Correlação entre qualidade de vida e morbidade do cuidador de idoso com acidente vascular encefálico

Este estudo objetivou descrever as características sociodemográficas e a qualidade de vida dos cuidadores de idosos com histórico de acidente vascular cerebral e correlacionar o número de morbidades com os escores de qualidade de vida do cuidador. Inquérito domiciliar transversal que entrevistou cuidadores de idosos utilizando-se de instrumentos: semi-estruturado; World Health Organization Quality of Life-abreviado e Brazilian Multidimensional Functional Assessment Questionnaire. Foi realizada análise descritiva e correlação de Pearson (p<0,05). Prevaleceu o sexo feminino (93,5%), idade média de 55,4±14,17 anos, casados ou moram com companheiro (58,7%), com 4| - 8 anos de estudo (28,3%) e renda de 1 salário mínimo (34,8%). O maior escore de qualidade de vida foi no domínio relações sociais (67,57) e, menor no meio ambiente (54,82). O maior número de morbidades do cuidador correlacionou-se com menores escores em todos os domínios de qualidade de vida.

In the last 15 years, the therapeutical options for the treatment of chronic inflammatory diseases in rheumatology have increased a lot. Nevertheless, some patients do not respond or respond partially to the current therapies--including to the biologics therapy. Tofacitinib (Xeljanz) is now on the Swiss market. It inhibits the JAK pathway. Tofacitinib--as monotherapy or with methotrexate--improves the control of rheumatoid arthritis (RA). In a comparative study, tofacitinib was as effective as adalimumab. Further, tofacitinib reduced structural damages in RA and is considered as an alternative, in case of non-response, to anti-TNF and probably to other biologics therapy. The side effects are upper respiratory tract and opportunist infections and tuberculosis. Blood count, lipids, kidney function, liver tests, CK and blood pressure have to be monitored.

Hoje vivemos as consequências sociais e ambientais de 30 anos de descontrolado crescimento populacional e económico. A expansão da AML, centralizada e polarizada na capital, desencadeou, nos anos 60-80, uma crescente ocupação periféricos de Lisboa. A par da ocupação terciária do centro da cidade, desenvolveu-se um processo de expansão e de densificação social das periferias. Por outro lado, o forte crescimento populacional, bem como a ausência de políticas urbanas consistentes, estimularam um crescimento metropolitano desordenado, em “mancha de óleo” e, consequentemente fortemente desqualificado em termos sociais e urbanísticos.

BACKGROUND: Cytomegalovirus (CMV) replication has been associated with more risk for solid organ graft rejection. We wondered whether this association still holds when patients at risk receive prophylactic treatment for CMV. METHODS: We correlated CMV infection, biopsy-proven graft rejection, and graft loss in 1,414 patients receiving heart (n=97), kidney (n=917), liver (n=237), or lung (n=163) allografts reported to the Swiss Transplant Cohort Study. RESULTS: Recipients of all organs were at an increased risk for biopsy-proven graft rejection within 4 weeks after detection of CMV replication (hazard ratio [HR] after heart transplantation, 2.60; 95% confidence interval [CI], 1.34-4.94, P<0.001; HR after kidney transplantation, 1.58; 95% CI, 1.16-2.16, P=0.02; HR after liver transplantation, 2.21; 95% CI, 1.53-3.17, P<0.001; HR after lung transplantation, 5.83; 95% CI, 3.12-10.9, P<0.001. Relative hazards were comparable in patients with asymptomatic or symptomatic CMV infection. The CMV donor or recipient serological constellation also predicted the incidence of graft rejection after liver and lung transplantation, with significantly higher rates of rejection in transplants in which donor or recipient were CMV seropositive (non-D-/R-), compared with D- transplant or R- transplant (HR, 3.05; P=0.002 for liver and HR, 2.42; P=0.01 for lung transplants). Finally, graft loss occurred more frequently in non-D- or non-R- compared with D- transplant or R- transplant in all organs analyzed. Valganciclovir prophylactic treatment seemed to delay, but not prevent, graft loss in non-D- or non-R- transplants. CONCLUSION: Cytomegalovirus replication and donor or recipient seroconstellation remains associated with graft rejection and graft loss in the era of prophylactic CMV treatment.

Do high levels of human capital foster economic growth by facilitating technology adoption? If so, countries with more human capital should have adopted more rapidly the skilled-labor augmenting technologies becoming available since the 1970 s. High human capital levels should therefore have translated into fast growth in more compared to less human-capital-intensive industries in the 1980 s. Theories of international specialization point to human capital accumulation as another important determinant of growth in human-capital-intensive industries. Using data for a large sample of countries, we find significant positive effects of human capital levels and human capital accumulation on output and employment growth in human-capital-intensive industries.

Ancient asexuals directly contradict the evolutionary theories that explain why organisms should evolve a sexual life history. The mutualistic, arbuscular mycorrhizal fungi are thought to have been asexual for approximately 400 million years. In the absence of sex, highly divergent descendants of formerly allelic nucleotide sequences are thought to evolve in a genome. In mycorrhizal fungi, where individual offspring receive hundreds of nuclei from the parent, it has been hypothesized that a population of genetically different nuclei should evolve within one individual. Here we use DNA-DNA fluorescent in situ hybridization to show that genetically different nuclei co-exist in individual arbuscular mycorrhizal fungi. We also show that the population genetics techniques used in other organisms are unsuitable for detecting recombination because the assumptions and underlying processes do not fit the fungal genomic structure shown here. Instead we used a phylogenetic approach to show that the within-individual genetic variation that occurs in arbuscular mycorrhizal fungi probably evolved through accumulation of mutations in an essentially clonal genome, with some infrequent recombination events. We conclude that mycorrhizal fungi have evolved to be multi-genomic.

Human exploitation of the forest in the northwest of São Paulo State has generated enormous fragmentation of that forest. Such disturbance has reduced the populations of insects in general. This work was a survey of social wasps (Hymenoptera, Vespidae; Polistinae) in three areas in different stages of regeneration: Paulo de Faria - SP (435 ha), Pindorama - SP (128 ha) and Neves Paulista - SP (1 ha). These three areas were chosen for comparative purposes. To capture the wasps, it was used: active collecting with attractant liquid (solution of water, salt and sugar) with the aid of a dorsal spray bag. During the period from July to December 2006, 414 social wasps were collected in Paulo de Faria, constituting seven species belonging to four genera; 111 social wasps were collected in Pindorama, constituting six species belonging to four genera, and 129 social wasps were collected in Neves Paulista constituting 12 species belonging to seven genera. In order to compare these three areas ecological indexes were calculated. Neves Paulista had the greatest diversity, and Paulo de Faria presented greater abundance. These factors were probably caused by neighboring areas and ecological corridors, which were limited in Paulo de Faria and Pindorama.
