Although the regular practice of physical exercise for the maintenance of the nutritional state and quality of life is important, it is not a common habit among the elderly, especially those of the lower income social bracket. The objective was to characterize and compare according to social-economic conditions and anthropometric indexes of the nutritional state of the elderly who regularly practice and those who do not practice exercises, offered at no cost, in the Northern zone of São José do Rio Preto-SP. In this study, 110 elderly women participated, of which 60 (group P) exercised regularly and 50 did not (group NP). Group P participated in a special gym program, free of charge, for at least 2 years, 2 to 3 times a week, one hour a day. Group NP was recruited from the same location as Group P. The comparison between the groups of elderly women was proportionately distributed according to marital status, level of education, means of locomotion, results of body mass index (IMC), waist to hip ratio (RCQ) and calf circumference, using the chi-square test. Comparisons were also made of average age, per capita family income, individual income, IMC, arm and triceps skin fold measured by t-Test. Differences were not found in all the comparisons made. The variables that stood out were the IMC (overweight) and RCQ (risk of chronic diseases) results, which were above what was expected, and the high frequency of walking and riding buses for both groups of elderly women (about 60%). We came to the conclusion that the low-intensity exercise program evaluated made no difference in the nutritional profile of the elderly who also used walking as a means of getting around.

Introduction: Sedentary behavior is characterized by individuals who designed much of the day sitting or lying down activities, even if such conduct regular physical activity. Over the years people tend to be less active, worsening the quality of life. For healthy aging is necessary for the individual to be constantly active, and moving activities improve mood and quality of life, though little is known of the influence of sedentary behavior in these variables. Aim: To investigate the association between sedentary time, quality of life and mood states of elderly women engaged in physical activity. Methodology: The study included 68 elderly women who belong to the Physical Activity Program for the Elderly (PROFIT), for at least 6 months. These answered the Sedentary Behaviour Questionnaire, the Pentacle Welfare and IPAQ Short Version. The Pep States List Reduced and Illustrated (LEA-RI) was applied before and after a workout and before and after three times of sedentary behaviors. To evaluate the difference of sedentary behavior time average for each domain of quality of life was conducted the analysis of the 95% confidence interval for the STATA program version 12.0 and to the moods list data a non-parametric test - test binomial using SPSS version 17.0 program and has adopted a p≤0,05. Results: A moderate intensity physical activity was the most practiced (939.5 ± 650.2 min / week) and the activity most frequently performed in sedentary time was watching television (187.6 ± 96,4min / day). In general, the quality of life for the elderly is considered good (98.39%) and the disease is most prevalent hypertension (29.47%). After a physical activity older felt less useless, less shy and less fear and after a sedentary behavior, felt less agitated and less useless. In relation between the domains of QOL and sedentary behavior time, there were no statistical differences. Conclusion: Elderly active even after a sedentary behavior...

To evaluate whether a history of falls is directly related to the quadriceps muscular function and body sway, 26 elderly women were divided on the basis of the presence or absence of a history of falls. Evaluation of muscular power and anteroposterior and mediolateral displacements of center of pressure during consecutive stand and sit 5 times were performed. Fallers exhibited higher mediolateral displacement than nonfallers. No differences were observed for quadriceps power and for sit-to-stand time between groups (P<.05). The fall history was not related to the quadriceps muscular function or to the anteroposterior displacement during sit to stand.

Background: Recent studies have shown an important reduction of joint overload during locomotion in elderly women with knee osteoarthritis (OA) after short- term use of minimalist shoes. Our aim is to investigate the chronic effect of inexpensive and minimalist footwear on the clinical and functional aspects of OA and gait biomechanics of elderly women with knee OA. Methods/Design: Fifty-six elderly women with knee OA grade 2 or 3 (Kellgren and Lawrence) are randomized into blocks and allocated to either the intervention group, which will use flexible, non-heeled shoes-Moleca (R)-for six months for at least six hours daily, or the control group, which could not use these shoes. Neither group is undergoing physical therapy treatment throughout the intervention period. Moleca (R) is a women's double canvas, flexible, flat walking shoe without heels, with a 5-mm anti-slip rubber sole and a 3-mm internal wedge of ethylene vinyl acetate. Both groups will be followed for six months and will be assessed at baseline condition, after three months, and after six months (end of intervention). All the assessments will be performed by a physiotherapist that is blind to the group allocation. The primary outcome is the pain Western Ontario and McMaster Universities Osteoarthritis (WOMAC) score. The secondary outcomes are global WOMAC score; joint stiffness and disability WOMAC scores; knee pain with a visual analogue scale; walking distance in the six-minute walk test; Lequesne score; amount and frequency (number of days) of paracetamol (500 mg) intake over six months; knee adduction moment during gait; global medical assessment score; and global patient auto-assessment score. At baseline, all patients receive a diary to record the hours of daily use of the footwear intervention; every two weeks, the same physiotherapist makes phone calls to all patients in order to verify adherence to treatment. The statistical analysis will be based on intention to treat analysis, as well as general linear models of analysis of variance for repeated measure to detect treatment-time interactions (alpha = 5%). Discussion: This is the first randomized, clinical trial protocol to assess the chronic effect of minimalist footwear on the clinical and functional aspects and gait biomechanics of elderly women with knee osteoarthritis. We expect that the use of Moleca (R) shoes for six months will provide pain relief, reduction of the knee adduction moment when walking, and improve joint function in elderly women with knee OA, and that the treatment, thus, can be considered another inexpensive and easy-to-use option for conservative OA treatment.

Este estudo aleatorizado não controlado teve como objetivo determinar o efeito de um protocolo de exercício com pesos ou aeróbico no desempenho das atividades da vida diária em mulheres idosas. Para tanto, a amostra foi constituída por 41 mulheres idosas aparentemente saudáveis na faixa etária de 60 a 85 anos de idade (x: 65,1 ± 7,9 anos) divididas aleatoriamente em grupo exercício com pesos (n: 22) ou aeróbico (n: 19). O grupo exercício com pesos consistiu na execução de três séries de oito a 12 repetições a 60% de uma repetição máxima no exercício leg press 45º. O grupo exercício aeróbico consistiu em pedalar em cicloergômetro durante 40 minutos a 60% da frequência cardíaca de reserva. Os dois protocolos foram realizados três vezes por semana durante cinco semanas. As atividades da vida diária selecionadas foram velocidade para se levantar de uma posição sentada (VLPS), velocidade para se levantar de uma posição deitada (VLPD), velocidade para subir escada (VSE) e velocidade para calçar e amarrar o tênis (VCAT). O grupo exercício aeróbico melhorou significativamente o desempenho em VCAT (19,1%), enquanto o exercício com pesos incrementou significativamente o desempenho em VSE (4,3%) e VLPS (8,9%). Os resultados deste estudo permitem concluir que tanto o exercício com pesos como o aeróbico induziram efeito positivo nas atividades da vida diária, sugerindo que ambas as modalidades de exercício devem ser associadas a um programa adequado de exercícios para a melhora da capacidade funcional de pessoas idosas.

Um dos maiores problemas de saúde pública na população idosa são as quedas, agravando-se quando relacionadas à presença de osteoporose. Dentre os vários fatores de risco, destacam-se a diminuição do equilíbrio, controle postural e força muscular. O objetivo deste trabalho foi comparar o equilíbrio, o controle postural e a força muscular em idosas osteoporóticas com e sem quedas referidas no último ano. Foram avaliadas 45 mulheres entre 65 e 85 anos, divididas em dois grupos com base no relato de quedas nos 12 meses anteriores à avaliação: grupo com quedas (GCQ; n=21) e grupo sem quedas (GSQ; n=24). O equilíbrio foi avaliado por meio da escala de equilíbrio de Berg; o controle postural pelo teste clínico modificado de interação sensorial no equilíbrio (mCTSIB), realizado no equipamento Balance Master®; e a força muscular dos flexores e extensores de joelho e dorsiflexores de tornozelo, com dinamômetro EMG System do Brasil®. Foi considerado nível de significância α=0,05. Houve diferença significativa no equilíbrio (p<0,01) e na velocidade de oscilação do Centro de Pressão (CP) durante o teste mCTSIB nas condições olhos fechados superfície estável (p=0,05) e olhos abertos superfície instável (p<0,01), com valores maiores para o GCQ. Os grupos foram semelhantes entre si em relação à força muscular (p>0,05). Nossos resultados indicam que idosas osteoporóticas com histórico de quedas nos últimos 12 meses possuem pior equilíbrio e controle postural em relação às osteoporóticas sem quedas referidas.

Introduzione: le Coliti Microscopiche, altrimenti note come Colite Collagena e Colite Linfocitica, sono disordini infiammatori cronici del colon che causano diarrea e colpiscono più frequentemente donne in età avanzata e soggetti in terapia farmacologica. Negli ultimi anni la loro incidenza sembra aumentata in diversi paesi occidentali ma la prevalenza in Italia è ancora incerta. Scopo: il presente studio prospettico e multicentrico è stato disegnato per valutare la prevalenza delle CM in pazienti sottoposti a colonscopia per diarrea cronica non ematica. Pazienti e metodi: dal Maggio 2010 al Settembre 2010 sono stati arruolati consecutivamente tutti i soggetti adulti afferenti in due strutture dell’area metropolitana milanese per eseguire una pancolonscopia. Nei soggetti con diarrea cronica non ematica sono state eseguite biopsie multiple nel colon ascendente, sigma e retto nonché in presenza di lesioni macroscopiche. Risultati: delle 8008 colonscopie esaminate 265 sono state eseguite per diarrea cronica; tra queste, 8 presentavano informazioni incomplete, 52 riscontri endoscopici consistenti con altri disordini intestinali (i.e. IBD, tumori, diverticoliti). 205 colonscopie sono risultate sostanzialmente negative, 175 dotate di adeguato campionamento microscopico (M:F=70:105; età mediana 61 anni). L’analisi istologica ha permesso di documentare 38 nuovi casi di CM (M:F=14:24; età mediana 67.5 anni): 27 CC (M:F=10:17; età mediana 69 anni) e 11 CL (M:F=4:7; età mediana 66 anni). In altri 25 casi sono state osservate alterazioni microscopiche prive dei sufficienti requisiti per la diagnosi di CM. Conclusioni: nel presente studio l’analisi microscopica del colon ha identificato la presenza di CM nel 21,7% dei soggetti con diarrea cronica non ematica ed indagine pancolonscopica negativa. Lo studio microscopico del colon è pertanto un passo diagnostico fondamentale per il corretto inquadramento diagnostico delle diarree croniche, specialmente dopo i 60 anni di età. Ampi studi prospettici e multicentrici dovranno chiarire ruolo e peso dei fattori di rischio associati a questi disordini.

The Multiple Affect Adjective Check List (MAACL) has been found to have five first-order factors representing Anxiety, Depression, Hostility, Positive Affect, and Sensation Seeking and two second-order factors representing Positive Affect and Sensation Seeking (PASS) and Dysphoria. The present study examines whether these first- and second-order conceptions of affect (based on R-technique factor analysis) can also account for patterns of intraindividual variability in affect (based on P-technique factor analysis) in eight elderly women. Although the hypothesized five-factor model of affect was not testable in all of the present P-technique datasets, the results were consistent with this interindividual model of affect. Moreover, evidence of second-order (PASS and Dysphoria) and third-order (generalized distress) factors was found in one data set. Sufficient convergence in findings between the present P-technique research and prior R-technique research suggests that the MAACL is robust in describing both inter- and intraindividual components of affect in elderly women.

Bisherige Forschung hat die Verwitwung entweder primär als soziales oder als individuelles Ereignis untersucht, selten jedoch wurden beide Perspektiven verbunden. Zudem ist wenig darüber bekannt, inwiefern bisherige Forschungsergebnisse Perioden- oder Kohorteneffekte wiederspiegeln. In diesem Beitrag wird die persönliche Bilanzierung nach der Verwitwung älterer Schweizer Frauen und Männer im Geschlechterund Zeitvergleich untersucht1. Die Datenbasis beruht auf Befragungen von 1.197 verwitweten Frauen und Männern (Alter: 65-102 Jahre), welche 1979, 1994 und 2011 durchgeführt wurden. Während sich die wirtschaftlichen und sozialen Rahmenbedingungen nach einer Verwitwung – namentlich bei Frauen – im Zeitvergleich verbessert haben, zeigen sich bezüglich psychischer Herausforderungen einer Verwitwung keine periodenspezifischen Veränderungen. Psychisch bleibt der Partnerverlust auch bei günstigen Sozialbedingungen ein kritisches Lebensereignis, das individualisiert bewältigt werden muss.

One of the major challenges in treating mental illness in Nigeria is that the health care facilities and mental health care professionals are not enough in number or well equipped to handle the burden of mental illness. There are several barriers to treatment for individual Nigerians which include the following: such as the lack of understanding of the root causes of mental illness, lack of financial support to get mental treatment, lack of social support (family, friends, neighbors), the fear of stigmatization concerning being labeled as mentally ill or being in association with the mentally ill, and the consultation of traditional native healers who may be unknowingly prolonging illness, rather than addressing and treating them due to lack of formal education and standardization of their treatments. Another barrier is the non-health nature of the mental health services in Nigeria. Traditional healers are essentially the mental health system. The elderly, women, and children are the most vulnerable groups in times of strife and hardships. Their mental well-being must be taken into account as well as their special needs in times of personal or societal crisis. ^ Nigerian mental health policy is geared toward forming a mental health system, but in actuality only a mental illness care system is the observed result of the policy. The government of Nigeria has drafted a mental health policy, yet its actual implementation into the Nigerian health infrastructure and society waits to be materialized. The limited health legislation or policy implementations tend to favor those who have access to these urban areas and the facilities' health services. Nigerians living in rural areas are at a disadvantage; many of them may not even be aware of services available to help them understand and treat mental illness. Perhaps, government driven health interventions geared toward mental illness in rural areas would reach an underserved Nigerians and Africans in general. Issues with political instability and limited infrastructure often hinder crucial financial resources and legislation from reaching the people that are truly in need of governmental leadership in regards to mental health policy.^ Traditional healers are a severely untapped resource in the treatment of mental illness within the Nigerian population. They are abundant within Nigerian communities and are meeting a real need for the mentally ill. However, much can be done to remove the barriers that prevent the integration of traditional healers within the mental health system and improve the quality of care they administer within the population. Mental illness is almost exclusively coped with through traditional medicine practices. Mobilization and education from each strata of Nigerian society and government as well as input from the medical community can improve how traditional medicine is utilized as a treatment for clinical illness and help alleviate the heavy burden of mental illness in Nigeria. Currently, there is no existing policy making structure for a working mental health system in Nigeria, and traditional healers are not taken into account in any formulation of mental health policy. Advocacy for mental illness is severely inadequate due to fear of stigmatization, with no formally recognized national of regional mental health association.^

Objetivos: Avaliar numa coorte de idosos se há diferenças entre as situações do curso de vida que contribuem para uma maior chance de sobrevida em homens e mulheres com 60 anos e mais. Métodos: 2.143 idosos entrevistados em 2000 foram acompanhados durante quase 15 anos. Utilizou-se na análise estatística o teste de associação para amostras complexas (Rao-Scott), o estimador produto limite de Kaplan-Meier, o teste de log-rank, análise univariada e multivariada do modelo de riscos proporcionais de Cox, sendo construídos, através deste último, dois modelos finais por gênero, ao nível de significância de 5 por cento . Resultados e discussão: Dos 836.204 idosos paulistanos representados pelo estudo 51,6 por cento continuavam vivos (43,0 por cento dos homens e 57,7 por cento das mulheres). Em ambos os gêneros, os/as que tinham boas condições materiais e que realizavam sem dificuldade e sem ajuda mais da metade das atividades básicas e instrumentais de vida diária (ABVD/AIVD) apresentaram maior chance de sobrevivência. O mesmo foi verificado naquelas mulheres com menos de duas limitações de determinadas doenças, nas casadas, nas que ofereciam algum tipo de ajuda e nas que realizaram ações preventivas em relação a sua condição de saúde. Entre os homens o mesmo ocorreu naqueles que se referiram ser chefe de família, nos que tinham participação comunitária, nos que residiam com algum outro morador que foi ou ia à escola, que trabalharam predominantemente como proprietário ou por conta própria e nos que relataram ter tido alguma doença em seus primeiros quinzes anos de vida. Conclusão: As chances de sobrevida são diferentes entre gêneros, e entre si, mesmo em situações aparentemente semelhantes do curso de vida vivenciados e presentes em homens e mulheres idosas.

Introdução - A abordagem da intervenção promotora da atividade física funcional em um grupo de idosas hipertensas consiste em considerar a pessoa na sua integridade física, psíquica e social. Objetivo – Avaliar o efeito da implementação de um programa de treinamento funcional sobre a pressão arterial em repouso em mulheres idosas hipertensas, submetidas ao treinamento de exercício físico durante um período de 24 meses. Métodos - Pesquisa de caráter exploratório, com orientação analítico-descritiva com a finalidade de analisar a ação da atividade física direcionada e implementada três vezes por semana no prognóstico da hipertensão arterial. Esta pesquisa foi suportada na recolha de dados da pressão arterial em repouso numa amostra de idosas brasileiras na faixa etária de 60 a 90 anos, em condições de excelente motivação por parte do grupo. Para as análises estatísticas foi utilizado o programa IBM SPSS 20, onde os dados foram tratados de forma descritiva e inferencial. Verificou-se o pressuposto de parametria quanto à normalidade dos resíduos para PAS e PAD ao longo das avaliações pressóricas na amostra pelo teste K-S, onde o mesmo não foi detectado (p<0,05). Assim, foram utilizados o teste de X2 e de Comparações Múltiplas para observações acerca de quais diferenças estatísticas eram significativas (p<0,05), além da Análise de Correlação de Pearson (rho) para apontar as devidas associações entre os níveis de PAS e PAD durante o estudo. Resultados – A maioria (100%) das idosas estavam hipertensas, constatando-se que durante o período de treino apresentaram pressão arterial sistólica e diastólica muito abaixo do padrão da normalidade para essa faixa etária. Tal decréscimo foi altamente significativo (p<00,1) e foi detectado tanto nas estatísticas comparativas quanto nas de associação. Conclusões - Após a intervenção do Projeto Prevenção de Saúde Estádio Vivo, as idosas apresentaram redução e maior controle dos valores da sua pressão arterial. Considera-se assim, que o treinamento físico ao exercer um efeito fisiológico especifica ao nível muscular e cardiocirculatório é protetor do estado de saúde global pelo que deve ser incentivado ao longo de todo o ciclo vital. Infere-se também que o Programa implementado pode ser replicado como medida de educação terapêutica, de avaliação e de auditoria de boas práticas em saúde. Palavra-chave: Idosos. Atividade Física. Purpose: Although the beneficial effects of estrogen use on cardiovascular and cognitive function in postmenopausal women have been recently discredited, controversy remains regarding its usefulness for maintaining skeletal muscle mass or strength. Therefore, the purpose of this study was to determine whether estrogen use is associated with enhanced muscle composition and, if so, whether this translates into improved strength and physical function. Methods: Cross-sectional analysis of 840 well-functioning community-dwelling white women (current estrogen replacement therapy (ERT) users = 259, nonusers = 581) aged 70-79 yr participating in the Health, Aging and Body Composition Study. Muscle composition of the midthigh by computed tomography included cross-sectional area (CSA) of the quadriceps, hamstrings, intermuscular fat and subcutaneous fat, and muscle attenuation in Hounsfield units (HU) as a measure of muscle density. Isometric hand grip and isokinetic knee extensor strength were assessed by dynamometry. Physical function was assessed using a summary scale that included usual 6-m walk and narrow walk speed, repeated chair stands, and standing balance. Results: In analyses of covariance adjusted for relevant confounders. quadriceps muscle CSA and HU were greater in Current ERT than non-ERT women (P < 0.05). Grip strength was also greater (P < 0.05) in women taking ERT while knee extensor strength approached significance (P < 0.10). However, differences in muscle composition and strength were modest at <= 3.3%. There was no difference by ERT status for the hamstring, muscles. fat CSA. or for physical function. Conclusion: The associations between ERT and muscle composition and strength were minor and did not translate into improved physical function. Initiation of ERT for preservation of muscle composition and function may not be indicated.
