Objectives: We sought to characterise the demographics, length of admission, final diagnoses, long-term outcome and costs associated with the population who presented to an Australian emergency department (ED) with symptoms of possible acute coronary syndrome (ACS). Design, setting and participants: Prospectively collected data on ED patients presenting with suspected ACS between November 2008 and February 2011 was used, including data on presentation and at 30 days after presentation. Information on patient disposition, length of stay and costs incurred was extracted from hospital administration records. Main outcome measures: Primary outcomes were mean and median cost and length of hospital stay. Secondary outcomes were diagnosis of ACS, other cardiovascular conditions or non-cardiovascular conditions within 30 days of presentation. Results: An ACS was diagnosed in 103 (11.1%) of the 926 patients recruited. 193 patients (20.8%) were diagnosed with other cardiovascular-related conditions and 622 patients (67.2%) had non-cardiac-related chest pain. ACS events occurred in 0 and 11 (1.9%) of the low-risk and intermediate-risk groups, respectively. Ninety-two (28.0%) of the 329 high-risk patients had an ACS event. Patients with a proven ACS, high-grade atrioventricular block, pulmonary embolism and other respiratory conditions had the longest length of stay. The mean cost was highest in the ACS group ($13 509; 95% CI, $11 794–$15 223) followed by other cardiovascular conditions ($7283; 95% CI, $6152–$8415) and non-cardiovascular conditions ($3331; 95% CI, $2976–$3685). Conclusions: Most ED patients with symptoms of possible ACS do not have a cardiac cause for their presentation. The current guideline-based process of assessment is lengthy, costly and consumes significant resources. Investigation of strategies to shorten this process or reduce the need for objective cardiac testing in patients at intermediate risk according to the National Heart Foundation and Cardiac Society of Australia and New Zealand guideline is required.

William J. Chambliss (Bill) is well-known for his path-breaking theories of lawmaking and for his innovative research on state-organized crime. However, rarely discussed is the fact that his study of the original vagrancy laws marked the birth of rural critical criminology. The main objective of this article is twofold: (1) to show how Bill helped shape contemporary rural critical criminology and (2) to provide suggestions for further critical theoretical and empirical work on rural crime and social control.

- Objective To compare health service cost and length of stay between a traditional and an accelerated diagnostic approach to assess acute coronary syndromes (ACS) among patients who presented to the emergency department (ED) of a large tertiary hospital in Australia. - Design, setting and participants This historically controlled study analysed data collected from two independent patient cohorts presenting to the ED with potential ACS. The first cohort of 938 patients was recruited in 2008–2010, and these patients were assessed using the traditional diagnostic approach detailed in the national guideline. The second cohort of 921 patients was recruited in 2011–2013 and was assessed with the accelerated diagnostic approach named the Brisbane protocol. The Brisbane protocol applied early serial troponin testing for patients at 0 and 2 h after presentation to ED, in comparison with 0 and 6 h testing in traditional assessment process. The Brisbane protocol also defined a low-risk group of patients in whom no objective testing was performed. A decision tree model was used to compare the expected cost and length of stay in hospital between two approaches. Probabilistic sensitivity analysis was used to account for model uncertainty. - Results Compared with the traditional diagnostic approach, the Brisbane protocol was associated with reduced expected cost of $1229 (95% CI −$1266 to $5122) and reduced expected length of stay of 26 h (95% CI −14 to 136 h). The Brisbane protocol allowed physicians to discharge a higher proportion of low-risk and intermediate-risk patients from ED within 4 h (72% vs 51%). Results from sensitivity analysis suggested the Brisbane protocol had a high chance of being cost-saving and time-saving. - Conclusions This study provides some evidence of cost savings from a decision to adopt the Brisbane protocol. Benefits would arise for the hospital and for patients and their families.

Desde 1945 la izquierda argentina no pudo resolver el enigma planteado por el hecho de que la clase obrera era peronista. Varios intentos teóricos fracasaron antes de que John William Cooke hiciera el suyo a mediados de los años sesenta. Influenciado por Gramsci, consideró que la clase obrera es una categoría histórica, por lo que el peronismo era la experiencia histórica de la clase obrera argentina y, como tal, revolucionario. Neutralizó el papel político de Perón convirtiéndolo en el símbolo de la identidad peronista. Por esto, la teoría de Cooke fue clave para que guerrilleros de izquierda se declararan peronistas sin que les importaran las opiniones e intenciones políticas de Perón. Las limitaciones de esa teoría quedaron expuestas cuando el general regresó a la Argentina y fue electo presidente por tercera vez.

We tagged a total of 14 yellowtail snapper (Ocyurus chrysurus Bloch 1790) and black grouper (Mycteroperca bonaci Poey 1860) inside the Conch Reef Research Only Area (a no-take marine reserve) in the northern Florida Keys National Marine Sanctuary in November 2001. Both species are heavily exploited in the region. Our objective was to characterize site fidelity and movement behavior along the reef tract to the north and south of the release point. Fishes were collected by baited hook and line from the surface, surgically-tagged with coded-acoustic transmitters, and returned to the reef by snorkelers. Tracking of fish movement behavior was conducted by five acoustic receivers deployed on the seafloor from Davis Reef in the south to Pickles Reef in the north. Fishes were tracked for up to eight months. Results indicated that the majority of signal detections for individual fish from both species were recorded at the two Conch Reef receivers. Limited movement from Conch Reef to Davis Reef was recorded, but no signal detections were recorded at the two sites to the north of Conch Reef. These results suggest that both species show site fidelity to Conch Reef. Future studies will seek to characterize this site fidelity with increased temporal and spatial resolution at Conch Reef. As câmaras municipais constituíram-se em um dos mais notáveis mecanismos de manutenção do vasto império ultramarino português. Originavam-se dos antigos conselhos medievais, aglutinavam os interesses das elites coloniais ao serem compostas pelos homens bons da colônia, detinham considerável poder sobre a sociedade local além de terem a liberdade de representar ao rei de Portugal seus anseios ou dificuldades. Paralelo, ao poder do senado da câmara municipal, encontravam-se as autoridades nomeadas pelo rei de Portugal: governadores coloniais. Este compartilhamento do poder na colônia gerava, muitas vezes, conflitos entre a câmara municipal e os funcionários régios. No Rio de Janeiro, setecentista, vários fatores internos e externos à colônia deterioraram as relações entre os governadores coloniais e os membros do senado.Tal situação agrava-se com as incursões corsárias francesas de 1710 e 1711 que demonstraram a fragilidade do império português que há muito deixara de ter um poder naval significativo, perdendo espaços para potências como a França, Inglaterra e Holanda. Incapaz de conter os inimigos no vasto oceano, desprovido de meios navais capazes de patrulhar os litorais de suas colônias na África, Ásia e América, em especial o do Brasil, o império português dependia cada vez mais dos recursos humanos de suas colônias para a manutenção do seu território ultramarino. A corte portuguesa sofreu duro impacto com a conquista da cidade do Rio de Janeiro por Duguay-Trouin e, ao longo dos próximos anos, procurou fortalecer o sistema defensivo de sua colônia com o envio de tropas e navios além da construção de novas fortalezas e o reaparelhamento do sistema defensivo já existente.Todo este esforço para a guerra era bancado, em sua maior parte, com recursos da própria colônia do Rio de Janeiro. Obviamente este ônus não agradava a incipiente elite mercantil que florescia na colônia resultando no fato de que a política de enclausurar o Rio de Janeiro entre muralhas e fortificações, ás custas da economia colonial, colocou em campos opostos os funcionários do rei e os membros do senado por várias vezes nas primeiras décadas do século XVIII. Surgiram inevitáveis conflitos pelo uso e posse do território urbano do Rio de Janeiro cada vez mais pontilhado por fortalezas, sulcado por extensas valas e trincheiras a impedir-lhe o crescimento urbano. Além do conflito territorial, em função da expansão da atividade mercantil desenvolvida pelos colonos, as disputas comerciais envolveram as elites locais, ávidas por lucros e impulsionadas ao comércio devido à descoberta do ouro na região das Minas, e as autoridades e comerciantes lusos, uns querendo controlar a atividade comercial que crescia em acelerado ritmo, outros querendo lucrar e disputar espaços com as elites coloniais locais. No meio destes embates encontrava-se a Câmara Municipal do Rio de Janeiro, objetivo maior desta pesquisa, a defender os interesses das elites da colônia, pois delas era representante. Era uma disputa em que, muitas vezes, seus membros pagaram com a perda da liberdade e dos seus bens frente a governadores coloniais mais intolerantes

Nesta tese abordamos alguns aspectos das inter-relações entre conhecimento, ética e valores dentro da atividade científica segundo as ideias do matemático-filósofo vitoriano William Clifford. O nosso tema geral coloca em jogo o envolvimento da produção, da avaliação e da transmissão de conhecimento científico com os comportamentos, as responsabilidades e os traços de caráter do investigador. Nosso objetivo é oferecer uma introdução ao pensamento e a algumas produções intelectuais de Clifford, um autor pouco familiar ao público filosófico brasileiro, bem como uma descrição comentada de seu escrito mais famoso, intitulado A Ética da Crença. Mediante esse objetivo, extraímos suas concepções a respeito das características e consequências éticas do empreendimento científico. As questões que orientam a tese são as seguintes: de que maneira a produção de conhecimento estaria condicionada à personalidade e ao comportamento ético de quem se lança àquela prática? Em que medida essa prática promove o cultivo de características pessoais socialmente desejáveis e favoráveis? Quais as conseqüências para a sociedade dessa inter-relação entre o caráter do investigador e os valores epistêmicos que estes colocam em ação e, sem os quais parece não ser possível a obtenção de conhecimento confiável?

William Francis Thompson (1888–1965) was a preeminent fishery scientist of the early to mid twentieth century. Educated at Stanford University in California (B.A. 1911, Ph.D. 1930), Thompson conducted pioneering research on the Pacific halibut, Hippoglossus stenolepis, from 1914 to 1917 for the British Columbia Provincial Fisheries Department. He then directed marine fisheries research for the State of California from 1917 to 1924, was Director of Investigations for the International Fisheries Commission from 1924 to 1939, and Director of the International Pacific Salmon Fisheries Commission from 1937 to 1942. He was also Director of the School of Fisheries, University of Washing-ton, Seattle, from 1930 to 1947. Thompson was the founding director in 1947 of the Fisheries Research Institute at the University of Washington and served in that capacity until his retirement in 1958. He was a dominant figure in fisheries research of the Pacific Northwest and influenced a succession of fishery scientists with his yield-based analysis of fishery stocks, as opposed to studying the fishes’environment. Will Thompson was also a major figure in education, and many of his former students attained leadership positions in fisheries research and administration.
