Los objetivos de la tesis son: 1.- Estudiar la relación entre la incidencia y mortalidad por cáncer y los factores medioambientales, en particular la contaminación atmosférica, controlando por factores socioeconómicos. 2.- Utilizar aquellos métodos de estadística espacial apropiados para cada tipo de diseño. 3.- Distinguir en los modelos las diferentes fuentes de extra-variabilidad espacial. 4.- Controlar el problema de exceso de ceros inherente a alguna de las neoplasias de interés medioambientales. Conclusiones: - Tanto la incidencia como la mortalidad de las neoplasias, presentaron dos fuentes de extravariación. La extravariaicón espacial, por la que unidades vecinas tienden a presentar razones de incidencia/mortalidad similares, y la heterogeneidad no espacial. En general la extravariabilidad espacial ha resultado ser mucho mayor que la no espacial. - Para suavizar las RIE/RME correspondientes a variables con un porcentaje de ceros superior al40-50% debe utilizarse un modelo que capture este comportamiento. - El mejor modelo en términos de ajuste para recoger el exceso de ceros en las variables de interés ha resultado ser el modelo mixto de riesgo relativo. - Las RIE/RME suavizadas presentan un patrón geográfico claro sólo en algunas neoplasias de interés medioambiental. - Parte de la variabilidad remanente en las RIE/RME suavizadas pudo ser explicada mediante la introducción de variables explicativas, en particular la contaminación atmosférica y variables socioeconómicas. -Como los contaminantes atmosféricos fueron observados en un diseño geoestadístico y las neoplasias de interés mediambiental lo fueron en un diseño en rejilla se modelizó la superficie de exposición. - El efecto del contaminante en cada municipio/sección censal se aproximó introduciendo en el modelo el valor promedio en cada área y la variabilidad intra-área. - El efecto del contaminante se consideró aleatorio, en el sentido de que podría ser diferente en cada una de las áreas. - Las condiciones socioeconómicas fueron otra de las variables que redujeron la variabilidad remanente en las RIE/RME suavizadas. -Las variables explicativas observadas con un diseño en rejilla, como el índice de privación, se introdujeron en el modelo como efectos fijos. - El efecto de la privación sobre la incidencia y/o mortalidad por cáncer de tráquea, bronquios y pulmón, controlando por contaminantes atmosféricos, fue mayor en las mujeres que en los hombres. -Altas concentraciones de contaminantes atmosféricos aumentan el riesgo de padecer neoplasias de interés medioambiental, controlando por condiciones socioeconómicas.

Objective: Evaluation of selective decontamination of the digestive tract (SDD) on late mortality in ventilated trauma patients in an intensive care unit (ICU). Methods: A multicenter, randomized controlled trial was undertaken in 401 trauma patients with Hospital Trauma Index-Injury Severity Score of 16 or higher. Patients were randomized to control (n = 200) or SDD (n = 201), using polymyxin E, tobramycin, and amphotericin B in throat and gut throughout ICU treatment combined with cefotaxime for 4 days. Primary endpoint was late mortality excluding early death from hemorrhage or craniocerebral injury. Secondary endpoints were infection and organ dysfunction. Results: Mortality was 20.9% with SDD and 22.0% in controls. Overall late mortality was 15.3% (57/372) as 29 patients died from cerebral injury, 16 SDD and 13 control. The odds ratio (95% confidence intervals) of late mortality for SDD relative to control was 0.75 (0.40-1.37), corresponding to estimates of 13.4% SDD and 17.2% control. The overall infection rate was reduced in the test group (48.8% vs. 61.0%). SDD reduced lower airway infections (30.9% vs. 50.0%) and bloodstream infections due to aerobic Gram-negative bacilli (2.5% vs. 7.5%). No difference in organ dysfunction was found. Concluson: This study demonstrates that SDD significantly reduces infection in multiple trauma, although this RCT in 401 patients was underpowered to detect a mortality benefit.

Measurements of weighted dietary intakes and plasma determinations of albumin, iron, zinc, ascorbic acid and TIBC were carried out on twenty female multiple sclerosis patients in a long-stay hospital for disabled people. The group included ten patients with a recent history of pressure sores, closely matched with ten patients without pressure sores. Mean daily intake of carbohydrate was found to be higher in the non-pressure sore group whilst intake of zinc was lower in this group. Intakes of all other nutrients were comparable between the two groups. For both groups, intakes of energy, folate, vitamin D, iron and zinc were less than recommended values. Mean plasma levels of albumin and iron were towards the lower limit of the normal range, whilst that for zinc was considerably less than the normal range. Plasma TIBC was slightly above the normal range. Levels of plasma iron and zinc were significantly lower in the pressure sore group. The data indicate that severely disabled hospitalized patients with multiple sclerosis may be at risk of poor nutritional status. The results suggest that in the presence of pressure sores, there are increased requirements for specific nutrients, notably zinc and iron. Consideration is given to the possible value of supplementation of these individuals.

Calculations using a numerical model of the convection dominated high latitude ionosphere are compared with observations made by EISCAT as part of the UK-POLAR Special Programme. The data used were for 24–25 October 1984, which was characterized by an unusually steady IMF, with Bz < 0 and By > 0; in the calculations it was assumed that a steady IMF implies steady convection conditions. Using the electric field models of Heppner and Maynard (1983) appropriate to By > 0 and precipitation data taken from Spiroet al. (1982), we calculated the velocities and electron densities appropriate to the EISCAT observations. Many of the general features of the velocity data were reproduced by the model. In particular, the phasing of the change from eastward to westward flow in the vicinity of the Harang discontinuity, flows near the dayside throat and a region of slow flow at higher latitudes near dusk were well reproduced. In the afternoon sector modelled velocity values were significantly less than those observed. Electron density calculations showed good agreement with EISCAT observations near the F-peak, but compared poorly with observations near 211 km. In both cases, the greatest disagreement occurred in the early part of the observations, where the convection pattern was poorly known and showed some evidence of long term temporal change. Possible causes for the disagreement between observations and calculations are discussed and shown to raise interesting and, as yet, unresolved questions concerning the interpretation of the data. For the data set used, the late afternoon dip in electron density observed near the F-peak and interpreted as the signature of the mid-latitude trough is well reproduced by the calculations. Calculations indicate that it does not arise from long residence times of plasma on the nightside, but is the signature of a gap between two major ionization sources, viz. photoionization and particle precipitation.

A global, time-dependent, three-dimensional, coupled ionosphere-thermosphere model is used to predict the spatial distribution of non-thermal plasma in the F-layer. It is shown that, even for steady-state conditions with Kp as low as 3, the difference between the ion and neutral velocities often exceeds the neutral thermal speed by a factor, D', which can be as large as 4. Theoretically, highly non-Maxwellian, and probably toroidal, ion velocity distributions are expected when D' exceeds about 1.5. The lack of response of the neutral winds to sunward ion drifts in the dawn sector of the auroral oval cause this to be the region most likely to contain toroidal distributions. The maximum in D' is found in the throat region of the convection pattern, where the strong neutral winds of the afternoon sector meet the eastward ion flows of the morning sector. These predictions are of interest, not only to radar scientists searching for non-thermal ionospheric plasma, but also as one possible explanation of the initial heating and upward flows of ions in the cleft ion fountain and nightside auroral oval, both of which are a major source of plasma for the magnetosphere.

Some vocal disorders in teachers are associated with occupational factors, but there are few studies that analyze the influence of vocal habits, fluid intake, mastication, and sleep on these disorders. The objective was to analyze the Occurrence of vocal fatigue, hoarseness, and dry throat in elementary and high school teachers and their association with vocal habits, fluid intake, mastication, and sleep. A sample of 422 elementary and secondary school teachers was Studied using a specific questionnaire. The multiple regression analysis showed that hoarseness was associated with absence of water intake (odds ratio (OR) = 1.7; P = 0.047), yelling/speaking loudly (OR = 1.6; P = 0.058), jaw-opening limitations (OR = 3.8; P = 0.003). average of: 6 hours of sleep/light (OR = 1.7; P = 0.039), and waking-up feeling replenished (OR = 2.0; P = 0.020). The presence of vocal fatigue was significantly associated with yelling/speaking loudly (OR = 2.2: P = 0.013), speaking excessively (OR = 2.4; P = 0.023), difficulty to open the mouth to masticate (OR = 6.6; P = 0.003), less than 6 hours of sleep (OR = 4.0; P = 0.008), and waking-up feeling replenished (sometimes OR = 2.8: P = 0.003; or never OR = 3.3 P = 0.002). The presence of dry throat was associated with being a former smoker (OR = 3.3; P = 0.011) and having jaw-opening limitations (OR = 3.9; P = 0.021). In recent years, speech and hearing interventions with teachers have focused on health-care promotion actions and prevention of vocal disorders, prioritizing issues related with hydration and healthy vocal use habits. However, the findings in the present study show the need to further focus on lifestyle habits related to sleep and eating habits.

This study focuses on morphological and molecular data analyses, misidentifications, and phylogenetic inconsistencies regarding Bradypus variegatus (the brown-throated sloth) and B. tridactylus (the pale-throated sloth). Misidentifications were recorded on 75 of 313 museum specimens of Bradypus. Almost 90% of the misidentified specimens were B. variegatus from north-central Brazil, erroneously attributed to B. tridactylus. These misidentified specimens are reported in taxonomic reviews as the southernmost records of B. tridactylus. A history of confusing nomenclature regarding sloth species exists, and these particular misidentifications could be attributable to the similarity in face and throat color between B. variegatus from north-central Brazil and B. tridactylus. The molecular phylogeny of morphologically confirmed sloth specimens exhibits 2 monophyletic lineages representing B. variegatus and B. tridactylus. The split time between these 2 lineages was estimated at 6 million years ago (mya), contradicting previous studies that estimated this divergence to be 0.4 mya. Taxonomic inconsistencies were detected when comparing the molecular phylogeny to previously published DNA sequences ascribed to B. tridactylus. Misidentification or introgression could underlie such phylogenetic incongruities. Regardless of their causes, these discrepancies lead to misstatements regarding geographic distribution, phylogeny, and taxonomy of B. variegatus and B. tridactylus.

Micronozzles with piezoelectric actuator were fabricated and investigated. The micronozzles were fabricated in glass substrates using a powder-blasting technique, and the actuator is a bimorph structure made from a piezoelectric polymer. The actuator was located at the nozzle outlet, and was driven in an oscillating mode by applying an alternating voltage across the actuator electrodes. With a pressure difference between inlet and outlet, the gas flow rate through the device was increased. This effect was quantified, and compared to a similar micronozzle with no actuator. The increase in the flow rate was defined as the gas flow through the micronozzle with actuator oscillating minus the gas flow without actuator, was found to depend on the inlet pressure, the pressure ratio, and the nozzle throat diameter. Syftet med denna litteraturstudie var att beskriva vilka omvårdnadsåtgärder som kan förebygga uppkomsten av trycksår samt vilken förmåga att identifiera patienter som riskerar att utveckla trycksår sjuksköterskan har med de bedömningsinstrument som finns. De vetenskapliga artiklarna (n = 20) som ingick i studien söktes via Högskolan Dalarnas databaser Blackwell Synergy, PubMed och ELIN@Dalarna. Sökorden som användes i olika kombinationer var pressure sore, pressure ulcers, nursing, routines, treatment, knowledge, methods, management, measures, risk assessment, scales, risk patients och prevention/preventing. Inklusionskriterierna var att de skulle vara empiriska, skrivna på svenska eller engelska samt vara publicerade efter 1997. Resultatet visade att det finns många olika förebyggande åtgärder som vidtas. I preventivt syfte var evidensbaserade bedömningsinstrument ett bra och användbart verktyg. Även tryckreducerande underlag som luft- eller trögskumsmadrasser, fårskinn eller vattenfyllda handskar användes som trycksårspreventiva åtgärder. Lägesändringar med hjälp av vändscheman samt tidig mobilisering var också förebyggande åtgärder som vidtogs. Patienternas näringsstatus har stor betydelse i det preventiva arbetet. Resultat visade att det förekommer näringstillägg som en förebyggande strategi. Kontinuerlig inspektion av hud samt noggrann hygien var även något som utfördes i preventivt syfte. Det finns en mängd olika bedömningsinstrument och resultatet av litteraturstudien visar att resultaten vad gällde skalornas förmåga att förutse trycksår varierade mellan olika studier. Uppsatsförfattarna anser att trycksårsprevention är viktigt då trycksår orsakar lidande för patienten och är kostsamma för samhället. Preventiva rutinmässiga åtgärder borde utföras inom de flesta patientgrupper och då är bedömningsinstrumenten en viktig del i det arbetet.

Background and aims: The population of older people in our society is increasing. Agerelated changes in the skin results in a diminished perception of pain and pressure and a decreased microcirculation in the skin affects its ability to adapt to injury. Occurrence of pressure sore on geriatrikal clinics are 5-10%, witch means that between five and ten thousand patients gets daily treat for pressure sores. When the patient gets a pressure sore the need for help increases. A common apprehension is that if the patient’s affects with pressure sores it’s because of deficiency in care. According to the law, all nursing interventions should be performed according to scientific and evidence and the nurse’s assistants are responsible for how they perform. The aim of this study was to examine how much knowledge the nurses assistants in community care services has about preventing, predicting and locate riskfactors for pressure sores and if they get the right education. Methods: A questionnaire based on 20 questions was maid and used for this purpose. Out of 99 persons the questionnaires was answered bye 65 nurses assistants working in community care service in a small town in Sweden. Results: The results shown that the nurses assistants don’t use risk assessment scales in attempt to identify patients vulnerable to pressure sores and they are not well associated with the riskfactors. The study even shows that they have little knowledge in how to prevent pressure sores from appearing. The nursing model are some times out of date and the nurses assistants personal view attends to decide witch care they will perform instead of scientific and evidenced based nursing.

The genetic and environmental factors that affect age at first conception (AFC) of gilts were evaluated, using 466 observations of Dalland animals (C-40). The software program MTDFREML was used to assess the genetic parameters on an animal model that included as fixed effect, contemporary group and, as random effects, the additive genetic, the common litter and the error. Mean value of AFC was 241.12±25.9 days, with CV of 8.5 %. Variance analysis showed that year (P<0.01), season (P<0.01) and herd effects (P<0.01) were important sources of variation of AFC. The throat effect (P=0.34) was not important for the AFC, but the mother of the gilts (P<0.01) was an important source of variation. The heritability estimate was 0.44±0.14 that this trait would yield genetic gain as a response to the selection.

OBJETIVO: o presente trabalho propôs-se a comparar o perfil tegumentar pós-operatório de pacientes Classe II, Padrão Face Longa, submetidos ao tratamento ortodôntico-cirúrgico, com os parâmetros descritos na análise cefalométrica de Legan e Burstone. METODOLOGIA: 32 telerradiografias pós-cirúrgicas, com um mínimo de 6 meses de acompanhamento, foram submetidas a traçado manual (repetido 4 vezes) e digitalização (também repetida 4 vezes) no programa DFPlus para análise cefalométrica. RESULTADOS: os resultados permitiram verificar que 9 das 11 medidas avaliadas encontravam-se estatisticamente diferentes da norma avaliada; contudo, ao se verificar o desvio padrão permitido na norma, os achados deste trabalho situam-se dentro da mesma, com exceção do ângulo mentocervical. CONCLUSÃO: as condições experimentais deste estudo permitiram concluir que, embora a população estudada tenha obtido resultados estéticos-funcionais satisfatórios, não se enquadrou nas normas da análise de Legan e Burstone, o que indica que a avaliação após a cirurgia ortognática deve ser principalmente clínica e que a estética facial não está totalmente relacionada com as medidas préestabelecidas na análise cefalométrica.

Medicamentos homeopáticos como o Symphytum officinalle e a Calendula officinallis são dotados de propriedades anti-sépticas, antiinflamatória, cicatrizantes e também agem como promotores da consolidação de fraturas ósseas. Neste trabalho, uniram-se esses dois medicamentos similares em um complexo para verificar o seu efeito no reparo em feridas de extração dentária em camundongos. O complexo Symphytum officinalle e Calendula officinallis nas potências de 6CH e 3CH, respectivamente, foi ministrado por via oral ao grupo tratado durante 5 dias antes e após a extração do incisivo superior direito. No grupo controle, administraram-se 5ml de álcool etílico a 70% diluídos em 30 ml de soro fisiológico. Após a proservação, os animais foram sacrificados, a maxila direita separada da esquerda, fixada e processada para inclusão em parafina. Após a microtomia, os cortes obtidos foram corados pela H/E. A análise histológica mostrou que, tanto no grupo controle como no tratado, o alvéolo dentário estava preenchido por tecido de granulação e tecido ósseo neoformado, com graus variáveis de maturação, rico em osteócitos. No entanto, nos animais tratados, o processo de reparo em feridas após extração dentária do incisivo superior direito mostrou um avanço progressivo de neoformação óssea mais acentuado quando comparado ao grupo controle, em tempos equivalentes. Estes resultados enfatizam as propriedades biológicas do complexo Symphytum officinalle e Calendula officinallis e sua possível utilização como recurso terapêutico na Odontologia.
