Background: The Lescol Intervention Prevention Study (LIPS) was a multinational randomized controlled trial that showed a 47% reduction in the relative risk of cardiac death and a 22% reduction in major adverse cardiac events (MACEs) from the routine use of fluvastatin, compared with controls, in patients undergoing percutaneous coronary intervention (PCI, defined as angioplasty with or without stents). In this study, MACEs included cardiac death, nonfatal myocardial infarction, and subsequent PCI and coronary artery bypass graft. Diabetes was the greatest risk factor for MACEs. Objective: This study estimated the cost-effectiveness of fluvastatin when used for secondary prevention of MACEs after PCI in people with diabetes. Methods: A post hoc subgroup analysis of patients with diabetes from the LIPS was used to estimate the effectiveness of fluvastatin in reducing myocardial infarction, revascularization, and cardiac death. A probabilistic Markov model was developed using United Kingdom resource and cost data to estimate the additional costs and quality-adjusted life-years (QALYs) gained over 10 years from the perspective of the British National Health Service. The model contained 6 health states, and the transition probabilities were derived from the LIPS data. Crossover from fluvastatin to other lipid-lowering drugs, withdrawal from fluvastatin, and the use of lipid-lowering drugs in the control group were included. Results: In the subgroup of 202 patients with diabetes in the LIPS trial, 18 (15.0%) of 120 fluvastatin patients and 21 (25.6%) of 82 control participants were insulin dependent (P = NS). Compared with the control group, patients treated with fluvastatin can expect to gain an additional mean (SD) of 0.196 (0.139) QALY per patient over 10 years (P < 0.001) and will cost the health service an additional mean (SD) of 10 (E448) (P = NS) (mean [SD] US $16 [$689]). The additional cost per QALY gained was;(51 (US $78). The key determinants of cost-effectiveness included the probabilities of repeat interventions, cardiac death, the cost of fluvastatin, and the time horizon used for the evaluation. Conclusion: Fluvastatin was an economically efficient treatment to prevent MACEs in these patients with diabetes undergoing PCI.

Aims: This study tested the impact of combining a motivational intervention based on protection motivation theory (PMT, Rogers, 1983 [18]) plus a volitional intervention based on action planning and coping planning, as a way to promote the prevention of type 2 diabetes among UK undergraduates. Methods: Eighty-four participants were randomly assigned to either a control group or one of three experimental conditions: motivational intervention (PMT), volitional intervention (APCP), or combined motivational and volitional intervention (PMT&APCP). PMT variables, dietary and exercise behaviours were measured at three time-points over a four-week period. Results: The motivational intervention significantly changed PMT variables. The combined motivational and volitional intervention significantly decreased fat intake and increased the frequency of exercise relative to all other groups, and significantly increased the amount of fruit and vegetables consumed relative to control and volitional intervention groups. Conclusions: These results suggest that motivational intervention is effective at changing cognitions but changing behaviour requires an intervention based on both motivation and volition.

The final disposal of municipal solid waste in unsuitable areas without an infrastructure that meets the health measures and environmental protection, coupled with the lack of technical criteria in phase and decommissioning of the dump can promote environmental degradation. Alternatively to minimize the impacts of this activity for the stabilization of the area by isolating the massive waste with implementation of an adequate and finished by a layer of soil for plant growth final cover system. In this context, the present study aimed to evaluate the quality of the final cover in the area of a disabled dump the tropical semi-arid region in order to assist the process of recovery of these areas. The study area is located in the tropical semi-arid region in São João do Sabugi /RN. Soil samples were collected in the dump area and bushland as a benchmark of quality. To which they were subjected to analysis of physical attributes (particle density, bulk density, grain size and porosity), chemical properties (pH, K + , Na+ , Ca2 + , Mg2 + and Al3 + exchangeable, potential acidity, available phosphorus, sum of bases, CEC, base saturation, aluminum saturation, saturation Na + and adsorption ratio sodium, total organic carbon and total nitrogen) and total and soluble concentrations of heavy metals (Mn, Pb, Zn, Cd, Cu, Mo, Co, Cr, Ba and Ni). The differences between physical and chemical soil under native forest and final cover showing reduction of soil quality in the area off to the dump, which hinders the development of native vegetation and the recovery of the area. The absence of superior waterproofing to allow vertical transfer between the solid waste and the final cover promoted enrichment by chemical elements and heavy metals in excess can impair revegetation. Deficiencies found in the construction process of the final cover point to the need for intervention to accelerate the process of stabilization and recovery of the area of the local ecosystem

A phytosociological study of the forests from Sierra Maestra is conducted, following the methodology of the Zurich- Montpelier School. They are transformed into a forest typology using the standards of the Institute of Agro-Forestry Research. In general, 35 types and/or subtypes are presented. From this group, the most abundant ones belong to semi-deciduous microphyll forest, followed by those from mangroves and mountain rainforest, respectively. Silvicultural treatments are needed; among them, the protection forests are those found above 800 m asl and mangroves.

EU’s deference to the Member State approaches in minority protection can intensify the oppression of
the vulnerable groups, its insistence on non-discrimination on the basis of nationality in the minority regions with special rights in place can equally produce injustice. Its inability to protect EU-wide minorities, like the Roma, is equally problematic. Although a ‘value’, minority protection functions incoherently, if at all. It is time to approach the EU as a highly specific minority protection arena not to be confused with its component parts – the Member States. The reform of the Member State-centred thinking should start at the level of approaching the core issues. It should include the assessment of such questions as what is a minority in the EU’s context of a missing majority, what is the appropriate depth of EU’s intervention in the area of minority protection, i.e. how much room for manœuvre should reasonably be left with the Member States without disrupting the effectiveness of EU’s regulation, as well as the approach to defining what a success in minority protection should be, in the
EU context. The latter should be done, in particular, with due regard to the division of competences between the EU and the Member States in this and other relevant fields. This paper briefly explores a series of diverse casestudies – from migrant EU citizens, Baltic Russians, and sexual minorities to, most importantly, Roma rights to make the first attempt to test the proposed synergetic approach.

Abstract The Coalition Government's new Transforming Rehabilitation (TR) agenda jeopardises the work undertaken with perpetrators of domestic abuse by highly skilled, qualified probation staff. Under new changes outlined by Grayling, Lord Chancellor and Secretary of State for Justice, probation clients who are assessed as posing a medium/low risk of causing harm will be assigned to private sector/voluntary organisations rather than come under the remit of the National Probation Service. This article argues that victims of domestic abuse, primarily women and children, will be placed at an increased risk of harm given this latest TR strategy. The majority of domestic abuse cases will be assessed as posing a medium risk of causing harm and will receive lower levels of intervention by a variety of disparate agencies and organisations. The Ministry of Justice states that the National Probation Service will directly manage offenders who pose a high risk of serious harm to the public, this article will argue that all perpetrators of domestic abuse should be considered as an important exception to this stance, and should remain under the auspices of Probation supervision, irrespective of statistical risk assessment, as has sex offender case management and sex offender treatment programme delivery.

Depuis le début du 20e siècle, plusieurs chercheurs (assez originaux il va s'en dire), provenant de disciplines différentes, ont exploré cette avenue de l'humour en démontrant certaines théories concernant son impact en intervention. De la psychanalyse à la sexologie en passant par la philosophie et la thérapie, pour n'en nommer que quelques-uns, tous ont mis leur grain de sel dans cet engrenage humoriste. Ainsi, dans le milieu hospitalier, on croit que les professionnels de la santé hésitent à utiliser l'humour avec les patients. Le contexte sérieux de la souffrance, l'image respectable du professionnel et la question du conservatisme religieux inhérent à la pratique infirmière constituent les trois facteurs expliquant ce fait. En psychanalyse, Freud (1929) indique que l'humour implique un processus de total défoulement. Bergson (1940), quant à lui, après l'avoir défini comme étant un geste social, nous explique que par l'humour, l'être humain réussit à se dégager émotivement de la dure réalité de l'existence, et en arrive, tout comme le mentionne la sexologue Claire Bouchard (1993), à dédramatiser les situations difficiles. Fry (1987) affirme pour sa part que le paradoxe fait partie intégrante de l'humour, et ce, dans le monde entier. Certains médecins, dont le Dr Drouin concluent quant à eux que l'humour, enfin le rire, en favorisant la synchronisation des deux hémisphères cérébraux, permet à l'humain de prendre des décisions à la fois logiques et intuitives. Et qu'en est-il de l'humour en lien avec la créativité ? Si pour Koestler (1964) l'humour constitue un type de créativité, Freud (1929) le relie au processus créateur par leur base commune, c'est-à-dire, le fantasme du retour à l’enfance. Le service social ne sera pas en reste, car aussi curieux, insolite et surtout paradoxal que cela puisse paraître aux yeux de certains, cet essai porte sur l'humour en protection de la jeunesse. Est-il possible ou illusoire de penser que cet outil qu'est l'humour peut contribuer, entre autres, à créer ou améliorer la relation de confiance entre l'intervenant et le client, et ce, dans le contexte tout à fait particulier qu'est la protection de la jeunesse ? À ce sujet, nous avons donné la parole à huit personnes (quatre clients et quatre intervenants) rattachés au Centre jeunesse de l'Estrie. Au terme de ces entrevues, nous avons analysé plusieurs éléments qui ont particulièrement capté notre attention. Nous avons relevé l’importance pour les intervenants de savoir doser l'humour. Nous avons donc défini quatre types de dosage l'humour, dont un dosage qualitatif, un dosage temporel, un dosage humaniste, et enfin, un dosage quantitatif. Nous avons également relevé un type de rapport à l'humour différent d'un participant à l'autre. Certains mettent ainsi l'emphase sur la complicité, d'autres sur le pouvoir que l'on peut obtenir par l'humour. Ah! Le pouvoir! L'humour implique donc, dans certaines situations une prise de pouvoir négative, mais aussi, et heureusement, dans certains cas, une prise de pouvoir positive. De même, l'humour procure à l'occasion une forme de protection positive ou négative (fuite) à son utilisateur, qu'il soit intervenant ou client. Enfin, ce qui ressort principalement de cette recherche exploratoire, c'est combien les participants, autant les clients que les intervenants ont un immense besoin d'humour dans leur vie. C'est une survie pour les clients face à ce qu'ils vivent par rapport à la protection de la jeunesse et à leur situation familiale, et pour les intervenants qui évitent entre autres l’épuisement professionnel.
