This study examined the clinical, histological, and immunohistochemical features as well as the differential diagnoses of oral intravascular papillary endothelial hyperplasia (IPEH) to aid clinicians and pathologists in its diagnosis. Clinical features of five oral IPEH cases were obtained from medical records, and all histopathological diagnoses were reviewed. Immunohistochemical reactions, including anti-CD-34, laminin, vimentin, estrogen receptor alpha, and Ki-67, were assessed. Microscopically, a reactive proliferation of vascular cells composed of small papillary structures with hypocellular and hyalinized cores arising in an organized thrombus was seen. CD-34, vimentin, and laminin staining were strongly positive, while estrogen receptor alpha was negative in all cases. A low percentage of cells were positive for Ki-67 in four of five lesions, but one case was strongly positive. A diagnosis of angiosarcoma was investigated and rejected. IPEH presents specific microscopic characteristics that along with clinical data lead to an accurate diagnosis. The general dentist, the first to participate in the diagnostic process, must share the responsibility for diagnosis with the pathologist, and they must work together to determine the correct diagnosis and management. Oral lesions of IPEH are uncommon. Their main significance is that they show a microscopic resemblance to angiosarcoma. Thus, clinicians should have more information regarding this benign entity. Finally, we suggest that in recurrent cases exhibiting strong immunolabeling of proliferative markers the possibility of angiosarcoma should be investigated.

Ameloblastic fibrosarcoma (AFS), regarded as the malignant counterpart of the benign ameloblastic fibroma, is an extremely rare odontogenic neoplasm with only 68 cases reported in the English literature up to 2009. It is composed of a benign odontogenic epithelium, resembling that of ameloblastoma, and a malignant mesenchymal part exhibiting features of fibrosarcoma. Due to the rarity of the lesion, little is known about its molecular pathogenesis; therefore, in the current study, we sought to evaluate the immunoexpression of Ki67, proliferative cell nuclear antigen, and Bcl-2 proteins in AFS, comparing the results obtained with its benign counterpart, as well as to report a new case of this rare entity affecting a 19-year-old female patient. The results obtained revealed that all the proteins evaluated were overexpressed in the malignant mesenchymal portion of AFS if compared with ameloblastic fibroma, suggesting that nuclear proliferative factors such as Ki67 and proliferative cell nuclear antigen, in association to histopathologic features, may be useful markers for identifying the malignancy and that, despite the lack of molecular analysis in the case reported, Bcl-2 alteration may play a role in AFS pathogenesis. Intravascular papillary endothelial hyperplasia: Report of 4 cases with immunohistochemical findings

Intravascular papillary endothelial hyperplasia (IPEH) is a benign endothelial proliferation, usually intravascular, that may mimic angiosarcoma. In this report, four new cases of IPEH involving the oral region are described. The affected sites were the lower lip, labial comissure and the submandibular region. After clinical evaluation, the complete removal of the lesions showed a circumscribed and soft mass. Histologically, the major feature was a reactive proliferation of endothelial cells composed of small papillary structures with hypocellular and hyalinized cores arising in an organized thrombus. Immunohistochemical staining for CD34 was strongly positive in endothelial cells. Vimentin and laminin immunolabelling were also consistent with a vascular origin. In order to verify the proliferative potential of the lesions, the Ki-67 antibody was used, revealing low percentage of labeled cells (<20%). No immunoreactivity for GLUT-1 was observed. Since the complete removal is curative, no additional treatment was necessary, and no signs of recurrence had been observed until now. Due to the particular features of IPEH, it is important for pathologists and clinicians to become familiar with this lesion. Additionally, the specific histological arrangement, including the absence of cellular pleomorphism, mitotic activity and necrosis, represents a guide to help in the differential diagnosis. Moreover, the vascular origin and the proliferative index should be assessed by immunohistochemistry in order to provide an accurate diagnosis.

Objective: To investigate the presence and distribution of substance P (SP) and neurokinin I receptor (NK-IR) in oral squamous cell carcinoma (OSCC) and their relationship with proliferation. Patients and Methods: Ninety OSCCs from 73 patients were immunohistochemically analyzed using monoclonal antibodies against SP, NK-IR and Ki-67 in a case and control study. Results: Seventy-one percent (n=49) of cases expressed SP on tumour cell membrane, 81.3% (n=69) in cytoplasm, 39.4% (n=28) in nucleus, 81.6% (n=71) in infiltrating lymphocytes, and 58.1% (n=43) in peritumoural or intratumoural blood vessels; 14% (n=12) of cases expressed NK-1R on tumour cell membrane, 50% (n=43) in cytoplasm, 48.3% (n=42) in infiltrating lymphocytes and 22.5% (n=18) in tumour blood vessels. All cases expressed Ki-67, which was expressed in >25% of tumour cells in 79.8% of cases (n=63). Direct significant associations were observed in SP expression between different tissue levels (p<0.01), between SP and NK-IR tumour cell membrane expression (p<0.01), and between joint,SP and NK-IR expression in tumour cell cytoplasm and a higher expression of Ki-67 (p<0.05). Conclusion: The ubiquitous presence of SP strongly suggests a role for SP/NK-1R complex in tumour development and progression and possibly for NK-IR antagonists, such as L-773060, in the management of patients with oral cancer.

The aim of the present study was to evaluate the in vitro osteogenic potential of subcultured human osteoblastic cells derived from alveolar bone on a titanium (Ti) surface produced by an anodized alkali treatment (BSP-AK). Primary osteoblastic cells were subcultured on BSP-AK and machined Ti discs (control) and grown for periods of up to 21 days under osteogenic conditions. Morphologic and biochemical methods were used to assess important parameters of in vitro bone-like tissue formation. Although no major differences were observed between the BSP-AK and the control Ti surface in terms of cell attachment and mineralized matrix formation, a significant increase in cell population, ALP activity, and collagen content was detected in cultures on BSP-AK surface. Our results demonstrate that human osteoblastic cells are sensitive to the BSP-AK-modified Ti surface during the transitional stage between the end of the proliferative phase and the onset of the differentiation /matrix maturation ones. Together with the good mechanical properties exhibited by the Ca- and P- coating, our findings suggest that BSP-AK treatment could be useful for the development of a new surface for dental and orthopedic implants. Cells from patients with the genetic disorder ataxia-telangiectasia (A-T) are hypersensitive to ionizing radiation and radiomimetic agents, both of which generate reactive oxygen species capable of causing oxidative damage to DNA and other macromolecules. We describe in A-T cells constitutive activation of pathways that normally respond to genotoxic stress, Basal levels of p53 and p21(WAF1/CIP1), phosphorylation on serine 15 of p53, and the Tyr15-phosphorylated form of cdc2 are chronically elevated in these cells. Treatment of A-T cells with the antioxidant alpha -lipoic acid significantly reduced the levels of these proteins, pointing to the involvement of reactive oxygen species in their chronic activation. These findings suggest that the absence of functional ATM results in a mild but continuous state of oxidative stress, which could account for several features of the pleiotropic phenotype of A-T.

OBJETIVO: Neste trabalho foram avaliadas as características histológicas e imunohistoquímicas (Ki-67 e CK-16) do colesteatoma espontâneo do meato acústico externo comparando-as com as do colesteatoma adquirido da orelha média. FORMA DE ESTUDO: Caso controle. MATERIAL E MÉTODO: Fragmentos de colesteatoma do meato acústico externo foram submetidos a estudo histológico e imunohistoquímico com o intuito de verificar a expressão da CK16 e do antígeno nuclear Ki-67 nas células de sua matriz, e os resultados foram comparados com os encontrados no colesteatoma adquirido da orelha média. RESULTADOS: Os aspectos histológico e imunohistoquímico quanto à presença da CK16 e do antígeno nuclear Ki-67 na matriz do colesteatoma do meato acústico externo foram idênticos àqueles encontrados no colesteatoma adquirido da orelha média. DISCUSSÃO: Acreditamos que a ocorrência do colesteatoma do meato acústico externo seja devido ao comportamento anormal de células com potencial hiperproliferativo no epitélio do meato. Este potencial estaria relacionado com a presença da CK16 em uma região onde estes marcadores não são usualmente encontrados. Esta característica hiperproliferativa do colesteatoma do meato fica patente pela presença do antígeno nuclear Ki-67 nas células suprabasais de sua epiderme. Possivelmente esta doença deve ser desencadeada pela interação desta citoqueratina (CK16) com citocinas presentes no tecido conjuntivo subepitelial inflamado, como a TGF-µ. CONCLUSÃO: As características histológicas (presença de cones epiteliais) e histoquímicas (expressão da CK16 e do antígeno nuclear KI-67) são idênticas no colesteatoma adquirido da orelha média e no colesteatoma espontâneo do meato acústico externo.

Amyotrophic lateral sclerosis (ALS) is a fatal neurodegenerative disease characterized by the pro-gressive loss of motoneurons (MN). Increasing evidence points glial cells as key players for ALS onset and progression. Indeed, MN-glia signalling pathways involving either neuroprotection or inflammation are likely to be altered in ALS. We aimed to study the molecules related with glial function and/or reactivity by evaluating glial markers and hemichannels, mainly present in astrocytes. We also studied molecules involved in mi-croglia-MN dialogue (CXCR3/CCL21; CX3CR1/CX3CL1; MFG-E8), as well as proliferation (Ki-67) and inflammatory-related molecules (TLR2/4, NLRP3; IL-18) and alarming/calming signals (HMGB1/autotaxin). We used lumbar spinal cord (SC) homogenates from mice expressing a mutant human-SOD1 protein (mSOD1) at presymptomatic and late-symptomatic ALS stages. SJL (WT) mice at same ages were used as controls. We observed decreased expression of genes associated with astrocytic (GFAP and S100B) and microglial (CD11b) markers in mSOD1 at the presymptomatic phase, as well as diminished levels of gap junction components pannexin1 and connexin43 and expression of Ki-67 and decreased autotax-in. In addition, microglial-MN communication was negatively affected in mSOD1 mice as well as in-flammatory response. Interestingly, we observed astrocytic (S100B) and microglial (CD11b) reactivity, increased proliferation (Ki-67) and increased autotaxin expression in symptomatic mSOD1 mice. In-creased MN-microglial dialogue (CXCR3/CCL21; CX3CR1/CX3CL1; MFG-E8) and hemichannel activ-ity, namely connexin43 and pannexin1, were also observed in mSOD1 at the symptomatic phase, along with an elevated inflammatory response as indicated by increased levels of HMGB1 and NLRP3. Our results suggest that decreased autotaxin expression is a feature of the presymptomatic stage, and precede the network of pro-inflammatory-related symptomatic determinants, including HMGB1, CCL21, CX3CL1, and NLRP3. The identification of the molecules and signaling pathways that are dif-ferentially activated along ALS progression will contribute for a better design of therapeutic strategies for disease onset and progression.

Müllerian adenosarcoma with sarcomatous overgrowth presented by a 52-year-old female patient after adjuvant tamoxifen treatment for breast carcinoma is described. The diagnosis was made on histological basis after curettage and complementary total hysterectomy with bilateral salpingo-oophorectomy. The immunohistochemical study showed high expression of estrogen receptors in the epithelial component of the lesion and irregularly positive findings in the stroma. The proliferative activity evaluated by Ki-67 immunoexpression was higher in the stroma than the epithelium. Some of the stromal cells showed rhabdomyoblastic differentiation. The association of tamoxifen use and development of mesenchymal neoplasms is discussed.

La poliaminas son moléculas esenciales para el crecimiento, diferenciación y transformación celular. Se encuentran presentes en todo tipo de células y sus niveles endógenos están perfectamente controlados. Estudios recientes indican que los niveles de la arginina regularían el metabolismo de las poliaminas putrescina y espermidina. Este aminoácido produce óxido nítrico (ON), segundo mensajero implicado en la respuesta inmune, en la neurotransmisión y en el proceso inflamatorio. En este tipo de trabajo se propone desarrollar un sistema de detección para determinar la concentración de ON en tiempos cortos que permita conocer su relación con el metabolismo de las poliaminas. Por otra parte, el papel específico de las poliaminas en la regulación del ciclo celular es desconocido y en nuestro laboratorio se está estudiando el rol de estas moléculas en el ciclo de células normales y malignas; se propone determinar la expresión de la ciclina D1 a lo largo de este ciclo y su implicancia en situaciones de carencia de poliaminas endógenas. También se han relacionado a las poliaminas con la apoptosis o muerte celular programada (MCP), por lo que se intentará estudiar las variaciones durante el ciclo celular de la MCP de células normales y malignas y su relación a las poliaminas. Los resultados de estos estudios ampliarán el conocimiento del rol de las poliaminas y permitirá diseñar experimentos utilizando inhibidores de estas moléculas como anticancerígenos. Desde el punto de vista biotecnológico, el sistema detección de ON posibilitará determinar su concentración en tiempos cortos, aportando información de su rol metabólico. Esta metodología podrá aplicarse en otros sistemas.

El carcinoma basocelular (CBC) representa el tumor maligno más frecuente de la piel en personas de raza blanca. Incide en mayores de 50 años de edad e infiltra los tejidos, pero rara vez ocasiona metástasis. En trabajos anteriores, observamos imágenes de cuerpos apoptóticos con MOAR (microscopía óptica de alta resolución), en pieles portadoras de CBC. Todas las células eucariotas poseen una maquinaria enzimática que interviene en la apoptosis. Estas enzimas son las proteincinasas pertenecientes a la familia de las caspasas. Se sintetizan como procaspasas, las cuales una vez activadas, actúan sobre otra caspasa en una reacción secuencial en cadena. Se reconoce que la apoptosis juega un rol importante en el balance y mantenimiento de los tejidos. Hipótesis: -Investigar si la expresión de metaloproteasas y caspasas en las células del CBC promueven y contribuyen a la progresión y extensión tumoral. Objetivos:-Profundizar el conocimiento de la conducta biológica del CBC, en referencia a la relación de los patrones de crecimiento macro y microscópicos del tumor, enfatizando en la expresión de metaloproteasas (colagenasa-3 y streptomelisina), CD-31, Ki-67, oncoproteína bcl-2 y caspasas, mediante la implementación de técnicas de inmunohistoquímica. -Analizar a nivel morfológico la presencia de cuerpos apoptóticos por medio de MOAR y determinar si influyen en la conducta biológica del tumor. Materiales y Métodos: Se realizará un estudio retrospectivo y prospectivo de pacientes asistidos en el Servicio de Dermatología del Hospital Nacional de Clínicas, desde el año 2000. Se utilizarán tomas incisionales de piel, que se fijarán en formol neutro al 10 por ciento y luego serán procesadas e incluidas en parafina y coloreadas con H-E (hematoxilina-eosina). Especimenes seleccionados, se utilizarán para técnicas de MOAR e inmunocitoquímica. El material para MOAR, se fijará en Karnovsky enfriado a pH 7,2 y se incluirá en resinas epóxicas, seccionados con un ultramicrótomo Porter BluMT1 (1micra) y coloreados con P.A.S. azul de toluidina, fuscina básica y metenamina-plata. La aplicación de las técnicas inmunohistoquímicas, se utilizaran sobre cortes desparafinados con xilol, que permitirán investigar en forma retrospectiva y prospectiva la matriz extracelular (MEC) y los elementos neoplásicos. Los anticuerpos monoclonales a utilizar serán: Colageno tipo IV clone CIV 22 M0785 monoclonal antibody (DAKO), CD44 clone E29 M0613 monoclonal antibody (DAKO), Oncoproteina bcl-2 clone 124 M0887 monoclonal antibody (DAKO), CD31: clone 35 BH11 MO631 monoclonal antibody (DAKO) K-167 clone PC10 M0879 monoclonal antibody.(DAKO) bclx y Caspasa 8. Metodología Estadística Los resultados serán evaluados mediante: a) Varianza Anova; para datos con distribución gausiana como media, SD. b) Krusal Walles; para rasgos de distribución no gausiana. c) Chi2 para determinar la diferencia significativa para proporciones. Resultados esperados: Del análisis histomorfológico cualitativo y cuantitativo de biopsias de piel con CBC, aportar determinaciones que clarifiquen la conducta biológica de este tumor.- Importancia del Proyecto: El CBC es una entidad que está aumentando su incidencia en forma exponencial, debido sobre todo a pautas sociales. No sólo ocasiona trastornos en los pacientes, sino que también, representa gastos en el sistema de salud. Asimismo, significa un desafío para la investigación. Esto permite explorar aspectos de la biología molecular, a partir de una entidad que posee características únicas.

Entre los tumores frecuentes en los caninos, se encuentra el denominado Tumor Venéreo Transmisible (TVT). El origen celular exacto del TVT no está claro, aunque estudios inmunohistoquímicos muestran las características de compatibilidad con células histiocíticas. En nuestra región se diagnostican con frecuencia aunque no se tienen datos precisos de su incidencia. El diagnóstico precoz es la mejor herramienta utilizada tanto en medicina veterinaria como humana para el control de tumores. El tumor puede ser muy agresivo localmente, rara vez metastiza. El manejo terapéutico no ha sido sencillo. Se han probado distintos tratamientos como cirugía, radioterapia, bioterapia y quimioterapia- La quimioterapia ha demostrado ser el tratamiento mas efectivo y la droga mas utilizada es el sulfato de vincristina, pero el tiempo del tratamiento hasta la remisión completa varía ampliamente entre individuos. Si bien existen estadificaciones clínicas y diagnósticos citológicos e histológicos que permiten establecer el grado de malignidad, los mismos tienen componentes subjetivos por su naturaleza cualitativa. Los marcadores tumorales se han utilizado especialmente en el establecimiento de un pronóstico acertado. Entre estos están las Regiones Organizadoras Nucleolares (NORs), que pueden colorearse con tinciones argénticas (AgNOR). dichas regiones corresponden a segmentos de cromosomas que contienen genes ribosomales. Por otro lado, existen marcadores de proliferación celular como el anticuerpo monoclonal Ki 67, el cual detecta un antígeno nuclear que se expresa en las células que entran al ciclo celular (fases G1, S, G2 y mitosis), proporcionando una medida directa de la fracción de crecimiento del tejido. El uso de estos marcadores fue eficiente en patología humana, mientras en veterinaria existen escasos reportes La mayoría de los marcadores se han usado sobre muestras histológicas, existiendo escasa información de su uso en muestras citológicas. Conocer el diagnóstico y el pronóstico del tumor relacionándolo con el tratamiento instaurado permitiría predecir las expectativas y calidad de vida del paciente. El contar en Medicina Veterinaria con una herramienta cuantitativa como son los marcadores tumorales que soporte las observaciones subjetivas que aporta el diagnóstico citomorfológico, podría contribuir a establecer de manera confiable el pronóstico del paciente con TVT, así como también definir el modelo terapéutico. Por esto sería importante la búsqueda de técnicas rápidas y seguras que permitan establecer pronósticos cada vez más precisos.

El limfoma de cèl•lules de mantell (LCM) és un limfoma de cèl•lules B incurable que presenta sobreexpressió de ciclina D1. Això fa necessari el desenvolupament de noves teràpies. Els gens supressors de tumors estan alterats en càncer pel silenciament epigenètic aberrant, com a conseqüència de la desacetilació de les histones dels seus promotors. Els inhibidors de les desacetilases d'histones (HDACi) són nous compostos amb resultats prometedors per al tractament de tumors. L'objectiu principal, i que ha durat 7 mesos, va ser analitzar l'activitat antitumoral de l'àcid hidroxàmic suberoilanílid (SAHA, vorinostat), un HDACi en fase d'assajos clínics per al tractament de varis tumors, en cèl•lules de LCM. Es va analitzar la sensibilitat al SAHA (Merck Pharmaceuticals) en nou línies cel•lulars humanes de LCM, que es diferenciaven en les alteracions genètiques, les característiques replicatives i la sensibilitat als fàrmacs; i cèl•lules primàries de 6 pacients. El SAHA va presentar un efecte citotòxic heterogeni amb DL50 (Dosi Letal 50) de 3.25 μM a &25 μM amb 24 d'incubació. Aquest efecte citotòxic s'incrementava notablement després de 48 hores d'incubació assolint una DL50 de 0.34 a 5.69 μM. Cal destacar que 5 dels 6 casos de les mostres primàries de LCM van mostrar una elevada sensibilitat (DL50 & 8.07 μM). A nivell mecanistic, el SAHA va augmentar l'acetilació de les histones H3 i H4, i va disminuir els nivells de proteïna de la ciclina D1 i c-Flip. La citometria de flux i els anàlisis per Western Blot van posar de manifest que l'efecte citotòxic del SAHA es dóna a través de l'activació de la via mitocondrial de mort cel•lular i la cascada de caspases. El SAHA indueix l'expressió transcripcional de la proteïna proapoptòtica Bmf. Aquests resultats suggereixen que el SAHA podria ser una nova teràpia prometedora per al tractament del LCM.

Several authors have demonstrated an increased number of mitotic figures in breast cancer resection specimen when compared with biopsy material. This has been ascribed to a sampling artifact where biopsies are (i) either too small to allow formal mitotic figure counting or (ii) not necessarily taken form the proliferating tumor periphery. Herein, we propose a different explanation for this phenomenon. Biopsy and resection material of 52 invasive ductal carcinomas was studied. We counted mitotic figures in 10 representative high power fields and quantified MIB-1 immunohistochemistry by visual estimation, counting and image analysis. We found that mitotic figures were elevated by more than three-fold on average in resection specimen over biopsy material from the same tumors (20±6 vs 6±2 mitoses per 10 high power fields, P=0.008), and that this resulted in a relative diminution of post-metaphase figures (anaphase/telophase), which made up 7% of all mitotic figures in biopsies but only 3% in resection specimen (P<0.005). At the same time, the percentages of MIB-1 immunostained tumor cells among total tumor cells were comparable in biopsy and resection material, irrespective of the mode of MIB-1 quantification. Finally, we found no association between the size of the biopsy material and the relative increase of mitotic figures in resection specimen. We propose that the increase in mitotic figures in resection specimen and the significant shift towards metaphase figures is not due to a sampling artifact, but reflects ongoing cell cycle activity in the resected tumor tissue due to fixation delay. The dwindling energy supply will eventually arrest tumor cells in metaphase, where they are readily identified by the diagnostic pathologist. Taken together, we suggest that the rapidly fixed biopsy material better represents true tumor biology and should be privileged as predictive marker of putative response to cytotoxic chemotherapy.
