In this study the first measurements of DMSP in six species of corals and ten species of benthic algae collected from four coral reefs in the Great Barrier Reef are reported, together with DMSP measurements made on cultured zooxanthellae. Concentrations ranged from 21 to 3831 (mean=743) fmol DMSP zooxanthellae(-1) in corals, 0.16 to 2.96 nmol DMSP cm(-2) (mean=90) for benthic macroalgae, and 48-285 fmol DMSP zooxanthellae(-1) (mean=153) for cultured zooxanthellae. The highest concentrations of DMSP in corals occurred in Acropora formosa (mean= 371 fmol DMSP zooxanthellae(-1)) and Acropora palifera (mean=3341 fmol DMSP zooxanthellae(-1)) with concentrations in A. palifera the highest DMSP concentrations reported in corals examined to date. As well as inter-specific differences in DMSP, intra-specific variation was also observed. Adjacent colonies of A. formosa that are known to have different thermal bleaching thresholds and morphologically distinct zooxanthellae, were also observed to have different DMSP concentrations, with the zooxanthellae in the colony that bleached containing DMSP at an average concentration of 436 finol zooxanthellae(-1), whilst the non-bleaching colony contained DMSP at an average concentration of 171 finol zooxanthellae(-1). The results of the present study have been used to calculate the area normalized DMSP concentrations in benthic algae (mean=0.015 mmol m(-2)) and corals (mean=2.22 mmol m(-2)) from the GBR. This data indicates that benthic algae and corals are a significant reservoir of DMSP in GBR waters. (C) 2002 Published by Elsevier Science Ltd.

Human cytomegalovirus (HCMV) can establish both nonproductive (latent) and productive (lytic) infections. Many of the proteins expressed during these phases of infection could be expected to be targets of the immune response; however, much of our understanding of the CD8(+)-T-cell response to HCMV is mainly based on the pp65 antigen. Very little is known about T-cell control over other antigens expressed during the different stages of virus infection; this imbalance in our understanding undermines the importance of these antigens in several aspects of HCMV disease pathogenesis. In the present study, an efficient and rapid strategy based on predictive bioinformatics and ex vivo functional T-cell assays was adopted to profile CD8(+)-T-cell responses to a large panel of HCMV antigens expressed during different phases of replication. These studies revealed that CD8(+)-T-cell responses to HCMV often contained multiple antigen-specific reactivities, which were not just constrained to the previously identified pp65 or IE-1 antigens. Unexpectedly, a number of viral proteins including structural, early/late antigens and HCMV-encoded immunomodulators (pp28, pp50, gH, gB, US2, US3, US6, and UL18) were also identified as potential targets for HCMV-specific CD8(+)-T-cell immunity. Based on this extensive analysis, numerous novel HCMV peptide epitopes and their HLA-restricting determinants recognized by these T cells have been defined. These observations contrast with previous findings that viral interference with the antigen-processing pathway during lytic infection would render immediate-early and early/late proteins less immunogenic. This work strongly suggests that successful HCMV-specific immune control in healthy virus carriers is dependent on a strong T-cell response towards a broad repertoire of antigens.

In a 2-yr multiple-site field study conducted in western Nebraska during 1999 and 2000, optimum dryland corn (Zea mays L.) population varied from less than 1.7 to more than 5.6 plants m(-2), depending largely on available water resources. The objective of this study was to use a modeling approach to investigate corn population recommendations for a wide range of seasonal variation. A corn growth simulation model (APSIM-maize) was coupled to long-term sequences of historical climatic data from western Nebraska to provide probabilistic estimates of dryland yield for a range of corn populations. Simulated populations ranged from 2 to 5 plants m(-2). Simulations began with one of three levels of available soil water at planting, either 80, 160, or 240 mm in the surface 1.5 m of a loam soil. Gross margins were maximized at 3 plants m(-2) when starting available water was 160 or 240 mm, and the expected probability of a financial loss at this population was reduced from about 10% at 160 mm to 0% at 240 mm. When starting available water was 80 mm, average gross margins were less than $15 ha(-1), and risk of financial loss exceeded 40%. Median yields were greatest when starting available soil water was 240 mm. However, perhaps the greater benefit of additional soil water at planting was reduction in the risk of making a financial loss. Dryland corn growers in western Nebraska are advised to use a population of 3 plants m(-2) as a base recommendation.

OBJETIVO: Conhecer a evolução do peso ao nascer e a mortalidade hospitalar de nascidos vivos em maternidade de Campinas, SP. MÉTODOS: Foram estudados 215.435 nascimentos ocorridos em maternidade do Município de Campinas, SP, de 1975 a 1996. As informações foram coletadas de livros de registro e relatórios de assistência médica. Foram analisados os óbitos ocorridos antes da alta hospitalar, independentemente da idade. Para a análise dos dados, utilizou-se o programa Epi Info 6.01. RESULTADOS: O baixo peso se manteve próximo a 9% desde os anos 70, concentrando82% dos óbitos hospitalares em 1996. O peso insuficiente foi observado em 24% dos nascidos vivos. Em 1975, 13,3% dos nascidos com peso entre 1.000 g e 1.499 g sobreviveram e 73,8% dos nascidos com peso entre 1.500 g e 1.999 g. Em 1996, a sobrevida foi 69,1% e 87,7%, respectivamente. Houve grande diminuição do número de óbitos especialmente na faixa de 1.000 g a 1.499 g (de 867 para 309 por mil nascidos vivos, redução de 64,5%) e de 1.500 g a 1.999 g (de 264 para 123 por mil nascidos vivos, redução de 53,4%). A mortalidade hospitalar entre aqueles de peso abaixo de 1.000 g permaneceu entre 78,9% e 100,0%, mesmo nos anos 90. Observou-se queda da mortalidade hospitalar de 17 para 11 por mil nascidos vivos. CONCLUSÕES: Não houve modificação na distribuição do peso ao nascer, a despeito deavanços apresentados por Campinas. Contudo, observou-se queda da mortalidade hospitalar. Outros estudos são necessários para compreender esse perfil.

The methods of molecular biology applied in epidemiological research lead us to the realm of molecular epidemiology, where there is immense potential for the establishment of associations between cancer and exposure to risk factors in lifestyle, profession, or pollution. Human biomonitoring consists, on the one hand, in research and identification of hazardous environmental conditions and, on the other hand, in the assessment of cancer risk following exposure to such conditions. Since carcinogenesis is a lengthy process, the biomarkers used to recognize biological abnormalities are selected and developed in the realm of molecular epidemiology. Such biomarkers are quantifiable and allow for the recognition of progression from normal to abnormal biological conditions at the molecular level. They can be categorized in biomarkers of exposure, effect, and genetic susceptibility. Genotoxicity biomarkers are a particular subset of effect biomarkers and are used to assess genomic instability caused by environmental or occupational exposure, being considered useful carcinogenesis predictors.

Introdução – A cintigrafia de perfusão do miocárdio (CPM) desempenha um importante papel no diagnóstico, avaliação e seguimento de pacientes com doença arterial coronária, sendo o seu processamento realizado maioritariamente de forma semiautomática. Uma vez que o desempenho dos técnicos de medicina nuclear (TMN) pode ser afetado por fatores individuais e ambientais, diferentes profissionais que processem os mesmos dados poderão obter diferentes estimativas dos parâmetros quantitativos (PQ). Objetivo – Avaliar a influência da experiência profissional e da função visual no processamento semiautomático da CPM. Analisar a variabilidade intra e interoperador na determinação dos PQ funcionais e de perfusão. Metodologia – Selecionou-se uma amostra de 20 TMN divididos em dois grupos, de acordo com a sua experiência no software Quantitative Gated SPECTTM: Grupo A (GA) – TMN ≥600h de experiência e Grupo B (GB) – TMN sem experiência. Submeteram-se os TMN a uma avaliação ortóptica e ao processamento de 21 CPM, cinco vezes, não consecutivas. Considerou-se uma visão alterada quando pelo menos um parâmetro da função visual se encontrava anormal. Para avaliar a repetibilidade e a reprodutibilidade recorreu-se à determinação dos coeficientes de variação, %. Na comparação dos PQ entre operadores, e para a análise do desempenho entre o GA e GB, aplicou-se o Teste de Friedman e de Wilcoxon, respetivamente, considerando o processamento das mesmas CPM. Para a comparação de TMN com visão normal e alterada na determinação dos PQ utilizou-se o Teste Mann-Whitney e para avaliar a influência da visão para cada PQ recorreu-se ao coeficiente de associação ETA. Diferenças estatisticamente significativas foram assumidas ao nível de significância de 5%. Resultados e Discussão – Verificou-se uma reduzida variabilidade intra (<6,59%) e inter (<5,07%) operador. O GB demonstrou ser o mais discrepante na determinação dos PQ, sendo a parede septal (PS) o único PQ que apresentou diferenças estatisticamente significativas (zw=-2,051, p=0,040), em detrimento do GA. No que se refere à influência da função visual foram detetadas diferenças estatisticamente significativas apenas na fração de ejeção do ventrículo esquerdo (FEVE) (U=11,5, p=0,012) entre TMN com visão normal e alterada, contribuindo a visão em 33,99% para a sua variação. Denotaram-se mais diferenças nos PQ obtidos em TMN que apresentam uma maior incidência de sintomatologia ocular e uma visão binocular diminuída. A FEVE demonstrou ser o parâmetro mais consistente entre operadores (1,86%). Conclusão – A CPM apresenta-se como uma técnica repetível e reprodutível, independente do operador. Verificou-se influência da experiência profissional e da função visual no processamento semiautomático da CPM, nos PQ PS e FEVE, respetivamente.

Recent integrated circuit technologies have opened the possibility to design parallel architectures with hundreds of cores on a single chip. The design space of these parallel architectures is huge with many architectural options. Exploring the design space gets even more difficult if, beyond performance and area, we also consider extra metrics like performance and area efficiency, where the designer tries to design the architecture with the best performance per chip area and the best sustainable performance. In this paper we present an algorithm-oriented approach to design a many-core architecture. Instead of doing the design space exploration of the many core architecture based on the experimental execution results of a particular benchmark of algorithms, our approach is to make a formal analysis of the algorithms considering the main architectural aspects and to determine how each particular architectural aspect is related to the performance of the architecture when running an algorithm or set of algorithms. The architectural aspects considered include the number of cores, the local memory available in each core, the communication bandwidth between the many-core architecture and the external memory and the memory hierarchy. To exemplify the approach we did a theoretical analysis of a dense matrix multiplication algorithm and determined an equation that relates the number of execution cycles with the architectural parameters. Based on this equation a many-core architecture has been designed. The results obtained indicate that a 100 mm(2) integrated circuit design of the proposed architecture, using a 65 nm technology, is able to achieve 464 GFLOPs (double precision floating-point) for a memory bandwidth of 16 GB/s. This corresponds to a performance efficiency of 71 %. Considering a 45 nm technology, a 100 mm(2) chip attains 833 GFLOPs which corresponds to 84 % of peak performance These figures are better than those obtained by previous many-core architectures, except for the area efficiency which is limited by the lower memory bandwidth considered. The results achieved are also better than those of previous state-of-the-art many-cores architectures designed specifically to achieve high performance for matrix multiplication.

Diagnosis of cytomegalovirus infections by qualitative and quantitative PCR in HIV infected patients

A high incidence of cytomegalovirus (CMV) infections is observed in Brazil. These viruses are causatives of significant morbidity and mortality among patients with advanced human immunodeficiency virus (HIV) infection. This work, shows the application of a PCR on determination of CMV load in the buffy coat and plasma. We analyzed the samples of 247 HIV infected patients in order to diagnose CMV infection and disease. We developed a semi-quantitative PCR that amplifies part of the glycoprotein B (gB) gene of CMV. The semi-quantitative PCR was carried out only in positive clinical samples in a qualitative PCR confirmed by a nested-PCR. CD4 lymphocyte count, HIV viral load and CMV disease symptom were correlated with CMV load. CMV genome was detected in the buffy coat of 82 of 237 (34.6%) patients, in 10 of these the CMV load was determined varying between 928 and 332 880 viral copies/mug DNA. None of these 237 patients developed any suggestive manifestation of CMV disease. For the other 10 HIV infected patients selected based on the suspicion of CMV disease, CMV genome was detected in only one case. This patient presented a high CMV load, 8 000 000 copies/mug DNA, and developed a disseminated form of CMV disease including hepatitis and retinitis. Our results were greatly influenced by the impact of the highly active antiretroviral therapy that reduced incidence of CMV viremia and occurrence of CMV disease in the HIV infected patients.

Seroprevalence of HCMV in Costa Rica is greater than 95% in adults; primary infections occur early in life and is the most frequent congenital infection in newborns. The objectives of this study were to determine the genetic variability and genotypes of HCMV gB gene in Costa Rica. Samples were collected from alcoholics, pregnant women, blood donors, AIDS patients, hematology-oncology (HO) children and HCMV isolates from neonates with cytomegalic inclusion disease. A semi-nested PCR system was used to obtain a product of 293-296 bp of the gB gene to be analyzed by Single Stranded Conformational Polymorphism (SSCP) and sequencing to determine the genetic polymorphic pattern and genotypes, respectively. AIDS patients showed the highest polymorphic diversity with 14 different patterns while fifty-six percent of HO children samples showed the same polymorphic pattern, suggesting in this group a possible nosocomial infection. In neonates three genotypes (gB1, gB2 and gB3), were determined while AIDS patients and blood donors only showed one (gB2). Of all samples analyzed only genotypes gB1, 2 and 3 were determined, genotype gB2 was the most frequent (73%) and mixed infections were not detected. The results of the study indicate that SSCP could be an important tool to detect HCMV intra-hospital infections and suggests a need to include additional study populations to better determine the genotype diversity and prevalence.

Cytomegalovirus (CMV) is a genus in the family Herpesviridae that has been associated with gastrointestinal syndromes. In this work we looked for a possible association of CMV infection with colorectal cancer and ulcerative colitis (UC). Blood and enteric tissue samples of 14 patients with colorectal cancer and of 21 with UC were subjected to a nested-PCR that amplifies part of the gB gene of CMV and also to immunohistochemistry using a specific monoclonal antibody to IE 76kDa protein of CMV. CMV was detected by nested-PCR in the blood and/or the enteric tissue of nine (64.3%) colorectal cancer and 16 (76.2%) ulcerative colitis patients. In the immunohistochemistry it was observed that 12 (12/21, 57.1%) positive enteric tissue samples of patients with UC and none from patients with colorectal cancer (0/14) were positive to CMV. The positivity of CMV infections in the UC patient group (12/21, 57.1%) showed by both techniques, was significantly higher (p = 0.015) than that observed for colorectal cancer patients (2/14, 14.3%). These results suggest an association of ulcerative colitis with CMV infection of the enteric tissue.

BACKGROUND: Cytomegalovirus (CMV) remains an important pathogen to immunocompromised patients even in the era of HAART. The present study aimed at evaluating the influence of CMV viral load and its gB genotypes on AIDS patients' outcome. METHODS: Blood samples of 101 AIDS patients were collected and tested for HIV load, CD4 - cell count and opportunistic pathogens, including CMV. Semi-nested PCRs were run to detect CMV genome and in the positive samples, gB genotyping and CMV load were established using enzymatic restriction and real time PCR, respectively. All patients were clinically followed for four years. RESULTS: In thirty patients (31%) CMV was detected and all fatal cases (n = 5) occurred in this group of patients (p = 0.007), but only two patients had CMV disease (1.9%). However, viral load was not statistically associated with any analyzed parameter. The most frequently observed CMV genotype was gB2 (45.16%) followed by gB3 (35.48%). gB2 genotype was more frequently found in patients with CD4-cell counts under 200 cells/mm³ (p = 0.0017), and almost all fatal cases (80%) had gB2 genotype. CONCLUSIONS: Our study suggests that CMV and its polymorphisms in biologically relevant genes, such as the gB encoding ORF, may still influence the prognosis and outcome of AIDS patients. The gB2 genotype was associated to patient's bad outcome.

Os autores realizando capturas de Triatomíneos na favela do Morro do Telégrafo (São Cristóvão - GB), onde as construções, na sua maioria são de barro ou estuque puderam, constatar: 1) - Todos os exemplares capturados eram da espécie Triatoma rubrofasciata (De Geer, 1.773). 2) - De 170 domicílios examinados, foram encontrados triatomíneos em 23, sendo que o mínimo de triatomíneos por casa foi de 1 exemplar e o máximo de 56 exemplares. Esta última cifra foi obtida em duas capturas consecutivas no mesmo local. 3 - Entre 115 exemplares capturados, 8 eram ninfas do 1º estágio, 15 de 2º, 10 de 3º, 5 de 5º, 11 de 6º, 42 eram adultos machos e 24 fêmeas 4) - Nenhum dos hemipteros examinados mostrou-se infectado por qualquer flagelado. 5) - A reação de precüpitina feita em 17 exemplares revelou resultado positivo para sangue humano em 15, para rato em 1 e para porco e galinha em 1.

Introdução: A migração, apesar de não ser um fenómeno recente entre as profissões de saúde assumiu nos últimos anos proporções significativas, sobretudo em África. Para a GB, a saída dos profissionais de saúde tem um impacto devastador no que concerne o desempenho do sistema de saúde e por conseguinte a saúde das populações. Portugal, pelas ligações histórias e pela proximidade que mantém com a GB, tem sido escolhido pelos profissionais de saúde como o país destino para a emigração. Neste sentido considerou-se importante aprofundar conhecimentos sobre o fenómeno migratório dos profissionais de saúde da GB para Portugal, incluindo as motivações dos profissionais de saúde, a realidade vivida em Portugal e os motivos para o regresso. Objectivos: Caracterizar o fenómeno migratório dos profissionais de saúde guineenses para Portugal, ao longo de todo o processo migratório, desde a decisão de emigrar, à decisão de retorno. Material e métodos: Este estudo teve por base uma abordagem qualitativa, optando-se pela realização de um estudo de caso único, com várias unidades de análise. O caso em estudo foi o fenómeno migratório, tendo como objecto de estudo os profissionais de saúde guineenses nele envolvidos. Utilizaram-se métodos não probabilísticos para a selecção das amostras. Foram realizadas um total de 4 entrevistas semi-estruturadas a profissionais de saúde, 3 grupos focais a enfermeiros e 3 entrevistas a informadores chave na GB. Em Portugal, foram realizadas 12 entrevistas semi-estruturadas. Posteriormente, realizou-se a análise de conteúdo e análise exploratória dos dados. Resultados: As condições de trabalho, o acesso a formação, o crescimento e valorização profissionais, assim como, a estabilidade político-militar e melhores condições de vida, são factores essenciais para a motivação e empenho dos profissionais de saúde, e por conseguinte, funcionam como factores impulsionadores da emigração quando não existentes. Enquanto imigrantes, os profissionais médicos e enfermeiros não têm o mesmo percurso em termos de integração profissional. Os enfermeiros recentemente emigrados não obtêm o reconhecimento das suas habilitações ao contrário dos médicos. Contudo, a melhoria das condições de vida é, na generalidade, alcançada. Existe na maioria dos profissionais o desejo de regressar ao país de origem, estando a sua concretização dependente de condições tanto na GB como em Portugal. Conclusão: O fenómeno migratório é algo complexo, com inúmeros factores e em constante evolução, dizendo respeito sobretudo à procura de melhores condições de trabalho e de vida. O presente estudo serviu para identificar alguns destes factores e caracterizar as etapas do processo migratório dos profissionais de saúde da GB para Portugal, desde a decisão de emigrar à decisão de regressar. Deixa também pistas para desafios que se impõem com a migração destes profissionais.
