastructural cardiac damage. We also reviewed the 22 cases of chloroquine-induced cardiopathy described to date as well as its pathophysiology.

L’entorn administratiu d’aquests últims anys ha evidenciat la proliferació dels codis de conducta de l’Administració pública, un instrument procedent de l’experiència anglosaxona, especialment de la nord-americana, que en el present està en apogeu a l’Estat espanyol. La realitat ha exigit el desenvolupament d’un mecanisme extrínsec capaç de procurar la comprensió de determinats estàndards ètic-institucionals, així com d’orientar l’actuació d’uns empleats públics que, amb caràcter general, es troben lluny d’encarnar aquella que podríem considerar característica del mite de l’administrador ben educat. Així, sorgeix la voluntat ferma de recórrer a una figura que alguns han qualificat d’organitzativa, la finalitat de la qual rau en cobrir aquelles àrees de l’activitat administrativa que presenten una situació especialment sensible, responent a la necessitat d’una reforma administrativa multidireccional que afronti els nous reptes. El fenomen actual dels codis ètics no ha romàs immune a la influència d’uns determinats corrents de pensament, que no només han definit els fonaments específics dels mateixos –subjacents en la formulació dels principis que incorporen−, sinó també la base del marc ètic-públic en el qual els codis es troben immersos. Els codis de conducta dels funcionaris públics anglosaxons estan arrelats, principalment, en les ètiques fundacionals (teleològica utilitarista i deontològica kantiana) i, de manera indirecta, en aquells preceptes jurídics que en el seu moment varen acollir els principis ètic-públics fundacionals, conformant el que avui denominem ètica institucional, inserida en un marc jurídic-positiu. Per aquest motiu podem afirmar la naturalesa híbrida dels codis de conducta, majoritàriament ètica però no exclusiva que, atenent als principis constitucionals, encapçalarà el diàleg entre la tradicional disjuntiva Ètica-Dret. La necessitat d’elaboració i d’implantació dels codis de conducta al servei del bé dels ciutadans ha conduït, sense més ni més, a la recepció del model nord-americà a l’Estat espanyol, desproveïda de qualsevol element distintiu entre ambdues cultures. D’aquesta manera, l’Administració pública espanyola ha incorporat uns codis ètics de fonamentació únicament fundacional, sense ni tan sols procurar un marc d’aplicació conforme a les exigències públiques del nostre Estat. En desavinença a aquest fet, proposem un model en el que, prescindint –i fins i tot assumint− d’una irreversible formulació fundacional dels codis, l’aplicació d’aquestes figures assenti la base en l’ètica de les virtuts. Les normes i els deures definits en els codis de conducta dels empleats públics no poden esdevenir realitats morals primàries, sinó que han d’estar en funció de la vida segons la virtut. Aquesta concepció ètica dirigeix l’home fins a un nivell de reflexió que li permet indagar racionalment sobre el bé veritable de la vida humana, obrint camí a la perfecció d’una conducta que, en última instància, li proporcionarà l’autèntica llibertat d’acord amb les seves pròpies conviccions.

Hypoxia, a condition of insufficient oxygen availability to support metabolism, occurs when the vascular supply is interrupted, as in stroke. The identification of the hypoxic and viable tissue in stroke as compared with irreversible lesions (necrosis) has relevant implications for the treatment of ischemic stroke. Traditionally, imaging by positron emission tomography (PET), using 15O-based radiotracers, allowed the measurement of perfusion and oxygen extraction in stroke, providing important insights in its pathophysiology. However, these multitracer evaluations are of limited applicability in clinical settings. More recently, specific tracers have been developed, which accumulate with an inverse relationship to oxygen concentration and thus allow visualizing the hypoxic tissue non invasively. These belong to two main groups: nitroimidazoles, and among these the 18F-Fluoroimidazole (18F-FMISO) is the most widely used, and the copper-based tracers, represented mainly by Cu-ATSM. While these tracers have been at first developed and tested in order to image hypoxia in tumors, they have also shown promising results in stroke models and preliminary clinical studies in patients with cardiovascular disorders, allowing the detection of hypoxic tissue and the prediction of the extent of subsequent ischemia and clinical outcome. These tracers have therefore the potential to select an appropriate subgroup of patients who could benefit from a hypoxia-directed treatment and provide prognosis relevant imaging. The molecular imaging of hypoxia made important progress over the last decade and has a potential for integration into the diagnostic and therapeutic workup of patients with ischemic stroke.

Oogenesis and oviposition can be inhibited in female of Rhodnius prolixus by means of short-term experiment (first reproductive cycle) of a single dose of ethoxyprecocene II given by ingestion. The inhibition is dose-dependent as measured by oocyte growth, egg maturation and egg deposition. In a long-term experiment (second and third reproductive cycles) egg production and oogenesis can be partially or totally re-established by subsequent blood meals without ethoxyprecocene II. These findings suggest that in female R. prolixus, damage caused to corpus allatum by ethoxyprecocene II, in certain cases, is not irreversible.

After ischemic stroke, the ischemic damage to brain tissue evolves over time and with an uneven spatial distribution. Early irreversible changes occur in the ischemic core, whereas, in the penumbra, which receives more collateral blood flow, the damage is more mild and delayed. A better characterization of the penumbra, irreversibly damaged and healthy tissues is needed to understand the mechanisms involved in tissue death. MRSI is a powerful tool for this task if the scan time can be decreased whilst maintaining high sensitivity. Therefore, we made improvements to a (1) H MRSI protocol to study middle cerebral artery occlusion in mice. The spatial distribution of changes in the neurochemical profile was investigated, with an effective spatial resolution of 1.4 μL, applying the protocol on a 14.1-T magnet. The acquired maps included the difficult-to-separate glutamate and glutamine resonances and, to our knowledge, the first mapping of metabolites γ-aminobutyric acid and glutathione in vivo, within a metabolite measurement time of 45 min. The maps were in excellent agreement with findings from single-voxel spectroscopy and offer spatial information at a scan time acceptable for most animal models. The metabolites measured differed with respect to the temporal evolution of their concentrations and the localization of these changes. Specifically, lactate and N-acetylaspartate concentration changes largely overlapped with the T(2) -hyperintense region visualized with MRI, whereas changes in cholines and glutathione affected the entire middle cerebral artery territory. Glutamine maps showed elevated levels in the ischemic striatum until 8 h after reperfusion, and until 24 h in cortical tissue, indicating differences in excitotoxic effects and secondary energy failure in these tissue types. Rotation-mediated aggregating brain cell cultures at two different maturational stages (DIV 11 and DIV 20) were subjected for 1 or 2 hours to ischaemic conditions by transient immobilization (arrest of media circulation). During recovery, cell damage was evaluated by measuring changes in cell type-specific enzyme activities and total protein content. It was found that in immature cultures (DIV 11), immobilization for 1 or 2 hours did not affect the parameters measured. By contrast, at DIV 20, ischaemic conditions for 1 hour caused a pronounced decrease in the activities of glutamic acid decarboxylase and choline acetyltransferase. A significant decrease in these neuron-specific enzyme activities was found at post-ischaemic days 1-14, indicating immediate and irreversible neuronal damage. The activity of the astrocyte-specific enzyme, glutamine synthetase, was significantly increased at 4 days post-treatment; equal to control values at 6 days; and significantly decreased at 14 days after the ischaemic insult. Immobilization of DIV 20 cultures for 2 hours caused a drastic reduction in all the parameters measured at post-ischaemic day 6. Generally, the ischaemic conditions appeared to be more detrimental to neurons than to astrocytes, and GABAergic neurons were more affected than cholinergic neurons.

Purpose: The aim of this educational poster is to introduce the technical principles of cerebral perfusion CT and to provide examples of its clinical applications and potential limitations in the everyday emergency practice. Methods and materials: Cerebral perfusion CT is a well established investigatory tool for many vascular and parenchymal brain dysfunctions. CT perfusion maps allow a semiquantitative assessment of cerebral perfusion. Results: Currently, cerebral perfusion CT has a pivotal role in differentiating reversible from irreversible ischemic parenchymal insult besides its integral role in grading vasospasm after subarachnoid hemorrhage. Furthermore, cerebral perfusion CT can be coupled to acetazolamide administration in order to assess the cerebrovascular reserve capacity before performing extra-/intra-cranial bypass surgery in patients with cerebral vascular insufficiency. Cerebral perfusion CT can also identify diffuse abnormalities of cerebral perfusion in children with traumatic brain injury showing a low initial GCS in order to predict the final outcome regarding the late occurrence of irreversible parenchymal damage. Cerebral Perfusion CT is also able to detect focal parenchymal perfusion abnormalities in acute epileptic seizures. Conclusion: Cerebral perfusion CT can be integrated in the management of many vascular, traumatic and functional disorders of the brain.
