Forty-three specimens of Leptodactylus podicipinus (Anura: Leptodactylidae) were collected in the south-eastern Pantanal, municipality of Corumba, State of Mato Grosso do Sul, Brazil in February and July 2007, and examined for endoparasites. Forty (93%) specimens were infected with at least one helminth species. The predominant parasites were nematodes (Aplectana sp., Cosmocerca podicipinus, Oswaldocruzia lopesi, Physalopteroides venancioi, Rhabdias sp.), but the trematode Catadiscus propinquus also showed high prevalence. The trematodes Infidum infidum and Travtrema stenocotyle were also found, but in only one specimen. Adult frogs showed higher parasite diversity than subadults. Leptodactylus podicipinus was preferentially infected by direct life-cycle parasites and was reported as a new host record for seven helminth species.

Fifty-two stream segments were sampled from 16 August to 13 September in 1993 in the eastern Atlantic Rainforest of São Paulo State, southeastern Brazil (22°55′-25°00′S, 44°48′-48°03′W). Forty-two macroalgal subgeneric taxa were found and the most widespread species were Audouinella pygmaea (21% of sites), Compsopogon leptoclados and Microcoleus subtorulosus (19%). Macroalgal species number per sampling site ranged from 0 to six (2.6 ± 1.7) and was positively correlated to species abundance, whereas species cover ranged from 0 to 70% of the stream bed (15.5 ± 20.8%). No significant correlation was found among macroalgal species number and abundance with any physical or chemical variable analyzed. Most sites were dominated by one or few macroalgal species, mainly, Audouinella macrospora, C. leptoclados and M. subtorulosus. No significant difference was found between the frequency distribution of variables measured for streams and for total macroalgae but the most widespread species (A. pygmaea) differed significantly for current velocity, specific conductance, turbidity and pH. Overall means for macroalgal occurrence include the following values: temperature (X̄ = 19.9°C), current velocity (X̄ = 45 cm s-1), oxygen saturation (X̄ = 66%), specific conductance (X̄ = 59.6 μS cm-1), turbidity (X̄ = 5 NTU) and pH (X̄ = 7.1). This pattern of patchy distribution and dominance by few species has been suggested as typical of stream macroalgal communities and has been ascribed to the rapid fluctuation of physical and chemical conditions. Total macroalgal species richness as well as mean species number per sampling site were considerably lower than found in similar studies of other regions. The Intermediate Disturbance Hypothesis was applied to explain these results: the same factor (high precipitation) responsible for the maintainance of the high species diversity in the surrounding forest can be, paradoxically, a constraint to the development of a more diverse macroalgal flora in streams. © 1996 Kluwer Academic Publishers.

Background: The increase in opportunistic fungal infections has led to the search for putative sources of contamination in hospital environments. Aim: Ants in a public hospital in Itabuna, north-eastern Brazil were examined for carriage of filamentous fungi. Methods: During a year-long survey, ants from different hospital areas were sampled. Preference was given to locations where it was possible to observe ants actively foraging. The fungi found on the ants' integument were cultured and identified. Findings: A total of 106 ant workers belonging to 12 species in 11 genera were collected. A total of 47 fungal strains was isolated from 40% of the ants (. N = 42). We found 16 fungal species in 13 genera associated with the ant workers. The prevalent fungal genera were . Aspergillus, . Purpureocillium and . Fusarium. The ants . Tapinoma melanocephalum, . Paratrechina longicornis and . Pheidole megacephala were associated with six fungal genera; and four genera of fungi were associated with . Solenopsis saevissima workers. Fungal diversity was higher in the following hospital areas: nursery, hospital beds, breastmilk bank and paediatrics. Conclusion: Ants act as carriers of soil and airborne fungal species, and ant control in hospital areas is necessary to prevent the dissemination of such micro-organisms. © 2012 The Healthcare Infection Society.

Specimens (n= 41) of the amphisbaenid Amphisbaena wuchereri taken from a population in Minas Gerais state, south-eastern Brazil, were examined for gastrointestinal parasites. A single nematode species was found, Paradollfusnema amphisbaenia. This was a new host record for this nematode species. This parasite was encountered in the large intestine (prevalence of 100%), in the stomach (prevalence of 2%) and in the small intestine (prevalence of 7.3%). The intensity of infection ranged from 1 to 457 individual parasites per host and was positively correlated with body size of both male and female amphisbaenians. The discrepancy index (D) indicated that P. amphisbaenia tended to an even distribution in this host population. The nematode, which did not affect fat body mass, induced inflammatory infiltrations in the small intestine, indicating that the parasites might injure the host's organs. INTRODUCTION: The aim of this study was to evaluate the therapeutic response of hepatitis C in patients coinfected with human immunodeficiency virus (HIV-1). METHODS: A retrospective study of 20 patients coinfected with HIV-1/HCV who were treated in the outpatient liver clinic at the Sacred House of Mercy Foundation Hospital of Pará (Fundação Santa Casa de Misericórdia do Pará - FSCMPA) from April 2004 to June 2009. Patients were treated with 180µg PEG interferon-α2a in combination with ribavirin (1,000 to 1,250mg/day) for 48 weeks. The end point was the sustained virological response (SVR) rate (HCV RNA negative 24 weeks after completing treatment). RESULTS: The mean age of the patients was 40±9.5 years, of which 89% (n=17) were male, and the HCV genotypes were genotype 1 (55%, n=11/20), genotype 2 (10%, n=2/20) and genotype 3 (35%, n=7/20). The mean CD4+ lymphocyte count was 507.8, and the liver fibrosis stages were (METAVIR) F1 (25%), F2 (55%), F3 (10%) and F4 (10%). The early virological response (EVR) was 60%, the end-of-treatment virological response (EOTVR) was 45% and the SVR was 45%. CONCLUSIONS: The median HCV viral load was high, and in 85% of cases in which highly active antiretroviral therapy (HAART) was used, none of the patients with F3-F4 fibrosis responded to treatment. Of the twenty patients treated, 45% achieved SVR and 45% achieved EOTVR. Studies that include cases from a wider region are needed to better evaluate these findings.

The primary health care has been recognized as one of the key components of an effective health system. In its most developed form, the primary health care is the first contact with the health system and the site responsible for the organization of health care over time: individuals, their families and the general population; seeks to provide balance between the two goals of a national health system, which are improving the health of the population and provide equitable distribution of resources. Hospitalizations for primary care sensitive conditions (HPCSC) may be associated with deficiencies of service coverage primary health care or its effectiveness. Hospitalization rates can and should represent a warning sign, triggering mechanisms for analysis and search for explanations for these problems. The use of hospitalization data for HPCSC can serve as indicators of inequality in the health system, contributing to the evaluation of the deployment and implementation of health policies.

Knowledge of hospital costs is highly important for public health decision-making. This study aimed to estimate direct hospital costs related to pneumococcal meningitis in children 13 years or younger in the city of Sao Jose dos Campos, Sao Paulo State, Brazil, from January 1999 to December 2008. Data were obtained from medical records. Hospital costs were calculated according to the mixed method for measurement of quantities of items with identified costs and value attribution to items consumed (micro-costing and gross-costing). All costs were calculated according to monetary values for November 2009 and in Brazilian currency (Real). Epi Info 3.5.1 was used for frequencies and means analysis. Forty-one cases were reported. Direct hospital costs varied from R$ 1,277.90 to R$ 19,887.56 (mean = R$ 5,666.43), or 10 to 20 times the mean cost of hospitalization for other diseases. Hospital staff labor was the highest cost, followed by medication, procedures, supplies, and lab tests.

Introduction: This study aimed to isolate and identify Candida spp. from the environment, health practitioners, and patients with the presumptive diagnosis of candidiasis in the Pediatric Unit at the Universitary Hospital of the Jundiai Medical College, to verify the production of enzymes regarded as virulence factors, and to determine how susceptible the isolated samples from patients with candidiasis are to antifungal agents. Methods: Between March and November of 2008 a total of 283 samples were taken randomly from the environment and from the hands of health staff, and samples of all the suspected cases of Candida spp. hospital-acquired infection were collected and selected by the Infection Control Committee. The material was processed and the yeast genus Candida was isolated and identified by physiological, microscopic, and macroscopic attributes. Results: The incidence of Candida spp. in the environment and employees was 19.2%. The most frequent species were C. parapsilosis and C. tropicalis among the workers, C. guilliermondii and C. tropicalis in the air, C. lusitanae on the contact surfaces, and C. tropicalis and C. guilliermondii in the climate control equipment. The college hospital had 320 admissions, of which 13 (4%) presented Candida spp. infections; three of them died, two being victims of a C. tropicalis infection and the remaining one of C. albicans. All the Candida spp. in the isolates evidenced sensitivity to amphotericin B, nystatin, and fluconazole. Conclusions: The increase in the rate of hospital-acquired infections caused by Candida spp. indicates the need to take larger measures regarding recurrent control of the environment.
