Os vírus linfotrópicos de células T humano do tipo 1 e 2 (HTLV-1 e 2) são retrovírus que causam o Leucemia / Linfoma de células T do adulto (LLTA) e a Paraparesia Espástica Tropical ou Mielopatia associada ao HTLV-1(PET/MAH). Outras manifestações neurológicas também têm sido atribuídas ao vírus, tais como distúrbios sensoriais e reflexos hiperativos. A prevalência da infecção pelo HTLV-1 no Brasil é alta (0,8% a 1,8%); os HTLV 1 e 2 são endêmicos na região Amazônica. A infecção pelo HTLV e suas doenças associadas ainda são pouco conhecidas dos profissionais de saúde. Trata-se de um estudo descritivo transversal, tipo caso-controle com uma amostra de 76 pacientes portadores do HTLV-1/2 assistidos no Núcleo de Medicina Tropical, em Belém-Pará. Foram submetidos a avaliações clínico-funcional (OMDS), neurológica, laboratoriais (contagem de linfócitos T CD4+, quantificação da carga proviral) e exame de imagem de ressonância magnética (RNM). Os pacientes com HTLV-1com avaliação neurológica foram considerados casos (n=19) e os pacientes assintomáticos sem alteração neurológica foram os controles (n=40). O sexo feminino foi mais prevalente (66,1%), a média de idade foi de 50.7 anos. A distribuição média da contagem de linfócitos T CD4+ nos dois grupos esteve dentro da faixa da normalidade, a carga proviral mostrou-se mais elevada no grupo de casos, a pesquisa do anticorpo anti-HTLV-1 no LCR foi positiva em 93,3% dos casos. A avaliação neurológica revelou 16 (84.2%) pacientes com PET/MAH (p<0.0001). Em 73.7% (14) dos casos, a duração da doença ficou entre 4 a 9 anos. A pesquisa da força muscular em flexão e extensão dos joelhos mostrou que 63.2% dos casos apresentavam grau 3 e 68.4% grau 4, respectivamente (p<0.0001). Normorreflexia em MMSS, além de hiperreflexia no patelar e no Aquileu, em 78.9% e 73.7%, respectivamente. Sinal de Babinski bilateral foi visto em 73.7% dos casos e o sinal de Hoffman em 26.3%. Clônus bilateral esteve presente em 13 pacientes. Sensibilidade tátil alterada (31.6%), hipertonia de MMII (63.2%) e sintomas urinários foram observados em 89.5% dos casos. Das 17 RNM realizadas, 13 (76.47) tinham alteração de imagem em medula torácica. Não houve associações entre carga proviral, OMDS, duração da doença e RNM. A maioria dos casos de doença neurológica associada ao HTLV-1 era compatível com PET/MAH; a carga proviral elevada perece ser um marcador de desenvolvimento de doença.

O Vírus Linfotrópico Humano de Células T é um oncoretrovírus responsável por doenças linfoproliferativas, inflamatórias, degenerativas do Sistema Nervoso Central e por algumas alterações imunológicas do ser humano. Embora tenha associações com várias outras patologias, a Paraparesia Espástica Tropical ou Mielopatia Associada ao HTLV (PET/MAH), doença progressiva e incapacitante do Sistema Nervoso, e a Leucemia/Linfoma de Células T do Adulto (LLcTA), doença linfoproliferativa maligna e letal, são os principais agravos consistentemente definidos como provocados pelo HTLV-1. A propagação do vírus acontece de forma silenciosa, especialmente de mãe para filhos e pela via sexual. No Brasil, onde existem regiões de alta prevalência, ainda são escassas informações oficiais sobre essa transmissão. O objetivo do presente trabalho foi determinar a soroprevalência de anticorpos contra o Vírus Linfotrópico Humano de Células T – tipos 1 e 2 (HTLV-1/2) entre familiares de portadores confirmados do vírus, matriculados no ambulatório do Núcleo de Medicina Tropical (NMT), para estudar as características da transmissão do HTLV nos grupos familiares da região metropolitana de Belém do Pará. Foi realizado um estudo transversal, de base ambulatorial, envolvendo 82 pacientes matriculados no NMT e seus respectivos familiares, os quais foram submetidos à pesquisa de anticorpos anti-HTLV-1/2, utilizando-se o teste de ELISA (Ortho Diagnostic System Inc., US), no período entre junho de 2007 e novembro de 2009. A Soroprevalência da infecção pelo HTLV-1/2 foi observada em 40,2 % (33/82) das famílias e 24,0 % (50/208) no total de familiares pesquisados. A transmissão de mãe para filho(a) ocorreu em 23,2 % (19/82) das famílias, com taxas de soropositividade de 22,4 % (17/76) para filhas e 15,2 % (7/46) para filhos (p > 0.05). A transmissão sexual provável ocorreu em 25,6 % (21/82) das famílias e em 42,0 % (21/50) dos casais, com taxas de soropositividade de esposas e maridos de 53,1 % (18/34) e 18,8 % (3/16), respectivamente (p < 0.05). Não houve diferença significativa de soroprevalência entre familiares de portadores sintomáticos e assintomáticos e entre HTLV-1 e HTLV-2. Conclui-se que existe agregação da infecção nas famílias investigadas e que os dados obtidos estão em acordo com os previamente relatados na literatura. Os serviços de atendimento precisam realizar, rotineiramente, a educação dos indivíduos portadores de HTLV e manter ativas as medidas de controle dos comunicantes familiares, para evitar a propagação do vírus principalmente através do contacto sexual e amamentação.

A etiologia do carcinoma epidermóide do pênis (CEP) está relacionada a múltiplos fatores de risco como a presença de prepúcio, pobre higiene, dermatites crônicas e tabagismo. Entretanto, o fator de risco mais extensivamente estudado, éa infecção pelo papiloma vírus humano (HPV). Está bem estabelecido que esteexerce um importante papel etiológico em neoplasias da cérvice uterina; entretanto, sua associação com CEP tem sido foco de debates, demonstrando-se presença viral variando entre 15% a 80% dos casos, sugerindo que apenas um subgrupo desses tumores seja causado pelo HPV. O prognóstico da doença é influenciado negativamente principalmente pela presença de metástases em linfonodos inguinais. Desta forma, a abordagem cirúrgica destes, assume importância decisiva na cura da doença. Poroutro lado, os métodos disponíveis para o estadiamento desses linfonodos são imprecisos e as linfadenectomias são acompanhadas de morbidade significativa. A descrição de outros marcadores histológicos é escassa, devido a raridade desses tumores.O objetivo do presente trabalho é avaliar a prevalência, distribuição e associação do HPV com parâmetros histológicos de pior prognóstico, no sentido de determinar seu possível valor preditivo para metástases inguinais, assim como avaliar os fatores prognósticos já descritos. Espécimes tumorais em parafina de 82 pacientes portadores de carcinoma epidermóide do pênis, foram testados para prevalência e distribuição do genótipo do HPV por PCR. O estado do HPV foi correlacionado com fatores histopatológicos e ocorrência de metástases inguinais. Foi também avaliada a influência de diversascaracterísticas histológicas tumorais, na sobrevida livre de doença inguinal em 5 anos. O seguimento variou entre 1 e 71 meses (média 20 meses). O DNA do HPV foi identificado em 60,9% da amostra, tendo maior prevalência dos tipos virais 11 e 6 (64% e 32%, respectivamente). Não houve correlação independente significativa das variáveis histológicas de pior prognóstico com o estado do HPV. A probalidade de sobrevida livre de doença inguinal em 5 anos, também não foi influenciada pelo HPV (log Rank teste=0,45). Os únicos fatores patológicos independentes para metástases inguinais foram estadiamento T≥T1b-T4 (p=0,02), invasão linfovascular (p=0,04) e fronte de invasão infiltrativa (p-0,03). O estado e a distribuição do HPV não mostrou correlação com fatores histológicos de pior prognóstico, nem mostrou importância na predição de metástases linfonodais em CEP.

As caderinas compreendem uma classe de moléculas de adesão celular expressa na superfície de todas as camadas epidérmicas. A E-caderina é a principal caderina envolvida na adesão celular epitelial. A redução de sua expressão está envolvida na progressão de alguns tipos de câncer, no potencial metastático e ainda na definição do prognóstico, principalmente nos carcinomas. O carcinoma de células escamosas e o tumor de células basais são neoplasias cutâneas malignas que afetam os cães. O objetivo deste estudo foi avaliar a expressão da E-caderina no carcinoma de células escamosas (n=20) e no tumor de células basais (n=15), buscando-se relacionar sua expressão ao comportamento biológico desses tumores. Os carcinomas de células escamosas apresentaram significativa redução da expressão da molécula comparado aos tumores de células basais, quando avaliado pelo teste de Fisher (P=0,0039). Também foi observado que células neoplásicas mais diferenciadas apresentaram coloração mais intensa que as menos diferenciadas. Em conclusão, sugere-se que a expressão reduzida da E-caderina em tumores cutâneos pode indicar maior poder infiltrativo e consequentemente mau prognóstico na espécie canina.

Lower gastrointestinal tract neoplasms are rare in domestic animals, representing 3% of all biopsies and necropsies. Our objective is to describe the histopathological and B-mode ultrasound findings in a case of duodenal adenocarcinoma in a dog, since it commonly occurs in the large intestine. On ultrasound examination, there was transmural wall thickening in the duodenum with loss of layer structure, focal peritonitis and adenopathy. Histopathological examination of the intestinal mass revealed a malignant epithelial neoplasm, densely cellular and infiltrative, extending through the lamina propria, submucosa, muscle and hypodermis, characterizing a duodenal adenocarcinoma. We conclude that adenocarcinomas may be included in the differential diagnosis of transmural lesions in the duodenum in B-mode ultrasound examinations, but a laparotomy and biopsy are required for definitive diagnosis.

Odontogenic cysts are considered as nonneoplasic benign lesions. Among the cysts, keratocyst odontogenic tumor (KCOT) is an intra‑osseous tumor characterized by parakeratinized stratified squamous epithelium and a potential for aggressive, infiltrative behavior, and for the possibility to develop carcinomas in the lesion wall. Thus, the aim of this study was to describe a clinical case of KCOT in a young patient and discuss the treatment alternatives to solve this case. A 15‑year‑old male was referred for treatment of a giant lesion in his left side of the mandible. After the biopsy, a diagnostic of KCOT was made, and the following procedures were planned for KCOT treatment. Marsupialization was performed for lesion decompression and consequent lesion size reduction. Afterward, enucleation for complete KCOT removal was performed followed by third mandibular molar extraction. After 5 years, no signs of recurrence were observed. The treatment proposed was efficient in removing the KCOT with minimal surgical morbidity and optimal healing process, and the first and second mandibular molars were preserved with pulp vitality. In conclusion, this treatment protocol was an effective and conservative approach for the management of the KCOT, enabling the reduction of the initial lesion, the preservation of anatomical structures and teeth, allowing quicker return to function. No signs of recurrence after 5 years were observed.

Introduction: The Keratocystic Odontogenic Tumor (KCOT) is a benign odontogenic tumor with an infiltrative and potentially aggressive behavior with high recurrence rates. The KCOT occurs more often in men than women, with a frequency of 2:1, being more frequent in the mandible with a predilection for the body and branch. Treatment of KCOT remains controversial. Treatment usually includes enucleation, marsupialization, peripheral ostectomy, curettage associated with Carnoy solution and resection. Objective: To report a case of a KCOT located in the mandible. Case report: male patient, 15 years, with a KCOT on the right side of the mandible treated by enucleation and peripheral ostectomy, with four years of preservation, with no signs of recurrence. Final Comments: The treatment by enucleation associated with peripheral ostectomy reduces the relapse rate, preserves anatomical structures and can avoid a second surgical procedure for reconstruction of bone defects generated in surgery en bloc resection.

The squamous cell carcinoma (SCC) is a neoplasm that affect pets and production animals and it’s very common in tropical countries like Brazil; develops in sparsely pigmented, stratified squamous epithelium and in mucosal surfaces exposed Ultraviolet action. The SCC is quite infiltrative but rarely causes metastases. Its occurrence in the female reproductive tract is recognized in the literature on cattle breeds from Europe. This case was a female bovine, Nelore, adult, who was referred to the "Hospital Veterinário Luiz Quintiliano de Oliveira" with dark brown fluid leakage and putrid odor, associated with ulcerative growth of the vagina. Because the extent of injury, the animal was euthanized and taken to the necropsy, which was observed on board structure, ulcerated and pus in the vaginal floor, infiltrated into the pelvic cavity to the serosa of the uterine body. The microscopic findings were detected neoplastic proliferation of epithelial cells in the floor and vaginal metastases in the lung parenchyma, and classified the primary tumor and metastasis as squamous cell carcinoma moderately differentiated.

The Squamous Cell Carcinoma is what more happens among the oral cancers, being considered sweats incidence in more than 90%. As important prognostic factor competes the early diagnosis and the immediate institution of the treatment, that it can involve radiotherapy and chemotherapy besides the surgery. The initial lesions don’t offer great diagnostic difficulties and now complemental laboratory exams define the diagnosis quickly, In the present report, the initial medical conduct was inadequate, with incomplete interpretation of the laboratorial result and adoption of inadequate therapeutic conduct, what resulted in constant evolution of the lesion. The patient came to the consultation with that medical history, after six months of the first attendance accomplished in another center. To the physical exam, infiltrative ulcerated lesion was observed in the lower lip, with having accentuated tissue loss superficially. The patient accused spontaneous pain and she did use an ointment, recommended six months ago for occasion of the first attendance. As diagnostic conduct was indicated the accomplishment of cytology and biopsy, being defined the diagnosis of Squamous Cell Carcinoma. The patient was immediately guided to the medical service of its municipal district for manegement of the specialized oncologic treatment. The patient was submitted the surgery followed by radiotherapy, being verified involvement of another structures by contiguity. Now, the patient meets abed, without viable perspectives of cure. The present report emphasizes the importance of the clinical knowledge and professional responsibility in attendance of cases of that nature.

The squamous cell carcinoma (SCC) is a neoplasm that affect pets, production animals and exotic animals, and it’s very common in tropical countries like Brazil, once it develops a sparsely pigmented, stratified squamous epithelium and on surfaces mucosa exposed to ultraviolet action. The SCC is quite infiltrative but rarely causes metastases. Its occurrence in the nasal epithelium is widely reported in cats. This case is a chinese hamster (Cricetulus griséus), female, young, who developed a nasal nodule, and their only clinical sign was intense itching. The animal underwent surgery to perform an incisional biopsy, which was referred to the Veterinary Pathology Service of UNESP, campus of Araçatuba (SP) for histopathological evaluation. The material showed features consistent with squamous cell carcinoma well differentiated.

Leukotrienes (LTs) are lipid mediators involved in several inflammatory disorders. We investigated the LT pathway in human T-lymphotropic virus type 1 (HTLV-1) infection by evaluating LT levels in HTLV-1-infected patients classified according to the clinical status as asymptomatic carriers (HACs) and HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) patients. Bioactive LTB4 and CysLTs were both increased in the plasma and in the supernatant of peripheral blood mononuclear cell cultures of HTLV-1-infected when compared to non-infected. Interestingly, CysLT concentrations were increased in HAM/TSP patients. Also, the concentration of plasma LTB4 and LTC4 positively correlated with the HTLV-1 proviral load in HTLV-1-infected individuals. The gene expression levels of LT receptors were differentially modulated in CD4(+) and CD8(+) T cells of HTLV-1-infected patients. Analysis of the overall plasma signature of immune mediators demonstrated that LT and chemokine amounts were elevated during HTLV-1 infection. Importantly, in addition to CysLTs, IP-10 was also identified as a biomarker for HAM/TSP activity. These data suggest that LTs are likely to be associated with HTLV-1 infection and HAM/TSP development, suggesting their putative use for clinical monitoring.

Human T-cell lymphotropic virus type 1 (HTLV-1) is-an RNA virus responsible for diseases such as HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and adult T-cell leukemia/lymphoma (ATL). Cell-to-cell contact and Tax-induced clonal expansion of infected cells are the main modes of virus replication, making virus detection during the viremic stage difficult. Consequently, the proviral load is the current virologic marker for disease monitoring, but the mechanisms of progression have not been established yet. Thus, this study investigated the presence of virus in plasma from asymptomatic HTLV-1 carriers and from HAM/TSP patients. Real-time PCR was performed on DNA from 150 plasma samples; 12(8%) had detectable DNA amplification, including 6(4%) asymptomatic HTLV-1 carriers and 14(26%) HAM/TSP patients (p < 0.005). Of the 33 samples submitted for nested PCR, six (18%, p = 0.02) were positive for HTLV-1 RNA in the plasma. Additionally, 26 plasma samples were treated with DNAse enzyme to eliminate any DNA contamination before RNA extraction. Two of them (8%) showed amplification for HTLV-1 (p = 0.5). Therefore, this study described for the first time the detection of free HTLV-1 RNA in plasma from HTLV-1-infected subjects, regardless of their clinical status. Thus, HTLV-1 viral replication does occur in plasma, and other transmission pathways for HTLV-1 should be investigated further. (C) 2011 Elsevier B.V. All rights reserved.

The authors report the use of photodynamic therapy with methyl aminolevulinate (Metvix (R)) in a patient with nodular and infiltrative basal cell carcinoma in the right lower eyelid. Side effects on the eye were evaluated weekly. After 12 weeks of treatment, to confirm cure the patient was submitted to a 2-mm punch biopsy, the anatomopathological findings of which were negative for neoplasia. Photodynamic therapy with methyl aminolevulinate was shown to be an attractive alternative to surgical excision(-)the current gold standard treatment worldwide.

A glioblastoma multiforme (GBM) is the highest grade glioma tumor (grade IV) and is the most malignant form of astrocytomas. Grade IV tumors, which are the most malignant and aggressive, affect people between the ages of 45 and 70 years. A GBM exhibits remarkable characteristics that include excessive proliferation, necrosis, genetic instability, and chemoresistance. Because of these characteristics, GBMs are difficult to treat and have a poor prognosis with a median survival of less than one year. New methods to achieve widespread distribution of therapeutic agents across infiltrative gliomas significantly improve brain tumor therapy. Photodynamic therapy (PDT) and hyperthermia (HPT) are well-established tumor therapies with minimal side effects while acting synergistically. This study introduces a new promising nanocarrier for the synergistic application of PDT and magnetic hyperthermia therapy against human glioma cell line T98 G, with cellular viability reduction down to as low as 17% compared with the control.
