Purpose: To determine the evolution of fear of falling, and its relationship with gait performance after a 10-week gait and balance training program. Population and methods: Participants (N=50) were community-dwelling elderly persons enrolled voluntarily in a 10-week, low intensity, gait and balance training program. At baseline, fear of falling was assessed using a previously validated version of Tinetti's Fall Efficacy Scale (FES, range 0-120, higher score indicating higher confidence), that assesses one's confidence in performing 12 activities of daily life without falling. Gait parameters were measured over a 20m walk at preferred gait speed, using the Physilog system (Aminian K, et al., J Biomechanics, 2002). This system uses 4 kinematics sensors attached to the lower limbs and a data logger carried by the subject. Follow-up data on fear of falling and gait were collected one week after completion of the program. Results: Overall, 43 (86%) of the participants completed the program. Mean age was 78.1 years, 79% were women. At baseline, mean FES score was 98.8 (range 58-120) and mean gait speed was 0.92 m/sec (range 0.43-1.47). At follow-up, participants modestly improved their FES score (98.8±17.0 vs 103.2±16.0, P=.04) and gait speed (0.92±0.27 vs 0.99±0.26 m/sec, P<.01). In secondary analyses stratified by subject's baseline FES, those with lower than average confidence (N=21) improved significantly both FES score (84.4±11.8 vs 94.5±17.9, P<.01) and gait speed (0.79±0.26 vs 0.90±0.28 m/sec, P<.01), while no similar improvement was observed in subjects (N=22) with higher baseline confidence (112.5±6.6 vs 111.5±7.5, P=.56 and 1.03±0.22 vs 1.07±0.21 m/sec, P=.41). After adjustment for age, gender and baseline gait speed, subjects with lower baseline confidence had higher odds than the others to improve their confidence (AdjOR=10.8, 95%CI 1.8- 64.8 P=.01) and gait speed (AdjOR=3.3, 95%CI 0.6-19.7, P=.19) at follow-up. Conclusions: This pilot program of low intensity exercise modestly improved participants' fear of falling and gait speed. Interestingly, subjects with higher baseline fear of falling seemed more likely to benefit. Despite methodological limitations (pre-post comparisons, small sample), these results suggest that measuring fear of falling might be useful to better target subjects most likely to benefit from similar programs.

Los ciclones tropicales son los fenómenos climatológicos más destructivos. El PDI y otros índices que se definen aquí permiten estimar la energía que éstos poseen. Tales índices ajustan una ley de potencias en una parte de su recorrido. En el presente trabajo se procede al ajuste de las respectivas leyes de potencias de los índices para los ciclones tropicales en el norte del Océano Atlántico y noreste del Pacífico en los periodos de 1988 a 2010 y de 2001 a 2010 respectivamente, a partir de los datos registrados por el NOAA.

Fins ara, el desenvolupament del mercat interior ha patit les limitacions derivades de la manca d'una política energètica comú i, en el pla internacional, la UE, fortament dependent dels aprovisionaments externs, no ha estat capaç de parlar amb una sola veu a l'hora de negociar acords de subministrament amb els seus principals proveïdors de petroli i gas, i amb els països de trànsit. Sens dubte, la visió intergubernamentalista de la integració europea és la més indicada per explicar les mancances assenyalades en la mesura que la política energètica, especialment, en les vessants de la seguretat dels aprovisionaments i la independència energètica, forma part del nucli dur de la sobirania estatal. Només cal comprovar que, fins ara, tot i els continus avenços del mercat interior, els tractats han mantingut el vot per unanimitat del Consell pel que fa a les decisions relacionades amb l'energia, excloent, és clar, al Parlament Europeu (PE).. El Tractat de Lisboa introdueix un canvi de tendència en vincular la política energètica, alhora, al medi ambient i al "esperit de solidaritat entre els Estats membres "amb l'objectiu de garantir el funcionament del mercat de l'energia i la seguretat de l'abastament energètic en la Unió, i fomentar l'eficiència energètica i l'estalvi energètic i el desenvolupament d'energies noves i renovables i la interconnexió de les xarxes energètiques. Per primera vegada, el Parlament Europeu i el Consell establiran conjuntament (codecisió), les mesures necessàries per assolir els esmentats objectius. aquesta formulació reflecteix els canvis que està experimentant la política energètica a la Unió i que constitueixen l'objecte central d'aquest llibre: la seguretat de energètica i seva connexió amb el canvi climàtic.

ABSTRACT: BACKGROUND: The prevalence of obesity has increased in societies of all socio-cultural backgrounds. To date, guidelines set forward to prevent obesity have universally emphasized optimal levels of physical activity. However there are few empirical data to support the assertion that low levels of energy expenditure in activity is a causal factor in the current obesity epidemic are very limited. METHODS: The Modeling the Epidemiologic Transition Study (METS) is a cohort study designed to assess the association between physical activity levels and relative weight, weight gain and diabetes and cardiovascular disease risk in five population-based samples at different stages of economic development. Twenty-five hundred young adults, ages 25-45, were enrolled in the study; 500 from sites in Ghana, South Africa, Seychelles, Jamaica and the United States. At baseline, physical activity levels were assessed using accelerometry and a questionnaire in all participants and by doubly labeled water in a subsample of 75 per site. We assessed dietary intake using two separate 24-h recalls, body composition using bioelectrical impedance analysis, and health history, social and economic indicators by questionnaire. Blood pressure was measured and blood samples collected for measurement of lipids, glucose, insulin and adipokines. Full examination including physical activity using accelerometry, anthropometric data and fasting glucose will take place at 12 and 24 months. The distribution of the main variables and the associations between physical activity, independent of energy intake, glucose metabolism and anthropometric measures will be assessed using cross-section and longitudinal analysis within and between sites. DISCUSSION: METS will provide insight on the relative contribution of physical activity and diet to excess weight, age-related weight gain and incident glucose impairment in five populations' samples of young adults at different stages of economic development. These data should be useful for the development of empirically-based public health policy aimed at the prevention of obesity and associated chronic diseases.

Objetivo: Comparar la distribución de la grasa corporal mediante DEXA, en pacientes VIH+ monoinfectados y coinfectados VHC, que reciben tratamiento antirretroviral basado en inhibidores de proteasa o no nucléosidos. Materiales y Métodos: Estudio retrospectivo, observacional, unicéntrico. Resultados: 80 pacientes, 60 hombres, 20 mujeres. 41 monoinfectados, 39 coinfectados. Edad 48 años, peso 71 kg, IMC 24, 39 IP, 41 NN. Los coinfectados tenían triglicéridos, colesterol y LDL-c inferiores a los monoinfectados (p <0.05). La DEXA no mostró diferencias en la comparación de mono/coinfectados y grupos de tratamiento. Conclusiones: No se encontraron diferencias entre grupos. Los coinfectados muestran lípidos plasmáticos inferiores.

Obesity results from the organism's inability to maintain energy balance over a long term. Childhood obesity and its related factors and pathological consequences tend to persist into adulthood. A cluster of factors, including high energy density in the diet (high fat intake), low energy expenditure, and disturbed substrate oxidation, favour the increase in fat mass. Oxidation of three major macronutrients and their roles in the regulation of energy balance, particularly in children and adolescents, are discussed. Total glucose oxidation is not different between obese and lean children; exogenous glucose utilization is higher whereas endogenous glucose utilization is lower in obese compared with lean children. Carbohydrate composition of the diet determines carbohydrate oxidation regardless of fat content of the diet. Both exogenous and endogenous fat oxidation are higher in obese than in lean subjects. The influence of high fat intake on accumulation of fat mass is operative rather over a long term. Several future directions are addressed, such that a combination of increased physical activity and modification in diet composition, in terms of energy density and glycemic index, is recommended for children and adolescents.
