The T2K collaboration reports a precision measurement of muon neutrino disappearance with an off-axis neutrino beam with a peak energy of 0.6 GeV. Near detector measurements are used to constrain the neutrino flux and cross section parameters. The Super-Kamiokande far detector, which is 295 km downstream of the neutrino production target, collected data corresponding to 3.01×1020 protons on target. In the absence of neutrino oscillations, 205±17 (syst.) events are expected to be detected and only 58 muon neutrino event candidates are observed. A fit to the neutrino rate and energy spectrum assuming three neutrino flavors, normal mass hierarchy and θ23≤π/4 yields a best-fit mixing angle sin2(2θ23)=1.000 and mass splitting |Δm232|=2.44×10−3 eV2/c4. If θ23≥π/4 is assumed, the best-fit mixing angle changes to sin2(2θ23)=0.999 and the mass splitting remains unchanged.

The T2K experiment has observed electron neutrino appearance in a muon neutrino beam produced 295 km from the Super-Kamiokande detector with a peak energy of 0.6 GeV. A total of 28 electron neutrino events were detected with an energy distribution consistent with an appearance signal, corresponding to a significance of 7.3σ when compared to 4.92 ± 0.55 expected background events. In the PMNS mixing model, the electron neutrino appearance signal depends on several parameters including three mixing angles θ12, θ23, θ13, a mass difference Δm232 and a CP violating phase δCP. In this neutrino oscillation scenario, assuming |Δm232|=2.4×10−3 eV2, sin2θ23=0.5, δCP=0, and Δm232>0 (Δm232<0), a best-fit value of sin22θ13 = 0.140+0.038−0.032 (0.170+0.045−0.037) is obtained.

Background The use of cancer related therapy in cancer patients at the end-of-life has increased over time in many countries. Given a lack of published Swiss data, the objective of this study was to describe delivery of health care during the last month before death of cancer patients. Methods Claims data were used to assess health care utilization of cancer patients (identified by cancer registry data of four participating cantons), deceased between 2006-2008. Primary endpoints were hospitalization rate and delivery of cancer related therapies during the last 30 days before death. Multivariate logistic regression assessed the explanatory value of patient and geographic characteristics. Results 3809 identified cancer patients were included. Hospitalization rate (mean 68.5%, 95%CI 67.0-69.9) and percentage of patients receiving anti-cancer drug therapies (ACDT, mean 14.5%, 95%CI 13.4-15.6) and radiotherapy (mean 7.7%, 95%CI 6.7-8.4) decreased with age. Canton of residence and insurance type status most significantly influenced the odds for hospitalization or receiving ACDT. Conclusions The intensity of cancer specific care showed substantial variation by age, cancer type, place of residence and insurance type status. This may be partially driven by cultural differences within Switzerland and the cantonal organization of the Swiss health care system.

Antiretroviral therapy (ART) has improved quality of life and increased life expectancy of HIV-infected individuals. Opportunistic diseases are less common, and mortality has declined. Consequently, patterns of mortality and morbidity are changing among the HIV-positive population. The focus of care has shifted to ART-related problems and to various non-AIDS diseases. Such comorbidities, often occurring sequentially or concurrently, may be the consequences of long term ART toxicity, a state of chronic inflammation due to HIV infection, lifestyle-related risks for disease, and aging. The emergence of non-AIDS related conditions highlights the important role of primary care physicians, especially of those with extensive experience in HIV management.

BACKGROUND The factors that contribute to increasing obesity rates in human immunodeficiency virus (HIV)-positive persons and to body mass index (BMI) increase that typically occurs after starting antiretroviral therapy (ART) are incompletely characterized. METHODS We describe BMI trends in the entire Swiss HIV Cohort Study (SHCS) population and investigate the effects of demographics, HIV-related factors, and ART on BMI change in participants with data available before and 4 years after first starting ART. RESULTS In the SHCS, overweight/obesity prevalence increased from 13% in 1990 (n = 1641) to 38% in 2012 (n = 8150). In the participants starting ART (n = 1601), mean BMI increase was 0.92 kg/m(2) per year (95% confidence interval, .83-1.0) during year 0-1 and 0.31 kg/m(2) per year (0.29-0.34) during years 1-4. In multivariable analyses, annualized BMI change during year 0-1 was associated with older age (0.15 [0.06-0.24] kg/m(2)) and CD4 nadir <199 cells/µL compared to nadir >350 (P < .001). Annualized BMI change during years 1-4 was associated with CD4 nadir <100 cells/µL compared to nadir >350 (P = .001) and black compared to white ethnicity (0.28 [0.16-0.37] kg/m(2)). Individual ART combinations differed little in their contribution to BMI change. CONCLUSIONS Increasing obesity rates in the SHCS over time occurred at the same time as aging of the SHCS population, demographic changes, earlier ART start, and increasingly widespread ART coverage. Body mass index increase after ART start was typically biphasic, the BMI increase in year 0-1 being as large as the increase in years 1-4 combined. The effect of ART regimen on BMI change was limited.

The use of cancer-related therapies in cancer patients hospitalized at the end of life has increased in many countries over time. Given the scarcity of published Swiss data, the objective of this study was to evaluate the influence of hospital type and other factors on the delivery of health care during the last month before death. Claims data were used to assess health care utilization of cancer patients (identified by cancer registry data of four participating Swiss cantons) who deceased between 2006 and 2008. Primary endpoints were delivery of cancer-related therapies during the last 30 days before death. Multivariate logistic regression assessed the explanatory value of hospital type, patient and geographic characteristics. Of 3,809 identified cancer patients in the claims database, 2,086 patients dying from cancer were hospitalized during the last 30 days before death, generating 2,262 inpatient episodes. Anticancer drug therapy was given in 22.2% and radiotherapy in 11.7% of episodes. Besides age and cancer type, the canton of residence and hospital type showed independent, statistically significant associations with intensity of care, which was highest in university hospitals. These results should initiate a discussion among oncologists in Switzerland and may question the compliance with standard of care guidelines for terminal cancer patients.

BACKGROUND Breast cancer (BC) is the most commonly diagnosed cancer and a leading cause of death in younger women. METHODS We analysed incidence, mortality and relative survival (RS) in women with BC aged 20-49 years at diagnosis, between 1996 and 2009 in Switzerland. Trends are reported as estimated annual percentage changes (EAPC). RESULTS Our findings confirm a slight increase in the incidence of BC in younger Swiss women during the period 1996-2009. The increase was largest in women aged 20-39 years (EAPC 1.8%). Mortality decreased in both age groups with similar EAPCs. Survival was lowest among women 20-39 years (10-year RS 73.4%). We observed no notable differences in stage of disease at diagnosis that might explain these differences. CONCLUSIONS The increased incidence and lower survival in younger women diagnosed with BC in Switzerland indicates possible differences in risk factors, tumour biology and treatment characteristics that require additional examination.

Condomless sex is a key driver of sexually transmitted diseases. In this study, we assess the long-term changes (2000-2013) of the occurrence of condomless sex among human immunodeficiency virus (HIV)-infected individuals enrolled in the Swiss HIV Cohort study. The frequencies with which HIV-infected individuals reported condomless sex were either stable or only weakly increasing for 2000-2008. For 2008-2013, these rates increased significantly for stable relationships among heterosexuals and men who have sex with men (MSM) and for occasional relationships among MSM. Our results highlight the increasing public health challenge posed by condomless sex and show that condomless sex has been increasing even in the most recent years.

Background.  Although acquired immune deficiency syndrome-associated morbidity has diminished due to excellent viral control, multimorbidity may be increasing among human immunodeficiency virus (HIV)-infected persons compared with the general population. Methods.  We assessed the prevalence of comorbidities and multimorbidity in participants of the Swiss HIV Cohort Study (SHCS) compared with the population-based CoLaus study and the primary care-based FIRE (Family Medicine ICPC-Research using Electronic Medical Records) records. The incidence of the respective endpoints were assessed among SHCS and CoLaus participants. Poisson regression models were adjusted for age, sex, body mass index, and smoking. Results.  Overall, 74 291 participants contributed data to prevalence analyses (3230 HIV-infected; 71 061 controls). In CoLaus, FIRE, and SHCS, multimorbidity was present among 26%, 13%, and 27% of participants. Compared with nonsmoking individuals from CoLaus, the incidence of cardiovascular disease was elevated among smoking individuals but independent of HIV status (HIV-negative smoking: incidence rate ratio [IRR] = 1.7, 95% confidence interval [CI] = 1.2-2.5; HIV-positive smoking: IRR = 1.7, 95% CI = 1.1-2.6; HIV-positive nonsmoking: IRR = 0.79, 95% CI = 0.44-1.4). Compared with nonsmoking HIV-negative persons, multivariable Poisson regression identified associations of HIV infection with hypertension (nonsmoking: IRR = 1.9, 95% CI = 1.5-2.4; smoking: IRR = 2.0, 95% CI = 1.6-2.4), kidney (nonsmoking: IRR = 2.7, 95% CI = 1.9-3.8; smoking: IRR = 2.6, 95% CI = 1.9-3.6), and liver disease (nonsmoking: IRR = 1.8, 95% CI = 1.4-2.4; smoking: IRR = 1.7, 95% CI = 1.4-2.2). No evidence was found for an association of HIV-infection or smoking with diabetes mellitus. Conclusions.  Multimorbidity is more prevalent and incident in HIV-positive compared with HIV-negative individuals. Smoking, but not HIV status, has a strong impact on cardiovascular risk and multimorbidity.

QUESTION UNDER STUDY/PRINCIPLES This study aimed to evaluate trends in the incidence of oesophageal and gastric cancer by anatomical location and histology using nationally representative Swiss data. METHODS We included all oesophageal and gastric cancers recorded in 10 Swiss population-based cancer registries 1982-2011. We calculated age-standardised incidence rates (ASIRs) per 100 000 person-years (PY) (European standard) for both cancer sites stratified by sex, language region (German, French-Italian), morphology and anatomical location. To assess time trends, we estimated annual percentage changes (APCs) with 95% confidence intervals (95% CIs). RESULTS ASIR of oesophageal adenocarcinoma increased in both sexes and language regions (p <0.001). The steepest increase occurred in males of the German-speaking region (APC 6.8%, 95% CI 5.8-7.8) with ASIRs of 0.8 per 100,000 PY in 1982-1987 and 3.9 per 100.000 PY in 2007-2011. Incidence of oesophageal squamous cell carcinoma decreased significantly in males of both language regions by around -1.5% per year. In contrast, a slight but significant increase (APC 1.4%, 95% CI 0.3-2.4]) of oesophageal squamous cell carcinoma was observed in females of the German-speaking region. We observed stable rates for cancer of the gastric cardia. The incidence of noncardia gastric cancer decreased substantially in both sexes and language regions (p <0.001). CONCLUSION In Switzerland, the incidence of oesophageal adenocarcinoma has risen whereas incidence of noncardia gastric cancer has decreased substantially as observed in other developed countries.

Number of days spent in acute hospitals (DAH) at the end of life is regarded as an important care quality indicator for cancer patients. We analysed DAH during 90 days prior to death in patients from four Swiss cantons. Claims data from an insurance provider with about 20% market share and patient record review identified 2086 patients as dying of cancer. We calculated total DAH per patient. Multivariable generalised linear modelling served to evaluate potential explanatory variables. Mean DAH was 26 days. In the multivariable model, using complementary and alternative medicine (DAH = 33.9; +8.8 days compared to non-users) and canton of residence (for patient receiving anti-cancer therapy, Zürich DAH = 22.8 versus Basel DAH = 31.4; for other patients, Valais DAH = 22.7 versus Ticino DAH = 33.7) had the strongest influence. Age at death and days spent in other institutions were additional significant predictors. DAH during the last 90 days of life of cancer patients from four Swiss cantons is high compared to most other countries. Several factors influence DAH. Resulting differences are likely to have financial impact, as DAH is a major cost driver for end-of-life care. Whether they are supply- or demand-driven and whether patients would prefer fewer days in hospital remains to be established.

La caries de la infancia temprana es un problema de salud pública que afecta con mayor agresividad a preescolares en riesgo social. El objetivo del presente trabajo fue establecer las asociaciones existentes entre estado dental, demanda de atención odontológica, percepción de salud bucal y condición socio-económica en niños preescolares del Gran Mendoza. Material y método: la población estuvo constituida por escolares en una muestra intencionada de 155 niños de nivel inicial residentes en la provincia de Mendoza, pertenecientes al ámbito escolar urbano- marginal. Se registraron los siguientes índices: ceod, CPOD, ceos, CPOS, y sus componentes discriminados, ICDAS II, íNTC (Bordoni, 1998) e índice de placa (Silness y Löe, 1967). Fue aplicado un cuestionario de percepción parental de salud bucal (ECOHIS) y se realizaron encuestas estructuradas sobre tipología de la demanda de atención odontológica y de condiciones socioeconómicas (NBI). Fue determinada la distribución de frecuencias y los intervalos de confianza para cada variable, las medidas de tendencia central y dispersión y se realizaron comparaciones entre grupos mediante la prueba de chi cuadrado con p=0.05. Resultados: 85.8% de la muestra presentó experiencia de caries. Para cada indicador se registraron los siguientes valores: ceod+CPOD= 5.716 +/- 4.08; cd+CD =5,39+3.79; ed+PD= 0.25+- 0.72; y od+OD= 0.08+-0.34; ceos+CPOS=8.89+-8.39; cs+CS=7.63+-6.25; es+PS= 1.23+-3.62; y os+OS= 0.12+0.46; IP= 1.04+-0.48; y INTC=5.65+-3.19. No se observaron diferencias significativas al agrupar los niños por sexo. Al analizar la demanda de atención bucal se observó que el 54,9% efectuó una consulta odontológica, 25% en el último año y 74% en el subsistema de salud pública. El análisis del cuestionario ECOHIS reveló que obtuvo el máximo porcentaje de respuestas cuando se ordenó según la categoría nunca. El único ítem que se relacionó con el estado dental en forma estadísticamente significativa fue el dolor. Conclusiones: Se observa alta prevalencia de caries dental sin que la demanda de atención odontológica haya producido impacto positivo sobre el estado dentario ni sobre la percepción de los padres respecto de la salud bucal.

En Mendoza se realizó un estudio clínico de intervención, prospectivo de dos años, dirigido a preescolares en riesgo social con alto índice de caries con el objetivo de evaluar la eficacia y efectividad de un programa preventivo basado en fluoruros. El programa preventivo se basó en acciones de educación para la salud, enseñanza de técnicas de higiene bucal y aplicación tópica de flúor fosfato acidulado realizadas dos veces durante el ciclo escolar. La muestra intencionada sumó 96 niños de sala de 4 años (2009) que fueron evaluados en su cursado posterior en sala de 5 años (2010) y en 1er año de EGB durante 2011. Niños asistentes a sala de 5 años y primer año EGB al momento del examen basal (2009) conformaron el grupo control. Se les aplicó el mismo programa por razones éticas. A 12 y 24 meses de aplicado el programa se evaluó: porcentaje de libres de caries, ceod+CPOD, cpos+CPOS y sus valores discriminados, medias de categorías ICDAS II, índice de Placa Bacteriana de Löe y Silness e índice de necesidad de tratamiento de caries de Bordoni. Se determinó la distribución de frecuencias e intervalos de confianza para cada variable; medidas de tendencia central y dispersión y se realizaron comparaciones entre grupos mediante la prueba T de Student y chi cuadrado con un nivel de p=0.05. Resultados: Comparando sala 5 años 2010 / sala de 5 años 2009: disminuyeron significativamente las medias de ceod +CPOD, ICDAS 6, Índice Placa Löe e índice de necesidad de tratamiento. Comparando 1er año 2011 / 1er año 2009: disminuyeron significativamente las medias de cs+CS, ceos+CPOS e ICDAS 4. El resto de los valores no mostró diferencias significativas. Conclusión: Un programa preventivo basado en fluoruros es efectivo y tiene efectos positivos sobre la prevalencia de caries en preescolares de Mendoza.

La problemática de los Residuos Sólidos Urbanos (RSU), en la actualidad, encuentra respuesta en la Gestión Integral de Residuos Sólidos Urbanos (GIRSU), la cual determina cómo debe tratarse la basura desde que se genera, se transporta y cuál debe ser su destino final. Esta política reconoce la recuperación de residuos como fundamental para una correcta gestión de la basura, y en este sentido, los Recuperadores Urbanos son quienes realizan esta tarea y reinsertan los residuos en el sistema productivo. En el presente trabajo se analiza cómo se construyen las relaciones entre las Cooperativas de Recuperadores Urbanos (CRU) y el Estado provincial, en la implementación de la política de Gestión diferenciada de RSU de grandes generadores (GG) de la provincia de Buenos Aires entre el año 2013 y 2014. Para esto nos basamos en la perspectiva de análisis de las políticas públicas, centrándonos en la fase de la implementación de la misma, pero necesariamente recorriendo la construcción del tema en la agenda de gobierno y los posicionamientos de los distintos actores sociales. En la provincia de Buenos Aires, particularmente en el Area Metropolitana y el Gran La Plata, los residuos se entierran en los rellenos sanitarios de la Coordinación Ecológica Area Metropolitana Sociedad del Estado (CEAMSE). La saturación de estos rellenos sanitarios en el año 2012, generó que el Organismo Provincial para el Desarrollo Sostenible (OPDS) implemente una política de Gestión diferenciada de RSU para los GG, con el objetivo de reducir los residuos que se entierran en la CEAMSE. Esta política implementada por el Organismo, tiene la particularidad de incluir a las Cooperativas bajo la figura de "Destino Sustentables", dando la posibilidad de que cobren el servicio de tratamiento de los residuos. Pero esta inclusión de las CRU trajo aparejada una gran cantidad de problemáticas debido al desconocimiento del OPDS de la heterogeneidad y diversidad de realidades en las que se encuentran las Cooperativas en la actualidad. En este contexto, se generó un espacio de participación entre las CRU y el OPDS denominado "Mesa de Trabajo", en donde centramos nuestro análisis
