Aiming at demonstrating a decrease of acquired immunity after chemotherapeutic cure, a group of mice was infected with 25 Schistosoma mansoni cercariae (LE strain). A part of these animals was treated with 400 mg/kg oxamniquine, at 120 days after infection. Challenge infections were carried out at 45, 90 and 170-day-intervals after treatment (185, 210 and 290 days after primoinfection, respectively). Recovery of worms at 20 days after reinfections showed that a residual immunity remains up to 90 days after treatment, and disappears at 170 days after cure. Using the ELISA method, it was possible to detect a decrease of antibody levels (total IgG) in the treated group, when antigens from different evolutive stages of S. mansoni were used. The epidemiological implications of the present results, and the possible mechanisms involved in the decrease of acquired immunity after treatment are discussed.

Plague caused by Yersinia pestis, has persisted in Brazil in several natural foci spread throughout rural areas in the States of Ceara, Paraiba, Pernambuco, Piaui, Rio Grande do Norte, Alagoas, Bahia, Minas Gerais and Rio de Janeiro. Nationwide surveillance of plague in Brazil based on serological testing started in 1983. We now present an update report of the examinations carried out in our laboratory from 1983 to 1992. The passive hemagglutination test for antibodies against fraction 1A antigen of Y. pestis and the passive hemagglutination inhibition control were employed for testing a total of 220,769 sera. Samples analyzed included 2,856 sera from clinically diagnosed plague cases or suspects, 49,848 sera from rodents of 24 species and 2 species of small wild carnivores (marsupials), 122,890 sera from dogs, and 45,175 sera from cats. Specific antibodies were found in 92 (3.22%) human sera; 143 (0.29%) sera from rodents of 8 species and from the two species of marsupials, 1,105 (0.90%) sera from dogs and 290 (0.64%) sera from cats. The presence of significant levels of specific anti-F1A antibodies among rodents and wild or domestic carnivores (dogs and cats) indicates that all the Brazilian plague foci remain active in spite of the absence of human cases in some of them.

This paper presents the applicability of a reinforcement learning algorithm based on the application of the Bayesian theorem of probability. The proposed reinforcement learning algorithm is an advantageous and indispensable tool for ALBidS (Adaptive Learning strategic Bidding System), a multi-agent system that has the purpose of providing decision support to electricity market negotiating players. ALBidS uses a set of different strategies for providing decision support to market players. These strategies are used accordingly to their probability of success for each different context. The approach proposed in this paper uses a Bayesian network for deciding the most probably successful action at each time, depending on past events. The performance of the proposed methodology is tested using electricity market simulations in MASCEM (Multi-Agent Simulator of Competitive Electricity Markets). MASCEM provides the means for simulating a real electricity market environment, based on real data from real electricity market operators.

The study of electricity markets operation has been gaining an increasing importance in the last years, as result of the new challenges that the restructuring process produced. Currently, lots of information concerning electricity markets is available, as market operators provide, after a period of confidentiality, data regarding market proposals and transactions. These data can be used as source of knowledge to define realistic scenarios, which are essential for understanding and forecast electricity markets behavior. The development of tools able to extract, transform, store and dynamically update data, is of great importance to go a step further into the comprehension of electricity markets and of the behaviour of the involved entities. In this paper an adaptable tool capable of downloading, parsing and storing data from market operators’ websites is presented, assuring constant updating and reliability of the stored data.

Cryptococcus neoformans is an opportunistic fungal pathogen that causes meningoencephalitis as the most frequent clinical presentation in immunocompromised patients, mainly in people infected by HIV. This fungus is an environmental encapsulated yeast, commonly found in soil enriched with avian droppings and plant material. A total of 290 samples of pigeon and the other avian droppings, soil, ornamental trees and vegetable material associated with Eucalyptus trees were collected to study environmental sources of Cryptococcus species in Goiânia, Goiás State. The determination of varieties, serotypes and the susceptibility in vitro to fluconazole, itraconazole and amphotericin B of C. neoformans isolates were performed. C. neoformans var. grubii (serotype A) was found in 20.3% (36/177) of pigeon dropping samples and in 14.3% (5/35) of samples of Eucalyptus. None of the environmental isolates of C. neoformans showed in vitro resistance to three antifungal agents. The knowledge of major route for human cryptococcal infection (inhalation of infectious particles from saprophytic sources) and a total of 60 C. neoformans isolates obtained from AIDS patients with cryptococcal meningitis between October 2001 and April 2002 justify the study of the habitats of these yeasts as probable sources of cryptococcosis in this city.

Traumatic lesions of the femoral veins during varicose veins surgery are very uncommon and they raise several therapeutic difficulties. They occur in 1/1000 cases of varicose veins operated. The authors describe four cases of lesions of this type that were seen and treated for the last six years, probably because there was an increase in the number of varicose veins operated on in the Lisbon area. The authors discuss the surgical options and they concluded that these complications can be minimized with good anatomical and surgical skills, specially of the saphenous-femoral and saphenous-popliteal junctions.

Slowed atrial conduction may contribute to reentry circuits and vulnerability for atrial fibrillation (AF). The autonomic nervous system (ANS) has modulating effects on electrophysiological properties. However, complex interactions of the ANS with the arrhythmogenic substrate make it difficult to understand the mechanisms underlying induction and maintenance of AF. AIM: To determine the effect of acute ANS modulation in atrial activation times in patients (P) with paroxysmal AF (PAF). METHODS AND RESULTS: 16P (9 men; 59±14years) with PAF, who underwent electrophysiological study before AF ablation, and 15P (7 men; 58±11years) with atrioventricular nodal reentry tachycardia, without documentation or induction of AF (control group). Each group included 7P with arterial hypertension but without underlying structural heart disease. The study was performed while off drugs. Multipolar catheters were placed at the high right atrium (HRA), right atrial appendage (RAA), coronary sinus (CS) and His bundle area (His). At baseline and with HRA pacing (600ms, shortest propagated S2) we measured: i) intra-atrial conduction time (IACT, between RAA and atrial deflection in the distal His), ii) inter-atrial conduction time (interACT, between RAA and distal CS), iii) left atrial activation time (LAAT, between atrial deflection in the distal His and distal CS), iv) bipolar electrogram duration at four atrial sites (RAA, His, proximal and distal CS). In the PAF group, measurements were also determined during handgrip and carotid sinus massage (CSM), and after pharmacological blockade of the ANS (ANSB). AF was induced by HRA programmed stimulation in 56% (self-limited - 6; sustained - 3), 68.8% (self-limited - 6; sustained - 5), and 50% (self-limited - 5; sustained - 3) of the P, in basal, during ANS maneuvers, and after ANSB, respectively (p=NS). IACT, interACT and LAAT significantly lengthened during HRA pacing in both groups (600ms, S2). P with PAF have longer IACT (p<0.05), a higher increase in both IACT, interACT (p<0.01) and electrograms duration (p<0.05) with S2, and more fragmented activity, compared with the control group. Atrial conduction times and electrograms duration were not significantly changed during ANS stimulation. Nevertheless, ANS maneuvers increased heterogeneity of the local electrograms duration. Also, P with sustained AF showed longer interACT and LAAT during CSM. CONCLUSION: Atrial conduction times, electrograms duration and fractionated activity are increased in PAF, suggesting a role for conduction delays in the arrhythmogenic substrate. Acute vagal stimulation is associated with prolonged interACT and LAAT in P with inducible sustained AF and ANS modulation may influence the heterogeneity of atrial electrograms duration.

To evaluate the short and mid-term results of prostatic artery embolization in patients with benign prostatic embolization. Retrospective study between March 2009 and June 2011 with 103 patients (mean age 66.8 years, 50-85) that met our inclusion criteria with symptomatic benign prostatic hyperplasia. The clinical outcome was evaluated by the International Prostate Symptom Score (IPSS), quality of life (QoL), International Index of Erectile Function, prostate volume (PV), prostate-specific antigen (PSA), peak urinary flow (Q(max)), and post-void residual volume (PVR) measurements at 3 and 6 months, 1 year, 18 months, and 2 years after PAE and comparison with baseline values was made. Technical and clinical successes, as well as poor clinical outcome definitions, were previously defined. In this review, we evaluate the short and mid-term clinical outcomes and morbidity of patients treated only with non-spherical polyvinyl alcohol. Six months after the procedure, the PV decreased about 23%, IPSS changed to a mean value of 11.95 (almost 50% reduction), the QoL improved slightly more than 2 points, the Q(max) changed to a mean value of 12.63mL/s, the PVR underwent a change of almost half of the baseline value, and the PSA decreased about 2.3ng/mL. In the mid-term follow-up and comparing to the baseline values, we still assisted to a reduction in PV, IPSS, QoL, PVR, and PSA, and an increase in Q(max). Prostatic Artery Embolization is a safe procedure with low morbidity that shows good short- and mid-term clinical outcome in our institution.

RESUMO:Introdução: Reviu-se o conhecimento epidemiológico, fisiopatológico e clínico atual sobre a doença coronária, da sua génese até ao evento agudo, o Enfarte Agudo do Miocárdio (EAM). Valorizou-se, em especial, a teoria inflamatória da aterosclerose, que foi objeto de grandes desenvolvimentos na última década. Marcadores de instabilidade da placa aterosclerótica coronária: Aprofundou-se o conhecimento da placa aterosclerótica coronária instável. Descreveram-se detalhadamente os biomarcadores clínicos e laboratoriais associados à instabilidade da placa, com particular ênfase nos mecanismos inflamatórios. Objetivos:Estão divididos em dois pontos fundamentais:(1) Estudar em doentes com EAM a relação existente entre as moléculas inflamatórias: Interleucina-6 (IL-6), Fator de Necrose Tumoral-α (TNF-α) e Metaloproteinase de Matriz-3 (MMP3), não usados em contexto clínico, com um marcador inflamatório já em uso clínico: a Proteína C-Reativa ultrassensível (hs-CRP). Avaliar a relação de todas as moléculas inflamatórias com um biomarcador de lesão miocárdica: a Troponina Cardíaca I (cTnI). (2) Avaliar, no mesmo contexto de EAM, a Resposta de Fase Aguda (RFA) . Pretende-se demonstrar o impacto deste fenómeno, com repercussão clínica generalizada, no perfil lipídico e nos biomarcadores inflamatórios dos doentes. Métodos:(1) Estudo observacional prospetivo de doentes admitidos consecutivamente por EAM (grupo EAM) numa única unidade coronária, após exclusão de trauma ou infeção. Doseamento no sangue periférico, na admissão, de IL-6, TNF-α, MMP3, hs-CRP e cTnI. Este último biomarcador foi valorizado também nos valores séricos obtidos 6-9 horas depois. Procedeu-se a correlação linear (coeficiente de Pearson, de Rho-Spearman e determinação do R2) entre os 3 marcadores estudados com os valores de hs-CRP e de cTnI (valores da admissão e 6 a 9 horas após). Efetuou-se o cálculo dos coeficientes de regressão linear múltipla entre cTnI da admissão e cTnI 6-9h após, com o conjunto dos fatores inflamatórios estudados. (2) Estudo caso-controlo entre o grupo EAM e uma população aleatória de doentes seguidos em consulta de cardiologia, após exclusão de eventos cardiovasculares de qualquer território (grupo controlo) e também sem infeção ou trauma. Foram doseados os mesmos marcadores inflamatórios no grupo controlo e no grupo EAM. Nos dois grupos dosearam-se, ainda, as lipoproteínas: Colesterol total (CT), Colesterol HDL (HDLc), com as suas subfrações 2 e 3 (HDL 2 e HDL3), Colesterol LDL oxidado (LDLox),Triglicéridos (TG), Lipoproteína (a) [Lp(a)], Apolipoproteína A1 (ApoA1), Apolipoproteína B (ApoB) e Apolipoproteína E (ApoE). Definiram-se, em cada grupo, os dados demográficos, fatores de risco clássicos, terapêutica cardiovascular e o uso de anti-inflamatórios. Procedeu-se a análise multivariada em relação aos dados demográficos, fatores de risco e à terapêutica basal. Compararam-se as distribuições destas mesmas caraterísticas entre os dois grupos, assim como os valores séricos respetivos para as lipoproteínas estudadas. Procedeu-se à correlação entre as moléculas inflamatórias e as lipoproteínas, para todos os doentes estudados. Encontraram-se os coeficientes de regressão linear múltipla entre cada marcador inflamatório e o conjunto das moléculas lipídicas, por grupo. Finalmente, efetuou-se a comparação estatística entre os marcadores inflamatórios do grupo controlo e os marcadores inflamatórios do grupo EAM. A regressão múltipla marcadores inflamatórios/perfil lipídico (grupo EAM) foi: hs-CRP (R=0,647, R2=0,418) p=0,004; IL-6 (R=0,544, R2=0,300), p=0,073; MMP3 (R=0,539, R2=0,290) p=0,089; TNF-α (R=0,595; R2=0,354) p=0,022. Da comparação entre os marcadores inflamatórios dos dois grupos resultou (mediana/intervalo interquartil, grupo controlo vs EAM): hs-CRP (0,19/0,27 vs 0,42/2,53 mg/dl) p=0,001, IL-6 (4,90/5,48 vs 13,07/26,41 pg/ml) p<0,001, MMP3 (19,70/13,70 vs 10,10/10,40 ng/ml) p<0,001;TNF-α (8,67/6,71 vs 8,26/7,80 pg/dl) p=0,805. Conclusões: (1) Nos doentes com EAM, existe correlação entre as moléculas inflamatórias IL-6, MMP3 e TNF-α, quer com o marcador inflamatório hs-CRP, quer com o marcador de lesão miocárdica cTnI. Esta correlação reforça-se para os valores de cTnI 6-9 horas após admissão, especialmente na correlação múltipla com o grupo dos quatro marcadores inflamatórios. (2) IL-6 está inversamente ligada às lipoproteínas de colesterol; hs-CRP e IL-6 têm excelentes correlações com o perfil lipídico valorizado no seu conjunto. No grupo EAM encontram-se níveis séricos mais reduzidos para as lipoproteínas de colesterol. Para TNF-α não foram encontradas diferenças significativas entre os grupos, as quais foram observadas para a IL-6 e hs-CRP (mais elevadas no grupo EAM). Os valores de MMP3 no grupo controlo estão mais elevados. ------------- ABSTRACT: Introduction: We reviewed the epidemiology, pathophysiology and current clinical knowledge about coronary heart disease, from its genesis to the acute myocardial infarction (AMI). The inflammatory theory for atherosclerosis, which has undergone considerable development in the last decade, was especially detailed. Markers of coronary atherosclerotic vulnerable plaque: The clinical and laboratory biomarkers associated with the unstable coronary atherosclerotic plaque vulnerable plaque are detailed. An emphasis was placed on the inflammatory mechanisms. Objectives: They are divided into two fundamental points: (1) To study in AMI patients, the relationship between the inflammatory molecules: Interleukin-6 (IL-6), Tumor Necrosis Factor-α (TNF-α) and Matrix metalloproteinase-3 (MMP3), unused in the clinical setting, with an inflammatory marker in clinical use: ultrasensitive C-reactive protein (hs-CRP), as well as a biomarker of myocardial injury: cardiac troponin I (cTnI). (2) To study, in the context of AMI, the Acute Phase Response (APR). We intend to demonstrate the impact of that clinical relevant phenomenon in the lipid profile and inflammatory biomarkers of our patients. Methods: (1) Prospective observational study of patients consecutively admitted for AMI (AMI group) in a single coronary care unit, after exclusion of trauma or infection. A peripheral assay at admission for IL-6, TNF-α, MMP3, hs-CRP and cTnI was performed. The latter was also valued in assays obtained 6-9 hours after admission. Linear correlation (Pearson's correlation coefficient, Spearman Rho's correlation coefficient and R2 regression) was performed between the three markers studied and the values of hs-CRP and cTnI (on admission and 6-9 hours after admission). Multiple linear regression was also obtained between cTnI on admission and 6-9h after, with all the inflammatory markers studied. (2) Case-control study between the AMI group and a random population of patients from an outpatient cardiology setting (control group). Cardiovascular events of any kind and infection or trauma were excluded in this group. The same inflammatory molecules were assayed in control and AMI groups. The following lipoproteins were also assayed: total cholesterol (TC), HDL cholesterol (HDLc) and subfractions 2 and 3 (HDL2 and HDL 3), oxidized LDL cholesterol (oxLDL), Triglycerides (TG), Lipoprotein (a) [Lp(a)], Apolipoprotein A1 (apoA1), Apolipoprotein B (ApoB) and Apolipoprotein E (ApoE). Demographics, classical risk factors, cardiovascular therapy and the use of anti-inflammatory drugs were appreciated in each group. The authors conducted a multivariate analysis with respect to demographics, risk factors and baseline therapy. The distribution of the same baseline characteristics was compared between the two groups, as well as the lipoprotein serum values. A correlation was performed between each inflammatory molecule and each of the lipoproteins, for all the patients studied. Multiple linear regression was determined between each inflammatory marker and all the lipid molecules per group. Finally, the statistical comparison between the inflammatory markers in the two groups was performed. The multiple linear regression admission cTnI/inflammatory markers produced: (R=0.638, R2=0.407) p<0.001 and 6-9 h cTnI/inflammatory markers (R=0.780, R2=0.609) p<0.001. (2) Significances of the multivariate analysis were found for age (p=0.029), IMC>30 (p=0.070), Aspirin (p=0.040) and group (p=0.002). Important differences between the baseline data of the two groups (control group vs AMI): age (47.95 ± 11.55 vs 68.53±12.70 years) p<0.001; gender (18.18 vs 22.80%) p=0.076; diabetes mellitus (9.09% vs 36. 84%) p=0.012; Aspirin (18.18 vs. 66.66%) p<0.001; Clopidogrel (4, 54% vs 66.66%) p=0.033; Statins, 31.81% vs 66.14%, p=0.078, beta-blockers 18.18% vs 56.14%, p=0.011; anti-inflammatory drugs (4.54% vs 33.33%) p=0.009. Significant differences in the lipid pattern of the two groups (mean±SD or median/interquartile range, control group vs AMI): TC (208.45±35.03 vs 171.05±41.63 mg/dl) p<0.001; HDLc (51.50/18.25 vs 42.00/16.00 mg/dl) p=0.007; HDL2 (8.50/3.25 vs 10.00/6.00 mg/dl) p=0.292; HDL3 (41.75±9.82 vs 31.75±9.82 mg/dl) p<0.01; oxLDL (70.00/22.0 vs 43.50/21.00 U/L) p <0.001; TG (120.00/112.50 vs 107.00/86.00 mg/dl) p=0.527; Lp(a) (0.51/0.73 vs 0,51/0.50 g/L) p=0.854; apoA1 (1.38±0.63 vs 1.19±0.21 g/L) p=0.002; ApoB (0.96± 0.39 vs 0.78±0.28 g/L) p=0.004; ApoE (38.50/10,00 vs 38.00 /17,00 mg/L) p=0.574. In the linear correlations between inflammatory variables and lipid variables for all patients, we found a negative relationship between IL-6 and TC, HDLc, HDL3, ApoA1 and ApoB. The multiple linear regression inflammatory markers/lipid profile (control group) was: hs-CRP (R= 0.883, R2=0.780) p=0.022; IL6 (R=0.911, R2=0.830) p=0.007; MMP3 (R=0.498, R2=0.248) p=0.943; TNF-α (R=0.680, R2=0.462) p=0.524. For the linear regression inflammatory markers/lipid profile (AMI group) we found: hs-CRP (R=0.647, R2=0.418) p=0.004; IL-6 (R=0.544, R2=0.300) p=0.073; MMP3 (R=0.539, R2 =0.290) p=0.089; TNF-α (R=0.595, R2=0.354) p=0.022. The comparison between inflammatory markers in both groups (median/interquartile range, control group vs AMI) resulted as: hs-CRP (0.19/0.27 vs 0.42/2.53 mg/dl) p=0.001; IL-6 (4.90/5.48 vs 13.07/26.41 pg/ml) p<0.001; MMP3 (19.70/13.70 vs 10.10/10.40 ng/ml) p<0.001; TNF-α (8.67/6.71 vs 8.26/7.80 pg/dl) p=0.805. Conclusions: (1) In AMI patients there is a correlation between the inflammatory molecules IL-6, TNF-α and MMP3 with both the inflammatory marker hs-CRP and the ischemic marker cTnI. This correlation is strengthened for the cTnI at 6-9h post admission, particularly in the multiple linear regression to the four inflammatory markers studied. (2) IL-6 correlates negatively with the cholesterol lipoproteins. Hs-CRP and IL-6 are strongly correlated to the whole lipoprotein profile. AMI patients display reduced serum lipid levels. For the marker TNF-α no significant differences were found between groups, which were observed for IL-6 and hs-CRP (higher in the AMI group). MMP3 values are higher in the control group.

Objectivo: Determinar os factores de risco, características clínicas e desfecho da queratite por acantamoeba. Métodos: Os autores revêem os casos de queratite por acantamoeba em 6 doentes (3 do sexo masculino e 3 do sexo feminino) com idades compreendidas entre os 18 e os 41 anos, vigiados no Departamento de Córnea do nosso Hospital. As suas características e curso clínicos foram estudados retrospectivamente. Resultados: Todos os doentes eram portadores de lentes de contacto. Quatro dos 6 casos tiveram início do quadro entre final de Maio e Julho. A suspeita inicial de queratite bacteriana, fúngica ou viral precedeu o diagnóstico final de queratite por acantamoeba em todos os doentes. O diagnóstico foi feito através de observação clínica, PCR (polymerase chain reaction) ou microscopia confocal. Em todos os casos a terapêutica dirigida foi iniciada com propamidina, com adição de clorohexidina em um doente. Em 3 doentes verificou-se necessidade de realização de queratoplastia penetrante: um caso por incapacidade de controlo da infecção e por sequelas noutros dois. Conclusão: É necessário um grande nível de suspeita de infecção por acantamoeba em indivíduos utilizadores de lente de contacto e falência inicial à terapêutica antimicrobiana ou antiviral. A microscopia confocal é um auxiliar importante no diagnóstico da queratite por este microrganismo.

Objective: To report a case of retinal toxicity associated with efavirenz in an adult. Methods: We describe a case of gradual-onset blurry vision in both eyes in a 37-year-old HIV Caucasian woman, on antiretroviral therapy (ART), including efavirenz. Results: The patient presented with a best corrected visual acuity of 20/100 for the right eye (RE) and 20/125 for the left eye (LE). Fundoscopy revealed mottled atrophic changes of the macular retinal pigment epithelium (RPE) in both eyes. Fluorescein angiography revealed an annular pattern of RPE atrophy in both eyes. Full-field electretinography (ERG) was normal. Conclusions: Based on our patient’s history and on previous reports, efavirenz seems to be the culprit in this case. Our report provides evidence in support of routine ophthalmological evaluation of patients on efavirenz.

Os autores apresentaram informações sobre 6.556 triatomíneos capturados na área pela SUCAM, de 1964 a 1974, os quais apresentavam T. cruzi em 4,7%. O estudo atual revelou 1.290 exemplares em 69 localidades trabalhadas, 63,8% com triatomíneos, das quais 52,3% apresentavam triatomíneos infectados, o que significa 33,3% da área. Foram pesquisados ecótopos artificiais distante até 200 metros das casas. Dos 37 ecótopos naturais pesquisados, 73,6% eram habitados por morcegos, 23,7% por preás e 5,2% por punarés, espécies que apresentam variáveis taxas de infecção por T. cruzi. O T. brasiliensis corresponde a 65,5% dos triatomíneos capturados e 942% dos triatomíneos infectados são desta espécie, considerada a principal transmissora de Doença de Chagas na região. Em ecótopos naturais é a única espécie encontrada (10 vezes em 55, os triatomíneos estavam infectados). Dentro das habitações a espécie é menos abundante e menos infectada (2,7%). A segunda espécie é o T. pseudomaculata, (34,3%) considerada sem importância na transmissão da Doença de Chagas; a terceira espécie é o P. megistus, da qual foram capturados apenas 2 exemplares (0,2%).

INTRODUCTION: Mitral regurgitation (MR) is the most common valvular disease and has recently become the target of a number of percutaneous approaches. The MitraClip is virtually the only device for which there is considerable experience, with more than 20,000 procedures performed worldwide. OBJECTIVE: To describe our initial experience of the percutaneous treatment of MR with the MitraClip device. METHODS: We describe the first six MitraClip cases performed in this institution (mean age 58.5 ± 13.1 years), with functional MR grade 4+ and New York Heart Association (NYHA) heart failure class III or IV (n=3), with a mean follow-up of 290 ± 145 days. RESULTS: Procedural success (MR ≤ 2+) was 100%. Total procedure time was 115.8 ± 23.7 min, with no in-hospital adverse events and discharge between the fourth and eighth day, and consistent improvement in the six-minute walk test (329.8 ± 98.42 vs. 385.33 ± 106.95 m) and in NYHA class (three patients improved by two NYHA classes). During follow-up there were two deaths, in two of the four patients who had been initially considered for heart transplantation. CONCLUSION: In patients with functional MR the MitraClip procedure is safe, with both a high implantation and immediate in-hospital success rate. A longer follow-up suggests that the clinical benefit decreases or disappears completely in patients with more advanced heart disease, namely those denied transplantation or on the heart transplant waiting list.

O estado nutricional de 290 crianças, desde o nascimento até a idade de uns seis anos, da população Yanomami do Médio Rio Negro, Amazonas, foi observado em 1998 e 1999. Essa população tem um modo de viver tradicional e dispõe de um largo espaço para as atividades de caça e de colheita. O índice peso/altura foi comparado aos valores de referência do National Center for Health Statistics. Todas essas crianças são de pequena morfologia. Uma desnutrição (índice <= -2 SD) foi observada em 20 delas, ou seja 7%, cinco apresentando uma forma grave (<= -3 SD). A importância dos contatos exteriores não parece influenciar significativamente nas diferenças que existem entre as comunidades. Esses dados descrevem uma situação sem penúria de alimentação. Os casos de desnutrição parecem ser o resultado conjunto de infecções e de uma situação particular de rejeição do grupo.
